Behandelingen in de hematologie.
Conventionele en nieuwe chemotherapie.
Supportieve behandelingen: transfusie, AB, Epo, G-CSF,
Targeted therapies: monoclonale antistoffen, tyrosine kinase inhibitoren,
Immunotherapie: vaccins, dendritische cellen,
Stamceltransplantaties.
Soorten stamceltransplantaties.
Afhankelijk van bron stamcellen:
Autoloog
Allogeen: familiaal (sibling, syngeen) of niet-verwant (MUD of VUD)
Afhankelijk van type stamcellen:
Beenmerg
Perifere stamcellen (bloed)
Navelstreng
Afhankelijk van HLA-typering: volledig-, partieel (half-) identiek (haplo-identisch).
Afhankelijk van conditionering: myelo-ablatief of niet-myelo-ablatief (reduced intensity, light, mini)
Belang van HLA typering.
Autologe stamceltransplantaties.
Voordelen: Minder risico op transplant- gerelateerde mortaliteit; geen gevaar voor graft versus host ziekte (GVHD), ook op hogere leeftijd toepasbaar. Donor ter beschikking.
Nadelen: Groter gevaar voor relaps ziekte.
Wie ? Vooral bij multipel myeloma, herval NHL of HD, soms eerste lijn bij jonge ptn met aggressief lymfoma; soms bij AL.
Allogene stamceltransplantaties.
Voordelen: Minder risico op herval ziekte (graft versus tumor effect).
Nadelen: Meer risico op transplant-gerelateerde mortaliteit; niet steeds goede donor ter beschikking; zoektocht naar donor is tijds- en arbeidsintensief.
Wie ? CML momenteel zelden; vooral AL; soms NHL/HD/MM indien jonge ptn met slechte prognose.

Veno-occlusieve ziekte, interstitile pneumonie, GvH ziekte, graft failure.

= destructie kleine intrahepatische venules => beschadiging omliggendecentrilobulaire hepatocyten en sinusoden => fibrose => atrofie.

Interstitile Pneumonie (IP)
- Frequentie? Tot 35% van aSCT.- Toegenomen risico bij: - MTX
- GVHD- Leeftijd > 21 jaar- > 6 maand tussen D en Tx- Karnofsky preSCT < 100%- Radiotherapie (> 4 cGy/min)
- Oorzaken: - infectieus: - CMV- Fungus- Pneumocystis Carinii- RSV
- niet-infectieus: -IIP- Diffuse alveolaire bloeding
GRAFT FAILURE
- > 2 x 106 CD 34+/kg
- Toegenomen risico bij: - myelofibrose- splenomegalie- cryopreservatie- sommige medicaties (ganciclovir,
trimethoprim, MTX, )- infecties- GVHD- MUD

GVHD: Inleiding.
= reactie van donor cellen tegen weefsels van de gastheer.
= direct resultaat van een van de hoofdfuncties van het immuun systeem: distinctie tussen self and non-self.
= vooral een T-cell gemedieerde ziekte.
Basis vereisten voor ontwikkeling : Ent moet immunologisch competente cellen bevatten.
Gastheer moet antigenen bezitten die afwezig zijn in de ent.
De gastheer kan gedurende een periode niet reageren tegende ent.
GVHD.
35 to 50 % van aSCT: aGVHD (gr II-IV).
< 100 dagen na aSCT of DLI. Maar kan later.
