• Primary physician sees patient/student and develops AP The AP is stored as a PDF document at the Beacon partner site (“M/W/O”) Attending School is collected by M/W/O

• School nurse will select student from list and … View Action Plan View MIIC Webpage (immunizations given and forecasted) Complete INCIDENT REPORT and send to physician

• What if student not found on list … School nurse will need consent to search outside of Portal repository NwHIN CONNECT Outbound Request to Beacon partners If found, added to repository
