Dear Sir

I would like to draw your kind attention that my Basic Salary is over and above the Provident Fund limits as defined in the PF Act. Therefore I desire to contribute to the Provident fund @ 12% on the statutory limit of Rs. 15000/- Per month only. We further request you to contribute the equal amount along with the administrative expenses of Provident Fund.
