INVITATION TO A DISCIPLINARY MEETING
Dear [EMPLOYEE'S NAME],
Disciplinary meeting
I am writing to invite you to attend a disciplinary
meeting in [PLACE] on [DATE] at [TIME]. The
purpose of the meeting is to consider an
allegation of [misconduct OR gross misconduct]
against you.
The allegation is that [SET OUT ALLEGATION -
FOR EXAMPLE, "you assaulted another
employee"
The basis for this allegation is that [GIVE
SUMMARY OF INFORMATION OBTAINED AS A
RESULT OF INVESTIGATIONS INTO THE
ALLEGATION - FOR EXAMPLE: "[NAME] has
complained that you assaulted him in the canteen
on [DATE]. We have carried out an investigation
and have received statements from [NAME] and
[NAME] who witnessed the assault taking place"
I enclose a summary of the findings of the
investigation, which sets out further detail of the
allegations, and copies of relevant witness
statements and other documents which may be
used at the disciplinary meeting. The documents
which I attach are:

The ACAS code states: ‘Where
an employer or employee
intends to call relevant
witnesses they should give
advance notice that they intend
to do this’ and ‘The Employee
should be given a reasonable
opportunity to ask questions,
present evidence, and call
relevant witnesses’.
The ACAS code states: ‘If it is
decided that there is a disciplinary
case to answer, the employee
should be notified of this in
writing. This notification should
contain sufficient information
about the alleged misconduct or
poor performance and its possible
consequences to enable the
employee to prepare to answer
the case at a disciplinary
meeting’.
The ACAS code states: ‘It would
normally be appropriate to
provide copies of any written
evidence, which may include any
witness statements, with the
notification’.

wish to be considered at the meeting, please
provide copies as soon as possible. If you do not
have those documents, please provide details so
that they can be obtained.
The meeting will be held in accordance with the
Disciplinary Procedure which is [attached OR set
out in the Staff Handbook]. If you are found guilty
of misconduct we may decide to [issue you with a
written warning or a final written warning OR
dismiss you with notice or pay in lieu of notice]. [If
you are found guilty of gross misconduct, you
may be dismissed without notice or pay in lieu of
notice.]
The meeting will be conducted by [NAME] and
the following people will also be present: [GIVE
NAMES OR JOB ROLES OF PARTICIPANTS].
You are entitled to bring a fellow employee or a
trade union representative to the meeting in
accordance with our Disciplinary Procedure. If
you wish to bring a companion, please let me

pending the outcome of the disciplinary meeting.]

and that you will attend at the time stated above.
