crane mechanic noted that the hoisting wire rope was misaligned within its

drum during set up. The crane operator fully extended the crane boom to

uncoil the wire rope and as he was about to coil back the wire rope, he slewed

the boom of the crane slightly to the right and caused the crane to collapse.



the crane operator was hoisting a clamp shell bucket out of its housing which

was located on the barge. He then slewed the crane to the right when the

boom suddenly started to fall and collapse on the deck of a nearby hopper

barge



the truck-mounted mobile crane was deployed outside a maintenance workshop

to assist in the assembly of a grab bucket. When the crane lifted the assembled

grab bucket to about 0.5m above ground, the crane structure suddenly broke

off at its slewing ring, and separated from its chassis

- Mini cranes are mobile cranes with safe working load not more than 5 tonnes

- Smaller in size, less complex, designed to lift significantly lighter loads and

generally have a lower risk as compared to typical mobile cranes

- a specialised training course for mini cranes will be made available in the first

quarter of 2016 to raise the competency of mini crane operators

- mini crane operators will no longer be required to hold a mobile crane license

from 1 January 2016. Instead, will be required to attend the specialised

training course

WSH Council has launched a co-funding scheme which covers 50% of the cost of installation up to $5,000 for each registered crane. The funding is available from 3 August 2015 on a first-come-first serve basis and it will cease on the 31 July 2018 or when the funds have been fully disbursed.

STEP 2 Engage an Authorised Engineer (AE) to inspect the crane to verify that DL installed is according to requirements
