BLUE CROSS BLUE SHIELD AND KAISER

In 2021, associates on UMR will be moved to the Blue Cross Blue Shield (BCBS) network. Kaiser Permanente will continue to be offered in states where available. Even though our BCBS plan is located in Alabama, associates across the country have access to a national network of providers and facilities.

ENHANCED FAMILY SUPPORT BENEFITS

In the spring, we’ll introduce a new partner to help you and your loved ones along your personal journey as you grow your family. From pre-conception, fertility and pregnancy to adoption, return-to-work or parenting, this benefit offers support to you along the way.

HIGH BLOOD PRESSURE MANAGEMENT

We’re introducing a new partner to assist with managing hypertension (high blood pressure). Participating in this program may help you reduce your risk of heart disease and stroke.

VIRTUAL BETTERHELP THERAPY P

Using TalkSpace as part of our calm program? The TalkSpace service has changed to provide you with more enhanced virtual therapy through BetterHelp. If you weren’t using TalkSpace, but are interested in receiving counseling or coaching services at no cost to you, BetterHelp therapy can connect you with a licensed therapist via text, live chat, phone or video.

SUBSTANCE USE CENTER OF EXCELLENCE

If you or someone in your family is struggling with a substance use disorder, we will have a Centers of Excellence (COE) program to provide you with support and resources. A COE program offers access to highly specialized care to treat a particular issue, in this case, substance use disorders. The program will provide a higher level of coverage than before for certain substance use disorder treatments at select national treatment facilities. The COE is here to assist full-time associates and their dependents enrolled in a Lowe’s BCBS medical benefit option.

Smart-ChoiceTM FOR SAVING AND SPENDING ACCOUNTS

We’re introducing a new platform – Smart-Choice – to administer your Healthcare Flexible Spending Account (FSA), your Health Savings Account (HSA) and your Dependent Care FSA (DFSA). Smart-Choice will replace Your Savings AccountTM (YSA) and be your one-stop shop for all your tax-favored savings and spending account needs.

Hyatt Legal Plans is being rebranded as MetLife Legal Plans. This rebrand will continue to provide the same customer service and access to experienced attorneys you’re used to.

Annual Enrollment for 2021 benefits is Oct. 5–23. This is the time to select the offerings that will work best for you in the coming year. Please review the information in this guide and keep it for reference throughout the year. The more you know, the better prepared you’ll be when you or a covered member of your family needs care in the coming year.

What’s New? We have several benefit enhancements that will be offered to

full-time and part-time associates in 2021.

For all part-time associates: Look for a green marker, which denotes benefits that are also available to you!

Reminders• Annual Enrollment begins Monday, Oct. 5 at 12 a.m. CT

and ends at 11:59 p.m. CT on Friday, Oct. 23. Log into MyLowesBenefits.com and select the Annual Enrollment tile to get started.

• Unless you make changes to your benefits during Annual Enrollment, your current coverage as of Dec. 31, 2020 will carry over into the most similar coverage option available on Jan. 1, 2021.

• However, Healthcare Flexible Spending Account (FSA), Health Savings Account (HSA) and Dependent Care Flexible Spending Account (DFSA) contributions, as well as completion of your Activation Reward do not roll over. That means you must take action to re-enroll each year in these benefits or they revert to $0 on Jan. 1, 2021.

• For hourly associates, Lowe’s will match 50% of your contributions to the Dependent Care Flexible Spending Account (DFSA) up to a maximum match of $1,000 per year.

▶ The DFSA can be used to pay for daycare, day camps, and other eligible expenses with pre-tax money, and Lowe’s will match half of your contributions up to the annual $1,000 limit to help! You must make an annual contribution during Annual Enrollment to receive the Lowe’s match.

Coverage for Additional Medical Costs for Full- and Part-Time Associates P

In addition to preventive care medical coverage, Lowe’s offers three Allstate® benefit plans to help cover additional medical costs if they arise.

Critical Illness: Receive a cash payout if a covered person has a serious disease or health condition like a heart attack, stroke or cancer.

Off-the-Job Accident: Receive a cash payout if a covered person is injured in an accident and incurs a health expense.

Fixed Indemnity: Receive a cash payout if a covered person has a hospital stay. Note: If you are expecting a baby in 2021 and delivering in a hospital, this benefit can help offset the cost of the hospital stay for you and your baby.

In addition, if any person covered under these plans has preventive care performed during the year, they can provide proof in a claim to Allstate and receive a $75 check for the Critical Illness plan and a $75 check for the Off-the-Job Accident plan (if enrolled in both). TIP: If you’re enrolling for the first time, wait 30+ days to have the preventive visit or the claim won’t be covered and you won’t receive the $75 check.

• A NEW Employee Stock Purchase Plan (ESPP) launches Dec. 1, with enrollment beginning Nov. 1. Enhancements include a new enrollment platform on the E*TRADE website and updated eligibility. Associates can purchase Lowe’s stock at a 15% discount. Participants in the previous ESPP will need to re-enroll for this new ESPP. Watch for more information in October.

• If you’re enrolled in Long-Term Disability (LTD), your plan includes free, 30-day money coaching services to support your household’s financial health. The LTD plan also includes will preparation services. Visit cigna.mysecureadvantage.com for more information.

Need a leave of absence?Full-time associates: The Lowe’s Disability, Leaves and Accommodations Service Center, managed by Sedgwick, is available to help you with your workplace accommodation and leave of absence needs. They can assist with at-work medical accommodations, religious accommodations, medical and personal leaves of absence, military leaves and short-term disability. You can

reach them via MyLowesBenefits.com by clicking on the tile that says “Need to request a leave of absence including an

FMLA? Sedgwick can help” to begin a claim. You can also call 844-HR-LOWES (844-475-6937) Monday

through Friday, 7 a.m. – 7 p.m. CT.

Part-time associates: Call Sedgwick at 844-HR-LOWES (844-475-6937) to initiate a leave of absence. If you’re enrolled in the part-time Short-Term Disability plan, you will then be advised to contact Allstate to start a disability claim.

Need legal advice? Enroll in the MetLife High (Plus Parents) or MetLife Low legal plan and you’ll receive:

• A customer service team to match you with the right legal help

• Legal advice from attorneys in person or by phone

• In-court representation for certain covered legal matters

• Access to a mobile app and online tools for your convenience

• No copays, deductibles, or claim forms when using a network attorney for a covered matter

Emotional Well-beingBecause emotional health impacts your whole life, including

your work, Lowe’s offers associates a variety of benefits to

help ease your mind when it comes to emotional concerns.

Caring for Children with Special Needs Caring for a child with learning, social, behavioral challenges or developmental disabilities like autism and ADHD can be challenging. Lowe’s offers eligible associates access to Rethink, a program that includes 24/7 virtual care through expert behavioral clinicians, as well as online evidence-based training and support for parents and caregivers of children of all ages.

Lowe’s calm program provides access to care in a variety of ways with enhanced benefit solutions like BetterHelp. You can receive counseling in person, through text messaging, live chat, over the phone or by video conference.

• To make it easier to access licensed counselors, we offer confidential, online services through BetterHelp. You can send a counselor a secure, confidential message (text, video or voice message) whenever you need, and the counselor will typically engage at least two times a day, five days a week. Visit betterhelp.com/magellan to get started.

• You still have access to in-person counseling and will continue to receive five sessions at no cost to you or your household members. For access to in-person counseling sessions, please call 877-543-3875.

• In addition to BetterHelp, calm also provides web-based, confidential care programs for support with day-to-day concerns. With calm, you can receive the support you need to take care of yourself and your loved ones.

To access calm’s online, self-paced programs: Go to MyLowesLife > myHR > My Health & Well-being > View Your Benefits. Click on the Wellness Tab and select the calm tile. The calm program is available 24 hours a day, 7 days a week, 365 days a year.

In addition to new Family Support benefits in 2021, we will continue our partnership with Progyny, a fertility benefit provider, to bring you an inclusive family building benefit and comprehensive coverage for every path to parenthood. Progyny is a fertility benefits Center of Excellence that provides member services, care management, claims processing and a secure member portal to full-time Lowe’s associates enrolled in a BCBS medical plan. Livongo is a health benefit for those living with pre-diabetes, diabetes and high blood pressure. It is offered at no cost to you and your family members who have medical coverage through BCBS.

Health & Wellness Plan Benefits

Ask Accolade*Accolade provides you with a designated Health Assistant who gets to know you and your healthcare needs. This personal resource can help you:

• Navigate your Lowe’s benefits and use them effectively

• Establish a relationship with your in-network primary care provider

1. Visit member.accolade.com and create an account.

2. Download the Accolade mobile app from Google Play or the Apple app store to send secure messages to your Health Assistant. Text ZLFK to 67793 to download. (Message and data rates may apply). Visit accolade.com for privacy policy.

3. Call 888-926-2404 to speak with your Health Assistant. Office hours are Monday through Friday, 8 a.m. – 8 p.m. ET. Nurses are also available after hours.

Preventive CarePreventive care is an important part of managing your health. Even if you feel healthy, you should still see a health care provider for regular checkups and recommended screening tests. These tests and checkups are two ways to prevent chronic diseases or detect them early.

If you work in a distribution center or store support center, you have access to preventive care at your work location. Visit the Premise Health Center at your location to gain insights into your health and receive help with managing chronic conditions such as diabetes or high cholesterol. Premise also has virtual visit options via video or phone. Our providers will also give you information on other Lowe’s programs, most available at no cost to you!
