Medicare Wellness Visit
– A Preliminary ROI review of 1 physician utilizing the PatientLink AMWV provided:
• Orders identified in the report were sent to Community Memorial Health System
Performing Locations for Radiology, Laboratory, and In Office Point of Care
• Pre PatientLink Medicare Wellness Visit Form – 524 orders over 50 clinic days
• Post PatientLink Medicare Wellness Visit Form – 624 orders over 50 clinic days
– An increase of 100 orders performed by Community Memorial Hospital
– Increase in billing for Medicare Wellness visit G-Code was $13,000 during the same
period
• Each of those visits had an increased E/M code (increase of $30K for physician)
• Physician was able to schedule 3 times as many Annual Medicare Wellness visits per day

Where Else Can PatientLink Assist?• CMHS is working with PatientLink to develop a comprehensive data collection form for all
practices enrolled in our ACO, (Accountable Care Alliance Ventura (ACAV).
– Approximately 200 participating physicians
• Over the past 3 years data collection in practices with limited or no EHR has been time
consuming and resource intensive
– Took Several weeks, OT and Weekends, Lots of staff
• CMHS will provide an MSSP Quality Measure Data Collection Sheet to all ACO practices
– The practice will complete the form for every applicable Medicare patient and submit to
the ACO
– Working with PatientLink to export this data into an Excel format for complication, analysis,
and reporting.
• CMHS is also working with PatientLink to develop an online form for Patient Access
Preregistration in our hospitals.
– In an effort to reduce demographic and registration errors
