Ministry of Climate Change & Environment (MOCCAE) Model of Veterinary Health Certificate for Exporting Cats and Dogs to the UAE This document contains the "model" health certificate required for exporting cats and dogs to the UAE. By "model", we mean that the UAE government has set out strict requirements regarding the information which must be provided for each animal coming into the UAE, but they do not prescribe how that information is presented by the shipper from the country of origin. This means that you must provide all the information required (as requested in the following pages) but that you may, if you wish, create your own redesigned forms/form structure, designed your way, for the purposes of presentation of the required information, if you (a pet owner or a pet shipping agent) determine that a change in presentation is somehow to your advantage. You may also, of course, simply use the "model" form designed by the UAE MOCCAE, as is. There are 8 pages which follow. The first 4 pages are the "model" MOCCAE health certificate as published by the UAE government but include DKC's own annotations provided to you as an aid to understanding what is required for all data fields. The second set of 4 pages are again the same "model" health certificate, but completely pristine and untouched by DKC. Model of Veterinary Health Certificate for Exporting Cats and Dogs to the UAE

This document contains the "model" health certificate required for exporting cats and dogs to the UAE. By "model", we mean

that the UAE government has set out strict requirements regarding the information which must be provided for each animal
