Half-lives do not predict the rate of recovery following drug administration. The terminal half-life only sets an upper bound on how much time

• The simulation suggests that the initial boluses of propofol and alfentanil to provide anesthesia for intubation should be given as follows: 0.7 mg/kg of propofol administered 2.3 min before intubation and 30 mg/kg of alfentanil administered 1.4 min before intubation. If the propofol dose sounds too modest (and who wants to see 50% of patients respond to intubation?) the initial propofol dose can be increased to 1 mg/kg given 2.3 min before intubation with a net increase of only 1 min in the time required for recovery.

• The maintenance infusion rates for propofol and alfentanil appear in . After intubation (at time 0) the propofol infusion starts at 180 mg×kg-1×min-1 for 10 min, decreases to 140 mg×kg-1×min-1 from 10—30 min, and then decreases to approximately 100 mg×kg-1×min-1 for the next 9.5 h. The alfentanil infusion is not turned on until 10 min after intubation. The alfentanil infusion rate for the 1st h is approximately 350 ng×kg-1×min-1, is decreased to 275 ng×kg-1×min-1 for the 2nd h, and is then decreased to 240 ng×kg-1×min-1 for the next 8 h.

• Background: When given as an intravenous bolus for induction of anesthesia, propofol can decrease postintubation hypertension but can also create moderate to severe postinduction, preintubation hypotension. The addition of fentanyl usually decreases the postintubation hypertension but can increase the propofol-induced preintubation hypotension. The goal of the study was to determine the relation between propofol and fentanyl doses and the hemodynamic changes postinduction, preintubation and postintubation.
• Methods: Twelve groups of 10 patients, ASA physical status 1 or 2, first received fentanyl 0, 2, or 4 mg×kg-1 and then 5 min later received propofol 2.0, 2.5, 3.0, or 3.5 mg×kg-1 as an intravenous bolus for induction of anesthesia. Arterial blood pressure was continuously monitored. The trachea was intubated 4 min after propofol administration.
• Results: The mean decrease in systolic blood pressure after propofol was 28 mmHg when no fentanyl was given, 53 mmHg after 2 mg×kg-1 of fentanyl (P < 0.05 vs. no fentanyl), and 50 mmHg after 4 mg×kg-1 (P < 0.05 vs. no fentanyl; no statistically significant difference 4 vs. 2 mg×kg-1). There was no statistically significant difference in hemodynamic response to intubation relative to propofol dose. Hemodynamic response to intubation was decreased by the administration of fentanyl; the mean increase of systolic blood pressure after intubation was 65 mmHg from preintubation value without fentanyl, 50 mmHg after 2 mg×kg-1, and 37 mmHg after 4 mg×kg-1 (P < 0.05 for 2 and 4 mg×kg-1 vs. no fentanyl and for 4 vs. 2 mg×kg-1). Hemodynamic changes postintubation were not statistically different with increasing doses of propofol.
• Conclusions: Hemodynamic changes after induction with propofol or propofol/fentanyl, pre- or postintubation, are not modified when the propofol dose is increased from 2 to 3.5 mg×kg-1. Maximal hypotension preintubation occurs with a fentanyl dose of 2 mg×kg-1, whereas the magnitude of postintubation hypertension is significantly decreased with an increase in the fentanyl dose to 4 mg×kg-1.

of Consciousness Anesthesiology87:253-9, 1997Background: This study evaluated the efficacy and safety of remifentanil, a
potent mu agonist opioid with a rapid onset and offset of effect, as a sole induction agent for loss of consciousness (LOC) and compared it with alfentanil.
• Methods: Remifentanil and alfentanil were administered intravenously over 2 min in ascending doses (remifentanil 2, 3, 4, 5, 6, 8, 10, 15, 20 mg/kg; alfentanil 40, 60, 80, 100, 120, 160, 200 mg/kg) to unpremedicated healthy patients. Patients were observed for rigidity and LOC for 30 s after the end of infusion. If patients had not lost consciousness, 2 mg×kg-1×min-1 thiopental was administered until LOC was achieved. Arterial blood samples, obtained at specified time intervals, were analyzed for remifentanil and alfentanil whole-blood concentration. Blood pressure and heart rate were also recorded at preset time intervals.
•
• Results: Neither drug could reliably produce LOC. With both drugs, there was a dose-dependent decrease in thiopental requirements and a dose-dependent increase in the incidence and severity of rigidity (P < 0.05). The median effective dose (ED50) for LOC with remifentanil was 12 mg/kg, and for alfentanil it was 176 mg/kg. The median effective concentration (EC50; whole-blood concentration) of remifentanil was 53.8 ng/ml and for alfentanil it was 1,012 ng/ml. Minimal hemodynamic changes were observed after either drug was given.
• Conclusions: Remifentanil is 15 times more potent than alfentanil, based on the ED50 to achieve loss of response to a verbal command and 20 times more potent than alfentanil based on the EC50. Neither opioid is suitable as a sole induction agent.

associated with intense haemodynamic variations, the supplementary administration of a potent m-receptor agonist is recommended. This study compares the efficacy of remifentanil in comparison with alfentanil to attenuate the haemodynamic response during laryngeal interventions.
• Materials and Methods: With approval of the institutional Ethics Committee, 120 adult patients under-going elective laryngeal microsurgery under TIVA were investigated. The patients were randomly allocated to receive either alfentanil (group A; n = 60) or remifentanil (group R; n = 60). Dosage of the opioids was adapted under consideration of the individual intraoperative needs. Propofol infusion rate was increased or decreased by 30 mg/kg/min if heart rate (HR) or mean arterial pressure (MAP) deviated ³25% from preinduction base-line values. HR and MAP as well as the required propofol dosage were recorded at 5 standardised intra-operative time periods (TI–T5). Values from both groups were compared with the non parametric Mann-Whitney U-test (p < 0.05).
• Results: Mean remifentanil requirement was 5.35 ± 1.90 mg/kg/min and mean alfentanil requirement 22.61 ± 9.01 mg/kg/min. While the induction dose of propofol was similar in both groups (group R: 1.80 ± 0.24 mg/kg vs. group A: 1.76 ± 0.20 mg/kg), for maintenance a significantly lower propofol infusion rate was necessary in group R (group R: 5.81 ± 1.67 mg/kg/min vs. group A: 7.45±1.70 mg/kg/min; p < 0.05). In group A significantly elevated HR values were recorded at T1–T3, and also MAP was higher throughout the complete intervention.
• Conclusions: Remifentanil allowed lower propofol infusion rates and simultaneously offered a better control of haemodynamic variables during laryngeal micro-surgery.

• We have studied the pharmacokinetics and pharmacodynamics of alfentanil, fentanyl and sufentanil together with propofol in patients undergoing coronary artery bypass graft surgery (CABG). Sixty patients (age 40–73 yr, 56 male) were assigned randomly to receive alfentanil, fentanyl or sufentanil and propofol. Plasma concentrations of these drugs and times for the plasma concentration to decrease by 50% (t50) and 80% (t80) after cessation of the infusion were determined. Times were recorded to awakening and tracheal extubation. Total dose and plasma concentrations of propofol were similar in all groups. Mean total doses of alfentanil, fentanyl and sufentanil were 443, 45 and 4.4 mg kg-1, respectively. Time to awakening did not differ significantly. In patients receiving fentanyl, the trachea was extubated on average 2 h later than in those receiving sufentanil and 3 h later than in those receiving alfentanil (P<0.05). The t80 of fentanyl was longer (P<0.05) than that of alfentanil or sufentanil, and there was a linear correlation between the t80 of the opioid and the time to tracheal extubation (r=0.51; P<0.01). However, the t50 values for these opioids were similar and did not correlate with recovery time. In conclusion, patients undergoing CABG and who were anaesthetized with fentanyl and propofol needed mechanical ventilatory support for a significantly longer time than those receiving alfentanil or sufentanil and propofol. On the basis of the interindividual variation observed, the time to tracheal extubation was most predictable in patients receiving alfentanil and most variable in patients receiving fentanyl, a finding which may be important if the patients are transferred to a step-down unit on the evening of the operation.
