DSS – The Process

Ideal documentation will:
Indicate date(s) of testing/examination, and be on official letterhead, typed, dated and signed.
Clearly state the names, titles, professional credentials, of the evaluators.
Include a diagnostic statement identifying the disability, describing the current “nature and extent” of the disability, current symptoms.
List recommendations for accommodations in the classroom.

Define “Accommodations”

Submit your class schedule EVERY QUARTER to request accommodations!
Without a list of classes and instructors, we cannot send out your Letter of Accommodation
Notify DSS of any changes to your schedule, contact information, or any problems receiving your accommodation.
During the first week of the quarter, we advise meeting with your instructors during office hours to discuss your accommodations with them.

Returns: All items borrowed from the DSS Office are due back by the last day of the quarter.
Alternative format textbooks, Furniture, and/or Equipment (if approved): Submit request before the quarter begins. It can take up to 6 weeks to process your request. You must provide proof of purchase for the requested course textbooks.
Exams taken in DSS: Schedule at least 10 school days in advance of your test date. Let your instructor know they will need to send the test over.
