WHAT IT ISThe Microlight ML830® is a handheld noninvasive, light- emitting medical device. Cleared by the FDA for sale and use in the treatment of a variety of soft and connective tissue disorders as well as certain neurological disorders, this device is a low energy laser also known as a “cold laser.” In contrast to general surgical or cosmetic lasers that produce heat and thermo-destructive laser energy, Low Level Laser Therapy (LLLT) produces no harm to human tissue in normal use and thus is classified by the FDA as a Class IIIB medical device. Cold lasers

used in LLLT have been in medical use for over 30 years and have a long record of successful clinical studies demonstrating medical efficacy and safety. The Microlight ML830® produces infrared (invisible) laser light at the 830 nm wavelength via three LASER diodes. A fourth visible LED (Light-Emitting Diode) is used to provide a visible red target light field for the clinician to target the desired tissue area. As a noninvasive contact medical device, the Microlight ML830® is completely portable, very easy to operate and provides reliable and effective treatment options to the clinician and patient.

WHAT IT DOESThrough the action of photo-stimulation of light reactive biological receptors (chromophores) in the body, Low Level Laser Therapy (LLLT) has demonstrated the ability to significantly accelerate and enhance the body’s natural defense and repair components in the presence of injury, inflammation and certain disease processes. By modifying the effects and limiting the duration of inflammation as well as enhancing specific repair and healing processes, LLLT is consistent in providing pain relief, reducing injury damage and loss of function, in addition to facilitating more rapid repair and stronger healed tissue structures.

HOW IT WORKSAs the light receptive chromophores in various tissues, in both cellular and sub-cellular locations, are irradiated with coherent laser light, energy transfers occur that stimulate more active levels of function and increased interaction. Laser light, penetrating much deeper than other light wave forms, activates increased mitochondrial ATP synthesis, produces increased mast cell inflammatory mitigation and stimulates increased Ca2+ ion presence and beneficial reactive oxygen species (ROS) production.

Through the action of photo bio-stimulation, oxidative metabolism is increased via cytochrome C oxidase and photosensitization of hemoglobin. Laser irradiation stimulates increased endorphin release and increased prostaglandin synthesis. Due to the powerful effect photo- stimulated enzymes exert, both localized and systemic cascade reactions occur that result in immunomodulation and inflammatory mitigation. Why Use The 830nm Wavelength? When evaluating the sometimes confusing and varied choices in low level lasers , the question is frequently asked, “why is the 830nm wavelength better”? The answer to this question is essentially one of penetration and delivered energy. In the treatment of soft and connective tissue injuries and disorders, the goal is to deliver the most effective laser wavelength to the target tissue.

Some 30+ years of scientific study and clinical research has provided us with detailed information on the reaction of biologic organelles to laser light. It has also shown us which wavelengths penetrate most deeply and what energy levels effect biological processes in the most desirable manner. We need to fully understand the relationship between the depth (from the skin surface) of the target tissue and the ability of the therapeutic laser light to reach that tissue.

We now have ample clinical evidence that in contrast to other wavelengths, the 830 nm laser light produces specific beneficial biological reactions that are not

produced by other wavelengths. This unique ability to stimulate greater response is the key to 830nm laser therapy.

Laser therapy has been used by leading scientists and clinicians around the world for over 30 years. In Japan, where laser therapy is considered a preferred medical practice, Dr. Noriko Mochizuki et al published a clinical study on the effect of laser therapy and ATP production.

In that study, the scientists found that tissue levels of ATP were significantly increased when the target area was irradiated with 830nm wavelength laser light. The study further stated that this beneficial increase in

ATP production was not the result of temperature modification, but rather a direct response to the effect of laser light photons.

What is LLLT?

LLLT (Low level laser therapy) is a painless, sterile, non-invasive, drug-free treatment which is used to treat a variety of pain syndromes, injuries, wounds, fractures, neurological conditions and pathologies. Laser therapy can be used

any time a patient requests or needs a drug free procedure for the control of pain, when conventional therapies have been ineffective, or when the acceleration of healing from injuries is desired.

Around the world, laser therapy is rapidly becoming a medical therapy that can heal wounds and fractures up to 60% faster and also reduce the cost

of treatment for many conditions. In the U.K., LLLT has become the treatment of choice for soft tissue “whiplash” injuries and for the treatment of painful post-herpetic neuralgia (shingles pain).

How Does Laser Light Heal? Healing with the use of light is not new. Light therapy was reported to be effective for many conditions by Hippocrates. With the development of the laser and its special properties, using light as a treatment has gained more popularity. This is because we can now use specific wavelengths of light and give accurately measured doses of energy directly to the appropriate treatment site, which was not possible with other light sources.

Low level lasers supply energy to the body in the form of non-thermal photons of light. Light is transmitted through

the skin’s layers (the dermis, epidermis and the subcutaneous tissue or tissue fat under the skin) at all wavelengths in the visible range. However, light waves in the near infrared ranges penetrate the deepest of all light waves in the visible spectrum. When low level laser light waves penetrate deeply into the skin, they optimize the immune responses of our blood. This has both anti-inflammatory and immunostimulate effects. It is a scientific fact that light transmitted to the blood in this way has positive effects throughout the whole body, supplying vital oxygen and energy to every cell.

What to Expect During a Laser Therapy Treatment Session

For most people, laser therapy is quite passive. There are no pulsating shocks felt, as in forms of electronic stimulation, nor heat used as with ultrasounds. The most noticeable sensation is the touch of the probe head of the laser, as it comes in contact with the skin.

Some patients (3-5% of those undergoing light therapy) have reported a slight tingling or tapping in a nerve or along a nerve pathway. Some have noted that they are able to sense a slight feeling of warmth. But for the most part, the treatment, which may last from 2 to 20 minutes, is not noticed at all.

Following (and even during) a laser therapy session, approximately 75-80% of patients being treated can notice an immediate improvement in their condition. This will depend primarily on the type of condition and the length of time the condition has been present.

Generally, the more chronic or severe the condition, the longer it takes to respond. The majority of conditions treated will take anywhere from 4-5 or 10-18 treatments. Once again, the number of treatments depends upon the severity of the condition and its duration. If your condition does not change immediately, it may take 3-4 sessions before a dramatic or marked change is perceived.

Difference between Lasers, LED & SLD light devicesSome manufacturers produce devices with super luminous diodes (SLD) or light emitting diodes (LED). Monochromatic non-coherent light can be helpful for superficial wound healing, however, numerous studies have shown that lasers are far more effective due to their unique properties and penetration depth. Many super luminous & light emitting diode products are usually very low powered (typically 5mW-15mW), and much of this energy is scattered, therefore requiring longer treatment times.

See the chart below to see how typical light penetrates the skin. LED’s will normally penetrate less then 1cm. SLD’s are more powerful and can penetrate about 2.5 cm. But Lasers at 830nm can penetrate 5 cm. Far more effective then SLD’s or LED’s.

FDA Clearances - LED vs LaserThe FDA uses a set of product codes (“procodes”) to classify LED and laser devices for review and clearance. One of those codes is “ILY.” Another is “NHN.” Why does the FDA use these codes, and what are the differences between them?

The FDA explains that it designates a device “ILY” only if the following is true:

For devices requesting clearance for indications under the ILY procode, that is, devices that use thermal energy to provide pain relief through local heating, we [at the FDA] require that these devices demonstrate the ability to generate temperatures of 40-42o C at the treatment site for a duration of 5-10 minutes.

From these criteria and the ILY title “Infrared Lamp” it is clear that ILY devices treat pain with heat, the same way as electric heating pads, hot packs or sunlamps.

When the MicroLight ML830® was cleared, the FDA added a new code, “NHN,” and its title is “Lamp, Non-Heating, For Adjunctive Use In Pain Therapy.”

The new FDA “NHN” code implies that NHN devices treat pain by means other than the mere topical heating of tissue, and studies show this to be true. The new treatment is with true laser diodes (which are “NHN”) as opposed to light emitting diodes or LED’s (which are “ILY”).

On the basis of my clinical observations of patients with (and without) fibromylagia, I have seen that at its core, FM is a disease of abnormal pain processing in the caudate nuclei of the thalamus. This area of the midbrain, through which essentially all pain impulses travel, becomes “hyperirritable” to incoming pain impulses (via a process known as wind-up) over time, after some type of peripheral injury in mostly genetically-susceptible individuals. This injury is almost always the development of muscle trigger points (TPs) in myofascial pain syndrome.

Myofascial pain syndrome is defined as pain at the site of active trigger points, referred pain to predictable areas, and associated muscle dysfunction. A myofascial trigger point is a hyperirritable spot, usually within a taut band of skeletal muscle that is painful on precise compression, and that can give rise to characteristic referred pain, tenderness, and atomic or other epiphenomena. Trigger points can be active, latent, primary, secondary, or satellite.

This “hyperirritability” in FM patients if usually seen as a near total-body alloying (the response to a stimulus as painful, when the stimulus-such as light touch, wind, or

clothing is not perceived as painful in “normal subjects”). This widespread alloying, combined with the pain from active and latent trigger points, and limitations of the range of motion of these affected muscles, can explain most if not all the clinical symptoms of FM.

I personally have seen the transition from myofascial pain to FM a number of times, and I have seen FM diagnosed in cases where patients actually had widespread myofascial syndrome. This latter circumstance must be recognized as it requires different treatments. In a patient with widespread pain that has been chronic, all the palpable trigger points must be found and inactivated in one of a variety of ways, including injection, ischemic pressure, acupuncture, heat, stretching, massage therapy, body work, low level laser therapy, or micro-current (this may require more than one visit).

In the past six years, use of the low level laser therapy (LLLT) in all of my patients with FM (and most with myofascial pain) has supplanted the use of TP in injections in my clinical practice. While this adds some time to the office visit, it is well worth it in the reduction in risk, increase in efficacy, and reduction in cost of the treatment.

The LLLT inactivates the TP pain, allowing for heat and stretching to further the patient’s progress. The muscles have memory, and therefore a major part of the therapy for myofascial pain includes in-office as well as at home application of heat (not ice packs that will reactivate the TPs) and stretching , once the TPs have been inactivated with the LLLT. As the patient’s pain begins to decline,

a reconditioning process needs to begin, tailored to the patient’s specific weaknesses. This also includes attention to the patient’s posture (which is often an impoded one), trying to keep the center of gravity in proper position.

LLLT has become the primary therapy I use to inactivate TPs. I use the Microlight ML830® which has a power rating of 90 milliwatts via three 30 milliwatt fiber bundles, at a wavelength of 830 nanometers (this penetrates the deepest: nearly three inches). As long as one wears the appropriate glasses, it is risk-free and cannot burn the skin or underlying tissues. It is a Class 111b laser (not 111a, which is a laser pointer, nor IV, which can be used for surgery and can burn or set fires!).

The LLLT inactivates trigger point pain, allowing for heat and stretching to further the patient’s progress.

The LLLT also results in an increased production of energy compounds from the cell’s mitochondria, which are then available for improved healing of the cell/tissue. All of these, over time with the mechanical treatment of posture control, muscle stretching and reduction, and reconditioning lead to a significant reduction in pain and improvement in function. Patients with FM see a gradual reduction in their overall FM symptoms (though not without setbacks, which is typical in treatment of most chronic conditions). If necessary, some of the relief from these symptoms can be helped along with a proper stretching and reconditioning home exercise program.

Recently, I have begun to see an increase in FM patients coming to my practice specifically to receive treatment with the low level laser. Any practitioner who is licensed by the their state to treat patients can purchase this device. Numerous boards that supervise massage therapists and chiropractors have begun to include this therapy in the scope of practice of their licenses. Even some of the major insurance companies have begun to pay for the treatments!

A myofascial trigger point is defined as a hyperirritable spot, usually within a taut band of skeletal muscle that is painful on precise compression, and that can give rise to characteristic referred pain, tenderness, and autonomic or other epiphenomena. They can be active, latent, primary, secondary, or satellite. Twitch responses are usual. They are found at areas with high concentrations of endplate. An illustration of a dysfunctional endplate region is shown in (Figure 1) and represents a site at which either a trigger point injection or inactivation with a low-level laser would be effective.

Myofascial Pain Syndrome vs Fibromyalgia

Myofascial pain syndrome is defined as pain at the site of active trigger points and referred pain to predictable areas along with associated muscle dysfunction. It is a regional syndrome that may involve many regions. It is clear to me, based on nearly 20 years experience, that the sympathetic nervous system is involved in the referred pain patterns. In a clinical setting, this can be clearly demonstrated in patients with cervical or thoracic

syringomyelia when there is no sympathetic innervations of one arm.

Fibromyalgia, on the other hand, is a very different condition. While, in my experience, all fibromyalgia patients have myofascial pain with associated trigger points, not all myofascial pain patients have fibromyalgia. The fibromyalgia syndrome involves central sensitization at the caudate nuclei of the thalamus in the mid brain. This central sensitization results in widespread allodynia (the perception of pain in response to a normally non-painful stimulus). These patients also demonstrate numerous neuroendocrine abnormalities, primarily involving the hypothalamo-pituitaryadrenal axis. An injury can develop into a chronic process of either myofascial pain syndrome, fibromyalgia, or CRPS as illustrated in (Figure 2).

In the treatment of chronic pain, we all know that the patient’s symptom of pain is very subjective. However, trigger points and their referred pain are objective, and can be consistently demonstrated by a trained clinician. What is most impor- tant in the treatment of chronic pain is making the proper diagnosis. Without the proper diagnosis, treatments are frequently unsuccessful and may, in fact, lead to a far more complicated problem.

Trigger Point Inactivation

Trigger point inactivation can be achieved a number of ways. These techniques can include injection, ischemic pressure, acupuncture, heat, stretching, massage therapy, body work, low level laser therapy, as well as the use of microcurrent. For the first 15 years of my practice, trigger point injections were the primary modality in the treatment of myofascial pain as described in my guest editorial in the October 2006 issue of Practical Pain Management. In the past six years, I have gradually begun to use low level laser therapy as the primary modality for inactivating trigger points in my practice. It is extremely important not to just simply inactivate the trigger point and do nothing else. It is very important to use heat and gentle stretching immediately after inactivating the trigger points and having the patient follow up with home stretching and application of heat.In terms of discussing trigger point injections, there are number of areas that need to be covered:• Solution makeup • Tools • Treatment sequence •

Quantity and frequency • Post-treatment • Pitfalls and complications

Low Level Laser for Inactivating Trigger Points

In my practice, over the past five years, my primary method of inactivating trigger points has become the infra-red low level laser. It is effective, essentially risk free, and less expensive for the patient. In addition, as the energy compounds (cyclic AMP, primarily) produced in the mitochondria and distributed around the cell are increased, the muscle cells will heal faster thus shortening the overall time until symptoms are relieved. The unit I use is the Microlight ML830®. It has three 30mw laser beams that slightly converge and cover approximately one square centimeter at a depth of 5 centimeters from the skin surface. The unit delivers nine joules/square centimeter over 96 seconds. It is a Class IIIb laser and non-heating to the tissues—unlike a Class IV laser that can burn tissue. Aside from having to wear appropriate glasses to protect the eyes, the unit is otherwise risk-free.

There are multiple advantages to implementing LLLT in oncology practice. Foremost, patients appreciate having nothing to swallow. Because LLLT may also lower the need for pain medication, laser therapy may provide a number of benefits. Obviously LLLT will not cause the constipation that opioids do. Perhaps more importantly, LLLT does not have the sedating action of most pain medications, allowing for increased alert quality time with family, which is especially precious to patients with advanced cancers. LLLT has no interactions with other oncology treatments, so it can be used when other treatments are precluded by interaction concerns. Considering these benefits to oncology patients, we are currently exploring the addition of LLLT here at CTCA.

Properly-administered laser treatment is painless and does not damage the skin. Best results occur with multiple treatments. Insurance reimbursement in the range of $45 per treatment area may be available via category III CPT codes. 88

Most LLLT treatments in the United States last 2-5 minutes per treatment area, with some authors raising the theory that shorter treatments biostimulate while longer treatments suppress. The Arndt-Schulz rule and some clinical evidence suggest a biphasic dose-response curve favoring frequent, brief treatments, though the ideal treatment schedule has not been conclusively established.

When purchasing a laser therapy system, consider the FDA approval category and wavelength. FDA approval categories describe the laser’s documented mechanism of action. NHN approval indicates that a laser has been proven to biostimulate cells via increases in ATP production. Example is the Microlight ML830® laser.

What is a Class IV Laser? FDA - Description of Class IV Laser (Laser Product Hazard): Immediate skin hazard and eye hazard from exposure to either the direct or reflected beam; may also present a FIRE HAZARD

Wikepedia: Class IV/4 lasers CAN BURN SKIN, and in some cases, even scattered light can cause eye and/or skin damage.

Europe (EN & IEC): Class IV is the highest class of laser radiation. Radiation in this class is very dangerous, and viewing of the diffused reflection may be dangerous. Class 4 laser beams are CAPABLE OF SETTING FIRE to materials onto which they are projected.

Are they safe?No, they are very dangerous. You should be very careful if you decide to buy one of these lasers. There is a reason the FDA classified them as Class IV lasers.

Class IIIa & IIIb lasers generate no heat, and are proven (20+ years) of being VERY SAFE. They are commonly referred to as “Cold Lasers”.They can be used for an unlimited amount of time, without generating heat or causing harm to your patients.

Class IV lasers are VERY DIFFERENT. If you use one of these devices for several minutes, you could cause serious harm to your patients. Most Class IV operating guides, warn about prolonged exposure. They consider a few minutes, prolonged exposure.

Buyer BewareClass IV lasers sell between $15,000 – $50,000 and pose a potential risk to your patients, insurance, and your business. So if safety and results are both important to you, buy a non thermal (NHN) FDA cleared laser.

Only Cold Lasers have the safety rating by the FDA as non-significant risk device, meaning that it is extremely safe for human application. Plus Cold Lasers have been used and tested around the world for over 35 years in over 30 countries.

If they were “Cold Lasers”, why the warning? “Cold Lasers” do not produce any heat and can be used for any duration of time.

Patriots’ Super Bowl VictoryThe New England Patriots won Super Bowl XXXVIII with some help from a little known form of laser technology that could change the way athletic injuries and chronic pain are treated.

In the week preceding the Super Bowl, Boston based registered nurse Ellen Spicuzza treated more than 10 Patriot players with cold laser therapy for tendon and muscle injuries.

“A couple of days prior to the Super Bowl weekend, I treated [Patriot wide receiver] David Givens, who had a locked-up hamstring,” she said. She rotated the $4,000, pen-like laser over the “belly” of his hamstring muscle for about five minutes, she said. “The laser released it.”

Spicuzza, an independent nurse/physical therapist in Boston, usually treats Patriot players’ injuries with medical massage. For the big game, she for the first time used low level laser therapy on the athletes’ most troublesome pain spots. “I

don’t think [the improved recoveries were] a coincidence,” Spicuzza said. “It did help. I used it on a flared-up sciatic nerve, and the player had relief soon after treatment.”
