Personal Details Form It’s important to ensure that all of these details are correct.

You must complete all fields that contain an asterisk (*) on the Personal Details Form.

This information is needed so that we can add you into our payroll system. We will never give out your personal information without your consent.

Team Member Details

This section must be based on your legal name. Generally, this is the way your name appears on your passport or your birth certificate.

If you have been married, please ensure you use your married name. Further documentation like a marriage certificate must be provided to verify the name change.

Please provide this at your induction if it was not provided at your interview.

What if I have a preferred name?

If you have a preferred name, that can be added to the field labelled Preferred Name.

My passport only shows one name – how do I fill out the form in the correct

way?

If your passport only shows either a given name or a surname, please enter both First Name and Last Name with the same name.

See the example on the following page.

For example:

Harjit Singh has just been offered a role with Coles Services. He prefers to be known as Harry.

Harry must type his legal name Harjit into the box labelled First Name. The name Harry can be placed into the Preferred Name box. His surname Singh will be typed into Last Name.

I have 6 names, how do I know what names to use on the form? Will they all fit?

There is a maximum of 40 characters that you can type into each of the name fields. It is important that you include the names that exactly match your passport or birth certificate.

Background Details

This section is not compulsory however we would love to know a little bit more about each of our team members.

Coles employs a diverse range of team members from different ethnic backgrounds, countries and religions. Help us to understand a little bit more about you.

Primary phone number

This is the contact number for which we can call you on regarding any matters relating to work.

For example:

Nishant has been offered a role with Coles Services. His surname on his passport is blank.

Nishant must type his legal name Nishant into the box labelled First Name. He must then again type Nishant into the box labelled Last Name.

Likewise, if Nishant’s given names on his passport were blank, then Nishant must be entered in as both First Name and Last Name on the form.

Most people list their mobile numbers as their primary number as their mobile is with them at all times. The format of a mobile number needs to be written as XXXX XXX XXX.

For example: 0412 345 678.

If you don’t have a mobile, then your primary number may be your landline number. In this case, ensure that the format is the area code followed by the 8-digit number XX XXXX XXXX.

For example: 03 9876 5432.

What does Silent mean?

A number that is silent means that your number is not listed anywhere (ie. in the phone directory).

Residential Address

From time to time, Coles will send you mail regarding work through the post. This section requires you to complete your permanent residential address and where you wish your mail to be sent.

Please ensure that you don’t abbreviate addresses.

For example: write Abbey Street instead of Abbey St, or Page Court instead of Page Crt.

If your mailing address is the same as your residential address, then simply tick the check box and this will automatically prefill your mailing address.

I live in a unit, how do I enter this?

If you live in a unit, leave a space, not a stroke, between the unit number and house number. For example: you live in unit 2A at 456 John Street. You will need to list your address like this:

Emergency Contact Details

In the event there is an emergency at work, we may need to reach your emergency contact. Coles will only speak with your emergency contact if the matter is urgent and absolutely necessary.

You may provide up to two numbers for this person. Most team members provide a mobile number as the primary phone number.

The format for mobile numbers needs to be written as XXXX XXX XXX. For example: 0412 345 678.

If the emergency contact does not have a mobile, a landline number is also acceptable. In this case, ensure that the format is the area code followed by the 8-digit number XX XXXX XXXX. For example: 03 9876 5432.

Banking Details form It is extremely important to get your banking details right.

Make sure you double check these details are correct – otherwise we won’t be able to pay you.

Account Holder Name

This needs to be the same name in which the bank has listed at their end. If this is slightly wrong, the bank could potentially reject the funds to be deposited.

Bank Name

This is simply the name of your bank, for example NAB, Commonwealth Bank, ANZ or Westpac.

Bank State Branch (BSB) Code

This is a six-digit number separated into two sets of three numbers. For example: XXX XXX. Most BSB numbers start with a 0, for example, 083. You will need to enter the first three digits in the first field, and the last three digits in the second field.

Account Number

This must be nine digits long. If your account number is shorter than this, you must type the appropriate number of zeros in front of the account number to make it nine digits long.

As a precaution, please ensure that you re-enter your account number.

For example:

Jane Citizen has an account number of 56789. As this is only five digits long, Jane will type her account number as 000056789.

Multi Work Site form Each team member is offered a role at one store. However, there are occasionally shifts available at other stores within the same area.

If you wish to be eligible to pick up these additional shifts, you are invited to nominate as few or as many sites other than your base store as you like. It is not mandatory to choose any other sites if you do not wish to.

Simply click on the tick box next to any site you wish to be eligible to pick up additional shifts.
