REQUEST FOR QUOTATION

The Bureau of Customs (BOC), through its Administration Office (AO), will undertake a Small ValueProcurement for the Renovation of Common Comfort Rooms at OCOM Building, in accordance with theRevised Implementing Rules and Regulations of Republic Act No. 9184. The details of the project are asfollows:

Approved Budget for the Contract Three Hundred and Twenty Thousand Pesos(P 320,000.00) inclusive of tax

QTY. SCOPE OF WORKS1 lot 1. Male and Female Common Comfort Room

1. Replacement of floor and wall tiles. Use 60 x 60 cm floor tiles and30 x 60 cm wall tiles (SOL tiles or approved equivalent).

2. Replacement of ceiling, electrical wirings and lightings.Submit sample of lighting fixture.

3. Replacement of plumbing fixtures and accessories (water closet withsloan valve, lavatory, bidet and faucets). Submit brochures and samplesfor approval. -

4. Replacement of lavatory counter top with granite slab. Submit sample ofgranite.

5. Replacement of vanity mirror.6. Replacement of partition doors and main door. Use modular partition door

and solid wood for main door or approved equivalent7. Rehabilitation of window and relocation of exhaust fan.8. See plans and drawings for specifications.9. Painting works.

Duration: Sixty (60) calendar days upon signing of Purchase Order

Interested suppliers are required to submit their valid and current Mayor's Permit, PHILGEPS RegistrationCertificate and DTI / SEC Registration, Bill of Quantity / Detailed Estimate and duly signed price quotationform (Annex "A'').

Submission of quotation and eligibility documents is on or before July 15, 2015 10:00 a.m. at GeneralServices Division (GSD), Ground Floor, OCOM Building, South Harbor, Gate 3, Port Area, Manila.

Award of contract shall be made to the lowest quotation, which complies with the minimum descriptionas stated above and other terms and conditions stated in the price quotation form.

The BOC reserves the right to accept or reject any or all quotations and to impose additional terms andconditions as it may deem proper.
