Evidence-based Amputee Rehabilitation

Robert S. Gailey, Jr., Ph.D., PT, ProfessorUniversity of Miami - Miller School of

Medicine, Dept. Physical Therapy—Coral Gables, FL

DescriptionAs prosthetic systems with microprocessor controls continue to improve, with features such as increased stance con-trol and greater cadence variance, prosthetic gait training enabling amputees to take full advantage of these enhance-ments becomes critical. Advanced gait training methods can

promote a more natural gait pattern with reduced effort. Exercises designed to increase strength in the closed chain environment of the socket can intensify the speed and power of muscular contractions for improved single limb stance control. Functional activities such as sitting, turning, fast walking as well as negotiating ramps and stairs can also be improved with some relatively simple training strategies. This presentation will discuss the biomechanics of gait and functional activities, identify specific training techniques that will enhance performance and illustrate clinically friendly assessment techniques that will enable amputees of any functional capacity to maximize their potential.

BiographyRobert S. Gailey, PhD, PT is a professor at the University of Miami Miller School of Medicine, Department of Physical Therapy and the Director of the Neil Spielholz Functional Outcomes Research & Evaluation Center. He has also been a Special Advisor to the Department of Defense for amputee rehabilitation. His Doctorate in Prosthetics and Orthotics was received from the University of Strathclyde in Glasgow, Scotland. He has authored over 60 peer reviewed publi-cations and book chapters. AFO’s offered by Allard USA are custom fit dynamic carbon composite shells that can be combined with custom SMO’s or Foot Orthoses to provide biomechanically sound gait function while maintaining structurally sound support at the foot and ankle.

The SWASH variable abduction hip orthosis is indicated for patients with poor sitting posture and stability, limited use of upper extremities, and difficulty with ambulation. SWASH is also used for patients with high tone and poor spinal posture which can often leave hips at risk for sublux-ation or even dislocation.

Factor Analysis of Upper Extremity Prosthetic Patient Acceptance

Gerald Stark, MSEM, CPO/L, FAAOP, Senior Upper Limb Specialist and Clinical LeaderOttobock—Signal Mountain, TN

DescriptionAlthough there have been a number of studies that have attempted to investigate prosthetic upper limb prosthetic acceptance, very few have provided conclusive evidence to assess the overall clinical probability of wear. This has impacted approval for these increasingly expensive devices that are coming under increased scrutiny for funding. In a recent survey of upper limb prosthetic practitioners collec-tive estimations were made with respect to the priority of the various factors that contributed to Upper Limb rejection as well as the rejection rates by level. From these factors a Bayesian forecasting model was constructed to provide an initial estimation of prosthetic acceptance.

Th e Impact of Outcomes and Current Research: Preparing forthe Future of Amputee Care and Your Practice

Robert S. Gailey, Jr., Ph.D., PT, Professor,University of Miami - Miller School of

Medicine, Dept. Physical Therapy—Coral Gables, FL

DescriptionThe need to objectively demonstrate outcomes is no longer an option worldwide but rather a requirement imposed by many funding agencies. This presentation will outline, quan-tify and discuss the most current research along with other evidence that supports or refutes the appropriate use of outcome measures within the field of prosthetics and ampu-tee rehabilitation. How these measures can be easily used in the clinic, from simple documentation of current functional status, to monitoring a patient’s performance and quanti-fying change of over time. Moreover, the most powerful use of performance-based outcome measures is treatment

prescription. By using outcomes to identify specific measur-able physical limitation of amputees and then applying the correct exercises and prosthetic care specifically targeted will enhance performance. Evidence will be offered showing how a coordinated effort by the physician, prosthetist and physi-cal therapist can easily allow the amputee to reach their optimal outcome with a prosthesis. In many cases, improved outcomes may mean simply moving from one K-level to the next, in other instances it might be the reduction of knee or back pain or just achieving better balance to eliminate the fear of falling. Each of these constructs are measurable, can be documented and may be shared with the referring physi-cian, patient, or paying agency.

Engineered Perception: Sensation, Cognition and Prosthetics

Paul D. Marasco, Ph.D., Associate Staff ScientistCleveland Clinic, Lerner Research Institute -

Department of Biomedical Engineering—Cleveland, OH

DescriptionThe more we understand interfacing with the nervous

system, the better off we are. We will be able to extend that beyond just the prosthetics realm. Prosthetics for years has been focused on motor control and it is interesting to see more options for sensory feedback as well. Realistically, people have been talking about sensory feedback since the early 1970s, but it has never necessarily moved forward. Now we are starting to see some technologies that are able to send neural signals back.

not only enhance comfort but benefit residual limb health as well. Here, we report an update on research efforts focused to quantitatively assess residual limb health in response to elevated vacuum suspension. Non-invasive probe and imag-ing platforms were used to assess residual limb circulation and skin health in response to use of an elevated vacuum suspension socket system. Key preliminary observations have identified specific conditions under which elevated vacuum suspension system is beneficial to residual limb perfusion and skin health.

Enhancing Upper Limb Prostheses with Wireless Technology

Martin Vilarino, MSE, Engineering Product ManagerInfinite Biomedical Technologies

—Baltimore, MD

DescriptionDexterous myoelectric prostheses are evolving very quickly. Hands offer the ability to access multiple grip patterns, and powered wrists and elbows allow for a greater range of mo-tion than before. However, as the number of motion con-tinues to increase, these devices become more complex and require more elegant control schemes. Wireless technology has the ability to enhance control by equipping prostheses with the capability to communicate with their environments. This can automate the control scheme by allowing the pros-theses to switch modes by simply approaching wireless tags that store prosthesis mode information.

BiographyMartin Vilarino received a B.S. in Bioengineering from the University of Maryland, College Park, MD in 2011, and a M.S.E. in Biomedical Engineering from the Johns Hopkins University, Baltimore, MD in 2014. During his M.S.E., Martin worked as a Research Engineer for Infinite Biomedical Tech-nologies, in Baltimore, MD, where he led the development and commercialization of morph, the first RFID controller for upper limb prosthetic devices. After finishing his M.S.E., Martin joined Infinite Biomedical Technologies as an Engi-neering Project Manager. Currently, he is involved with the marketing and regulation of Infinite’s products, as well as the engineering of the next generation wireless technology.
