About MPCA
WHO WE ARE, WHAT WE DOMichigan Primary Care Association (MPCA) is a leader in building a healthy society in which all residents have convenient and affordable access to quality health care. Our mission is to promote, support, and develop comprehensive, accessible, and affordable community-based primary health care services to everyone in Michigan.
For over 30 years, MPCA has been the voice for Health Centers and other community based providers in Michigan. MPCA got its beginnings in the late 1970s as a networking organization of Health Centers and stakeholders interested in fostering primary care in rural underserved communities across the state. With the realization that urban communities also had great health care needs, the Association quickly broadened its scope to include urban Michigan communities as well. Today, 34 Michigan Health Centers provide quality, affordable, comprehensive primary and preventive health care for over 600,000 patients annually at more than 200 sites across the state. information about Health Centers begins on page 11.
MEMBERSHIPActive MPCA membership is open to organizational providers of primary care that support the mission of MPCA and includes a seat on the Board of Directors and full voting privileges. Associate membership is open to individuals or organizations that support the mission of MPCA but do not qualify for active membership.
OUR SERVICESu Clinical Support & Quality Improvement A core focus of MPCA’s
work is to support quality primary health care services in Michigan through training and support in patient safety, Patient-Centered Medical Home adoption, quality assurance and improvement plans, UDS clinical measures, chronic disease management, preventive/wellness health care, and improvement models.
u Communications MPCA communicates internally and externally through our website, blog, weekly eUpdate, and social networking sites. We also host communication networks to promote information sharing, educational opportunities, and peer-to-peer networking among Health Center staff.
u Community Development With the goal of 100% access to health care in Michigan and zero health disparities, MPCA assists communities in determining the best approach to providing access to care.

• 6 •u Community HealthCorps This statewide program focuses on Healthy Futures
through AmeriCorps member service in Health Centers and with other partners. its priority areas include access to care, disease prevention, health promotion initiatives, health literacy, childhood obesity, other unmet health needs, and volunteer recruitment and development.
u Data Analysis MPCA analyzes a variety of data to highlight the need for redistributing the supply of primary care, dental health, and mental health professionals to areas with the highest need; support proposals presented
to grantmakers, policymakers, legislators, and partner organizations; and identify trends and interventions to improve the health of underserved communities and vulnerable populations.
u Education & Training MPCA hosts education and training events throughout the year to strengthen and improve the ability of Michigan Health Centers to deliver high-quality health care to medically underserved communities.
u Emergency Management MPCA works with local, state, and national partners to assist Michigan Health Centers in emergency management planning and response, and provides information to partners about the role of Health Centers in their community at all times, and the potential role of Health Centers in the event of an emergency.
u Outreach & Enrollment MPCA is committed to serving Michigan families by enrolling children in the state’s MiChild and Healthy Kids programs. it places Community navigators across the state, and hosts enrollment events to connect eligible children with these programs. MPCA also works with Health Centers to help children currently enrolled retain their health coverage through innovative usage of technology and quality renewal assistance.
u Policy & Advocacy MPCA works with policymakers and elected officials helping to create an environment in Michigan that assures all residents have access to medical, dental, and behavioral health services within their communities. Each year MPCA identifies legislative priorities that will improve the ability of Health Centers to provide quality health care services to all in need, and analyzes policies that may affect Health Centers and access to care. MPCA also advocates at the state and federal level, and educates its members about policy and legislative issues impacting Health Centers.
u Special Populations MPCA works with members and partner agencies to better understand and respond to the needs of Michigan’s homeless individuals and families, residents living with HiV, and migrant and seasonal farmworkers.
u Technical Assistance & Consulting Services MPCA’s technical assistance and consulting services help maintain and strengthen Michigan Health Centers, provide surveillance, facilitate Health Center growth, improve clinical quality, leverage and enhance Health Center revenue, promote workforce development, and enhance the role of Health Centers as collaborating partners.

u Workforce Development MPCA places a strong emphasis on developing and monitoring workforce development initiatives to increase the effectiveness of health professional recruitment and retention programs for MPCA and Michigan Health Centers.

The fundamental principles on which Health Centers were established over 45 years ago set them apart from other providers of health care:
u Located in or serve medically underserved areas or populations
u governed by a community board composed of 51 percent or more of Health Center patients who represent the population served
u Provide comprehensive primary health care services as well as support services (e.g. education, translation, transportation) that promote access to health care
u Services available to all with fees adjusted based on ability to pay
u Meet strict performance and accountability requirements regarding administrative, clinical, and financial operations as established by the federal government
Michigan Health Centers are part of a nationwide network of 1,200 Health Centers spread across 50 states and all U.S. territories that provide care for over 20 million people at over 8,000 delivery sites.

About Health Centers
“Health Center” is used to refer to Health Center Program grantees and Federally Qualified Health Center Look-Alikes
Of Michigan’s 34 Health Centers, 30 are Health Center Program grantees, 2 are FQHC Look-Alikes, and 2 are both a Health Center Program grantee and an FQHC Look-Alike.
Each Health Center’s staffing models, facilities, scope of services, and approaches are tailored to meet the unique needs of its patients and surrounding communities. Many individuals have difficulty accessing medical care because of where they live, the language they speak, and their complex health care needs. Health Centers provide culturally appropriate health care that is close to where patients live, at times that are convenient, and in languages patients can understand.
Health Centers have professionally trained health care teams. These teams may include board-certified or board-eligible physicians, nurses, nurse practitioners, physician assistants, certified nurse midwives, medical assistants, mental health and substance abuse professionals, dentists, and dental hygienists. The clinicians of each team work together to provide a core of primary and preventive health services to individuals and families throughout all life stages, from pediatric through geriatric.
Four types of Health Centers as described on page 12 are funded under Section 330 of the Public Health Service Act. These Health Centers are also called Federally Qualified Health Centers (FQHCs) and they must meet the following requirements:
u Be located in or serve a Medically Underserved Area/Population (MUA or MUP) as designated by the Health Resources and Services Administration; health care is needed but scarce in these areas
u Operate under a patient-majority board of directors
u Provide comprehensive primary health, oral health, and mental health/substance abuse services, as well as supportive services such as translation and transportation assistance, that promote access to health care to everyone in the community in all stages of the life cycle
u Charge uninsured patients for services on a sliding-fee scale that is based on their family size and income

• 12 •SECTION 330E Community Health CentersCommunity Health Centers were developed in 1965 in response to community need for improved health services for inner city populations. As part of the federal government’s War on Poverty, funding was made available for communities to establish primary care centers to provide comprehensive health services, regardless of ability to pay. Although there have been many changes in the Health Center program over the years, its foundation remains the same—to provide high-quality primary and preventive health care to people in rural and urban medically underserved areas. Medical underservice is defined by a shortage of professionals or health services and high rates of poverty and infant mortality. it may also include special considerations, such as lack of Medicaid providers, lack of bilingual services, or a large percentage of uninsured persons.
SECTION 330G Migrant Health CentersMigrant Health Centers were established in 1962 to provide comprehensive primary care services to migrant and seasonal agricultural workers and their families in a culturally sensitive environment. Like Community Health Centers, Migrant Health Centers provide comprehensive health care services to their target population at a price that is affordable and regardless of insurance status. Migrant Health Centers use culturally-sensitive clinical protocols, bilingual health personnel, and lay outreach workers. In 2011, five Migrant Health Centers served 15,350 migrant and seasonal farmworkers in Michigan (approximately 17% of the state’s migrant and seasonal population).
SECTION 330H Health Care for the Homeless CentersModeled after a demonstration project jointly funded by The Robert Wood Johnson Foundation and Pew Charitable Trust, the Health Care for the Homeless program was initiated in 1987 as part of the Stewart B. McKinney Homeless Assistance Act. in 1996, Congress passed the Health Centers Consolidation Act, which joined the Health Care for the Homeless program to the community, migrant, and public housing primary health care programs under a single authority called the Consolidated Health Center Program. The Health Care for the Homeless program provides federal grants to non-profit organizations to deliver primary health care and substance abuse services to homeless individuals and families. The Health Care for the Homeless program provides a multidisciplinary approach to the delivery of care by combining systems of primary care, substance abuse and mental health services, patient management, and education with aggressive outreach. Either through direct provision or through established links with other community agencies, eight Health Centers in Michigan operate Health Care for the Homeless programs and serve 19,345 homeless individuals (approximately 19%, of the state’s homeless population).
SECTION 330I Public Housing Health CentersThe Public Housing Primary Care Program was established to support the provision of accessible and comprehensive preventive and primary health care services to residents of public housing. Health promotion and disease prevention are emphasized, and primary health care services are provided on the premises ofpublic housing developments or at other locations immediately accessible to residents of public housing.

About Health Centers
FQHC LOOK-ALIKESFederally Qualified Health Center (FQHC) Look-Alikes are Health Centers that have been certified by the Centers for Medicare and Medicaid Services (CMS), based on recommendations provided by the Health Resources and Services Administration (HRSA)/Bureau of Primary Health Care (BPHC), as meeting all Health Center Program requirements. FQHC Look-Alikes do not receive funding under the Health Center Program. This certification makes them, like Health Center Program grantees, eligible for fair reimbursement through Medicaid and Medicare, participation in the 340B federal drug pricing program, and assists in recruiting and retaining providers through the national Health Service Corps and the State Loan Repayment Program.
INDIAN HEALTH SERVICESThe most prominent provider of health services to American indians is the indian Health Service (iHS). The iHS provides both direct care and contract care to members of federally recognized tribes on or near federal reservations. The iHS funds two MPCA members—American indian Health & Family Services of Southeastern Michigan, inc. and Bay Mills Health Center—to provide urban indians in Southeast Michigan and Chippewa County respectively with comprehensive health care services, which include medical, community health, dental, and mental health/substance abuse services.
RyAN WHITE CARE TITLE III CLINICSHealth care organizations funded under the Ryan White CARE Act Title iii Early intervention Services Program provide a comprehensive continuum of outpatient HiV primary care services including HiV counseling, testing, and referral; medical evaluation and clinical care; other primary care services; and referrals to other health services. Community Health Centers, Migrant Health Centers, and Health Care for the Homeless sites funded under Section 330 of the Public Health Service Act may receive Title iii grants.
