Certification / Recertification Process

• NO GRACE PERIOD – clients must come prepared with their documents to complete the Certification form at the time of certification or recertification

• The Site Certifier must complete the form, not the client• The Site Certifier must sign and date the form before returning it

to FBR

CSFP Processes

Memorandum of Understanding (MOU) An MOU is an agreement between Food Bank

of the Rockies and our Sub-Agencies who distribute CSFP on our behalf

They are filled out annually We will request the 2014-15 MOU in June 2014

Sign-In Sheets The driver will bring your sign-in sheets with your boxes each

month. The sign-in sheets reflect clients who are current. If a client will expire at the end of the current month, his/her name

will be highlighted and with the word RECERT next to their name. Each client must sign and date the sheet (unless they have a

proxy, which requires written permission), or they have moved out or passed away.

If a client is expired, and we have not received their recertification paperwork, then they will not appear on the sign-in form. They will NOT receive a box until they have recertified.

If a client has not picked up/received benefits within the last two months, they will be terminated from the program - - they will be required to certify again to start receiving benefits.

NO RETROACTIVE BOXES – a missed box, is a missed box. They may pick up for the next month – reflect this on the sign in sheet (picked up for November and December)

Paperwork

As we work with a large volume of CSFP recipients, we need to ensure that paperwork is handled in a timely manner.

New and recertifying applications and sign-in sheets are entered the FRIDAY before each delivery date.

Civil Rights

We serve a diverse clientele and it is imperative that we do not discriminate due to age, race, income levels, etc. We will be setting up a link on our website (www.foodbankrockies.org) which will provide you with a quick training to make sure that we are fairly distributing our CSFP boxes. All sites will need to take this Civil Rights training - we will let you know when the link is complete.

Nutrition Education

FBR is required by the State to complement the CSFP distribution, nutritional education and information must be distributed to all clients picking up CSFP benefits.

Recipes, upcoming health fairs, health screenings, SNAP benefits, and other senior resources are examples.

Flyers and handouts will be provided to you via our driver to distribute to your clients.

If you know of health & wellness resources in your area, please share them with FBR and your clients.

It is our goal to give seniors resources which will benefit their health and well-being.
