DellpartnerswiththeGovernmentofIndiatodevelopahuman-centeredtechnologysolutiontoovercomethechallengesofprovidingconsistent,preventivecareinruralareas.

Approximatelytwo-thirdsofIndia’s1.3billionpeopleliveinruralareas,accordingtotheWorldHealthOrganization.Imaginethechallengeofdeliveringpreventivehealthcaretoover800millionremotevillagersandtrackingpatientrecordswithouttheuseoftechnology.

India’snetworkof200,000auxiliarynursemidwives(ANM)serveasthefrontlinesofIndia’shealthcaresystem,providingvillagerswitheverythingfrommanagingdiabetestochildbirthassistance.ANMsintheseareasrelyprimarilyonapaper-basedsystemforscreening,referringandtrackingpatients.Thissystemofrecordingpatients’informationispronetoerrors,makingitdifficulttoprovidecontinuityofcare.

Withnoncommunicablediseases(NCDs)likecancer,diabetesandheartdiseaseontheriseinIndia,theGovernmentofIndiaembarkedonamissionforpopulation-basedprevention,screeningandmanagementbyhealthworkerstoincreaseearlydetectionofcommonNCDs.TheNCDprogramisakeycomponentofcomprehensiveprimaryhealthcareunderAyushmanBharat,aflagshipnationalhealthinitiativeannouncedbytheGovernmentofIndiainearly2018.DellworkedwiththeMinistryofHealthandFamilyWelfareasitstechnologypartner,takingontheformidablechallengeofbuildingatechnologysolutionthatwouldhelpANMsanddoctorstoscreenandmanageNCDs,whilealsohelpingthegovernmenttostandardizeandautomatecareandrecordkeeping.Thesolutionalsoneededtohavetherightdashboardstoenablethepolicymakerstousedataanalyticstospothealthtrendsandneedsacrossthecountry.

In2014,Dellbuiltanddeployedamoderndigitalhealthcaresolution,DigitalLifeCare,asapilotinpartnershipwiththenonprofitKarunaTrustintheStateofKarnataka.Later,DellworkedwithstategovernmenthealthdepartmentstocustomizeanddeployDigitalLifeCareacrosstheStateofAndhraPradeshin2016andinsevendistrictsintheStateofTelanganain2017.InpartnershipwithTataTrusts,

Sincelate2017,DellhasbeenworkingwiththeMinistryofHealthandFamilyWelfaretocustomizetheDigitalLifeCaresolutionfordeploymentinhealthfacilitiesinmorethan150districtsacrossallofIndia’s29statesandsevenunionterritories.Toachievethisambitiousgoal,theDellteamisworkingwiththeMinistryandpartnerinstitutionsincludingtheWorldHealthOrganization,AllIndiaInstituteofMedicalSciences,IndianCouncilforMedicalResearchandNationalHealthSystemsResourceCenter.ThecomprehensivefeedbackfromtheMinistryandexpertsishelpingshapethedesignanddevelopmentofasophisticatedapplicationwithreferraloptions,remindersandworkplans;protocol-basedcareattheprimarylevelforassessingrisk,screening,treatmentandfollow-up;andauniquehealthIDforeveryindividual.Thisdata,capturedinanelectronichealthrecord,willenableacontinuumofcareovertimeacrosshealthfacilitiesatalllevels.

ThissolutionwaslaunchedbytheHonorablePrimeMinisterofIndiainApril2018,andtheGovernmenthealthdepartmentsacrossthecountrywilluseittoinitiallyserveatargetpopulationofnearly37millionpeopleovertheageof30.OurdeploymentpartnerisTataTrusts,oneofthelargestfoundationsinIndia.TataTrustswillsupporttheGovernmentinthedeploymentandtrainingforthousandsofANMsonthetechnologysolution.

Screeningshelpimprovehealthcareinvillages,lesseningthedemandsonoverburdenedhospitals.

TheDigitalLifeCarePlatformisarchitectedtosupportmoderndigitalhealthcareapplications.Itincludesmobileandwebappswithreal-timedashboardsbuiltonahighlyscalable,secure,robust,modular,open-APIplatformbasedonamicro-servicesarchitectureensuringseamlessintegration,rapidcontinuousdeploymentandportabilityofdata.

Thecoreteamhasbeensupportedbymorethan70DellvolunteersoverthreeyearstodevelopDigitalLifeCare’ssoftwareplatform.DellGivinghasfundedthisinitiativesince2017,enablingdeploymentintheStateofTelanganaandthecurrentefforttoscaleacrossallstates.

DigitalLifeCareistailoredtotheneedsofANMsanddoctorsinprimary-,secondary-andtertiary-levelGovernmentfacilities.ANMsdonothavemedicaldegreesbutrathera10thgradeeducationfollowedbytwoyearsofANMtraining.ToassistANMswiththeirscreeningsandcarerecommendations,DigitalLifeCare’smobileappfeaturesaseriesofinteractivemodulestoleadworkersthrougheverystepofavisit.Weincludedpromptstoobtainfamilyandmedicalhistory,videosdemonstratingproperscreeningtechniques,andhealtheducationcontenttohelppatientslearnaboutthingslikenutritionorconductingbreastself-exams.Primaryhealthdoctorsaresometimesoverburdened,andDigitalLifeCarehelpsthembyguidingthemthroughthepatientexaminationandofferingrecommendedactionsandalternativesateachstep.TheportalisbasedontheGovernmentofIndia’sdiseaseprotocolsforexamination,referral,diagnosis,treatmentandfollow-up.ThisisparticularlyimportantgiventhatNCDsaresilent,chronicdiseasesthatneedfollow-upforlife.






Theprotocol-basedsystemensuresthatpatientsreceivethesamelevelofcare.ThehealthrecordcreatedbytheANMforeachindividualisstoredonthecloudandaugmentedduringeverydoctorvisit.ThereisacomprehensiveplanningtoolthatallowsANMsanddoctorstotrackfollow-upvisitsofpatients,andmostimportantly,focusonthosewhohavefallenthroughthecracks.

“ThisprojectfulfillsDell’svisiontodeveloptechnologiesthatdrivehumanprogress,anditreallytappedintoourteam’spassionforservice,”saidSunitaNadhamuni,directorofDell’sCenterforTransformationalInnovationinBengaluru,whichspearheadedDigitalLifeCare’sdevelopment.“Ourteammembersworkedhardtounderstandthechallengeshealthcareworkersfaceinthefieldsotheycoulddesigntechnologythathelpsthemsucceed.ThisinitiativewillmakearobustsystemofhealthmanagementandchangethelandscapeofmedicalservicesdeliveryinruralIndia.”

Wecontinuetolearnfromworkers’useofDigitalLifeCareandmakechangesaccordingly.Forexample,datashowedusthatthescreeningswereoftenunintentionallyleftincomplete,soweconsultedwithworkersandaddedsmartremindersandtutorialvideostonudgeuserstowardcompletescreenings.

SaidNadhamuni,“Workingonthisnation-buildingprojectisatremendousinspirationforourvolunteersandteammembers,whoaretotallycommittedtomakingDigitalLifeCareasuccessforthecommunitiesweserve.”
