ATTENDANCEPOLICIESANDPROCEDURES1. Studentsarrivinglatetoschoolshouldreportdirectlytoattendanceoffice.3. Ifnoparentalcallisreceivedonthedayoftheabsence,thestudentwillbetemporarilylistedasunexcusedabsentandforfeittherighttomakeuptheworkmissed.Iftheparentalcallisnotreceivedduringthenext24hourperiod,theunexcusedabsencemarkbecomespermanent.4. Missingaschooldaywithoutanacceptablereasonmaysubjectthestudenttodisciplinaryaction,possiblyrequiringaparenttohaveaconferencewithanadministrator.Repeatedabsencesmaysubjectthestudenttocourtreferral.5. Students should not miss a class without first checking with the classroom teacher or principal. personalerrands,etc.,onSaturdays,schoolholidaysorafterschool.Studentsshouldmakeaconcertedefforttobeinattendanceeveryschoolday.”ABSENCESDEFINITIONOF1/2DAYABSENCEMissingtwo(2)tofive(4)classperiodsisa½dayabsence.Missingfive(5)classperiodsormorewillbedeterminedasafulldayabsent.EXCUSEDABSENCEThefollowingreasonsareapprovedbytheOhioRevisedCodeandtheFairviewParkCitySchoolsasreasonswhichwillbeconsideredexcusedandwhichwillallowstudentstomakeupworkmissed:1. personalillnessofastudent2. illnessinastudent'sfamily3. deathinthefamily4. specialreligiouseventswhicharescheduledonlyduringschoolhours5. emergencyreasonsacceptabletoabuildingadministrator6. vacationdays(five(5)maximumfortheyear)Anystudentwhohasanexcusedabsencemust,uponreturningtoschool,makearrangementswithhis/herteacher(s)tomakeuptheworkmissed.Forexcusedabsences,thestudentwillhave“two(2)calendardaystomakeupworkforeachdayabsent."(BoardofEducationPolicyJED)TRUANCYTheprincipalshallclassifyanabsenceastruancyif,intheirjudgment,thestudentisabsentwithoutjustifiablereasonorwithoutparentalconsent.Studentswhofailtoreporttoclasswillreceiveadditionaldisciplineandwillbeunabletodoassignmentsforthoseperiods.Whenastudentistruant,thetruancychartwillbeapplied.TRUANCYPOLICYTruanciestoclass(es)orschoolandtheirpunishmentsarecumulativethroughouttheschoolyear.Astudentwho isunexcused(truant) fromoneormoreclasseswill receiveazeroforthatclassandnotbeallowedtomake up any assignments or tests that occurred during the unexcused (truant) time. MISSINGAREGULARLYSCHEDULEDCLASS

1. Astudentmaynotmissaclasswithoutfirstobtainingpermissionfromhis/herregularclassroomteacher.

2. If the student is completingwork for his/her regular teacher and remains after the bell rings, thuspreventinghim/her fromattendinghis/hernext scheduledclass, thestudent'snext teachermustbenotifiedassoonaspossible.Itisthestudent’sresponsibilitytoseethatthisisdone.

3. Studentsmissingaclasswithoutpriorpermissionfromtheteacherwhoseclassisbeingmissedwillbeconsideredastardyortruant.

PROCEDUREFORVACATIONSDURINGTHESCHOOLYEAR

1. Onlyfamilyvacationscanbeapproved.Thismeansthatthestudentisonvacationwithhis/hermotherand/orfather/guardian.

2. Vacationsareapprovedforuptoten(10)daysperyear.3. Vacationsextendingbeyondtheten(10)dayswillbeconsideredunexcused.

4. Vacation forms can be found on the school’s website( www.fairviewparkschools.org ), Parents tab,Vacationforms,andalso inthemainoffice.Vacationformshouldbeobtainedat leastone(1)weekbefore leaving. The completed vacation form must be turned in to main office with teachers’signaturesandapprovalatleastthree(3)schooldayspriortoyourvacationdeparturedate.

Mayer Middle School subscribes to the belief that "guidance is foreveryone." Itexists to servestudentsand theirparents. Theguidancedepartment isprepared toassist indealing with academic needs, occupational planning and personal concerns. APPOINTMENTSAppointments to meet with a counselor can be made by visiting the counseling office or via email.Appointmentsshouldbemadeone(1)dayinadvanceifatallpossible.Ifastudentneedstoseeacounselorwithoutanappointmentduetoanimmediateproblem,apassisneededfromtheclassroomteacher.COLLEGECREDITPLUSStudents in grades 7-12may participate in the College Credit Plus Program (CCP) to access both the highschoolandcollegecreditsimultaneously.Seetheschoolcounselorforfurtherdetails.SCHEDULECHANGESWhen the parent, student and counselor have signed the course selection sheet,a commitment has beenmadetopursuethechosencoursetocompletion;therefore,thestudentisunderobligationtocontinuewiththechosencourse.Thefearoffailureisnotavalidrequestforacoursechangeorwithdrawal.Consequently,schedulechangesshouldbelimitedtothefollowing:(a)clericalerrorinscheduling,(b)incaseofprolonged

illnesswhereexcessiveabsencemakesanormal load tooheavy. ATHLETICSATHLETICELIGIBILITY-LewisF.MayerMiddleSchoolAfterastudenthasbecomeamemberofasquad,he/sheshallnottakepartinacontestonanindependentteam,orasanindividualinthesamesportuntilhis/hersquadhascloseditsseasonforthatsport.Penalty-individualloseseligibilityinthatsportfortheensuingseason.Forthefirstnine(9)weeksoftheschoolyear,7thand8thgradersmustbepassingfive(5)creditstowardsgraduationfromthepreviousyear’s4thquartergrades. The incoming9thgradersmusthavepassedfive(5)coursesduringthepreviousnine(9)weeksof8thgrade. TheG.P.A.requiredforallstudentathletes is2.0,whichisnon-cumulative.Note:Thefive(5)creditsofpassingdonotincludePhysicalEducationcourses.Astudentwillbeeligibleorineligibleforthenextgradingperiodbeginningwiththefifthschooldayaftertheendofeachgradingperiod.Eligibilityorineligibilitywillremainuntilthefifthschooldayaftertheendofthenextgradingreportperiod.Attheendofeachgradingperioda7thor8thgradestudentwhopasseslessthanfive(5)ofhis/herclasseswillbeineligibleduringthenextgradingperiod.Allincoming7thgradersareeligibleforthefirstgradingperiodoftheschoolyearregardlessoftheirprioracademicachievement.If,duringthecourseofaseason,astudent’sgradesdropanddonotmeetbasiceligibilitystandards,thestudentmaybewithheldfromparticipationuntiltheirgradeshaveimproved.OHSAA(OhioHighSchoolAthleticAssociation)statesthat6thgradestudentsarenoteligibleatanytimetoparticipateinLewisF.Mayersanctionedathletics.

EXTRA-CURRICULARCODEOFCONDUCTParticipation in extra-curricular student activities is a privilege afforded to the students of Lewis F.Mayermiddleschool. Theseactivitiesaresponsoredtoprovidestudentswithanopportunitytoexcelanddevelopoutsideoftheclassroom.Asamemberofanorganizationorathleticteam,yourepresentandthetraditionofpridethatitsymbolizes.ThefollowingregulationsapplytoallFairviewParkstudentsasmembersofstudentactivities,organizationsorathleticteams.Astudentactivityisanyactivitythatmeets,performsorpracticesattimesotherthan,orinaddition to, the regular schooldayand school yearand that isnotapartof the regular school curriculum.Compliance is required during the school year and also at times when students are engaged in summerstudentactivities.Studentactivityviolationsmayaccumulatefromoneschoolyeartothenextschoolyear.Consequencesaredeterminedbythenumberofviolationsastudentaccumulatesduringthepasttwelve(12)monthperiodoftime.Studentssuspendedfromastudentactivity,organizationorathleticteamshallhavenocontactwiththestudentactivity,organizationorteamoradvisor(s)/coach(es)orthefacilitieswhichsaidorganizationmayuse.STUDENTACTIVITYCODEOFCONDUCT-LewisF.MayerMiddleSchooll.Anypossessionand/oruseoftobaccobyastudentinvolvedinstudentactivitiesshallbeissuedthefollowingconsequences:

FirstOffense:Theparentswillbenotified.Seven(7)daysuspensionfromanystudentactivities.SecondOffense:Theparentswillbenotified.Fourteen(14)daysuspensionfromanystudentactivities.ThirdOffense:Theparentswillbenotified.Removalfromanystudentactivitiesforatwelve(12)monthperiodoftimewithreinstatementpredicateduponreceiptofastatementdetailingsuccessfulcompletionofapprovedsmokingcessationprogramprovidedatparents’expense.

PursuanttotheOhioRevisedCode(lawontobaccouse,possession,etc.,section2151.87),Personsunderage18,whoareinviolationoftheabovelaw,willbereportedtotheFairviewParkPoliceDepartmentandmay,inlieuofacourtappearance,havetopaya$100fine.

II. Anypossessionand/oruseofalcoholand/ordrugsbyastudentinvolvedinstudentactivitiesshallbeissuedthefollowingconsequences:

FirstOffense:Theparentswillbenotified.Fourteen(14)daysuspensionfromanystudentactivities.Thestudentmustattendandshowevidenceofsuccessfullycompletinganapprovedinterventionprogramattheparent's´expenseandthestudentmustwriteanessayfromtheinformationobtainedfromtheinterventionprogramoftheharmfuleffectsofdrugsandalcohol.Failuretocompletetheprogramwillresultinremovalfromallstudentactivityorganizationsandteamsfortheremainderoftheschoolyear,oreighty-eight(88)schooldays,whicheverislonger.SecondOffense:Theparentswillbenotified. Twenty-eight(28)daysuspensionfromanystudentactivities.Thestudentmustattendaprofessionalassessmentprogramattheparent's´expenseandshowevidenceoffollowingtheassessmentrecommendation. Failuretocompletetheprogramwill result inremoval fromallstudentactivityorganizationsandteamsfortheremainderoftheschoolyear,oreighty-eight(88)schooldays,whicheverislonger.

NOTE:Returntoactivityafteratwelve(12)monthsuspensionwillbeunderthefollowingprovision:Return to student activities must be accompanied by a parental/student consent form stating thatreinstatement toactivities is subject to randomtest,at theparent's’expense, for illegal substancesatalaboratorycertifiedbytheStateofOhioforsuchpurposes.

ThirdOffense: The parentswill be notified. Removal from any student activities for a twelve (12)monthperiodoftime.

III. AdditionalPenalties:Specialresponsibilityisinherentinall leadershippositions.Studentleaders,bydefinition,guideandinfluencetheconductofotherstudents.Therefore,tomaintainaleadershipposition,astudentmustconductoneselfinamannerwhichpromotesadherencetoorganizationand/orteamrules,aswellastoschoolrules.Theconsequencessetforthaboveinthissectionshallnotapplytoanycasewherethestudent'sconductispartofacourseoftreatmentbyalicensedphysicianorpartofthepracticeofanestablishedreligion.

ATHLETICFEEThefeeforparticipationatMayerMiddleSchoolwillbe$40.00persport.Followingthesecondweekofpractice,norefundswillbemade.Additionally,ifastudentisdismissedfromateam,theirathleticfeewillnotberefunded.STUDENTACTIVITYSUSPENSION/REMOVALAPPEALPROCEDUREIt is the responsibility of the head coach/advisor of an extracurricular activity to enforce extra-curricularactivity rules and regulations. The coaches/advisors must inform students under their jurisdiction of thetrainingrules,regulationsandpenaltiesandacopyofthosearetobesenttotheparents.Acopyoftheactivityrules,regulationsandpenaltieswillbeplacedonfileintheAthleticDirector'sofficeforall athletic activities and in the high school ormiddle school principal’s office for all other extra-curricularactivities.Thecoachoradvisorofanextra-curricularactivitywillsuspendastudentfromthatactivityifthestudenthasviolatedtheBoard'sStudentExtra-CurricularActivityCodeofConduct.Eachcoachshallhavetheauthoritytosuspendornotsuspendastudentwhohasviolatedteamtrainingorstudentactivityrulesandregulations.Priortosuchsuspension,thestudentshallbegivenwrittennoticeoftheintentiontosuspendhimfromtheextracurricular activities, including the reasons. The student shall have an opportunity to appear at aninformal hearing before the coach or advisor to challenge the reason for the intended suspension or tootherwiseexplainhisactions.Uponcompletionofthehearing,thecoachoradvisorshallinformthestudentofthedecision.Atnotimeshallthisdecisionbegivenlaterthanseventy-two(72)hoursfromthetimeofthehearing.Ifthestudentfeelsthatthedecisionbythecoachoradvisorwasincorrectorunfair,heshallbeaffordedtheopportunityforahearingbeforetheassociateprincipalorathleticdirector.Theassociateprincipalorathleticdirectorshallarrangeforahearingtobeconductednolaterthanseventy-two(72)hoursafterthedecisionbythecoachoradvisor.Thestudentisentitledtobeaccompaniedbyhisparentorguardianatthishearing.The

decisionbytheassociateprincipalorathleticdirectorshallbemadeknowntothestudentand/orhisparentswithinseventy-two(72)hoursofthishearing.Anystudentmayappealadecisionoftheassociateprincipalorathleticdirectortotheprincipalwithinthreedaysofthereceiptofthedecision.Theprincipalshallmeetwiththestudent,hisparentorguardianandthecoachoradvisorandmakeadecisionbasedupontheinformationpresented.Theprincipalshall informthestudentandhisparentorguardianofthedecisionwithinseventy-two(72)hoursofthehearingAnystudentmayappealadecisionoftheprincipaltotheSuperintendentofSchoolswithinthreedaysofthereceiptof theprincipal’sdecision. TheSuperintendentshallmeetwith thestudent,hisparentorguardian,principal,associateprincipalorathleticdirector,andcoachoradvisorandmakeadecisionbasedupon theinformationtherepresented.TheSuperintendentshallinformthestudentandhisparentsorguardiansofthedecisionwithinseventy-two(72)hoursofthehearing.Anystudentmayappeal theSuperintendent’sdecision to theBoardofEducation. TheBoardshallmeet inexecutive sessionwith the student, his parent or guardian, Superintendent and others appropriate for thishearingandmakeadecisionbasedupon the informationpresented. TheBoardpresident shall inform thestudentandhisparentorguardianwithinseventy-two(72)hoursofthehearing.The timeline at all levels is the maximum and all parties are encouraged to expedite the hearings anddecisions. RULESOFSTUDENTBEHAVIORItisthebeliefofthestaffandadministrationofFairviewHighSchoolandLewisF.MayerMiddleSchoolthatour students are young adults. We believe that all students have a right to be treated with respect anddignity.Consequently,studentshavearesponsibilitytoconductthemselvesinamannerwhichcontributesto

anappropriateschoolatmosphere. This impliesanobligation tobeawareof therightsofothersandhaveknowledgeofschool rulesandregulations. Also,studentshavea responsibility to treatallmembersof theeducationcommunitywithrespectanddignityandtorespecttheschool'sconstitutedauthority.

Truancy,missingclasses,usingorpossessingdrugsortobacco,bolddisrespecttostaff,insubordination,usingobscenities,vandalism,fighting,stealing,sexualandothertypesofharassmentanddefacingschoolpropertyareconsideredseriousschooloffensesandwillnotbetolerated.Anyonewhowillfullydamagesordestroysschoolorpersonalpropertywillberequiredtopaythetotalcostofreplacementorrepairandmaybesubjecttoprosecutionincourt.

TheBoardofEducationhasadoptedaGoodNeighborPolicywhichrequiresourconsiderationofthosewholivenear theschool. Studentsareaskedto respect lawns,keepnoisedown,andrefrain fromsmokingandloiteringonadjacentproperties.StudentsarenottoloiterintheseareasincludingtheCountyLibrary.

Duringschoolhours,studentsarenottobeoutsideinthecountylibraryarea,parkinglots,orontheW.213thStreetorCampusDr.sidesofthebuilding.Theseareasareofflimitsexceptwhencomingtoandgoingfromschool.CODEOFCONDUCTUnderOhiolaw,theBoardofEducationhasadoptedthefollowingCodeofConductforStudents:StudentsoftheFairviewParkSchoolsareexpectedtoconductthemselvesinapropermannerinschool,onschoolproperty,atschool-sponsoredevents,andwhiletravelingtoandfromschool.Studentsshallbesubjecttodisciplinaryaction,includingsuspensionorexpulsion,forthefollowingoffenses:

therulescontainedintheCodeofConduct.E.DamagetoPropertyAstudentshallnotcauseorattempttocausedamagetoschoolproperty.F.Disrespect/DisobedienceAstudentshallnotbedisrespectfultowardanyauthorizedstaffperson.G.DisruptionofSchool/DisorderlyConductAstudentshallnotcauseanydisruptionofanyclassroomactivity

ortheoperationoftheschool,andshallnotengageinanyactthatispotentiallyharmfultoothers.H.DistributionorSaleofUnauthorizedMaterialsAstudentshallnotdistributeorsellunauthorizedmaterials

onschoolproperty.I.DressorAppearanceAstudentshallnotdressinanobsceneorsuggestivemannerorinanyfashionthat,in

thejudgmentoftheadministration,isinappropriate.J.FailuretoPayTuitionAstudentshallpromptlypayanynecessarytuitionforschoolattendanceandother

charges.

K. Failure to Serve School Discipline Refusing to serve an ISR, Wednesday school, or any other form ofdiscipline, misbehavior while serving school discipline, failure to report for an assigned detention, andwalkingoutoftheprincipal'sorassistantprincipal'soffice.

L.FalseAlarms/BombThreatsAstudentshallnotgivefalsealarmoffire,bomb,orotherhazard,ormisusetheschool'sfirealarmsysteminanymanner.

M.FalseReports/ForgeryStudentsshallnotmakefalseaccusationsorgive false informationonanyschoolformsorcorrespondenceorothercommunications.

N. Fighting A student shall not engage in any fight or other act of violence or force that causes harm orthreatenstocauseharmtoanotherperson.

O.GamblingAstudentshallnotengageinanygamblingactivities,suchasplayingorgamblingformoneyorotherstakes.

P.HazingAstudentshallnotparticipateinhazingorotherdegradingordisgracefulacts.Q. IllegalPossessionofBuildingKeysandUnauthorizedEntryAstudentshallnotpossess,use, transmitor

concealanybuildingkeyswithoutproperauthorizationorpermission.R. Insubordination/Defiance A student shall not refuse to comply with reasonable requests, orders and

directionsofschoolpersonnel.S. Intimidation/Harassment/Menacing A student shall not intimidate, insult, or in any manner abuse or

harass,verballyorinwriting,anystudentorstaffmember.T. Loitering/Trespassing/Leaving School Property A student shall not loiter or delay in any way that may

causedisruptionofanactivity.Astudentshallnotleaveschoolpropertywithoutofficialpermission.U.MisuseofaComputer/CellPhoneStudentsshallnotuseacomputerorcellphonetoobtainaccesstoor

transmitlewd,obscene,scandalous,orotherunauthorizedinformation.V.MisuseofVehiclesonSchoolPropertyAstudentshallnotviolatetheprescribedrulesandregulationsfor

useofvehiclesonschoolproperty.W. Narcotics, Alcoholic Beverages, Drugs, and Paraphernalia A student shall not use, sell or distribute,

possess,buy,beundertheinfluenceof,orsmellof,alcoholicbeveragesorillegaldrugsonschoolproperty.X. Participation in Extra-curricular Activities A student participating in extra-curricular activities shall not

violatetherulescontainedintheStudentCodeofConductorintheAthleticCodeofConduct.Y.PrescriptionorNonprescriptionDrugsAstudentshallnotsellordistribute,buy,orpossessprescriptionor

nonprescriptiondrugs.Astudentmustfollowtheprocedureforuseofsuchmedications.Z. Profane, Obscene or Vulgar Language/Gestures A student shall not use profane, obscene or vulgar

languageorgesturesatschool,onschoolbusesorwhileengagedinorpresentatanyschoolevent.AA.PublicDisplayofAffectionStudentsshallnotengageinpublicdisplaysofaffectiononschoolproperty.BB.ProhibitedArticlesAnyobjectthat,inthejudgmentoftheadministration,disruptsorinterfereswiththe

educational process or endangers the health, welfare or safety of students or staff is prohibited. Thisincludes, but is not limited to, gambling and gambling related items, cell phones and other handheldcommunication devices, handheld gaming systems, iPods, and laser pointers. An exception to thisprohibition includes students who are active members of a volunteer fire fighting organization or avolunteeremergencymedicalserviceorganization.

CC.PunctualityandTardinessAll studentsareexpectedtobeontimeforschoolandforeachclass,unless

illnessoradelayedschoolbuspreventstimelyattendanceatschool.DD. Repeat Offenses A student shall not repeatedly fail to comply with school rules and regulations or

directionsofteachers,studentteachers,teacheraides,principals,orotherauthorizedschoolpersonnel.EE.SchoolBusesAstudentshallnotviolatetheprescribedrulesandregulationsforstudentconductonschool

buses.FF. Sexual Harassment A student shall not sexually harass another student, staff member, or any other

person.Thismayinclude,butnotlimitedto:graphicorsuggestivecommentsaboutanindividual'sdressorbody,theuseofsexuallydegradingwordstodescribeanindividual,displayingsexuallyaggressiveobjectsorphotographs,andobscenejokes.

GG.Shakedown/StrongArm/ExtortionA student shallnot forceanotherperson togivehim/hermoneyorarticlesofvalue.

HH.TheftAstudentshallnottakeorattempttotakethepropertyofotherswithouttheirconsent.II.ThrowingofObjectsAstudentshallnotthrowanyobjectwithoutauthorization,including,butnotlimited

tosnowballs.JJ.Tobacco/NicotineAstudentshallnotpossess,use,transmit,conceal,smoke,smellof,orotherwisemake

useoftobaccoproducts,anyalternativenicotineproduct, includingelectronicvapor,orothersubstituteformsofcigarettes,cigars,cigarillos,orpipesoranytobaccoornicotinecessationproductsintheschoolbuilding,onschoolbuses,atschoolsponsoredactivities,oronschoolpropertyatanytime.

KK.TruancyAstudentshallnotbeabsentfromschoolwithoutparentalandschoolapproval.LL.VandalismAstudentshallnotattempttoactoractinawaythatresultsinthedestructionordefacement

ofschoolorprivateproperty.MM. Violation of Federal or State Statutes Students shall not violate federal or state statutes, rules or

regulationsonschoolpremisesoratschoolactivities,includingbutnotlimitedtorequiredimmunizations.NN.Weapons and Dangerous Instruments A student shall not bring to school, possess, handle, transmit,

threatentouse,orconcealanyobjectcapableofinjuringhimselforothers.Thisincludesfirearms,knivesand objects made, constructed, or altered so that to a reasonable person the object appears to be afirearm.

OO.Anyotherformofbehaviorwhichisdetrimentaltoaproperschooland/orschoolactivityatmosphereasprescribedbytheadministrationandasoutlinedinthestudent/parenthandbookforthebuildinginwhichthestudentisenrolled.

ALCOHOLANDDRUGSThe Board recognizes that alcohol and drug abuse continues to cause serious problems throughout ourcountry.Theseproblemspersistdespitethemanyyearsofintensiveeffortsatalllevelsofgovernment.TheBoardalsorecognizesthatitsresources,andthepowersandjurisdictionconferreduponitbylaw,arelimitedandinsufficienttosolvetheseproblemscompletely.Butthereisnodoubtthatillegalorinappropriateuseofalcohol or drugs iswrong, harmful and contrary to thepositivedevelopmentof all students. TheBoard isdeterminedtodowhatitcantodeterstudentsfromsuchinvolvementwithalcoholanddrugsandtosupportandencourageallstudentstobeandremainalcoholanddrugfree.CONDUCTPROHIBITED

Nostudentshalluse,consume,possess,conceal,keep,transmit,sellorofferforsaleanyalcoholicbeverage,controlled substance, counterfeit controlled substance, dangerous drug, drug paraphernalia, harmfulintoxicant,illegaldrug,marijuana,narcoticdrug,oropiate:1)inanyFairviewParkpublicschoolbuilding;2)onthegroundsofanyFairviewParkpublicschool;3)adjacenttothegroundsofanyFairviewParkpublicschool;4)inanyareaunderthecontroloftheFairviewParkBoardofEducation;5)onschoolbusesorothervehiclesownedbyorunderthecontroloftheFairviewParkBoardofEducation;6)atanyoff-campuseventsponsoredwhollyorpartlybyanyFairviewParkpublicschoolorbytheFairviewParkBoardofEducation;7)atanyoff-campusFairviewParkpublicschool-relatedevent;8)atanyactivitysupervisedbyanyFairviewParkpublicschoolorbytheFairviewParkBoardofEducation.No student,while at any of the places listed above, shall show evidence of having used or consumed anyalcoholicbeverage,controlledsubstance,dangerousdrug,harmfulintoxicant,illegaldrug,marijuana,narcoticdrug,oropiate,regardlessofwheresuchuseorconsumptionoccurred.Thisruleshallnotbeconstruedtoprohibitpossession,useorconsumptionofasubstancelegallyprescribedforthestudent.Someexamples:allergymedication,antibiotics,asthmamedicationinsulin,ritalin.Thisruleshallnotbeconstruedtoprohibitpossession,useorconsumptionofasubstancelegallyavailabletothe student over the counter, if suchpossession, use or consumption conforms tomaker's instructions forproperuseofsuchsubstance,andtheamountinvolvedisreasonable. Someexamples:allergysuppressant,coughsuppressant,decongestant,feverreducer,painreliever,vitamin/mineral/nutritionsupplement.DEFINITIONS"ControlledSubstance"meansadrug,compound,mixture,preparation,orsubstanceincludedinScheduleI,II,III,IV,orVsetforthintheOhioRevisedCodesection3719.41asamended."Counterfeit Controlled Substance" means non-alcoholic beer, look-alike drugs, or any substance that areasonable person would believe to be a controlled substance because of shape, size, color, markings,labeling,packaging,distribution,orthepriceforwhichitissoldorofferedforsale."Dangerous Drug" has the samemeaning as in the Ohio Revised Code section 4729.02, including but notlimitedtoanydrugthatmaybedispensedonlybyprescriptionunderfederallaworOhiolaw."DrugParaphernalia"hasthesamemeaningasinOhioRevisedCodesection2925.14(A),suchdeterminationtobemadeusingallrelevantfactorsandthosefactorslistedintheOhioRevisedCodesection2925.14(B)."HarmfulIntoxicant"hasthesamemeaningasintheOhioRevisedCodesection2925.01,butshallnotapplytotheproper intendeduseofsuchsubstances,toproperuse inthecourseofeducational instruction,ortoinvoluntaryoraccidentalexposure."Marijuana"meansanypartofaplantof thegenuscannabis, to thesameextentas inOhioRevisedCodesection3719.01(P)."NarcoticDrug"hasthesamemeaningasinOhioRevisedCodesection3719.01(Q),includingbutnotlimitedtococaineinpowderorrockform."Opiate" has the samemeaning as inOhio Revised Code section 3710.01 (U), including but not limited tomorphine,heroin,andsimilaraddictivesubstances.

4.PersonalResponsibility:UsersareresponsiblefortheirbehaviorontheNetworkjustastheyareinaclassroom,schoolhallway,orotherSchoolDistrictproperty.EachUserisresponsibleforreadingandabidingbythisPolicyandanyandallfutureamendments,whichwillbemadereadilyavailableinbothelectronicandprintedform.Anonymoususeisnotpermittedandaccess(includingpasswords)maynotbesharedortransferred.IfaUsersuspectsthatapasswordisnotsecure,heorshemustinformdistrictadministrationimmediately.Anyimproperuseofyouraccount,evenifyouarenottheUser,isyourresponsibility.5.ReportingMisuseoftheNetwork:UsersmustreportanymisuseoftheNetworktodistrictadministration."Misuse"meansanyapparentviolationofthisPolicyorotheruse,whichhastheintentoreffectofharminganotherpersonoranotherperson'sproperty.6.ViolatingPolicywithPersonalEquipment:Theuseofpersonalequipmentand/orpersonalInternetaccesstoviolatethisPolicyortoassistanothertoviolatethePolicyisprohibited.Exceedingpermission(suchasabusingaccesstounfilteredInternetconnectivity)isaviolationofthisPolicy.Usingprivateequipmenttodivertstudenttimeand/orattentionfromschedulededucationalactivities,ortodivertpaidworktimefromitsproperpurpose,isalwaysstrictlyprohibited.PersonalequipmentusedtoviolatethisPolicyonschoolpropertyissubjecttosearchrelatedtotheviolationandseizureforaperiodofuptothirty(30)days.7.DisciplineforViolationofPolicy:ViolationsofeachoftheprovisionsofthisPolicyareconsideredviolationsoftheStudentCodeofConduct(orifanemployee,ofthecontractofemployment),andeachviolationisaseparateinfraction.Violationsmayresultindisciplinaryactionforstudentsuptoandincludingsuspensionorexpulsionand/orreferraltolawenforcement,oruptoterminationandreferraltolawenforcementforemployees.TheDistrictreservestherighttoseekreimbursementofexpensesand/ordamagesarisingfromviolationsofthesepolicies.Disciplinaryactionrelatingtoemployeesisalwayssubjecttotheprovisionsofanyapplicablecollectivebargainingagreement.8.WaiverofPrivacy:ByacceptingNetworkaccess,UserswaiveanyandallrightsofprivacyinconnectionwiththeircommunicationsovertheNetworkorcommunicationsachievedthroughtheuseofDistrictequipmentorsoftware.Electronicmail(e-mail)andotherformsofelectroniccommunication(includinginstantmessagingofallformsandSMSmessagesoriginatingfromemail)arenotguaranteedtobeprivate.TheDistrictownsalldatainthesystem.Systemsmanagershaveaccesstoallmessagesforpurposesofmonitoringsystemfunctions,maintainingsystemefficiency,andenforcingcomputer/networkusepoliciesandregulations,Districtpolicies,andstateandfederallaws.Illegalactivitiesorsuspectedillegalactivitiesmaybereportedtotheauthorities.9.ConfidentialityandStudentInformation:Usersareresponsibleformaintainingsecurityofstudentinformationandotherpersonallyidentifiabledatathattheyaccess,eveniftheyaccesssuchdataaccidentallyorwithoutpermission,andforupholdingFERPA(20U.S.C.§1232g),thestudentconfidentialitylaw(OhioRevisedCodeSection3319.321),theOhioPrivacyAct(Chapter1347oftheOhioRevisedCode),andanyotherapplicableprivacypoliciesandregulations.UsersareresponsiblewhethersuchdataisdownloadedfromtheNetworktotheircomputerscreen,transmittedbye-mail,storedonaflashdrive,portabledeviceorlaptop,copiedbyhandwritingorbyanyorallotherdevices,formsofstorageormethods.Negligencewithrespectto

protectingtheconfidentialityofsuchdatawillbeconsideredaviolationofthisPolicywhetherornotsuchnegligenceresultsinidentitytheftorotherharm.10.District-OwnedEquipment:Desktopcomputers,laptops,portabledevices,andotherequipmentbelongingtotheDistrictareyourresponsibility.Anymisuse,failure,damageorlossinvolvingsuchequipmentmustbereportedtodistrictadministration.Periodicmaintenanceonlaptopsandotherhardwareisrequired.Itisyourresponsibilitytomakesuchequipmenttimelyavailableformaintenanceattherequestofdistrictadministration.Youmaybeheldfinanciallyresponsiblefortheexpenseofanyequipmentrepairorreplacement.11.UnacceptableUsesoftheNetwork:AllUsersmustusetheNetworkinanappropriateandresponsibleway,whethertheirspecificactionsaredescribedinthisPolicyornot.Examplesofunacceptableusesinclude,butarenotlimitedto,thefollowing:OFFENSIVEORHARASSINGACTSCreating,copying,viewing,transmitting,downloading,uploadingorseekingsexuallyexplicit,obscene,orpornographicmaterials.Usinglanguageinappropriatetotheschoolenvironment,includingswearing,vulgaritiesorlanguagethatissuggestive,obscene,profane,abusive,belligerent,harassing,defamatoryorthreatening.Making,distributingorredistributingimages,jokes,storiesorothermaterialthatwouldviolatethisPolicyortheSchoolDistrict'sharassmentordiscriminationpolicies,includingmaterialthatisbaseduponslursorstereotypesrelatingtorace,gender,ethnicity,nationality,religion,sexualorientation,orotherprotectedcharacteristics.Engaginginharassment,stalking,orotherrepetitiveunwantedcommunicationorusingtheInternetinsupportofsuchactivities.VIOLATIONSOFPRIVACYUnauthorizedcopying,modifying,intruding,orattemptstocopy,modifyorintrude,intothefolders,files,data,work,networks,passwordsorcomputersofothers,orinterceptingcommunicationsintendedforothers.Copying,downloading,uploading,ortransmittingstudentorSchoolDistrictconfidentialinformation.CREATINGTECHNICALPROBLEMSKnowinglyperformingactionsthatcausetechnicaldifficultiestothesystem,otherusersortheInternet.AttemptingtobypassschoolInternetfiltersorto"hack"intootheraccountsorrestrictedinformation.Uploading,downloading,creating,ortransmittingacomputervirus,worm,Trojanhorse,orotherharmfulcomponentorcorrupteddata.Attemptingtohack,alter,harm,destroyorinterferewiththenormaloperationofsoftware,hardware,data,otherDistrictNetworkresources,orusingtheDistrictNetworkortodoanyofthesameactsontheInternetoroutsideNetworks.Downloading,saving,and/ortransmittingdatafileslargeenoughtoimpedethenormalfunctioningofthecomputerortheNetwork(suchasmanymusic,video,image,

POLITICALUSECreating,transmittingordownloadinganymaterialsthatsupportoropposethenominationorelectionofacandidateforpublicofficeorthepassageofalevyorabondissue.SolicitingpoliticalcontributionsthroughtheNetworkorconductinganytypeofofficialcampaignbusiness.GENERALMISCONDUCTUsingtheNetworkinamannerinconsistentwiththeexpectationsoftheSchoolsfortheconductofstudentsandemployeesintheschoolenvironment.UsesthatimproperlyassociatetheSchoolDistrictwithUsers'personalactivitiesortoactivitiesthatinjuretheDistrict'sreputation.Usesthatmisleadothersorviolatethestandardsofacademicorpersonalintegrity,includingbutnotlimitedtoplagiarism,disseminatinguntrueinformationaboutindividualsorgroups,orusinganother'spasswordorsomeotheruseridentifier.12.SpecificLimitsonCommunicationOvertheDistrictNetwork:~ExpressingOpinion:TheNetworkhasbeencreatedatpublicexpenseandexistsforpurposesrelatingtoeducationandadministration.Itdoesnotexisttoserveasapersonalblogfortheexpressionofopinionsorasapublicforumofanykind.ItisnottheintentionoftheDistricttoallowthepublic,staff,orstudentstousetheNetwork,includingthewebhostingorlinkingability,forpurposesofexpressionsofprivateopinions,ortosupportprivateorpubliccausesorexternalorganizations.~LargeGroupMailings:ThesendingofmessagestomorepersonsthanisnecessaryforeducationalorschoolbusinesspurposesisamisuseofsystemresourcesandUsertime.Largegroupmailings,suchas"alldistrict"or"allbuilding"arereservedforadministrativeuse,subjecttoanyexceptionswhichmaybedevelopedbytheAdministrationortheSystemAdministrator.Usersmaynotsendemailstomorethanten(10)recipientsinasinglemessage,subjecttoexceptionsdevelopedbytheAdministrationortheSystemAdministrator.TheSystemAdministratormayalsodevelopspecificlimitationsontheuseofgraphics,thesize,number,andtypeofattachments,andtheoverallsizeofemailmessagessentonthesystem.Theuseofmultiplemessages,non-systemaddresses,orothertechniquestocircumventtheselimitationsisstrictlyprohibited.~PersonalE-mail:LimitedpersonaluseofDistricte-mailbyemployeestocommunicatewithfamily,friends,andcolleagueswhoarewillingrecipientsispermittedasapersonalconvenience,butmustnotimpactpaidworktimeandissubjecttoalloftheprovisionsofthisPolicy.Misuseoftheprivilegeisprohibited,andincludesbutisnotlimitedtoexcessivevolume,frequency,inappropriatecontent,mailingtounwillingaddresses,orusesthatmaybringtheDistrictintodisrepute.ViolationswillbedeterminedinthesolediscretionoftheSuperintendent."Limitedpersonaluse"shallbedefinedasnomorethanten(10)messagesduringanyoneday,withnoattachmentslargeenoughimpedethenormalfunctioningofthecomputerortheNetwork,asdeterminedbytheSystemAdministrator.Exceptionstothislimitationmaybepermittedforpersonalemergenciesandotherextenuatingcircumstances.~ElectronicSignatures:Usersshallnotlegallyverifydocumentsoruse"electronicsignatures"inanywayunlesstheyhavebeentrainedinanapprovedverificationorsignaturesystemapprovedbytheAdministration.

OUTOFSCHOOLSUSPENSION(OSS)AnOutof School Suspension is reserved for usewhena student's unacceptablebehavior is detrimental tohimself/herselfor isadisruptionof the learningprocess. Astudent’s initialOSSduringtheschoolyearwillallow for make-up of work missed in the classes during the suspension and credit will be granted. Anysubsequent suspensions will result in denial of credit for completed work outside of class during thesuspension.Duringthesuspension,thestudentisnotallowedonFairviewParkBoardofEducationpropertyatanytimenorcanthestudentattendanyschooleventathomeoraway.IN-SCHOOLRESTRICTION(ISR)In-schoolrestriction isadisciplinaryactionusedforoffensesthatdonotwarrantahomesuspension,but isstillconsideredserious.Thein-schoolrestrictionallowsstudentstocompleteworkthatwillcountforagradeinclass.Thein-schoolsuspensionrulesareasfollows:

Students not reporting for ISR will be considered truant and will facefurtherdisciplinaryaction.3.StudentsabsentondaysofISRaretobe"calledin"bytheirparentbythestartofschoolorthedaymaybeconsideredasadayofOSS.4.Studentsaretohaveallschoolbooksandnecessarymaterialsforcompletingassignments.5.StudentsdismissedfromISRduetopoorbehaviorwillbesenthomefortheremainderoftheday.ThedaywillbedeterminedasOSSandnocreditwillbegivenforschoolwork.Furtherdisciplinaryactionmaybeassessed.6.Students are to sit facing forward, and in an acceptable posture study position (lying down,hunchedover,headresting,etc.,arenotacceptable).7.Studentsaretoremainawakeatalltimes.8.Studentsaretokeeptothemselves.Nocommunicationwithothers.9.Hats,coats,walkmansandotheritemsnotallowedthroughouttheregularschooldayarenottobeintheISRroom.10. Studentsaretobringlunch/beverageswiththemandmayonlyeat/drinkduringthedesignatedlunch time in the ISR room. No cafeteria accesswill be given. Those forgetting their lunchwillreceiveatregularschoolrateacartonofmilkandapeanutbutterandjellysandwich.13. The ISR supervisor/administration may apply any other rules necessary to operate anacceptableISRroom.14. Allotherschoolrulesapply

Note:Inmostcases,ISR'swillbeassignedatleasttwo(2)daysaftertheoffensetoenableteachersenoughtimetoprepareneededmaterialsforstudents.Thestudent’steachermayrequestthatthestudentattendtheteacher’sclassduringISR.Thoserequestswillbereferredtoanadministratorforafinaldecision.

AFTERNOONDETENTIONAfternoondetentionswillbeassignedtostudentsbaseduponstudentconductviolations.Staffthatissuedetentionswillcontactparents/guardianstodiscussoffense.LFMdetentionsareheldinthehighschoolinRoom139from3:05pm-3:25pm(Mondayonly)/3:20pm-3:40pm(Tuesday-Friday).Note:Studentsareallowedthree(3)schooldaysafteradetentionhasbeenissuedinwhichtoservethedetention.The1stand2ndtimeastudentforgetsorfailstoservewithinthegraceperiod,thenumberofdetentionswilldouble.Onhis/her2ndoffense,athree(3)hourdetentionwillbeissued.Ifyoubelieveanextenuatingcircumstanceexists,besuretoseeeithertheprincipalorassociateprincipalbeforethedetentionbecomesoverdue.EXTENDEDDETENTIONExtended Detention is a disciplinary action that is utilized for offenses that do not warrant an In-SchoolRestriction (ISR) or Home Suspension (OSS). Students should report with mandatory books and/orreading/studymaterialsby3:05pmandmayberequiredtoremainuntil6:00pm.Studentswhofailtoreportby3:05pmwillbeconsidered truantand furtherdisciplinewillbe issuedup toand includingOutofSchoolSuspension.IfastudentisremovedfromExtendedDetentionbecauseofmisbehavior,he/shewillbesubjecttoadditionaldiscipline,uptoandincludingOutofSchoolSuspension.

Thestudentandparent(s)aretobeawareofthefollowingguidelinesinrelationtoExtendedDetention:1. An unexcused absence to Extended Detention can result in two (2) days of out-of-schoolsuspension.IfthestudentcontinuestobeunexcusedfromExtendedDetentions,additionaldaysofout-of-schoolsuspensionmaybeapplied.

2.IfthestudentisillorotherfamilyemergenciesconstitutethestudentbeingabsentfromExtendedDetention, a call from the parentmust be received by 2:00 pmon the afternoon of the ExtendedDetention for the absence to be considered excused. 4.Studentsaretobringalltextbooks,paper,pencilsandanyotheritemsneededtocompleteschoolassignments. Appropriate books andmagazines are also permitted. Sitting, drawing,writing ofnotesorreadingofnewspapersexclusivelyarenotpermitted.

5.Studentsarenotpermittedtosleep,playIPods,MP3,BluetoothTechnology,radio/tapeplayers/CDplayers,playcardsoranyothergames. StudentsshouldmakesurethatcellphonesareturnedoffbeforeenteringtheExtendedDetentionroom.

6. Students will not be permitted to go to their lockers; plan accordingly before checking intoExtendedDetention.

7.Behaviorwhichisnotappropriatewillresultinbeingsenthomeearlyandfacingfurtherdisciplinaryaction.

8.Allotherschoolrulesapply.

1. Clothingandhairwillbeneatandclean,notconstituteathreattostudenthealthandsafety,andnot damaging to school property. In reference to whatmay be detrimental to school property,clothes and footwear (withmetal rivets, cleatsor chainswhich candamage furnitureor floorsormaygetcaughtonanotherobject)arenotpermitted.

2. Studentswillwearfootwearatalltimesthatarebothcleanandsafe.

3. Any appearance creating disruption or distraction of the educational process will be consideredimproper.Modesofdresswhichmaybeconsideredimproperare:exposingofthemidriff,halters,severelystyledT-shirts,muscleshirtsandtanktops(sleevelesstopsarepermissible).Anyexposureofundergarmentsisalsoconsideredinappropriate.Sheer,cut-outortatteredgarmentsarenottobe worn. Additionally, clothing is expected to be worn to fit. Clothing too big or too tight isconsideredinappropriate.

4. Studentsarepermitted towear regularpants, full-lengthsweatpants, shortsorskirts. Guideline:whilethestudentisstandingtallandstraightwitharmsathis/herside,shortsandskirtsmustreachbelowPantsaretobewornwiththewaistbandatwaistheightandnotdraggingontheground.Inaddition,boxers,Spandexshortsorpantsareinappropriate.

5. Hats, sweatbands and head-coverings are not allowed to beworn during the school day. (Note:hoodedsweatshirtsare consideredheadcoverings). Jacketsarenot tobewornduring the schoolday. These items aswell as hatsmust be stored in a locker during the school day. During coldweather,sweatshirtsorsweatersmaybeworn.

6. Apparel(includinglook-a-likes)advertisingactivitiesoragenciesconsideredillegaltoyoungpeoplethatarerelatedtodrug,alcoholandtobaccouseorpromiscuityand/orsexual implicationarenotpermissible.Likewise,anygarmentwhichdisplaysanobscenewordorsymbol,racialslurs,ethnicorsexistattitudes,orisgang-relatedisinappropriateandnotpermitted.

7. Onlyneckwearorbraceletsthatareintendedasjewelrymaybeworn.8. Additionaldressadjustmentsmayberequired forhealthandsafetypurposes incertainclassesorcertainsituations.

Violations of the student dress code may incur changing or forfeiting of the inappropriate item, parentalcontactordisciplinaryaction.Forreasonsofspiritorstudentmorale,modificationsmaybemadeonspecialdaysbytheAdministration.ARTICLESPROHIBITEDINSCHOOLProblemsoftenarisebecausestudentsbringarticlestoschoolwhicharehazardoustothesafetyofothersorwhich interfere in some way with school procedure. Such items as weapons, lasers (any type), chains,cigarettelighters,fireworks,toyguns,waterpistols,beanshooters,slingshots,tarotcards,knives,hardballs,canes,Frisbees,andplayingcards,etc.willbeimpoundedandappropriatedisciplinewillbeadministered.Theitemswillnotbereturnedtothestudent/parent. Otherprohibited itemsincludemagazines/booksthataresexuallyorientedordrugoriented,etc. Students inpossessionofthese itemswillbesubjecttodisciplinaryaction.HALLCONDUCTStudentsareexpectedtofollowallschoolruleswheninthehallsbefore,duringandafterschool.Horseplaysuchashitting,grabbingandchasingotherscannotbetolerated. Usingormaking inappropriatecommentssuchasswearing,etc.,willresultinareferraltotheAssociatePrincipal.Studentsarepermittedtohangsigns/postersindesignatedareasaroundtheschoolthatarerelatedtoschoolactivities,birthdaysorothereventsasapprovedbytheadministration.Anysigns/postersotherthanpositive

school-relatedactivitiesorbirthdaysaretobeapprovedbytheadministrationattheirdiscretionfortheoperationofasafeandnon-disruptiveschoolenvironment.HARASSMENTHarassmentmaytakedifferentformsincluding,butnotlimitedto,thefollowing:sexualharassment,gender,ethnic, religious, disability, group,weight. Verbal, non-verbal and physical aspects can all be part of suchmisconduct.Any studentwhobelievesheor shehasbeenharassedshouldcontacta teacher, counselororprincipal.Additionally,anypersonwhobelievesheorshehasbeenharassedmayfileawrittencomplaintattheBoardofEducationoffice.GRIEVANCEPROCEDUREAgrievanceisanallegationbyastudentorbyastudent'sparents,onhisorherbehalf,orbyanyfullorpart-time employee of the school district, that he or she on the basis of sex, race, color, national origin, orhandicap,hasbeenexcludedfrom,deniedthebenefitsof,orbeensubjectedtodiscriminationunderanyoftheprogramsoractivitiesoftheschooldistrict.INFORMALPROCEDUREWhenastudentorparent,onthestudent'sbehalf,feelsheorshehasagrievance,itshallfirstbediscussedwiththepersondirectlyinvolved,generallytheclassroomteacher.Iftheproblemisnotsettledsatisfactorily,itistobediscussedwiththeprincipaloftheschool.Ifitisnotsettledsatisfactorilybytheprincipal,itistobediscussedwiththesuperintendentofschools.Theobjectiveofallpartiesistoresolvethematterquicklyinaninformalmanner.FORMALPROCEDURE

1.Ifagrievanceisnotsettledsatisfactorilythroughtheinformalprocedure,thecomplainantwillfileawrittengrievancewiththepersondirectlyinvolvedwhowillarrangeameetingwiththecomplainantwithinthreeschooldaysandrespondwithadecisionandreasonsinwritingwithinthreeschooldaysafterthemeeting.

2.If thegrievance isnotsettledsatisfactorily, thecomplainantshall fileawrittengrievancewiththeprincipal of the school within three school days. The principal will arrange a meeting with thecomplainantand,ifnecessary,withthepersonorpersonsallegedtohavecausedthegrievanceandrespondwithadecisionandreasonsinwritingwithinsixschooldaysafterthemeeting.

3. If thegrievance isnotsettledsatisfactorily, thecomplainantshall fileawrittengrievancewiththesuperintendentofschoolswithinsixschooldays. Thesuperintendentwillarrangeameetingwiththe complainant andwith anyotherpersonsnecessarywithin six schooldays after receiptof thegrievance and respond with a decision and reasons in writing within six school days after themeeting.

Anyparentobjectingtotheabove-offeredservicesshouldindicatetheobjectioninwritingtotheschoolnurse,incareoftheoffice,nolaterthanOctober1,2012.MEDICATIONADMINISTRATIONATSCHOOLTherearesituationsinwhichstudentsareabletoattendschoolregularlyonlywhiletakingmedicationforadisability, illness or when otherwise prescribed by a physician for the student's well-being. No student ispermittedtocarry,takeoruseontheirown,anymedication(prescription,non-prescription/over-the-countermedicine, includingcoughdrops, throat lozenges, topicalcreamsorointments.) Exceptionswillbemade inspecialcircumstanceswithself-contained,self-administeringdevices,likeintravenousorinfusionssystemsorwhenaphysiciansoindicatesinwritingasdescribedbelow.Ifpossible,allmedicationsshouldbegivenbytheparentsathome, or theparentmay come to school andgivemedication to their children. Whenanystudent must take medication at school, all medications will be administered by school personnel inaccordance with theMedication Administration policy and guidelines. No studentmay give, distribute ormakeavailabletootherstudentsanyprescription,non-prescription/over-the-countermedications.Forthepurposeofthispolicy,thetermmedicationisdefinedasanyprescription,non-prescription/over-the-countermedication.Theremaybestudents,specificallyingrades6-8,whorequiretheuseofaninhalerorothermedicationtobeself-administered during the school day. These studentsmust have a ¨Self-Administration of MedicationForm¨completedandsignedbythedoctorandparentsonfilewiththeschoolnurse.Allformswillbekeptintheschooloffice.Allmedicationadministeredbyschoolemployeesmustbeprescribedbyalicensedphysician,whoalongwiththeparent,mustcompletetheParent/PhysicianAdministrationofMedicationForm.Nomedicationshallbedispensed, administered ormade available to students by school personnel unless in compliancewith thispolicy.Incaseofemergency,anyemployeeisauthorizedtoadministermedication. Forpurposesofthispolicy,anemergency is defined as any life-threatening or other serious situation where the medication must beadministered immediately, or if the medication must be administered at a particular time and no otheremployeeauthorizedtoadministerthemedicationisavailable.

Allschoolpersonnelmustbeinformedthattheadministrationofanymedication(prescriptionornonprescription/over-the-counter)withouttheorderofthephysicianandthepermissionoftheparentorguardiancouldbeinterpretedaspracticingmedicineandisprohibitedbylaw.2. TheFairviewParkBoardofEducationauthorizesbuildingprincipals, incollaborationwith theschoolnurse;to identifywhichBoardhiredemployeeswilladministermedicationduringtheschoolday. Thesedesignatedpersonswillbetrainedbytheschoolnurseanddelegatedtheresponsibilityaccordingtopolicy.Theannualtrainingspecifiesguidelines,proceduresanddocumentationrequiredforadministeringofmedications to students. In compliancewithState Law (ORC.3313,713C), thefollowingrequirementsmustbemetbeforetheemployeemayadministerthemedication:

· ParentmustcompletetherequiredParent/PhysicianAdministrationofMedicationschoolformthatincludesawrittenrequestfromthephysicianformedicationtobeadministeredtothestudent.

· Anyspecialinstructionsforadministeringthemedication,suchasstoragerequirementsorsterileconditions3. Theparent or guardianof the studentmust agree to submit a new revisedParent/PhysicianAdministrationofMedicationformifanyofthe informationrequiredundersubsection2(above) ischanged.

4. Themedicationmust be received in the original container inwhich itwas dispensed by thedoctororpharmacist.5. Parentsmustagreetobringthemedication(andanyrefills)totheschoolofficeandnotsenditwiththestudent.Emptybottlesaretobegiventoparentswhentheybringinthenewmedication.Attheendoftheschoolyear,anymedicationsnotpickedupbytheparentaftertwo(2)weekswillbediscarded.6. Eachbuildingprincipalshallestablishalocationineachschoolbuildingtostorethemedicationsthatare tobeadministered. Themedicationsare tobekept ina secured, locked storage cabinet.Medicationthatrequiresrefrigerationmaybekeptinarefrigeratorinanareanotcommonlyusedbystudents.Exceptionsmaybemadewheninhalersneedtobekeptinaclassroom.7. An individual studentmedication logwill bemaintained andwill include the date, time andinitialsofthepersonadministeringeachdoseofmedication.

The lawprotects thosepersonsauthorized toadministermedication from liability forcivildamagesand foradministeringorfailingtoadministerthemedicationunlesshe/sheactedinamannerthatwouldconstitutegrossnegligenceorwantonorrecklessmisconduct.TheSchoolBoardwillnotrequireanemployeetoadministermedicationtoastudentiftheemployeeobjectstoadministeringthemedicationonthebasisofreligiousconvictions.(Adoption date: March 8, 1993; revision March 11, 1996; revision May, 1999. VISITORSPersonswishingtovisitFairviewHighSchoolorMayerMiddleSchoolmustreporttotheschooloffice foravisitor'spass.Onlythosestudentvisitorsengagedinschoolbusinessmayrequestsuchpasses.Visitorsfromotherschoolswillnotbeadmittedduringtheirvacations,etc. Allvisitorsshouldgiveone (1)day'snoticebeforevisiting. ASSEMBLIESAssembliesarepresentedperiodicallyforthepurposeofencouragingschoolspirit,presentingspeakers,andrecognizingspecialachievements. Studentsareexpected tobequiet,attentive,andcooperative. Studentswhocauseadisturbancewillberemovedfromtheassemblyandreferredtotheofficefordiscipline. Theirattendanceatfutureassembliesmaybedenied.All students must report to their classes before all regular assemblies unless otherwise notified by theadministration.Seatingintheauditoriumand/orgymwillbebyassignedseatsunlessannounceddifferently.Students are required to attend all assemblies unless prior arrangement is made with an administrator.Cuttinganassemblywillbetreatedasaclasscutforthatperiod.ATTENDANCEAttendancewillbetakenbyeachteacheratthebeginningofeachperiod.BEHAVIORFORSUBSTITUTETEACHERSSubstituteteachersareguests inourschoolsandplayanextremely importantrole inourschoolday. Drinkswillbeallowedduringtheschooldayprovidedthecontainercanbesealedwhennot inuse.
