Before a person with a disability is allocated funds (I.S.P Individual Support Package) a “planner” is

allocated to work with the person or family

The Planning should be guided by the person with a disability, taking into account the person’s

aspirations, needs, wants (P.C.P Person Centred Plan)

Independent of services; minimises conflict of interest

Not focused on locking people into disability or segregated services

Has a focus on community involvement / citizenship

Includes persons wants and strengths not just needs and deficits

Looks beyond what is available at present

Seeks to utilize “natural supports” i.e. join a choir rather than purchase support to attend singing lessons

Person Centred Planning (PCP) concept started in the late 80’s

Aim is to empower people who have traditionally been disempowered by ‘specialist’ or segregated services.

For people with disabilities who require support – without planning / expectations, nothing happens

Some of us may be involved in teaching people skills but how as life changed for some of the people you know who attend

services?

Circles of Support

MappingThe maps process allows people to express their hopes and

fears. An action plan is developed to work towards the dream and away from the nightmare.

Essential Lifestyle Planning (ELP)ELP is detailed planning which focuses on the individuals life

now and how that can be improved. ELP specifies the way the support is to be provided on a day to day basis.

All people have aspirations regardless of disability. Important to have expectations for all people

No such thing as ‘too hard’ or ‘to disabled’ to support choice

Money is not the only option. We need to seek ‘natural supports’ if planning is to be sustainable

We need to consider what most people do at X age and consider the same things for people with

disabilities

The service system may not provide the best options or opportunities for people to fulfill their

aspirations.

Life is a journey, all changes have ups and downs, so things may not be perfect the first time.

Planning is an ongoing process
