Sliding Fee Program• A sliding fee program for income-eligible patients ensures that no one who needs care is turned away because of a lack of insurance or other financial obstacles• In 2008, 93% of PCHS uninsured patients participated in the sliding fee program• That represents nearly 25% of our total patient population

Sliding Fee ProgramHow it works:• Patient brings income verification to first clinic visit.

- Examples of income verification are: current pay stub; income tax forms; unemployment, social security or disability

voucher; letter of support• Patient is required to pay a minimum of $20 per visit• Patient meets with financial specialist to complete

application for income-based fee for visit• Often the visit is fully covered after the $20 payment• Patient can set up payment plan for as little as $5/month

Behavioral Health ServicesSupport When It is Needed• Help clients through difficult times and empower them to deal with life’s

challenges• Help individuals, couples and families address mental health needs through

ongoing therapy with a licensed mental health worker• Assist with evaluation, diagnosis and medication referrals

We Offer• Psychological and diagnostic assessments• Post Traumatic Stress Disorder treatment• Individual, couples and family therapy• Social services, including assessments required for government documents• Community advocacy and referrals

Diabetes ProgramOur experienced team gives the support patients need to

control diabetes and shape their health futureWe Offer:• Diet planning• General diabetes education• Connections to free or low cost fitness programs• Reminders when lab work needs to be completed• Monthly meetings to monitor blood glucose levels• Appointments with a clinical pharmacist• A Healthy Eating Luncheon• An accompanied grocery store trip

Clinical PharmacyWe are proud to be one of the few medical clinics with a

licensed clinical pharmacist on staffWorking closely with the patient and their physician, a clinical

pharmacist can:• Help ensure that a patient has access to the proper medications• Provide education about all of a patient’s medications: their side effects,

proper use and intended outcomes• Assess entire drug therapy, looking for unintended interactions or potential

complications• Modify dosages or change medications as needed• Help manage chronic diseases• Help find the most cost effective prescriptions

Nutrition Counseling• Our nutrition counselor can help enhance a patient’s physical and mental health by helping them eat the right foods. • Patients may be referred by their doctor, or make an appointment without a referral

A PCHS Dietitian Can:• Assess a patient’s diet and nutritional needs • Help with weight loss or weight gain• Help plan a diet to manage diabetes, high blood pressure, cholesterol

imbalances, or other diseases• Create an allergy-free diet plan• Give sports nutrition advice• Help improve children’s nutrition

Shape Up - Healthy Lifestyle ProgramA program designed to help patients live the healthiest life possible.

Attendees will learn:• Healthy lifestyle choices• Exercising at home• Tracking weight• Fitness• NutritionWho is Shape Up for?• We have two classes!

- One for all adults, ages 18 and older

- One for all youth, under 18 years of age

Shape Up for Adults! • Every Wednesday from 10:30 – 11:45 am

Shape Up for Youth!• Every Wednesday from 4:00 – 6:00 pm

Both classes are offered onsite at the People’s Center
