Ascend Optimal Wellbeing has been designed around two simple facts:1. We all deal with life issues from time to time and being proactive is key to becoming healthier and saving money you can use in the areas you want to.2. Everyone can be healthier – no matter how healthy you are!The programs that we provide are proven to solve things faster, and with less overall impact on your personal cost. Call today to ask how Ascend Optimal Wellbeing might be right for you.

Why Case Management?Behavioral Health can be complicated and confusing. If you are facing a behavioral health illness or problem, having an expert to guide you through it can bring peace of mind, knowing you are getting the right level of care. Your case manager will also help you save money by not putting you in levels of care that you do need to get better. No one needs a medical bankruptcy.

Levels of CareThe various levels of treatment for behavioral health are:

• Inpatient Hospitalization – Highest level of care. Used for stabilization and safety.

• Residential – A longer term stay in a facility that monitors progress for a period of time. Used for chronic problems that are not going to be helped in an inpatient setting.

• Partial Hospitalization – Used to provide structure and more intense help during the day, where you’re at a hospital for 8 or so hours a day and return to your home each day.

• Intensive Outpatient – Usually three times a week for 2-3 hours. Meeting with a therapist and others that are facing similar stresses as you.

• Outpatient therapy – Once a week, once a month, whatever you need to continue getting your life back.
