(If NO = 0, if YES = number of events) Qualified SMS events include: HHH, Clean Up, WWMD, FFA, Stakeholder Meeting, Learners to Leaders, and Environmental Project Application Submission). The number of SMS events in your surveys must match the number of “Incident Notifications”. Be sure to provide a short description of the event when reporting via the incident notification system.
