IMPORTANT: (1) All the relevant documents as per requirement of the Tender must also be

All such documents should be strictly submitted by RPAD / speed post only. Otherwise the offer will not be considered and no any further communication in the matter will be entertained.

INSTRUCTION TO BIDDERS 01. Prescribed EMD will have to be paid by the tenderer, which will be accepted by crossed

Demand Draft drawn on Bank of Baroda or SBI payable at Ukai. Tender submitted without EMD are liable to be rejected. All tenderers are strictly instructed that Xerox copy of DD No. of EMD & Tender fee to be sticked on tender cov er without which it will be resumed that EMD is not paid and tender will not be considered / accepted.

02. Tenders received late after prescribed due date & time will not be accepted. Any alternation / modifications to the bids received after opening of the tender shall not be considered unless called for by the GSECL in negotiation.

BID QUALIFICATION CRITERIA FOR TECHNICAL BID Offers of only those well-experienced and resourcef ul tenderers, who meet at least the following criteria, will be considered. i. Tenderer should have carried out the same or similar natured Ash plant/ Boiler related

maintenance and R & M work etc. on as and when required basis or on piecemeal basis having single or aggregate order value not less than 4 Lac in any year during last 03 years in any power station of GSECL or NTPC or state owned utility or in any IPPS (Independent power producers).

ii. Past executed order copies of similar work or ash plant / Boiler related maintenance /R&M work carried out in GSECL or any other State Electricity Board, NTPC/IPPS.

iii. Please furnish your address, Phone No., Fax No., e mail ID, CST/VAT registration certificate (if available), PAN No., Copy of separate P. F. Code Number and receipt of EMD paid & Signature of the party with Company’s seal.

iv. Service Tax Registration Certificate. v. List of tools and tackles lying with them for carry out the work. vi. Evidence of having successfully/satisfactorily carried out the same type of works

satisfactorily at the thermal Power Station of SEBS,NTPC & Public Sector within last 05(five) years period. The experience certificate with order copy & satisfactory work completion certificate from such organization where the work has been carried out by the bidder should be submitted.

vii. Chief Engineer (GEN) UKAI TPS reserves the right to either accept or reject any or all tenders without assigning any reason any thereof.

viii. The offer without any of the above documents or non acceptance of any clause shall be liable for rejection. In this regard the decision of CE (G) Ukai TPS shall be final & binding to the bidder.

ix. Subletting of tender is not permitted. x. Service tax, VAT, excise or any other taxes - applicable must be mentioned in price bid only

& if mentioned in technical bid or elsewhere are liable to be IGNORED. xi. In no circumstances price should be quoted in Technical bid. Bidder has to quote their rate

only in price Bid in schedule –B. Tenderer shall furnish enough details/ documents in the technical bid regarding their experience, capability and resourcefulness meeting above requirements, solvency certificate minimum of 20% of estimated cost issued by any nationalized Bank etc. Tenderer shall also furnish as per list. Tenderer shall specifically mention with supporting documents how they meets qualifying requirements i.e. tenderer shall furnish copy of past orders executed (mentioning detailed scope) along with their respective performance certificate from the end user by which he claims to have met tender qualification requirements.

Scope of Work 1. The contract is proposed mainly for mechanical maintenance of ash handling plant with

associated systems & equipments as per standard procedure issued by manufacturers for mechanical, natured works.

2. Jurisdiction of contract consists and includes, bottom ash systems, fly ash system and HCSD/LSD systems from boiler bottom ash hoppers to silo to dyke area.

3. Maintenance of equipments installed at AWPH, BAS, bottom ash conveyor, HCSD pump house, BA silo & fly ash silo etc erected for above mentioned system.

4. Repairing/replacement of disposal /conveying pipes, air pipes & water pipes, joints and couplings, structure/ supports associated with system shall be in scope of contractor.

5. The housekeeping of ash handling plant with Air jetting/cleaning of spillage ash from AHP auxiliaries i.e. BAS, Silos & ECO, APH, Duct hoppers and ESP hoppers

6. Scope of work consist / includes of safe, smooth, trouble free, energy efficient maintenance of equipment cover under the jurisdiction of plant premises falls under scope of works.

7. Loading, unloading, transportation of new materials required for maintenance and crediting the scrap in main store shall be in scope of contract.

8. All lubricants (Special designated oils only) & spare required for ash handling system will be provided by GSECL free of cost. Also GSECL provides electricity at free of cost to attend defect. Party has to arrange for transportation of spares/lubrication from GSECL main store to site. Consumables are to be provided by Party at his own cost.

9. For maintenance of equipment /systems for major/minor work if required arrangement for issuing the PTW /LC will be done by GSECL.

10. Replacement, erection, testing, commissioning of the equipment (i.e pumps, blowers, motors, compressors) shall be in the scope of contractor.

11. The evacuation of ash in ECO hopers, APH hopers, DUCT hopers from slant portion of hoppers and in ESP hopers below rapping mechanism shall be in the scope of contractor.

12. The efficient maintenance shall be carried out to prevent pollution due to fly ash / bottom ash leakages from pipelines etc. in course of operation of the units. The contractor has to follow GPCB norms otherwise penalty will be imposed.

13. Flushing & cleaning of ash slurry sumps and ash water sumps shall be carried out Periodically or at opportunity available and or as per the requirement of the system/instruction of EIC.

14. The routine, preventive and planned maintenance works are to be carried out (as per O

& M manual of auxiliaries) normally in general shift. i.e. During day times only. While regular defect, breakdown, emergency overhauling and AOH/COH maintenance works are to be carried out round the clock, and same should be completed in specified shut down period. Hence contractor shall have to engage his man power accordingly, whether unit / plant is running or not.

15. The contractor shall deploy sufficient qualified/ experienced cadre supervisors in round the clock {working hrs-08hrs/shift} .

� MAINTENANCE PART:

(A) ROUTINE MECHANICAL MAINTENANCE OF FLY ASH SYSTE M: The routine maintenance of FA system includes following works- 1. Regular maintenance/ repair/ rectification/ replacement/alignment of various valves such

as, pneumatically operated valves, manually operated valves, knife gate valves(KGV), bellows, all the valves of the fluidizing air blower lines, lever, line crossover valve, pneumatic air cylinder maint, replacement of seal kit for air cylinder etc.

2 . The maintenance of all the line of dry ash, HCSD up to the ash dykes, all associated pumps discharge valves, all the ash water pipelines, fluidizing air blower lines. Attending/ rectifying leakage of joints and replacement of defective pipes and pipe fittings.

3. On line preventive/ breakdown maintenance/repair/rectification of all auxiliaries/equipments of silo system which includes bag filter, silo tank pressure relief valve, dump valve, rotary feeder, spout and all other equipments and systems fall under the dry fly ash collecting system.

5. Carrying out all necessary maintenance of the plant during the shut-down of the units for major overhauling / inspection i.e. annual overhauling or Capital overhauling.

6. During the annual/capital overhauling of the FA system/ BA system the contractor has to carry out the complete maintenance of all the valves of the system that includes, dismantling of the valves; complete overhauling as per the standard maintenance practice of the valves and reassembly/ erection of the valves.

7. The complete maintenance of the fly ash pipe line/water line which includes repair/replacement/modification of the pipes, repairing of coupling joints, flange joints and attending of line puncture etc.

8. The complete maintenance of external fluidizing system during the shutdown period with flushing/cleaning the FAB line, attending leakages of FAB lines.

9. Routine mechanical maintenance/defective parts replacement/ of all compressor as per OEM manual or as per Instruction of EIC.

10. Mechanical maintenance and defect work of fly ash rotary feeder spout up-down mechanism. Regular cleaning of area of rotary feeder, silo operating panel area, silo top side etc. 11. Dispatching of fly ash from silo and to unload the fly ash silo as & when required as per instruction of EIC. (B) MECHANICAL MAINTENANCE OF VARIOUS PUMP S: The maintenance/ overhauling of various pump i.e GEHO pump, Charge pump, LP pump, HP pump, seal water pump slurry pump, sump pump, drain pump, BA make up pump, Ash conditioning pump, Ash water make up pump @ RWPH, includes following works-

1) De-coupling, dismantling, lifting & shifting to a workable place. 2) Opening/ inspection of bearing, impeller etc and overhauling (routine, breakdown & annual)

as per detail procedure laid down in O&M manuals and same shall be adopted invariably, to ensure optimum performance of equipment with replacement/ repair of parts/bearing/ impeller if required as per instruction of EIC.

3) Re-fitting, coupling, alignment, re-connection and trial of the pump after completion of work.

4) The work shall be completed and pump made ready in least possible time. The entire stand by Equipments should be kept in working condition for immediate use whenever required.

5) Regular checking/ maintenance of all pumps such as oil topping & greasing, attending gland Leakages from pumps and valves.

6) The dismantling/replacement of isolating valves if required (C) MAINTENANCE OF BOTTOM ASH SYSTEM : The maintenance of bottom ash system includes following works: 1. Hydraulically operated dump gate & BA hoppers quenching nozzles checking/cleaning/

maintenance for smooth operation shall be carried out at available opportunity. 2. Oil level and greasing of equipments/bearings/where ever required shall be checked regularly

and maintained as per manual. 3. Maintenance of Submerged Scraper Conveyor (SSC): • This consist cleaning of gear box, oil topping necessary, greasing of all bearings of chain, idler

and system as per schedule, tightness checking of the chain, tightening of all scrapers, replacement of scrapers, chains, maintenance of driving chain to gear box, guide rollers, sprocket etc. as per requirement. Also, includes complete gear box maintenance / overhauling, complete bottom ash system to keep chain tightening mechanism free and clean as per satisfaction & instruction of EIC. The work is to be completed within least time to avoid any generation loss.

• In case of scrapper chain break down or slipping of chain from guide roller/ sprocket the work of realigning the chain is to be carried out in the least minimum time,

• Doors provided on SSC casing must be leak proof to avoid ash leakage from the doors in case ash leakages found should be attended immediately. If doors are damaged/deformed due to erosion and corrosion or other reasons the same is to be repaired/fabricated as per instruction of EIC.

4. Maintenance of Clinker Grinder: This consist complete maintenance / overhauling of gear box, flap gate and fluid coupling as per requirement. The cleaning of gear box, oil topping, greasing of all bearings as per schedule and, tightness checking of the chain, replacement of chains, maintenance of driving chain to gear box, sprocket etc. as per requirement as per requirement of system/instruction of EIC.

5. The maintenance/regular inspection/repairing/replacement of all the line of Bottom ash, HCSD up to the ash dykes area.

6. Maintenance of Belt conveyors: • This consist of maintenance/overhauling/replacement of Drive pulley, Head pulley, Drive motor,

Gear Box, idlers, Scrapper guard, as per requirement & instruction of EIC. • It also involves daily checking/maintenance of belt alignment, belt tightness. Greasing at all

pedestal bearings and cleaning/oil topping of gear box if required or as per schedule given by EIC.

(D) MAINTENECE OF DISPOSAL LINES: 1. All the M.S /CI/DI/CB piping should be inspected regularly for any worn out / erosion which is

normally concentrated at the fittings/bends etc. And also pipes are properly supported/secured with clamps to be ensured to prevent distortion/leakage and accordingly preventive maintenance shall be done.

2. Ash collected over the disposal lines / trench / air release valve should be cleared immediately and disposed off to a site at a distance place as per instruction EIC.

4. The chocking of ash disposal lines is to be cleared immediately without fail to put in service. While clearing chocking up of pipe lines, heavy slurry flowing over the pipe line or falling over bridge shall be avoided.

5. Repair/replacement/rectification of defectives ash slurry valves and ash disposal pipes, bends etc. from ash slurry pump house to disposal points without affecting the operation of unit.

6. Arresting of leakage through joints by replacing gasket or coupling sleeve. The pipes to be replaced during shutdown shall be jointly decided by GSECL/contractor.

7. Regular patrolling of ash disposal pipelines from HCSD Pump house to dyke area shall be carried out and for any damage/fault shall be reported to main control room immediately.

8. In case of failure of running line, standby disposal line to be taken in service at the earliest, to keep both the fly ash and bottom ash system normal.

General aspect of scope of work :-

1) The line clear shall be issued to the contractor or their authorized technical supervisor for maintenance work and work shall be treated as completed when the equipment is accepted by operational staff and put into service. Any defects noticed on putting Silo/Ash handling system into operation after repair shall have to be rectified as part of this contract immediately.

2) The scope of work consist, dismantling/removal of defective parts and refitting the same after overhauling/repairing/fabrication or fitting of new parts as per instruction of Engineer In Charge.

3) The transportation and handling of old/new ash plant/Silo components, equipment, from any plant of Silo unit No. 6 available in working condition as a part of this contract without any extra cost as per instruction of Engineer In-charge. i. The scope of work shall also include transportation, handling & crediting of old/new

materials, equipment’s, lubricants, spares etc. from GSECL Main store/sub store to ash plant area of the units as directed by Engineer-in-charge. The dismantled items/equipment/spares shall have to be shifted immediately from working area & credited to sub store/main store or other department/areas of Unit No.6 as part of this contract without any extra cost and as directed by Engineer-in-charge.

ii. After the repairing/maintenance, the old spares should be removed immediately from the site and working area should be kept cleaned and cleared. Replacement of parts, the welding, grinding is in contractors’ scope.

4) In case of breakdown maintenance, the contractor shall have to mobilize all his resources immediately on notice of defect and take-up the required jobs on emergency basis and shall have to complete within least possible time. Work started shall be carried out nonstop till completion of job and safe commissioning of the Silo/Ash handling system to the entire satisfaction of Engineer In Charge. Any negligence/disobedience and or delay on the part of the contractor would be penalized appropriately and decision of the CE(G) Ukai TPS in this regard shall be final and binding to the contractor.

5) All the jobs shall be carried out as per standard maintenance practices and as instruction of GSECL site representative to meet the system requirement up to satisfaction of Engineer-in-charge.

6) Contractor has to deploy 07Nos. manpower per shift (i.e.01 Supervisor + 01 technician + 01 welder, + 02 fitter+ 02 helpers) ro und the clock for Scope of the Work described in Schedule B (Sr. No 1).

7) Party has to provide additional skilled manpower for Sr. No.2; unskilled manpower for Sr No.3 as per schedule-B as & when required basis as per oral or written instruction from EIC. GSECL will pay for additional man power as described in schedule-B. Party has to submit detail record/report for present of additional man power and work carried out by them dully signed by Party’s supervisor and GSECL representative/GSECL shift operator.

8) Party must keep record of work allotment, work carried out and present of employee duly signed by GSECL Shift Operator and has to submit to AHP for bill recording. Failure of submitting the present report, payment will not be made for the same day/shift.

9) Necessary scaffolding / juggad shall have to be prepared by the contractor for which materials like pullies, Kathies, Pipes, wooden planks; steel supports etc. shall be arranged by the contractor as part of this contract. After completion of work, contractor shall have to dismantle scaffolding / juggar etc. as per instruction of EIC.

10) During the maintenance work, any damage to valuable parts of GSECL or damage to the parts due to mis-handling and excessive wastage of material will be considered seriously and appropriate amount will be deducted from the bill. The amount will be decided by CE (Gen) and binding to the contractor.

11) The contractor shall submit monthly bill in triplicate regularly along-with material account. The payment shall be made on actual quantity basis per day basis for whole month.

12) Contractor has to keep surrounding area clean (Removal of dust /ash /debris etc.) before starting any welding work in area & shall take all precaution to avoid fire hazard in this area.

13) Any other minor activities which are required to be carried out but not mentioned in the scope of this work shall also have to be carried out by the party without any extra charges as per instruction of Engineer –in-charge.

14) Contractor has to deploy manpower as mentioned in Technical terms & condition, failing of which deduction of proportionate amount on ‘PRO-RATA BASIS’ will become admissible.

15) Poor workmanship: i) Occurrence of poor workmanship, frequent repeated maintenance will be viewed

seriously. The amount of Rs. 1000/- per occurrence shall be deducted from the bill with the approval of competent authority.

ii) If scrap material not removed from silo, hydro vector tower, ESP ground floor etc even after instruction of Engineer-In-Charge. The amount of Rs.500/- per day will be deducted from the bill.

iii) Contractor shall have to use proper tools tackles for the maintenance works. Removal of parts, bearings, shafts, gear by hammering is not allowed. If such occurrences are noticed, penalty of Rs. 500/- per occurrence shall be deducted from the bill.

16) If required by the GSECL, the contract shall discon tinue during extended Shut

down, Break down, any major force outage, Reserve S hut down etc. No claim/compensation/payment shall be made to the con tractor .

17) Regular Cleaning of Silo from top to bottom including Weigh Bridge & its surrounding area is in contractor’s scope. If ash is dumped on Weigh Bridge due to silo cleaning is to

be remove by contractor by manual lifting, water jetting etc. The silo surrounding area cleaning is in contractor’s scope.

18) Repairing of old parts of Ash Handling palnt such as Valve’s seat and slides, slide, dump gate, pipe pieces, elbows hard facing, preparation of pipe short pieces or any material related to the scope is in the scope of contract.

19) Contractor should be resourceful with sufficient skilled staff and labors every day from existing staff in such a way that breakdown work can be carried out within short time, otherwise appropriate amount will be recovered from the bill if the down time is more for each type of maintenance.

20) Area of Silo/Ash handling system shall be kept and maintained neat and clean free of debris, scrap/coal dust/lubricant all the times so that working is not held-up at any time.

21) All old/worn out parts or new spares of Silo/Ash handling system shall be immediately removed from working area and credited either to store or stacked properly in area as directed by Engineer-in-charge on completion of work.

22) In case of breakdown maintenance the contractor shall have to mobilize all his resources within one hour of notice and take-up the required jobs as directed by Engineer-in-charge on emergency basis and shall have to complete within least possible time. The extra labour force is to be arranged by contractor on need basis as per job requirement.

23) The contractor shall have to carry out the work as and when required and the work started shall be carried out non-stop till completion of job and safe commissioning of the Silo/Ash handling system to the entire satisfaction of Engineer In charge. Any negligence/disobedience and or delay on the part of the contractor would invite penalty for and decision of the competent authority in this regard shall be final and binding upon the contractor.

24) To improve the efficiency of the silo, modifications as suggested by Engineer I/c. or OEM are to be carried out by the contractor within quoted rates.

25) All old/worn out parts or new spares of Silo/Ash handling system shall be immediately removed from working area and credited either to store or stacked properly in area as directed by Engineer-in-charge on completion of work.

26) Small bearings, oil seals, O-Rings, rubber parts, R ubber Sheet, Belts, Fasteners, rubber bush, Teflon bush, Gaskets, minor spares etc . if not available with GSECL and if instructed by Engineer In Charge, contractor have to purchase the same and the payment of the same shall be reimbursed to contractor as per “PAKKA’ invoice from supplier in the name of GSECL.

27) Authorized & technically qualified and experienced representative (Supervisor) must remain present at site round the clock.

28) The round the clock activities such as ash tanker filling in co ordination with GSECL operator, Ash hopper pocking etc must be done invariably for better evacuation of hopper and maximum ash collection as desired by GSECL .

29) The preservation of issued material shall be done by the contractor. In case of loss/theft of material, contractor is responsible. In such case GSECL shall recover the cost of lost materials.

30) The contractor shall be responsible for care and safe custody of GSECL’s property entrusted and being handle by him otherwise cost of damaged /shortage will be recovered from him.

31) Party has to come-up with concrete suggestion in writing to solve any problem observed in running unit previously related to work awarded. If no improvement observed in performance, dealing with party will be stopped in future.

32) Any work related to contract awarded is not mentioned in the scope and is required to be carried out than same will be in the party’s scope at no extra cost.

33) GSECL reserves the right to forfeit the Security Deposit in case of any breach of contract.

34) Insurance and labour license as per the legal requirement shall be required to be taken care by the contractor.

35) Fly ash line & slurry discharge line coupling tightening work up to line cross over station if required to maintain the healthy system should be attended as per instruction of EIC.

36) Necessary cleaning by air jetting if required in the area of ash plant like silo area , ESP Area, Bottom Ash Area etc. should be attended.

37) During forced outage of boiler if ESP hoppers are found filled with fly ash necessary manual pocking or water jetting is to be done as per instruction of EIC.

38) For the above scope of work contractor have to engage required man power 24 hrs. round the clock.

39) The pocking ,chock removal ,dewatering, cleaning should be done as per site requirement and as per the instruction from Engineer In charge from maintenance /Shift In charge from operation.

40) Replacement/repairing/overhauling of Filter bags of Vent Filter, Vent Fan, Rotary Feeder, Telescopic Chute, Wire Rope Winch, Fly Valve of Silo,

41) Complete Cleaning, Ash Removal & Disposal from Silo, Checking of Fluidizing Pads, Repairing, Replacement and Refitting of Pads and Fluidizing Air Test,

42) Complete Overhauling, Repairing and Replacement of Equalizing/Vent Valves and its connected Piping, Bends, Reducers & Flanges, Checking & Replacement of Fluidizing Pads of ESP Hoppers and Overhauling of individual Hopper’s Fluidizing Air system Valves & NRVs :

43) Any other minor work related to this work not specified in the scope of work, but as per site requirement must be attended by the party in this quoted rates only.

Note: 1. Contractor has to arrange minimum 03 numbers 3-phase welding machine to cover entire

available including appropriate overhead charges. Issue of Consumables: In any emergency case any consumable required will be issued on chargeable basis if available with GSECL.

Tools & Tackles:

All required Tools & Tackles, welding machine & accessories, gas cutting/ heating set, chain Block, Hook Chuck, Winch Machine etc. shall be arranged by party.

Site Facilities: Power supply for electrical equipment and energy consumption for the activities associated within scope of work shall be provided at max. three point in the unit free of cost . All equipments must have over load earthing and other electrical protection switching devices. Man power: The contractor has to deploy 07 (Seven) Nos. manpower (Supervisor-1,Technician-1, Welder-1, Fitter/Rigger-2 & Helper-2) per shift round the clock to attend day to day defects as per scope of work.

The contractor has to arrange additional man power as per instruction of EIC on telephonic, orally or written instruction as described in schedule A.

Co-ordination: (1) Maintenance shall be carried out as per instruction of J.E./D.E./E.E of respective

maintenance departments of Mech. Construction/ Ash Plant. (2) Contractor has to report about daily progress. In case of round the clock work, the work

progress shall be reported at least once in a day.

Time Limit: This work order is of 100 Days. However the Time limit of the order is 30 0 Days from commencement of work. If machine remains out of bar for more than 03 days , Letter of discontinuation shall be issued to withdraw the man power from the site. Letter of commencement shall be issued upon the start of machine which shall be intimated to the party before 01 day & accordingly party has to mobilize the manpower. Billing & Payment:

(A) Payments will be made by running accounts bills monthly after taking joint measurements and following the GSECL procedure. (B) The final joint measurement will also be completed as early as possible after handing over the completed work in all respects and final bill will be prepared and paid for as soon as possible thereafter.

Mobilization period: Party shall have to mobilize the site within 03 days on receipt of letter of commencement of this work order.

Penalty:

Any delay that may take place beyond contractual cut off date/ stipulated period in the order shall be subjected to penalty (not liquidated damages) at the rate of ½ % of the contract price/order price per week or part thereof subject to a maximum of 10% of the total contract value/order value (i.e. End cost including taxes & duties).

General Terms & Conditions

1) All the maintenance activities are to be carried as per directive & procedures laid down in (O&M) manuals of OEM / OES it should be.

2) All the maintenance records are to be maintained by the contractor as per documentation formats issued by GSECL maintaining records in standard formats are necessary for certification. Day to Day maintenance report should be submitted to AHP office dully signed by control Room#6 representative.

3) Quantity show in work order is approximate, liable to change. Payment will be made according to actual work done only.

4) During the course of maintenance if possible some parts may be utilize after minor modification and for that if required workshop facility will be provided at free of cost.

5) First aid facility will be provided as per GSECL prevailing rules. 6) The contractor shall have to pay 5% of order value as security deposit by account payee

DD in favour of GSECL-Ukai payable at BOB / SBI at Ukai within 7 days after receipt of Letter Of Intent (LOI).

7) The contractor shall have to execute agreement on stamp paper worth Rs. 50/- and to sign the contract booklet of contract. The cost of stamp paper completion of agreement shall be borne by contractor.

8) The contractor shall have to execute indemnity bond on stamp paper worth Rs. 100/- and should be submitted along with agreement. The cost of stamp paper should be borne by contractor.

9) The contractor shall have to submit the bill in triplicate at the end of every month along with material account in the prescribed Performa as directed by EIC and payment will be made as per rules within 30 days after receipt of a RA bill.

10) Contract price are fixed for entire contract period and no any price escalation admissible. However taxes will be paid at actual on submission of paid evidence.

11) If generation loss occurred due to non availability of Ash handling system for lack of operation / maintenance of system / equipment, at the discretion of CA, the amount maximum up to 5% of the total contract value will be deducted from the bill of contractor.

12) Any pump/auxiliaries taken out for overhauling shall be made ready in least possible time by deploying the man power continuously, in case of emergency pump should be made available within a shift as directed by EIC, otherwise penalty at the rate of Rs.500/Hr will be charged.

13) In the event of any disputes regarding responsibility/ work scope for operation failure/mal-operation/ faulty operation/ equipment failure/ or other causing evacuation problems of fly /bottom ash the decision of Chief Engineer, UTPS will be final and binding to the contractor

14) Standard safety practices and standard engineering methods shall be adopted for O&M works. All the safety tools and personal protection equipments shall be provided to personnel by the contractor.

15) The efficient maintenance shall be carried out to prevent pollution due to fly ash / bottom ash leakages from pipelines etc. in course of operation of the units. The contractor has to follow GPCB norms otherwise penalty will be imposed.

16) “If units under RSD, AOH, COH or any long period outage, after satisfactory completion of defects, party’s order will be discontinued immediately & same work will be continued after unit taken on bar”

17) “Party has to engage their own labours and the full responsibility in respect of their supervisors and workers will be taken by them.

18) The payment shall be made through RTGS. The contractor shall be required to furnish the bank details.

19) The income tax will be deducted from bill as per prevailing rates and necessary certificates will be given by GSECL as per rules.

20) GSECL Ukai TPS is situated in Gujarat, all the rules and regulation regarding labour laws, safety laws etc. of Gujarat Government if amended shall be applicable for the said contract.

21) In case of any incidence occurred in the plant premises i.e fatal/ non fatal accident, the contractor has full liability for accident.

22) The contractor shall have to produce solvency certificate from nationalized / schedule bank.

23) The contractor has to produce copy of labour license before starting of work if applicable. He should produce/ comply with all labour laws of Gujarat government in force and he should have to submit the PF code.

24) Canteen facility will be available at prevailing rate available to GSECL’s employees 25) The contract period shall be of 300 Days from the date of commencement; however same

can be extended for further 6 months at the same terms and conditions. 26) The whole contract shall not be assigned to any other agency or any individual in any

circumstances. Violation of this leads to cancellation of contract by GSECL. However for any minor services and/or labours, the same shall be done with consultation with EIC to ensure credentials of persons and safety of plants.

27) The contractor shall have to comply with all safety standards including those established at our plant as well as those in industrial safety department. The details of general terms and conditions, factory act, labour laws, safety and securities enclosed with order copy is part of contract.

28) GSECL reserve right to stop the work at any time if Machine out of Bar due to major outage. Contactor will be informed in advance before 01 to 02 days to stop the work during this period no payment will be paid to contractor.

TERMS & CONDITIONS REGARDING INDUSTRIAL LAWS AND OTHER RELATED MATTERS TO BE INCORPORATED IN TH E TENDER DOCUMENTS 1.1 Wages to be paid and time of payment etc. by the Contractor. 1.2 The contractor shall pay minimum of wages per day or as may be specified hereafter or

rates fixed under the Minimum wages Act, whichever is higher. The wages of every contract labour employed by him under this contract shall be paid him before the expiry of 7th day of the last day of the month in respect of which the wages are payable ( i.e. wages of a month have to be paid by him in the first week of the next month). The payment shall be disbursed in the presence of management Representative during the working hours in factory premises and the contractor shall get the entries certified in the register of wages by the representative of the Corporation. Any default will result in cancellation of contract forthwith or else the contractor shall be punishable to the extent of Rs. 400/- fine per each day.

1.3 The contractor shall give his telephone number and address to the Corporation so that in case of labour troubles etc. the contractor can be contacted. The contractor shall arrange to have his office outside the factory premises and the contractor keep himself present throughout the working hours “As contractor you will be responsible and liable to pay difference in wages if any or observed the revised service condition that may be awarded by the Honorable Industrial Tribunal effective from the date directly in the Award”.

02 Labour Laws: 2.1 Person below the age of 18 years shall not be employed for the work. 2.2 No female worker shall be employed in the night shift between 7 PM to 6 AM. 2.3 Contractor shall maintain a valid labour license under the Contract Labour (Regulation

and Abolition) act for employing necessary manpower to be required by him. In the absence of such license the contract shall be liable to be terminated without assigning any reasons thereof.

2.4 The contractor shall at his own expenses comply with all labour laws and amendment made from time to time keep the Corporation indemnifies in respect thereof. Some of the major liabilities under various labour and industrial laws which the contractor shall comply with are as under:-

2.5 Payment of contribution by way of Employees (contractor Labour) as well as employer’s )contractor) Contribution towards Provident Fund, Family pension scheme, Employees Deposit linked insurance scheme, Administrative charges, inspection charge etc. at the rates made applicable from time to time by government of Gujarat/Government of India or other statutory Authority.

2.6 Labour engaged by you shall be entitled for Earn leave as per the provision laid down in factories Act. 1948. Contractor shall extend the facility of E.L. as per the provision laid down in factories Act. And shall also have to maintain leave records in prescribed form under factories Act.

2.7 No. labours are allowed to enter in power station premises who do not have Identity card prescribed under Factories Act.

2.8 Contractor shall have to comply with the Maternity Benefit if he happens to engaged female labours as per the provisions of the Maternity Benefit Act.

2.9 Contractors shall have to obtain Insurance of the labourers engaged buy him to carry out contractual work before commencement of work under workmen’s compensation Act.

2.10 P. F. code Number: The contractor shall have to submit the details of his own P.F code number along with offer from appropriate authority. Without P.F. code No. the tender is likely to be disqualified.

2.11 Provident Fund & Family Pension Schemes The Contractor shall submit along with his bill (month-wise) a statement regarding

deductions at the rate of 12% of the wages against employees provident Fund & Family Pension Scheme in respect of each concerned employee. (or at the rate made applicable by the Government from time to time) of the wages. The contractor’s contribution and his workers contribution towards Provident Fund and Family Pension Scheme shall be deposited by the Contractor with Regional Provident Fund Commissioner, Ahmedabad or to the authority prescribed under the Act. Along with the other charges applicable. P.F. Challan submit with RA bill

2.12 If any workman engaged by contractor meets with fatal or non-fatal accident while on duty then contractor shall be liable to pay compensation Act.

2.13 Employees Deposit Linked insurance Scheme: The contractor shall have to deposit 1/2 % of the wages in respect of employees who is member of the Provident Fund as the contribution to the Deposit Linked insurance scheme with Regional Provident Fund Commissioner, Ahmedabad or concerned P.F. Office.

2.14 The contractors should pay prevailing minimum wages to the labourers engaged by him as per the minimum wages Act. In presence of the corporation officer of representative.

2.15 Administrative Charges: Administrative charges for maintaining provident fund A/c. shall be deposited by the

Contractor with Regional Provident Fund Commissioner, Ahmedabad or concerned P.F. office at the rates applicable.

2.17 Labour License for Labour Contract: The contractor shall have to obtain the copy of labour contract license under contract

Labour (Regulation & Abolition) Act. from the appropriate authorities for commencement of work

2.18. Contractors shell have to issue the contractual appointment letter for a specified period to their labour engaged during the contract period each and every instant & should narrated clear out terms and conditions for the liabilities of the concerned employees.

2.19. After completion of contract work, the work completion certificate is required to be produced before the DGM/IRO for the purpose of no dues certificate and cancellation of gate passes issued to the contractors work men at the time of execution of the contract work.

2.20. The contractor shell have to maintain all the relevant records under labour laws as mentioned here in above and keep ready for verification by the corporation authority as well as the authority of the Govt. official a and when asked for the checking.

2.21 The contractor shell have to produce/submit the original copy of wedges register/muster roll and relevant record under labour law for the contract work as & when demanded by the corporation statutory completion if arises in future. The above are some of the major liabilities of the contractor in addition to other liabilities prescribed under the various labour laws in force from time to time from statutory Authorities like stage Government/Government of India which the contractor shall have to comply with.

03 Paid Leave Facility: Paid leave facility at the rate of one day for every twenty days worked by the contract labour shall be provided by the contractor to his workers. He shall maintain Leave Records/Leave Cards, for individual laborers which shall be duly verified and approved/certified by the authorized officer of the Corporation.

04 Workmen’s Compensation Fund & Employer’s Liability Insurance: The contractor shall cover all his employees under workmen’s compensation Fund & under the employer’s liability insurance.

05 Maintenance of registers & records: The contractor shall employ adequate number of experienced staff at site for daily supervision and for maintenance of various registers and records required under the law and contract. No payment for supervision shall be admissible.

06 Contractor to indemnify the Corporation: The contractor shall indemnify the Corporation and every member officer and employees of the Corporation also. Engineer-in-charge and his staff against all action, proceedings, claims, demands, costs and expenses whatsoever arising out of or in connection with matters referred in herein above elsewhere and against all actions, proceeding claims, demands, costs and expenses which may be made against the Corporation or Government for or in respect or for performance of his obligation under the contract documents. The Corporation shall not be liable for or in respect of or in consequence of any accident or injury to any workmen or other person in the employment of the contractor of his sub-contractor, and the contractor shall indemnify and keep indemnified the Corporation against all such damage and composition and against all claims, demands, proceedings costs, charges and expenses, whatsoever in respect thereof or in relation thereto.

07 Workmen’s Compensation and Employer’s Liability Ins urance: Insurance shall be effected for all the contractor’s employees engaged in the performance of this contract. If any of the work is subject, the contractor shall require the sub contractor to provide workmen’s compensation and Employer’s Liability Insurance for the latter’s employees unless such employees are covered under the contractor’s insurance.

08 Corporation’s Right to Terminate Contracts: The Corporation reserves the right to terminate this rate contract at any time during its pendency without giving notice of termination or any reasons thereof.

09 Corporation’s Right to Deduct from Contractor’s bil ls: The Corporation will be entitled to deduct directly from the bills, to be paid to the contractor any sum or sums payable by you & which sum/sums the Corporation is required to pay as a principal employee on account of your default in respect of all liabilities referred to in above clauses.

UTPS – SAFETY - 002 REV - 00 15\12\2008 Ukai TPS “GENERAL SAFETY RULES TO BE OBSERVED BY THE

CONTRACTORS”

(1) All the contractors working at Ukai thermal Power Station shall have to strictly observe following

safety rules. The contractors shall be responsible for informing and observing these rules by their supervisors / employees /labours as well as the supervisors / employees / labours of their Sub-agencies / sub-contractors engaged, if any. Prior to commencement of the work, contractor shall have to submit a written assurance on their letterhead to the concerned Sectional Head / Engineer-in-charge that they have thoroughly gone through these rules, have educated their employees / employees of their sub contractor and will strictly observe the said rules while execution of work under work contract awarded to them. They will have to indemnify the corporation for any loss or damage / accident / injury to the corporation’s property / employee or employee of their own in default of non - observing these rules.(01)Persons to be employed for carrying out the work shall possess required qualification, be fully trained and conversant for works to be done. All persons should have gate pass. Register consisting the full details (i.e. address, phone no, details of nearest relative, etc.) of all persons is to be maintained .during the work execution, one trained & competent supervisor should always remain present at site.

(2) The contractor shall all the required safety measures prior to commencement of work on dangerous substances, machineries or area at which cautionary notice is displayed and obtain “line clear” or “work permit” through the concerned Department / Section.

(3) Prior to carrying out welding, gas cutting, furnace heating or any other hot work job, the contractor shall remove all the inflammable material lying at nearby worksite or cover it properly by suitable protective covering. Also, special care shall be taken before carrying out such job & see that all possible contributing factors to set fire shall be removed / vanished prior to commencement of the work. Advance intimation shall be given to concerned section / fire section to commence the work in fire prone areas. they should also keep ready all the first aid fire extinguishers / equipments & fire extinguishing media / material like sand / water buckets or other appropriate equipment at such place.

(4) While carrying out work in confined space or inside vessel, the contractor shall obtain necessary “confined space / vessel entry permit” from concerned department prior to commencement of the work. For lighting in such areas, 24 volt (ISI certified & with proper guard) hand lamp shall be used. For taking care of the persons working inside the confined space / vessel, a supervisor / person capable to keep continuous watch on person(s) working inside, assist them incase of emergency or arrange to get immediate outside help. Shall remain present at entry point shall remain present at entry point and shall use full body safety belt without fail.

While working inside sewage, trench or in-depth, a person to warn outsiders / entrants/ passers etc shall remain available near entry point or the entry point shall be cordoned by a barricaded tape with a cautionary notice. After completion of the works, all the lids / covers / grills / grits opened, shall be re-fixed / re- placed in the original position as it were prior shall be re- fixed / re-fixed / re-placed in the original position as it were prior to commencement of the work and leave the work place in safe condition in all respect, so as to prevent accident to fellow workers.

(5) The contractor shall see that he / his persons do not block (by stacking material, spare parts, tolls-tackles, equipments etc), any passages / walkways / gangways / aisles / staircases / ladders / lift or any other approaches / roads leading to plants or its auxiliaries, on which there is a traffic movements in case of emergency . if it is utmost necessary to carry out work in such area with blocking of passage, prior permission of competent authority or the engineer- in- charge shall be obtained. To demarcate /declare the area as UNSAFE, cordon it using barricading type & display suitable caution notice or keep a person to restrict / divert the traffic route through other safe passage.

(6) Prior to use power / electrically operated hand tools / equipments / machines / gadgets like welding machine, hand grinder , hand drill etc, ensure its safe operation & use it only if it is found safe to use . do not use defective, unsafe or improperly maintained equipments.

The electrical power supply required to run such equipments shall not to be taken directly at their own but shall be obtained through concerned electrical maintenance departments or their authorized persons or under their observations / guidance only. The electrical section shall provide temporary electrical connection up to contractor’s mains board on which it is compulsory to install mains switch, ELCB & fuses of adequate capacity. all such equipments shall invariably be earthed adequately to prevent electrical shock, sparking, short circuit etc. power cord to be used shall be of adequate capacity, without any joint & shall consist of earth wire also. Hence, it is necessary to use adequate capacity 3-wire power cord for single & 5-wire power cord for three phase power connections. The plugs, receptacles, pins, holders etc shall be of adequate capacity & safe to use.

All electrical & mechanical equipments / tools-tackles viz. welding machine, cutting machine, grinder drill, chain pulley blocks, hook chooks etc required to be used during work execution shall be of standard make & bear ISI certification mark on it. The consumables like welding electrodes, grinding wheels / discs etc which has specific prescribed life span shall not be used in any case if its expiry date is over.

(7) It is compulsory to use standard make personal protective equipments (P.P.Es.) as per the job requirement. Do not work without use of required P.P.Es contractor is responsible to provide standard make (ISI approved) personal protective equipments / safety gadgets suitable to give sufficient protection against hazards involved in their work / job to their staff, as per the job to their staff, as per the job requirement and insist / enforce their work / job to their staff, as per the job requirement and insist / enforce their staff to put on the same while at works.

The Ongoing work is liable to be stopped at any time if the contractors’ staff is found working without P.P.Es. Following is the list of various P.P.Es. To be used for various works / worksites.

01 Industrial Safety Helmet For protection of Head against falling objects or during fall of person from height.

02 Safety Goggles (Grinding, Welding, etc).

For protection of Eyes against flying particles / dust, chemical splash, spark, arc, flashover etc.

03 Face shield (Half or full) For protection of Face against flying particles / dust chemical splash, spark, arc, flashover etc.

04 Ear plug/Ear muffs For ears/ hearing system protection while working in high noise level area.

05 Apron(Rubber/PVC/Asbestos/Leather/Cotton)

For body protection against chemicals, oils, sharp edges, heat, hot objects etc.

06 Gloves(Rubber/PVC/Asbestos/Leather//Electrical shock proof)

For protection of hands against chemicals, sharp edges objects, heat, hot metals. objects, electricity etc.

07 Safety / Leather / Asbestos shoes, Gum Boots etc.

For protection of legs & feet against falling objects, sharp edged objects, heat, hot metals / objects, electricity etc.

08 Safety Belt(fully body) /rope /Life line / fall prevention system etc.

For working at heights or depth, working in vessel or in confined space.

09 Dust Respirator/Scarf Protection of respiratory system against dust. 10 Chemical cartridge Respirators Protection against chemical fume, vapor etc. 11 Canister Gas mask Protection against toxic/poisonous fumes/gases. 12 Air supply respirators Working in oxygen deficient zone. (8) Before using lifting machines / tackles (like C.P. Bs., Hook chooks, winch, forklift, mobile crane,

EOT crane etc.) & its attachments (like D-shackles, slings, U-clamps, Eye bolts or any fixtures), it shall be checked and used only if found safe to use. Also, ensure that these are tested, examined & certified in form no.9 / 10 by competent person as per the Factory act-1948 and its validity is not expired. Further, it shall be fixed properly and firmly prior to lifting the weight.

(9) Scaffoldings to be used for working at height shall be adequate size & capacity. Obtain the work permit when working at height. While climbing on such scaffolding or working on any structure at height, use of full body safety belt & helmet is compulsory. It is also necessary to fasten chinstraps of the helmet.

(10) Contractor or their employee shall not interfere in day-today routine plant activities / works except the work assigned to them, shall not loiter in the areas other than their work jurisdictions, as well as shall not temper / operate / touch the machineries / equipments / auxiliaries with which they are not concerned. Also, the contractor shall strictly instruct their staff not to sit or take rest at / near / below running plants, auxiliaries, systems or any place which is risky, hazardous & prone to accident.

(11) The cylinders containing poisonous / toxic or inflammable / explosive gas like oxygen, acetylene, LPG, hydrogen, ammonia, chlorine, co2 shall be handled safety taking due care. To handle / shift such cylinders a special trolley / cage meant for it must be used but in no case it should be rolled. Domestic LPG cylinder shall not be used / permitted.

(12) On completion of the work, cotton waste, spilled oil / grease, pieces of welding rod & other waste material shall be removed from work site and the area shall be left safe, neat & clean.

(13) In case of any injury / accident while working, it shall immediately be reported to safety department through concerned sectional head / Engineer. The prescribed from no.21 may be obtained from concerned section or safety officer. For any incident occurred but have no injury to any persons should also be informed to safety officer as “Near Miss Incident.”

(14) In all risky jobs, before starting work, contractor should obtain general safety work permit from concerned section well in advance.

(15) In case of noticing smoke or fire during their work execution, they shall make immediate efforts to extinguish / control it and simultaneously inform the fire brigade on phone no.4444 or 4344 or 5555 or 02624-290049, shall shift the casualty to nearby hospital after rendering first aid in case of accident.

(16) Over & above these, contractor shall have to follow all the safety requirements / rules & regulation / norms and legal provisions laid down in various statues, particularly the provisions of The factories act-1948 & The Gujarat states factories rules-1963 (Amended up to date)shall be followed strictly. The contractor shall also obey the rules / regulations / instructions of the local competent authority for safety requirements.

(17) No women or young person shall be allowed to clean, lubricate or adjust any part of a prime mover or of any transmission machinery while the prime mover or any transmission machinery is in motion. Examination or operation of motion machinery shall be made or carried out only by a specially trained adult male worker wearing tight fitting clothing as per section 22 of factories act 1948.

(18) No women or young person shall be employed or permitted to work in lead-compound area like battery room etc. as per schedule VI of GFR 1963.

(19) All the relevant labour and industrial laws shall also be followed compulsorily.

(20) It is felt necessary to deploy safety officers / safety supervisors by contractors / agencies involved in carrying out hazardous activities / operations inside TPS to have better and constant supervision in terms of health safety and environment activities.

(21) In case, it comes to the notice of GSECL UTPS management that the safety guidelines / safety rules / safety norms are not being followed then a minimum sum of ropes one thousand shall be penalized on the contractor / Agency and for subsequent such violation, a server penalty / action as deemed fit shall be imposed, which may please be noted.

(22) The above rules shall be scrupulously followed and where required, they may contact the safety officer for any ambiguity / further guidance in this regard.

(23) For performance evolution of contractor, safety factors of work accident, fire incident & near miss accident will be considered. Steps can be taken to review the job assignment up to cancellation for negligence.
