This Knowledge Builder will walk an employee through enrolling in or cancelling benefits due to a marriage.
Follow the steps below to enroll in or cancel benefits due to a marriage. You must scan and save your marriage certificate before starting as you will be prompted to upload before you can go further. Please keep in mind that you only have a 30 day window from the qualifying event in order to make changes to your benefits.
1. In Workday, click Benefits.
2. Click Benefits under the Change column.

3. Select Marriage from the Benefit Event Type dropdown menu
4. Enter the date of that you were married in the Benefit Event Date
5. Attach marriage certificate to your event
•Drag and drop your document into the gray area, or click select files, and choose the documentation from your computer
6. Click Submit.

7. Click Open to begin the process of entering your benefit changes.
8. If you are enrolling in medical, dental and/or vision benefits, select Elect.
If you are cancelling medical, dental and/or vision benefits, select Waive.
9. Select your dependents from the prompt button in the Enroll Dependents section.
If a dependent you need to cover is not listed, you will need to add the dependent by following steps 10 through 13.
If your dependent(s) is listed, you can select them from the list and move to step 14.

10. Click Add my dependent from enrollment.
11. If your dependent is a beneficiary or emergency contact, click Yes, and then select who your
dependent is in the drop down box.
a. If they are not a beneficiary or emergency contact, select No
TIP: If you wish to use your dependent as a beneficiary for your life insurance, select Yes when asked if you want to use your dependent as a beneficiary.

12. Follow the prompts to enter information about your dependent. Items marked with a red
asterisk are required fields.
13. Click OK when you have added your dependent’s information.
14. Repeat steps 9-13 for each dependent you wish to cover for medical, dental and/or vision.
Note: Once you’ve added a dependent to one benefit, you do not need to create a new
dependent profile for them. They will be listed under your existing dependents button
TIP: You can select your newly created dependent by clicking the Prompt Button in the Enroll
Dependents, click Dependents, and click the name of the dependent.
15. Click Continue to go next screen.

16. You may be asked to enter your dependents Social Security Number. If you do not have this
information, you can enter a reason why the number is not available and then click Continue.
Note: The Affordable Care Act (ACA) requires that everyone have qualifying insurance, qualify for an exemption, or potentially pay a tax penalty. As part of the ACA, employers must file an annual report with the IRS that includes Social Security numbers for all employees, spouses, and dependent children enrolled in your medical insurance. One purpose of the report is to show the IRS that employees and their dependents have qualifying health coverage so they won't be subject to ACA's individual-mandate tax penalty. Therefore, it is important to provide your dependent’s Social Security Number.
17. If you have enrolled in the Consumer Driven Health Plan (CDHP), you may be eligible to enroll in a Health Savings Account (HSA) administered by HSA Bank. If you are enrolling in the HSA account, you can enter your annual election in the “How much do you want to contribute for the total year?” or you can enter the amount you wish to come out of each pay check.

19. If you have enrolled in the Copay Plan, you may be eligible to enroll in a Flexible Spending Account
(FSA) administered by Lifetime Benefit Solutions. If you are enrolling in the FSA plan, you can enter
your annual election in the “How much do you want to contribute for the total year?” or you can
enter the amount you wish to come out of each pay check.
Note: Any employee who works 20+ hours can enroll in the Child Care FSA. This benefit is used for
day care expenses.
20. Click Continue
21. Life Insurance and Accidental Death & Dismemberment insurance are available to employees
scheduled to work 20+ hours per week. Depending on your scheduled hours, you may not have
any elections to make on this screen.

22. Click Elect for the benefit plan you would like to enroll in.
23. Click the Prompt Button and select the amount of coverage you want
Note: If you enrolled in the employee optional life, AD&D, you can elect spousal life insurance- only up to 50% of your optional life insurance amount. Please be aware that you may be subject to providing evidence of insurability to Liberty Mutual.
24. Click Continue
25. If you are enrolled in voluntary life and/or voluntary AD&D insurance, you will be prompted to
enter or review your Beneficiaries.
a. To add a beneficiary, click the + button to the left of the Beneficiary column.

26. Click the prompt on the right hand side of the field, select Beneficiary Persons and choose the beneficiary’s name from the drop down list.
27. If the beneficiary is not yet listed, go back and click Create > and follow the prompts to add the beneficiary’s information.
28. To remove a beneficiary, click the - next to the individual’s name.
29. You may list different beneficiaries for Supplemental Life Insurance and Accidental Death & Dismemberment, but it is not required. At minimum, supply beneficiary information for the Basic/Core Life Insurance coverage provided to you.
30. You can designate a combination of primary beneficiaries and contingent beneficiaries. In the event of your death, benefits would be payable to your primary beneficiary. If no primary beneficiary is living at that time, benefits would be payable to your contingent beneficiaries. Assign a percentage amount to each beneficiary. The percent allocation for primary and contingent categories must each total 100%.

31. Click continue to proceed.
32. HSA Enrollment Attestation: If you enrolled in the HSA, you must certify that you meet the eligibility requirements to participate in an HSA as outlined on the prior HSA page.
33. Click continue to proceed.
34. On the last page, review your benefit elections. If applicable, pay attention to any messages
regarding the need to complete an Evidence of Insurability form.
TIP: If you need to make changes to your elections, click Go Back.
If you need more time to make your elections, click Save for Later and the changes you have made will be saved. Your elections will not be finalized until you submit them. Make sure you go back into the system and submit your elections by the Submit By date shown at the top of the screen.

35. When you are satisfied with your elections, scroll to the bottom of the page, review the
information in the Electronic Signature and click I Agree.
36. Click Submit to submit your benefit elections.
37. Click Print for a PDF copy of your Benefit Confirmation Statement. Then click Done.
Remember
If you elect an amount of Life Insurance that requires Evidence of Insurability you will receive the notification below. Liberty Mutual will send you the proper documentation, complete it and send it directly back to them. You will be notified if they require additional information or if your elected amount was approved.
