assertive management to protect him or her from possible injury.Voice control (speaking calmly but firmly) usually takes care of it.Mild sedation, such as nitrous oxide/oxygen or a sedative, may alsobenefit an anxious child. If a child is especially fearful, other seda-tion techniques or general anesthesia may be recommended.

Choosing a healthy, safe place for your child to thrivewhile you’re at work is a daunting decision. Daycare hor-ror stories making national news may suddenly keep youawake at night. But take a deep breath. Safe, healthyoptions for childcare are out there. It’s important to gatherinformation, do the legwork, and ultimately trust your gut.

Get the Facts: Narrow the PossibilitiesThere are regulated daycare centers, regulated home

daycares, unregulated home daycares, and preschools.Begin your search early. Ann Douglas, author of ChoosingChildcare for Dummies, suggests embarking upon yourquest between eight weeks and a year before your back-to-work date.Contact your local child care referral agency by check-

ing Child Care Aware, a national consumer educationparent hotline (1-800-424-2246) and web delivery system(www.childcareaware.org). The National Child CareInformation Center (1-800-616-2242 or www.nccic.org) isalso a good start.Ask the referral agency about licensing requirements in

your area and how to collect information about com-plaints or licensing violations. Ask about financial assistanceprograms for which you may qualify.

Home DayCare or Center?Home daycare may be the best fit for your child’s “home

away from home.” These providers tend to have more flexi-ble hours (especially helpful if you work an atypical workschedule). They may have less rigid schedules and policieswhich set you more at ease. If it is important to you thatyour caregiver be open to making lots of individual accom-modations (irregular naptimes, for example), a homeprovider might suit you and your child best. Some homeproviders are regulated, but many are not so it’s a personalchoice dependent on whether you’re comfortable.Licensed daycare centers offer many advantages. Centers

may hire certified early childhood educators with training inchild development and have more clearly outlined policiesso everyone knows what to expect. Also, having more thanone caregiver on the premises provides peace of mind

(especially if you’re concerned about TV watching or unfa-miliar adults in a home setting). Centers offer reliability soyou won’t have to scramble for care if one caregiver is ill oron vacation. The spaces in a center are typically kid friendlyand designed with safety in mind. Because of governmentregulation, child to caregiver ratios, safety requirements, andcriminal record checks are standard.

Do the Homework: Visit and Assess QualityThe American Academy of Pediatrics (AAP) recommends

you consider the following 13 guidelines as you evaluatethe quality of a provider:

• Supervision. Are children supervised at all times, evenwhen they are sleeping?Is discipline positive, clear, consistent, and fair?• Handwashing/Diapering. Hands should be scrubbed

with soap and water for at least 10 seconds and then rinsedand dried. The faucet should be turned off with a papertowel.

sider how the provider’s philosophy of childrearing, discipline, education, and nurturingmeshes with your own. As Ann Douglas says inher book, “You can’t count on anyone else toguarantee your child’s health, safety, and well-being in a particular child care setting. Who can forget their child’s first word? It was likelyyour name, but maybe it was “bawww” (ball) whichlit up your faces with a smile. It feels natural to cele-brate, praise, and applaud our toddlers as theyacquire new language skills daily. In turn, such enthu-siasm reinforces a child to speak additional words. However, University of Iowa psychology professor

Bob McMurray, Ph.D. says parents stop emphasizinglanguage as their kids leave toddlerhood. Since a pre-school child’s vocabulary is a critical predictor ofschool preparedness and reading comprehension, it isimportant parents do what they can to boost it. McMurray’s research indicates a vocabulary explo-

sion (or “word spurt”) is dependent upon a childlearning a mix of words, both easy and not easy andall at once. Interestingly, vocabulary explosionsspecifically require “more difficult words than easywords.” The professor and other language profession-als make the following suggestions to boost yourchild’s verbal skills.

Mealtime Magic (Hold the Thesaurus)A child’s vocabulary can be enhanced by our talk

at the dinner table. Use moments at mealtimes tointroduce new words, especially challenging onessince you’ll have their attention in a pleasant setting.What to discuss? In a mealtime study, Dr. Diane Beals

and her colleagues of the University of Tulsa discov-ered that 3- and 4-year-olds who were exposed touncommon words such as “boxer,” “wriggling,” or“tackle,” scored higher on later standardized tests atage 5. Beals says forget about serving up a thesaurusat the table and instead discuss “your day or some-thing cool you saw at the store.” Bringing in newwords helps them form connections between wordsand real-life events.

Lovely Language from Lit Reading your child a story creates magic for both

of you and is beneficial to their growing vocabularywhen you ask lots of questions during the story,checking for understanding. If your preschooler doesnot recognize a word when you quiz her, ask her tostudy the illustration for clues. Classics like Where

the Wild Things Are contain new words yourchild may not otherwise hear and accommo-date such as “rumpus” and “gnashed.”Throughout the book, continue to interact withyour child, asking open-ended questions suchas “What do you suppose will happen next?”and “Why do you suppose Max felt so angry?”Such interactions involving novel vocabularywill improve the quality of their languageskills, setting them up for increased success inschool.

Guessing for SuccessConsider a little restraint to challenge your

child. McMurray suggests instead of automati-cally doling out definitions for your preschool-er when they stumped on a word’s meaning,you should give clues and allow her to figure itout on her own. If, for example, the word inquestion is “equestrian,” give her hints such assaddle, mane or stable. It can also be helpful to“tell them what it isn’t” So you could say “notcows, but…”

Stage Your Own ‘Show & Tell’ Who says “show and tell” is just for school?

You can easily use this format to describe anddiscuss an object to expand their vocabularyoutside of school. You might demonstrate thehand chopper you use to dice vegetables,explaining how the appliance functions andsaves you time. You could show your child thechecks in your checkbook, discussing how thesmall sheets of paper may be used for pay-ments. The important thing is to have yourchild’s attention and provide something totouch and to see to anchor vocabulary withintheir memories.

Make Believe & MimeEngaging your child in pretend play will

introduce her to a wider variety of vocabularywords. If you are playing restaurant, for exam-ple, there are all sorts of unfamiliar wordswhich can be integrated into the session –

A mother’s nurture may pro-vide powerful protectionagainst risks that her babyfaces in the womb, accordingto an article recently pub-lished online in the journalBiological Psychiatry. Theresearch shows that fetuses exposed tohigh levels of stress hormone – shown to be a precursorof babies’ poor cognitive development – can escape thisfate if their mothers provide them sensitive care duringinfancy and toddlerhood.The new study represents the first direct human evi-

dence that fetuses exposed to elevated levels of thestress hormone cortisol may have trouble paying atten-tion or solving problems later on. But what may bemore intriguing is the study’s second finding: That thisnegative link disappears almost entirely if the motherforges a secure connection with her baby.“Our results shape the argument that fetal exposure to

cortisol – which may in part be controlled by the moth-er’s stress level – and early caregiving experience com-bine to influence a child’s neurodevelopment,” saysstudy author Thomas O’Connor, Ph.D., professor ofPsychiatry and Psychology at the University ofRochester Medical Center, and director of the WynneCenter for Family Research.For the study, researchers recruited 125 women at an

amniocentesis clinic in an urbanmaternity hospital, taking a sampleof their amniotic fluid so that stresshormones in it could be measured.The mothers were at 17 weeks ges-tation on average; only motherswith normal, healthy pregnanciesand subsequent deliveries were fol-lowed.When their children reached 17

months of age, researchers adminis-tered a Bayley infant developmentalscale test, which relies on puzzles,pretend play, and baby “memory”challenges to gauge youngsters’ cog-nitive development. They alsoobserved the baby and mother usingthe Ainsworth “Strange Situation” test,which judges childrearing quality, cate-

gorizing mom-baby pairs as either showing secure orinsecure attachment to each other.With cortisol levels, relationship quality results, and

cognition scores in hand, researchers analyzed how thefirst two measures might influence the third. Indeed, forchildren showing “insecure attachment” to their moth-ers, a high prenatal cortisol level was linked with short-er attention spans and weaker language and problem-solving skills. But interestingly, for kids who enjoyedsecure relationships with their moms, any negative linkbetween high prenatal cortisol exposure and kids’ cog-nitive development was eliminated.“This is such refreshing news for mothers,” O’Connor

says. “Pregnancy is an emotional experience for manywomen, and there is already so much for mothers to becareful of and concerned about. It’s a relief to learn that,by being good parents, they might ‘buffer’ their babiesagainst potential setbacks.”

They’re plenty confusing to parents – these myste-rious pains that plague kids’ legs at night and reli-ably vanish by morning. Medical tests show thatthere’s nothing wrong, and your child’s doctor hasno tips on preventing or treating the pain.“Growing pains” are a common childhood ail-

ment. And yet, they’re not even caused by a child’sgrowth.“It is true that muscles, tendons, ligaments and

bones grow continuously, but the growing processalone seldom causes pain,” explains Francis Y. Lee,M.D., an orthopedic surgeon at Children’s Hospital ofNew York and Columbia University. In fact, growing pains are most common from ages 3

to 12, when kids aren’t having rapid growth spurts. Sowhy call them “growing pains”? Because all childreneventually outgrow them, says William Oppenheim,M.D., a pediatric orthopedic surgeon with the LuskinChildren’s Clinic of Los Angeles Orthopaedic Hospitaland the UCLA Department of Orthopedics.

What They AreGrowing pains are the third most common type of

pain in children ages 3-12 (headache and stomachacherank first and second), impacting between 15 percentand 30 percent of this age group.

The pains tend to occur:• at night,• for a few nights at a time, every one to three months,

• in both legs, and• in the thighs, calves and behind the knee.

In the morning, kids who had been suffering fromgrowing pains get around just fine; you’d never knowthey had a problem. “During the day they’re totallynormal,” says Meghan Imrie, M.D., associate clinicalprofessor of orthopedic surgery at Stanford University.Any lab tests that your pediatrician performs will alsocome back normal if growing pains are the problem.

What They Aren’tThe most important thing to know about growing

pains is that this is a “diagnosis of exclusion,” Imriesays. That means growing pains are diagnosed afterdoctors rule out everything else.

Here are some symptoms that do not indicate grow-ing pains and should be reported to your child’s doctor:

• pain that is focused in one spot or one leg only(localized);

• pain focused in the joints, making a child feel

“stiff” in the mornings;• pain that’s persistent, rather than coming

and going in spates;• pain accompanied by fever, swelling or ten-

derness;• pain with injury or bruising;• pain that causes a child to limp during the

day, or curtail his physical activities.

Similar to growing pains is a condition calledtransient synovitis. Its cause is also unknown, butthis condition brings day and nighttime joint painin the knee or hip. Kids with transient synovitisdon’t have fever or other signs of infection, but thepain may become so severe they walk with a limp.The condition usually clears after three or fourdays, with no long-term effects. Painful joints with swelling and fever, on the

other hand, are a big red flag, and you shouldhave your child seen by a doctor immediately. Thiscan be a sign of joint infection, which Oppenheimwarns can cause permanent damage within a dayor two. A hopeful sign, believe it or not, is if your

child’s pain stays the same. “More worrisomethings like a tumor or arthritis become worse overtime,” says Imrie. Those are two of the seriousconditions connected with nighttime leg pain inkids. Others include leukemia (cancer of the whiteblood cells) and lymphoma (cancer of the lym-phatic system).Less-serious causes of leg pain include injury or

overuse of muscles (strains), ligaments (sprains),bones (stress fracture) and joints (cartilage bruise).Long-distance runners can develop pain in thethigh, which may indicate stress fracture.Basketball players can have pain over the front ofthe knee, a condition called Osgood-Schlatter’sdisease (apophysitis). But these things usually getbetter when physical activity is reduced, explainsLee.

CausesNo one knows what causes growing pains, “and

that makes it a bit of a problem,” saysOppenheim. Especially frustrating since the condi-tion was first described in 1823! He points to aJournal of Foot and Ankle Research article from2008 that shows that research on the topic hasbeen plentiful, but “lacking in scientific rigor.”Still, there have been plenty of theories about thecause of growing pains:

• Muscle fatigue – This theory hasn’t beentested, but parents of kids with growing painsoften report that they’re linked to increased physi-cal activity.

• Bone fatigue – Studies have shown that thebones of kids with growing pains are less dense(and so weaker) than those of other children, lead-ing some to suspect the pain comes from bonefatigue.

• Genetics – Experts suspect that low painthresholds run in families, and some studies showthat kids with growing pains have lower painthresholds than their peers without.

• Joint hypermobility – Another untested theo-ry is that children with growing pains are “doublejointed,” a trait also common to children withpainful conditions such as fibromyalgia.Oppenheim’s personal theory is that because

children in this age range are so physically active,they make the periosteum (a membrane containingblood vessels and nerves that covers the bones)sore. It’s much thicker in kids than in adults, andcould be less flexible. But Oppenheim says hedoesn’t have research to back this up. “Nothinghas really been pinned down,” he says.

What You Can DoThere aren’t any proven treatments for growing

pains, in part because it’s difficult to provewhether a given treatment worked or whether thechild just outgrew the condition. Still, some thingsdo seem to help:

• Reassurance,• Heat and massage, if that feels good to the

child, and• Over-the-counter pain medications such as

Tylenol and Advil.

“It’s basically just rubbing where it hurts, kissingthe boo-boo,” says Imrie.Oppenheim puts it another way. “You can be

sympathetic and carry them through it,” he says.“And you can realize it’s not the end of the world,and they will outgrow it.”

Emergencies can happen anywhere, atany time, and often without warning.Therefore, it is critical to discuss – andpractice – emergency and disaster prepared-ness plans, such as a home fire escaperoute and severe weather safety plan.A national survey released by

Underwriters Laboratories, an independentproduct safety organization, reveals thatchildren’s initial reactions might actuallyput them in danger during an emergency.While more than 90 percent of children saythey would know exactly what to do ifthere was an emergency like a fire, only 47percent chose the safest option: get out ofthe building immediately.UL urges families to consider the follow-

ing safety tips in preparing for the unex-pected:Children should know their full name,

parents’ full names, address (including cityand state), home phone number (includingarea code) and parents’ work phone num-bers or cell phones before leaving thehome.Designate an out-of-town relative or

friend to be your family’s emergency con-tact and keep their information with you atall times.Prepare an emergency kit, including: five

days worth of non-perishable food and water, a canopener, flashlight, portable emergency radio (hand-crank, solar-powered or battery-operated), batteries,any prescription medication needed by family mem-bers, a first-aid kit, list of phone numbers for relatives,neighbors and utility companies, and pictures anddescriptions of your family. We’re so accustomed to hearing about girls lag-ging behind boys in math. Now comes the news thatboys’ poor reading skills have led to a much biggergender gap in our schools.A new study from the public education advocate

Center on Education Policy (CEP) finds that boys aresignificantly behind girls in reading at the elemen-tary, middle and high school levels. Researcherslooked at scores from standardized tests in all 50states between 2002 and 2008, and found that boysare significantly behind girls in reading. In somestates, the percentage of boys scoring “proficient” inreading is more than 10 percentage points below thatof girls.For their part, girls appear to be doing much better

in math. The study found that at the proficient level,the number of states where girls outperform boys isabout equal to the number of states where boys out-perform girls. At the “advanced” score level, boys inthe fourth grade still outperform girls in most states.In a statement released with the report, CEP

President Jack Jennings urged “much greater atten-tion” to boys’ reading skills beginning in the earlygrades.Overall, many states have narrowed the gap

between male and female students in both math andreading, the CEP study found. But while gaps in ele-mentary school reading have narrowed in 24 states,they’ve widened in 14. And while boys and girlshave improved on fourth-grade reading exams since2002, more girls than boys reached all three achieve-ment levels – basic, proficient, and advanced – in2008.
