QHow does a pediatric dentist help with dental anxiety?

AAt Alamo Heights Pediatric Dentistry our office is set up to

create a place that your child will love to visit. From our 250 gal-lon salt water fish aquarium where your children can meet thereal live Nemo and all of his friends to our unique coral reef playcave. Pediatric dentists have special training in helping anxiouschildren feel secure during dental treatment. And staff membersare people who like children and have chosen to work with them. This combination ensures that most children are calm, comfort-able and confident in a pediatric dental office.

QHow will a pediatric dentist help my child feel comfortable?

APediatric dentists are trained in many methods to help chil-

dren feel comfortable with dental treatment. For example, inthe “Tell-Show-Do” technique, a pediatric dentist might name adental instrument, demonstrate the instrument by using it tocount your child’s fingers, and then apply the instrument intreatment.

There are many other techniques applied by pediatric dentists tocomfort a patient. They include modeling, coaching, distractionand parent participation. But by far the most preferred tech-nique is praise. Every child does something right during a den-tal visit, and pediatric dentists let children know that.

QWhat if a child misbehaves during treatment?

AOccasionally a child’s behavior during treatment requires

assertive management to protect him or her from possible injury.Voice control (speaking calmly but firmly) usually takes care of it.Mild sedation, such as nitrous oxide/oxygen or a sedative, may alsobenefit an anxious child. If a child is especially fearful, other seda-tion techniques or general anesthesia may be recommended.

When deciding on child care fortheir infants, many parents strugglewith questions about quality.Childcare research has shown thatthe setting (in-home, family or cen-ter) matters less than what happenswithin that setting. Babies need to beable to form a trusting relationshipwith providers, which allows for anemotional bond to form.

Low provider-to-child ratios arekey. Equally important are questionson what staff salaries and benefits arelike. Low pay is responsible for ram-pant turnover within the childcareindustry, thus making it harder foryour baby to build a comfortable bond withhis or her caregiver(s).

Here are some tips to keep in mindwhen investigating childcare options foryour baby:• Visit the daycare site before enrolling

your child.Visit at times when you canobserve the interactions of children witheach other and with adults.• Spend some time observing the infant

room.Visit several times to judge how care-givers respond to tears or times of stress.• See if infants are allowed to have their

naps when it feels right for them. Unlikeolder children, babies have a harder timebeing treated as part of a group and need tofollow their own inner schedules.• Make sure providers are interacting

with babies. According to a study by the

National Institute of Child Health andHuman Development, “the amount of lan-guage that is directed at the child in childcare is an important component of qualityprovider-child interaction.”• Ask how care providers handle babies’

special needs, such as food allergies orattachment to a pacifier.• Check the qualifications and creden-

tials of the caregivers. Degrees in earlychildhood education and accreditation fromthe National Association for Family ChildCare and the National Association for theEducation of Young Children provide goodstarting points.• Get names of parents whose children

have attended or are attending and callthem to inquire about the childcare centeror providers.

Choosing Child Care,Early Learning Programsfor Toddlers andPreschoolers

Early spring is the time many families begintheir search for an early learning childcare pro-gram for fall placement. It also is a time whenmany early childhood education programs areholding open houses and welcoming prospectiveparents who may have a lot of questions aboutchoosing the best program for their child, according to the National Association for theEducation of Young Children (NAEYC).

Consider the following checklist of tips fromthe NAEYC, the American Academy of Pediatricsand Child Care Aware when visiting programs forthe first time:1. Are there opportunities to visit the center?

Can you visit and stay awhile to get a good senseof the environment? If your child is enrolled inthe program, can you visit the center anytime itis open? Can you see all the areas that your childwill use? Are visitors screened or is their identifi-cation checked, so that only approved adults canvisit the center and pick up children?

2. What is teacher to student ratio? Ask aboutand observe group sizes and the number ofteaching staff in each classroom. NAEYC recom-mends no more than 12 toddlers with at leasttwo teaching staff. For preschoolers, group sizeshould not exceed 20 children with at least twoteaching staff always present.

3. Is play, including dramatic play and blocksand active play outdoors, integrated into class-room topics of study? Play not only supports chil-dren’s intellectual development; it is also veryimportant for the physical development of chil-dren.

4. Does the programs promote the health andnutrition of children?

5. Are the children in the program engagedwith one another and their teachers? Listen andwatch for a happy buzz of activity – neither tooquiet nor too loud.

6. Do teachers use positive speech? Are theyloving and responsive to the children?

7. Is the program able to adapt to the needs ofeach individual child? A good program should be

BABY SAFETY

New parents are obsessed with keeping their babies safe. Many will tell you thatthey check on their sleeping infants repeatedly just to be sure the babies are stillbreathing. The fact that, each year, 2,500 infants in the United States are victims of sudden

infant death syndrome (SIDS) is a big reason why. SIDS, the sudden, unexplainabledeath of a baby under 1 year old, is the leading cause of death for infants from 1month to 1 year of age. Typically, parents will check on a sleeping infant and findher dead – a tragedy that haunts them for the rest of their lives, according to theAmerican Sudden Infant Death Syndrome Institute. And yet, despite 16 years of public health messages urging parents to follow a set

of guidelines proven to help prevent SIDS, a recent study published in the journalPediatrics found that 26 percent of mothers of 3-month-old babies don’t adhere torecommendations to:• always put an infant to sleep on his back,• keep the crib free of soft objects like stuffed toys, pillows and quilts, and• not have the baby sleep in the same bed with his parents. These guidelines from the American Academy of Pediatrics (AAP) are based on

the idea that infants may be suffocating after turning their faces into soft bedding ornearby soft objects. And they’ve been shown to work. From 1992 to 2003, the rateof SIDS deaths actually dropped by more than half (from 1.2 deaths per 1,000 livebirths to 0.53 deaths per 1,000). The decline was attributed to health recommenda-tions, which began in 1992, to put babies to sleep on their backs instead of theirstomachs.

New SIDS Prevention TipsNow, new research points to the use of fans and the training of childcare

providers in SIDS prevention as other effective ways to reduce the incidence ofSIDS.A recent study by Kaiser Permanente’s research division found that using a fan in

a baby’s sleeping room cut an infant’s risk of SIDS by 72 percent, compared to

by DEIRDRE WILSON Keeping Your BabySafe from SIDS

rooms with no fan. Kaiser Permanenteresearchers also found that infants using a pacifi-er while sleeping were at reduced SIDS risk, per-haps because the pacifier prevented an infantfrom pressing completely up against soft beddingor a mattress.Meantime, the AAP is reporting that training

childcare providers in SIDS prevention also helpsreduce risk. An estimated 20 percent of SIDSdeaths occur while a baby is in the care of some-one other than the parent, the AAP reports. In a recent study, 1,212 providers in 264 child-

care programs received AAP-approved SIDS pre-vention training that emphasized placing infantsto sleep on their backs. Following the training, the providers reported an increased understand-ing (from 59.7 percent to 87.8 percent) of theneed to do this. And the percentage of providerswho actually did place infants on their backsincreased from 51 percent to 62.1 percent. The study underscores the need for continued

education of parents, expanded training of par-ents and caregivers and even statewide mandatesregarding SIDS prevention in childcare settings, the AAP reports. The American SIDS Institute recommends the

following additional tips to help reduce SIDS risk:• Keep the baby’s crib in the parents’ room

until the infant is at least 6 months old (studiesshow that infants are safest when their beds areclose to their mothers).• Have your sleeping baby dressed in justenough clothes to keep her warm, without hav-ing to use a blanket. And keep the room temper-ature at a comfortable setting. Overheating mayincrease the risk of SIDS.• Don’t expose your infant to tobacco smoke.

The greater the exposure, studies have found, thegreater the risk of SIDS.• Breastfeed your baby, if possible, since stud-

ies have shown that breastfed babies have alower incidence of SIDS than formula-fed babies.• Don’t expose your baby to people with res-

piratory infections; SIDS has often been associat-ed with minor colds or gastrointestinal infections.• Talk to your pediatrician immediately if your

baby experiences periods of not breathing, going limp or turning blue, or if your baby stopsbreathing or gags excessively after spitting up.

of guidelines proven to help prevent SIDS, a recent study published in the journalPediatrics found that 26 percent of mothers of 3-month-old babies don’t adhere torecommendations to:• always put an infant to sleep on his back,• keep the crib free of soft objects like stuffed toys, pillows and quilts, and• not have the baby sleep in the same bed with his parents.

Keeping Your BabySafe from SIDS

rooms with no fan.

The American SIDS Institute recommends the

following additional tips to help reduce SIDS risk:• Keep the baby’s crib in the parents’ room

until the infant is at least 6 months old (studiesshow that infants are safest when their beds areclose to their mothers).• Have your sleeping baby dressed in just

enough clothes to keep her warm, without hav-ing to use a blanket.

Children play because it’s fun. When a child presses a button and a toy respondswith lights and sounds, this cause and effect rewards the child for what she can do onher own. Success with toys drives engagement, generates self-confidence, encouragesrepetition and earns accolades from peers and parents. Most importantly, it’s fun.Kids with disabilities may learn at a slower pace or with special assistance, but they

have the same need to discover that success is fun. Success drives their eagerness to tryharder, do more and develop new skills. All of the toys that won National Parenting

Publications Awards (NAPPA) in 2008 introduce chil-dren to the delightful experience of success. For begin-ning learners, the Smart Builder’s Piano takes the age-old building blocks concept to a new level. Essentiallyan electronic keyboard, children create new sounds byadding blocks. The more they build, the more the piano’s

sounds reflect their choices. As children finger the keysand grasp the blocks, they are working on fine motor

skills and eye-hand coordination, in addition to expressing their musical cre-ativity.

For little thinkers, the Double Shutter, Shut the Super Box! is ano-fail way to learn numbers and develop strategic thinking.Children roll dice and easily choose which corresponding easy-to-manipulate levers to flip. The more they play, the more funthey have as they consider which lever options will help them

win. On a much grander scale, the Uberstix Scavenger Series introduces children to

recycling and to being innovative in the way that they see the world around them.Success is defined both by achieving the toy’s building goal and by learning the valueof recycling. By repurposing paper clips, straws, plastic bags, etc., from around thehouse, children learn to build with uncon-ventional materials, think outside the boxand hone their problem-solving skills. For more details of the 2008 NAPPA-win-

ning children’s toys, first described in SanAntonio’s November and December issues, head online to www.nappa.parenthood.com.

Modern DadsMore dads are going along on OB-GYN

appointments, attending ultrasounds and singingto the baby in-utero, says Greg Bishop, founderof the nationally known mentoring and orienta-tion program, Boot Camp for New Dads. “Youhave a lot more men becoming a lot moreinvolved a lot earlier,” Bishop says. And after abit of time off for the birth and more bonding, many men don’t feel right leaving that new babyall day.

“By the time they go back to work, they knowwhat they’re missing,” he says.

Unfortunately, while dads want to be asinvolved as possible, society hasn’t quite caughtup yet. Dads are still supposed to be the primaryproviders, and for the most part, they still fulfillthat role. “For all the hoopla given to stay-at-home dads,” Bishop says, “the fact is that we’restill a fairly traditional society.”

An American Time Use Survey released in2008 by the Labor Department backs this asser-tion. It found that on an average weekday inhouseholds with kids under age 6, mothers spentmore than four times as much time caring forthose children as did dads. On the weekend, dads did a bit better, but women still put in twiceas much time.

Bishop, whose organization provides educa-tion and support to fathers-to-be at more than260 sites across the United States and interna-tionally, says moms are the biggest factor in howinvolved the father of a child will be and howearly he’ll get involved. Bishop advises womento encourage their partners to jump in, and thento step back and give men space to care for thebaby on their own. His book, Crash Course forNew Dads (Dads Adventure Inc., 2008), offerseven more tips.

Working It OutStill, with long work days and ever-longer

commutes, where’s a dad to find the time?Bishop recalls that during his first child’s first 6months of life, his commute meant he didn’tmake it home on weeknights until after the babywas asleep.

One way for dads to carve out some time is totry for a more flexible work schedule by working

Working dads – especially newdads – have more to come home tothan ever before. But the economicdownturn means they might also feelmore pressure to stay at work, ifthey’re lucky enough to have work.As of early February, the federalLabor Department reported that 3.6million jobs had been lost sinceDecember 2007. Half of those losses

came during November andDecember of 2008 and January of2009; experts and regular folks alikeare still feeling for the bottom of thelong, steep slide.

This means that people with jobsare doing what it takes to keep them.So how can men balance their jobsas “providers” with their new roles as“hands-on dads”?

WORKING DADS

at home part of the time, even just one day aweek, and fitting in some baby time betweenemails and other tasks. Dads could also try tonegotiate a later start time at their companies.Babies tend to be early risers, so taking themorning shift with the baby could give mom anextra half-hour of sleep and dad an extra 30minutes of one-on-one time. Dads could alsoget to the office extra early and then make ithome in time for some bonding in the evening.

Not all bosses, however, will be baby-flexible.“If your employer doesn’t have any kids, findanother employer,” jokes Bishop. But he pointsout that it’s unreasonable to expect an employerto cut a man some slack just because he haskids. New dads need to present their plans tobosses as win-win situations, making the casefor how flexibility can make them more efficientand productive.

Balancing the BudgetBishop understands how tough it can be to

manage a job and family, especially in today’sfinancial world. “It’s mind boggling the respon-sibilities these young men today have as theybecome fathers,” he says.

The pressures can really mount once you addall the new-baby spending into the mix.Diapers, childcare and baby gear can get pricey. And it all comes at a time when mom – oftenone of the family wage earners – will be awayfrom work, at least temporarily. Among compa-nies who offer paid maternity leave, some don’tpay a full salary during the leave.

To help keep things from getting out of hand,Bishop suggests putting together a budget anddiscussing your financial and work situationstogether. Try to approach this realistically, sothat you can avoid spending more than you’rebringing in.

And remember – new fathers trying to holddown jobs and still make time for a baby areblazing a fairly new trail. Both parents need torealize that no one gets it right all the time. “Wetend to forget,” says Bishop, “that it was just ageneration or so ago that [dads] weren’t sup-posed to do these things.”

Christina Elston is a senior editor withDominion Parenting Media.

Six Tips for DadsGreg Bishop offers these suggestions to help dads stay con-

nected to their babies:1. Take a photo. Keep your baby’s picture at the office

where you can see it often.2. Keep in touch. Call or text home at lunchtime for a baby

update. Most camera phones will even let mom email a photoon the spot to keep you in touch. 3. Change diapers! “Changing diapers is just part of the ter-

ritory,” Bishop says. “When you go trout fishing, you clean thetrout.” 4. Go solo. Be prepared for alone time with the baby, which

Bishop says is essential – especially for dads with little time tospare.5. Be creative. Find your own way of doing things. Don’t

worry too much about something going wrong while mom isaway, and realize that her way isn’t necessarily the only way. 6. Play. Even when mom is around, make time to play with

your baby every day. This has benefits for both baby and mom. – Christina Elston

Is your family ready for a crisis? What do your kids needto know and to whom should they turn in an emergencysituation when they can’t reach you? Whether it’s a naturalor man-made disaster, Jeffrey Upperman, M.D., director ofthe Disaster Preparedness Project at Children’s Hospital LosAngeles, is an expert with plenty of experience. He offersthese four readiness tips for all of us:

1. Create Crisis Contacts. Contacts too close to homemight be caught up in the disaster themselves, so havesomeone out of town in mind. Have your child chat on thephone with these folks monthly, so that they’re comfortablecalling them if they cannot reach you in an emergency.

Meet your neighbors and get to know other parents at yourchild’s school. Enlist them as alternative contacts for yourfamily. “Those people are going to be your local contacts,”explains Upperman. “If there’s an issue, it’s nice to havetwo or three numbers that you could call.”

Jot all these contacts down on a “business card” for yourchild to carry in a wallet or backpack.

2. Get Set at School. Talk to other parents and your kids’teachers, and arrange for each of your children to have a“disaster buddy” whom they should stay close to in anemergency. Create a list of “helpers” – teachers, doctors, nurses, firefighters and police – to alert your children aboutwhom they can approach and count on in an emergency. For young children, include photos or drawings of thesekinds of “helpers.”

3. Have Supplies Ready at Home. Review your disastersupply checklist – the items you need on hand in emer-gency situations that could last several days. Don’t have alist? Head online to the American Red Cross Web site,www.redcross.org, or the Department of HomelandSecurity Web site, www.ready.gov.

4. Role Play and Review. Act out what might happen if adisaster occurred while your child was at school or athome. Review your plan as a family every year, updatingcontact information, re-stocking supplies and makingchanges as needed.
