WELCOME TO LEWIS F. MAYER MIDDLE SCHOOL HANDBOOK Lewis F. Mayer Middle School is a great school where students receive quality academic instruction in a safe, caring and respectful environment. The middle school is a special place because of all the wonderful people that learn and work here and because of the community that actively support it. We desire the best for all of our students and look forward to partnering with parents and community members to help our students be their best in and out of the classroom. With everyone working together, we are sure to achieve our goals and be successful. Students . . . Come to school with a willingness to work hard, expecting to learn, and with a positive attitude. Our teachers will provide the quality and positive instruction that you deserve and that your parents expect. Parents . . . Encourage your children often, talk to them about school and their work, and support them in every way that you can. Parental involvement shows children that school is important and that you are committed to their success. I encourage you to volunteer your time and abilities, visit often, and communicate regularly with your child’s teachers. Communication is extremely important and critical to his or her success. Please use the student handbook as a guide to success. Together, we can accomplish great things! With this in mind, we introduce you to the 2017/2018 Lewis F. Mayer Middle School Student Handbook. Please take the time to read through this very useful and very important tool which will enhance the performance of all students. Responsibilities of a Lewis F. Mayer Student 1. To attend school regularly, to be prompt, and to comply with school rules and regulations. 2. To respect authority of school personnel . 3. To provide his/her name when requested by school personnel. 4. To refrain from behavior which endangers the safety of self or others. 5. To follow all cafeteria rules. 6. To work for the betterment of the school environment . 7. To dress in an appropriate manner and to accept correction for dress judged to be so extreme as to cause undue attention or distraction or which may cause a health or safety hazard. 8. To respect the educational services designed to prepare students to become contributing members of society. 9. To adhere to the Fairview Park City Schools Code of Pupil Conduct. 10. To actively work toward graduation. ATTENDANCEPOLICIESANDPROCEDURES1. The student is expected tobe inattendanceeach schooldayunlesshe/she ispersonally ill or there is aseriousprobleminthehome.Ifthestudentisabsent,he/shemusthaveaparentcalltheattendanceofficetoreporttheabsencebefore8:30am.2. Studentsarrivinglatetoschoolshouldreportdirectlytoattendanceoffice.3. Ifnoparentalcallisreceivedonthedayoftheabsence,thestudentwillbetemporarilylistedasunexcusedabsentandforfeittherighttomakeuptheworkmissed.Iftheparentalcallisnotreceivedduringthenext24hourperiod,theunexcusedabsencemarkbecomespermanent.4. Missingaschooldaywithoutanacceptablereasonmaysubjectthestudenttodisciplinaryaction,possiblyrequiringaparenttohaveaconferencewithanadministrator.Repeatedabsencesmaysubjectthestudenttocourtreferral.5. Students should not miss a class without first checking with the classroom teacher or principal. Ifcompletingworkinanotherclass,etc.preventsthestudentfrommakinghis/herscheduledclass,thenhe/sheisresponsibleforcheckingwiththatclassroomteacherbeforeclassbegins.6. Studentshavinganappointment(medicalorcourtrelated)outsideofthebuildingmusthaveaparentcallthe attendance office before dismissal. Middle school students must be signed out by a parent. Allappointments require a confirmationwith a doctor’s note upon return. Failure to check with attendanceoffice before leaving the school for any reason will result in an UNEXCUSED ABSENCE even if parentalpermissionhasbeengivenanddisciplinewillbeissued.WHENASTUDENTISTOBEABSENTFROMSCHOOL1. Yourparent/guardianmustcallinyourabsencebefore8:30am.2. Thefollowinginformationisneededwhenyourparent/guardiancallsin:

personalerrands,etc.,onSaturdays,schoolholidaysorafterschool.Studentsshouldmakeaconcertedefforttobeinattendanceeveryschoolday.”ABSENCESDEFINITIONOF1/2DAYABSENCEMissingtwo(2)tofive(4)classperiodsisa½dayabsence.Missingfive(5)classperiodsormorewillbedeterminedasafulldayabsent.EXCUSEDABSENCEThefollowingreasonsareapprovedbytheOhioRevisedCodeandtheFairviewParkCitySchoolsasreasonswhichwillbeconsideredexcusedandwhichwillallowstudentstomakeupworkmissed:1. personalillnessofastudent2. illnessinastudent'sfamily3. deathinthefamily4. specialreligiouseventswhicharescheduledonlyduringschoolhours5. emergencyreasonsacceptabletoabuildingadministrator6. vacationdays(five(5)maximumfortheyear)Anystudentwhohasanexcusedabsencemust,uponreturningtoschool,makearrangementswithhis/herteacher(s)tomakeuptheworkmissed.Forexcusedabsences,thestudentwillhave“two(2)calendardaystomakeupworkforeachdayabsent."(BoardofEducationPolicyJED)TRUANCYTheprincipalshallclassifyanabsenceastruancyif,intheirjudgment,thestudentisabsentwithoutjustifiablereasonorwithoutparentalconsent.Studentswhofailtoreporttoclasswillreceiveadditionaldisciplineandwillbeunabletodoassignmentsforthoseperiods.Whenastudentistruant,thetruancychartwillbeapplied.TRUANCYPOLICYTruanciestoclass(es)orschoolandtheirpunishmentsarecumulativethroughouttheschoolyear.Astudentwho isunexcused(truant) fromoneormoreclasseswill receiveazeroforthatclassandnotbeallowedtomake up any assignments or tests that occurred during the unexcused (truant) time. Upon returning to school after an appointment, all studentsmust check inwith the attendance office andreceiveapasstoclass. PROCEDUREFORLEAVINGSCHOOLEARLY(illness,etc.)Ifyoubecomeillduringtheschoolday,theprocedureisasfollows:1.Askyourteacherforapasstogototheclinictoseethenurse.2.Thenursewillcontactyourparent/guardian.3.Adeterminationwillbemadeastowhetheryouwillremaininschoolorbesenthome.4.Studentsmustremaininschoolintheeventnoonecanbecontacted.Nostudentwillbereleasedwithoutparentorguardianpermission.

Note: Students leaving school without first receiving the schools permission will face disciplinaryaction.Statelawgivestheschoolanditspersonneltherighttoactinplaceofyourparent/guardian.Onceastudentstepsonschoolpropertyeachday,he/shebecomestheschool’sresponsibility.Leavingschoolgroundswithouttheschool'spermissionisaviolationofschoolrulesandstatelaw.

MISSINGAREGULARLYSCHEDULEDCLASS

1. Astudentmaynotmissaclasswithoutfirstobtainingpermissionfromhis/herregularclassroomteacher.

2. If the student is completingwork for his/her regular teacher and remains after the bell rings, thuspreventinghim/her fromattendinghis/hernext scheduledclass, thestudent'snext teachermustbenotifiedassoonaspossible.Itisthestudent’sresponsibilitytoseethatthisisdone.

3. Studentsmissingaclasswithoutpriorpermissionfromtheteacherwhoseclassisbeingmissedwillbeconsideredastardyortruant.

PROCEDUREFORVACATIONSDURINGTHESCHOOLYEAR

1. Onlyfamilyvacationscanbeapproved.Thismeansthatthestudentisonvacationwithhis/hermotherand/orfather/guardian.

2. Vacationsareapprovedforuptoten(10)daysperyear.3. Vacationsextendingbeyondtheten(10)dayswillbeconsideredunexcused.

4. Vacation forms can be found on the school’s website( www.fairviewparkschools.org ), Parents tab,Vacationforms,andalso inthemainoffice.Vacationformshouldbeobtainedat leastone(1)weekbefore leaving. The completed vacation form must be turned in to main office with teachers’signaturesandapprovalatleastthree(3)schooldayspriortoyourvacationdeparturedate.

COMPLETINGMAKE-UPASSIGNMENTSStudentsareresponsibletoseeeachindividualteacheraheadoftimeformissedassignments/quizzes/tests.Uponreturningtoschool,thestudentwillbereadytohandinallassignmentsandbereadytotakequizzes/tests

ACHIEVEMENTANDRECOGNITIONAttheendofeachgradingperiod,arecognitionassemblyisheld.Studentswillberecognizedforearningoneofthefollowing:MeritRoll–studentswhoearnaGradePointAverage(GPA)of3.0to3.499HonorRoll–studentswhoearnaGradePointAverage(GPA)of3.5to3.999Principal’sList–studentswhoearnaGradePointAverage(GPA)of4.000(StraightA’swithnoA-’s)StudentoftheMonth-Eachmonth2studentsfromeachgradewillbechosenas“StudentsoftheMonth”selectedbystaffandwillreceiverecognition.COUNSELINGThe guidance department of Lewis F. Mayer Middle School subscribes to the belief that "guidance is foreveryone." Itexists to servestudentsand theirparents. Theguidancedepartment isprepared toassist indealing with academic needs, occupational planning and personal concerns. APPOINTMENTSAppointments to meet with a counselor can be made by visiting the counseling office or via email.Appointmentsshouldbemadeone(1)dayinadvanceifatallpossible.Ifastudentneedstoseeacounselorwithoutanappointmentduetoanimmediateproblem,apassisneededfromtheclassroomteacher.COLLEGECREDITPLUSStudents in grades 7-12may participate in the College Credit Plus Program (CCP) to access both the highschoolandcollegecreditsimultaneously.Seetheschoolcounselorforfurtherdetails.SCHEDULECHANGESWhen the parent, student and counselor have signed the course selection sheet,a commitment has beenmadetopursuethechosencoursetocompletion;therefore,thestudentisunderobligationtocontinuewiththechosencourse.Thefearoffailureisnotavalidrequestforacoursechangeorwithdrawal.Consequently,schedulechangesshouldbelimitedtothefollowing:(a)clericalerrorinscheduling,(b)incaseofprolonged

Requests fordroppinga coursemustbeapprovedbeforetheendofthesecondweekofthecourse,orthestudentwillbecommittedtocompletingthecourse.Afterthestartofthethirdweekoftheclass,astudentwillbeexpectedtocompletethecourseand failure to do so will result in a failing grade on the student's permanent record. It is imperative thatparents/studentsunderstandthedifficultytochangeastudentscheduleandinmostcasescannotbedone.TRANSCRIPTSTranscriptsareavailable intheofficewithawrittenrequest. EXTRA-CURRICULARCODEOFCONDUCTParticipation in extra-curricular student activities is a privilege afforded to the students of Lewis F.Mayermiddleschool. Theseactivitiesaresponsoredtoprovidestudentswithanopportunitytoexcelanddevelopoutsideoftheclassroom.Asamemberofanorganizationorathleticteam,yourepresentandthetraditionofpridethatitsymbolizes.ThefollowingregulationsapplytoallFairviewParkstudentsasmembersofstudentactivities,organizationsorathleticteams.Astudentactivityisanyactivitythatmeets,performsorpracticesattimesotherthan,orinaddition to, the regular schooldayand school yearand that isnotapartof the regular school curriculum.Compliance is required during the school year and also at times when students are engaged in summerstudentactivities.Studentactivityviolationsmayaccumulatefromoneschoolyeartothenextschoolyear.Consequencesaredeterminedbythenumberofviolationsastudentaccumulatesduringthepasttwelve(12)monthperiodoftime.Studentssuspendedfromastudentactivity,organizationorathleticteamshallhavenocontactwiththestudentactivity,organizationorteamoradvisor(s)/coach(es)orthefacilitieswhichsaidorganizationmayuse.STUDENTACTIVITYCODEOFCONDUCT-LewisF.MayerMiddleSchooll.Anypossessionand/oruseoftobaccobyastudentinvolvedinstudentactivitiesshallbeissuedthefollowingconsequences:

II. Anypossessionand/oruseofalcoholand/ordrugsbyastudentinvolvedinstudentactivitiesshallbeissuedthefollowingconsequences:

FirstOffense:Theparentswillbenotified.Fourteen(14)daysuspensionfromanystudentactivities.Thestudentmustattendandshowevidenceofsuccessfullycompletinganapprovedinterventionprogramattheparent's´expenseandthestudentmustwriteanessayfromtheinformationobtainedfromtheinterventionprogramoftheharmfuleffectsofdrugsandalcohol.Failuretocompletetheprogramwillresultinremovalfromallstudentactivityorganizationsandteamsfortheremainderoftheschoolyear,oreighty-eight(88)schooldays,whicheverislonger.SecondOffense:Theparentswillbenotified. Twenty-eight(28)daysuspensionfromanystudentactivities.Thestudentmustattendaprofessionalassessmentprogramattheparent's´expenseandshowevidenceoffollowingtheassessmentrecommendation. Failuretocompletetheprogramwill result inremoval fromallstudentactivityorganizationsandteamsfortheremainderoftheschoolyear,oreighty-eight(88)schooldays,whicheverislonger.

III. AdditionalPenalties:Specialresponsibilityisinherentinall leadershippositions.Studentleaders,bydefinition,guideandinfluencetheconductofotherstudents.Therefore,tomaintainaleadershipposition,astudentmustconductoneselfinamannerwhichpromotesadherencetoorganizationand/orteamrules,aswellastoschoolrules.Theconsequencessetforthaboveinthissectionshallnotapplytoanycasewherethestudent'sconductispartofacourseoftreatmentbyalicensedphysicianorpartofthepracticeofanestablishedreligion.

ATHLETICFEEThefeeforparticipationatMayerMiddleSchoolwillbe$40.00persport.Followingthesecondweekofpractice,norefundswillbemade.Additionally,ifastudentisdismissedfromateam,theirathleticfeewillnotberefunded.STUDENTACTIVITYSUSPENSION/REMOVALAPPEALPROCEDUREIt is the responsibility of the head coach/advisor of an extracurricular activity to enforce extra-curricularactivity rules and regulations. The coaches/advisors must inform students under their jurisdiction of thetrainingrules,regulationsandpenaltiesandacopyofthosearetobesenttotheparents.Acopyoftheactivityrules,regulationsandpenaltieswillbeplacedonfileintheAthleticDirector'sofficeforall athletic activities and in the high school ormiddle school principal’s office for all other extra-curricularactivities.Thecoachoradvisorofanextra-curricularactivitywillsuspendastudentfromthatactivityifthestudenthasviolatedtheBoard'sStudentExtra-CurricularActivityCodeofConduct.Eachcoachshallhavetheauthoritytosuspendornotsuspendastudentwhohasviolatedteamtrainingorstudentactivityrulesandregulations.Priortosuchsuspension,thestudentshallbegivenwrittennoticeoftheintentiontosuspendhimfromtheextracurricular activities, including the reasons. The student shall have an opportunity to appear at aninformal hearing before the coach or advisor to challenge the reason for the intended suspension or tootherwiseexplainhisactions.Uponcompletionofthehearing,thecoachoradvisorshallinformthestudentofthedecision.Atnotimeshallthisdecisionbegivenlaterthanseventy-two(72)hoursfromthetimeofthehearing.Ifthestudentfeelsthatthedecisionbythecoachoradvisorwasincorrectorunfair,heshallbeaffordedtheopportunityforahearingbeforetheassociateprincipalorathleticdirector.Theassociateprincipalorathleticdirectorshallarrangeforahearingtobeconductednolaterthanseventy-two(72)hoursafterthedecisionbythecoachoradvisor.Thestudentisentitledtobeaccompaniedbyhisparentorguardianatthishearing.The

“Bath Salts/Spice/K2”means adrug, compound,mixture, preparation, or substance includedOhioRevisedCodesection2925.02;2925.03;and2925.11"Possess"meanstohavecontroloverthesubstance.Thisincluded,butisnotlimitedto,havingorholdingthesame:1)inone'smouthorhand;2)onone'sperson;3)inoronone'swearingapparel;4)inapurse,handbag,backpack,lunchbox,orsimilaraccessory;5)inone'shalllockerorgymlocker;6)inavehicleonschoolgrounds.Thedriverofavehicleonschoolgroundsmayalsobedeemedtobeinpossessionofanythinginthevehicle."Show Evidence"means to exhibit some or all of the physical and/or behavioral characteristics associatedwith such use or consumption. CONSEQUENCESForviolationsofthisrule,thefollowingpenaltiesshallapply:FirstOffense:Theparentsandlocalpolicewillbenotified.Ten(10)daysofout-of-schoolsuspensionwillbeissued. OUTOFSCHOOLSUSPENSION(OSS)AnOutof School Suspension is reserved for usewhena student's unacceptablebehavior is detrimental tohimself/herselfor isadisruptionof the learningprocess. Astudent’s initialOSSduringtheschoolyearwillallow for make-up of work missed in the classes during the suspension and credit will be granted. Anysubsequent suspensions will result in denial of credit for completed work outside of class during thesuspension.Duringthesuspension,thestudentisnotallowedonFairviewParkBoardofEducationpropertyatanytimenorcanthestudentattendanyschooleventathomeoraway.IN-SCHOOLRESTRICTION(ISR)In-schoolrestriction isadisciplinaryactionusedforoffensesthatdonotwarrantahomesuspension,but isstillconsideredserious.Thein-schoolrestrictionallowsstudentstocompleteworkthatwillcountforagradeinclass.Thein-schoolsuspensionrulesareasfollows:

ASSEMBLIESAssembliesarepresentedperiodicallyforthepurposeofencouragingschoolspirit,presentingspeakers,andrecognizingspecialachievements. Studentsareexpected tobequiet,attentive,andcooperative. Studentswhocauseadisturbancewillberemovedfromtheassemblyandreferredtotheofficefordiscipline. Theirattendanceatfutureassembliesmaybedenied.All students must report to their classes before all regular assemblies unless otherwise notified by theadministration.Seatingintheauditoriumand/orgymwillbebyassignedseatsunlessannounceddifferently.Students are required to attend all assemblies unless prior arrangement is made with an administrator.Cuttinganassemblywillbetreatedasaclasscutforthatperiod.ATTENDANCEAttendancewillbetakenbyeachteacheratthebeginningofeachperiod.BEHAVIORFORSUBSTITUTETEACHERSSubstituteteachersareguests inourschoolsandplayanextremely importantrole inourschoolday. Plasticbottlesofwater/softdrinkscanbepermittedintheclassroomwiththepermissionoftheclassroomteacheraslongastheyhavethescrew-ontypecaps.Notallteachersallowdrinksintheirrooms;therefore,drinksmayneedtobeleftinthestudent’sbookbagorlockerduringcertainclasses.Whenthisdrinkruleisviolated,theteacherwillaskthestudenttoputthedrinkawayorleaveitwiththeteacheruntiltheendoftheperiodatwhichtimethedrinkwillbereturned.
