ebbie and Larry Weber's five daughters started playingcompetitive basketball in Taylorville, Ill. Four of them suf-fered a type of serious knee injury that plagues many female

athletes.They each tore their ACL, or anterior cruciate ligament, which is a

part of the knee that helps stabilize the joint during twisting maneu-vers and changes in direction that are common in sports like basket-ball and soccer.

Biology, hormones and neuromuscular differences make femaleathletes more likely than males to tear their ACLs, but doctors sayresearch is helping point out specific risk factors and refine tech-niques that can almost totally eliminate the disparity.

"While we perform very successful surgery and can get the athleteback to high-levelcompetition, it is bet-ter to prevent the in-jury in the firstplace," said Dr. Di-ane Hillard-Sembell,an orthopedic sur-geon at SpringfieldClinic in Illinois whohas conducted exten-sive research on ACLinjuries and hopes tobegin publishing theresults in medicaljournals this year.

Emerging researchHillard-Sembell can empathize with in-

jured athletes because she was a standouthigh school and college basketball player whounderwent ACL surgery in her late 20s andearly 30s on both of her knees.

She began a study in 2002 to examine theo-ries surrounding ACL risks in females andlearn what works best to prevent injuries.

Hillard-Sembell and her colleagues foundthat the relatively weaker hamstrings in fe-males put them at increased risk of knee in-juries.

"The hamstrings are a protector of theACL," Hillard-Sembell said.

In males, the quadriceps, muscles at thefront of the thigh, typically are stronger thanthe hamstrings, at the back of the thigh, by aratio of 3 to 2. But in females, the quads maybe twice as strong as the hamstrings, Hillard-Sembell said.

But her research uncovered something new,which has since been confirmed by other re-searchers: the importance of muscles at theside of the hips. If those muscles are weak, thepelvis will collapse inward, making it morelikely for the thigh to angle inward, she said.

It's different for girlsAn estimated 150,000 ACL injuries occur

in the U.S. each year, generating more than$500 million in medical costs, according tothe American Orthopaedic Society for SportsMedicine. One ACL tear can cost about$30,000 when all the costs for surgery, hospi-talization and rehabilitation are included.

The 30 percent of ACL injuries that occurwhen athletes collide and land afterward can'tbe prevented in most cases. But when it comesto "non-contact" injuries, females are four toeight times more likely than males to injuretheir ACL, Hillard-Sembell said.

The peak ages for injuries in females are 14and 15, then again at 19 and 20.

The main factors that put females at in-creased risk of ACL injuries are anatomy andbiomechanics, Hillard-Sembell said. Females'relatively wider hips and pelvis — built forchildbearing — create more potential for"knock-knee" landings that can put specialpressure on the ACL.

Females also tend to be more flatfooted,and the anatomy of their knees is slightly dif-ferent from males.

How to prevent ACL injuries■ Plyometrics is a rapid, powerful movementthat first lengthens a muscle (eccentricphase) then shortens it (concentric phase).The length-shortening cycle increases mus-cular power. An example would be an athletejumping off a small box and immediatelyjumping back into the air after contact withthe floor.

■ Balance training commonly involves theuse of wobble or balance boards. On-fieldbalance exercises may include throwing aball with a partner while balancing on oneleg.

■ To improve single-leg core strength andstability, athletes perform exercises such asjumping and landing on one leg with theknee flexed and then momentarily holdingthat position.

A report released from The Children's Hospi-tal of Philadelphia and State Farm InsuranceCompanies shows the impact of teen drivercrashes extends far beyond teen drivers' fami-lies and friends. In 2008, 681,000 people wereinvolved in crashes where a teen driver was be-hind the wheel. More than 40,000 were in-jured, and nearly 30 percent of those who diedin these crashes were not in cars driven byteens.

"When most people think about those affect-ed by teen driver crashes, they think of the teensbehind the wheel. We must also consider thesignificant impact of these crashes on ... occu-pants of other vehicles, pedestrians, cyclists andother road-users," says Dr. Dennis Durbin, co-author of the report. "Reducing speeding andalcohol use, increasing seat belt use, and elimi-nating distractions for teen drivers are the fourcalls-to-actions we see in this report that wouldhave great impact on reducing injuries and fa-talities for all road users.”

"Although excess risk in the immediatefamily might indicate evidence of a geneticcontribution, it could also simply indicateshared environment risks or household expo-sure that may be contributing to the disease,"noted study author Dr. Alpesh A. Patel, assis-tant professor of orthopaedic surgery at theUniversity of Utah School of Medicine. "Con-versely, excess risks in second- and third-de-gree relatives strongly support a genetic con-tribution to disease, given the measurable ge-netic sharing in these more distant relativesand the relative absence of shared householdrisks."

Hazards of Texting While Driving While driving recently, I caught myself trying to answer

an e-mail on my cell phone that I thought, at the time, was urgent. As I was attempting to type a response, shifting my attention from the road, to my Blackberry, and back to the road again, the incredible stupidity of what I was doing struck me.

The National Highway Safety Administration recently reported that almost 80% of all auto crashes are caused by distracted drivers. Further, 37% of teens in a recent study rated texting while driving as “extremely” distracting or “very” distracting, yet over 90% of all teens do so.

According to a recent study conducted by researchers at Eastern Virginia Medical School and Children’s Hospital of the Kings’ Daughters in Norfolk, driving while text messaging or fiddling with an MP3 player is an even greater hazard than talking on a cell phone while driving.

In the study, the researchers had 21 teenagers between the ages of 16 and 18 take part in a series of simulated driving experiments. Donald Lewis, MD, chief of Eastern Virginia Medical School’s department of pediatrics and co-researcher, said that each teen was asked to get behind the wheel of a simulated vehicle and drive through simulated scenery, in rural, then urban settings, in 10-minute intervals.

In the tests involving distractions that required finger movement, the participants steered the virtual vehicles erratically, weaving in and out of lanes and running over virtual people. Lewis said that the worst driving came when the participants were text messaging, likely due to being required to look down in addition to moving their fingers.

Set a good example for your kids and keep your phone off while driving. No one wants another car accident tragedy caused by texting while driving.
