InCharge Healthcare 2016 represents Orlando Medical News’ fourth annual compendium of Central Florida’s healthcare leaders who have guided the region through unprecedented industry changes that continue to significantly impact the practice of medicine.

The new InCharge comes on the heels of two important milestones for Orlando Medical News. In 2015, our relaunched website created a 360-degree profile around readers via print and digital content. Soon after, our innovative analytics platform received accolades for the way optimized in-view impressions are measured, and has been heralded by advertising partners for its valuable Return on Engagement (ROE) reports.

Secondly, the 2016 edition of InCharge parallels with Orlando Medical News’ 10th anniversary! Today, vein disease, or venous insufficiency, is still regarded as a cosmetic problem by many, including some doctors. There is a lack of understanding of the consequences. Progression is usually subtle, and skin and tissue changes frequently aren’t appreciated until later stages of the disease.

In the earliest stages, spider and varicose veins are seen. If the person is overweight or obese, he or she may not be aware of the varicose veins because they are hidden by the fatty tissue of the leg. People with a family history of vein problems or swelling may see or feel a bulging vein as a teenager, but usually it becomes evident during or after pregnancy in a woman. Pregnancy not only causes extra fluid in the veins, but the gravid uterus presses on the pelvic veins resisting the outflow of blood from the legs. Men usually don’t develop varicosities until a little later unless they have a family his-tory or stand a great deal in their job.

Venous disease over time causes skin changes. These can start when people are in their 30s or 40s, but often aren’t appreciated un-til the 50s. Initially there may be some thickening of the skin in the lower leg when compared with skin higher on the calf. Skin may be dry and scaly and may be confused with aging skin, especially in people who have had a lot of sun exposure. There may be trace swelling- best noted by pressing the thumb over the ankle bone at the end of the day. If this leaves an impression, swelling is present. Small patterns of spider veins may appear around the ankle, but not necessarily. These changes are more dramatic in people who are overweight. Color changes may also begin to appear, especially in the overweight or in those who stand for long periods. Initially, this might be a light pinkish discoloration along the inner aspect

of the lower leg and ankle. Usually this appears after long periods of time on the feet and can be warm and tender. This picture is often confused with cellulitis, an infection, and is treated by many primary care and ER physicians with antibiotics. The condition is stasis dermatitis, an inflammation, not infection. The treatment is elevation and, if possible, compression if the patient can tolerate it. Cold soaks or compresses can help. Sometimes a Doppler exam is ordered to rule out a deep vein thrombosis. The condition is caused by malfunctioning valves in the veins, not clots, so the test invari-ably comes back negative and the physician frequently says “there is nothing else to do.” This patient needs to see a vein specialist.

As venous disease progresses, the skin of the lower legs turns a tannish color and the tissue becomes firmer. Later this can progress to a dark brown, and underlying tissue becomes firm. This is espe-cially true and progresses more rapidly in the overweight patient. We see many patients with these changes who have been told by several physicians that there is nothing to be done or have been referred to a dermatologist. This is lipodermatosclerosis and is a pre-ulcerative condition that needs to be dealt with using compres-sion stockings or garments and treatment of the underlying venous insufficiency.

An ulcer in the ankle region is the ultimate result of untreated ve-nous insufficiency. These ulcers are painful and are avoidable unless the patient has traumatized the tissue by dropping something on or hitting the lower leg on something. When an ulcer occurs it will take a prolonged period to heal, but treating the underlying vein problem can speed the healing process and importantly decrease the chances of another ulcer occurring.

This is lipodermatosclerosis and is a pre-ulcerative condition that needs to be dealt with using compres-sion stockings or garments and treatment of the underlying venous insufficiency.

An ulcer in the ankle region is the ultimate result of untreated ve-nous insufficiency. These ulcers are painful and are avoidable unless the patient has traumatized the tissue by dropping something on or hitting the lower leg on something. When an ulcer occurs it will take a prolonged period to heal, but treating the underlying vein problem can speed the healing process and importantly decrease the chances of another ulcer occurring.

Seminole Hospital in Longwood, where, under his leadership, the hospital became the organization’s first to install and activate digital quality boards in public areas to provide medical outcomes data directly to patients and families. The hospital was also the first Orlando Health facility to utilize the Xenex germ-fighting robot to keep healthcare associated infection rates low and patients safe. Both initiatives have since been introduced at all Orlando Health hospitals. Dr. Kelley graduated from the Medical College of Georgia and completed his internship and residency at St. Vincent’s Medical Center in Jacksonville. Dr. Zehngebot has served as the Director of Clinical Research, Florida Hospital Cancer Institute in Orlando since 2007 and, in 2008, he helped Florida Hospital earn a Clinical Trials Participation National Award from the American Society of Clinical Oncology (ASCO).

Linda Zinkovich, Chief Operating Offi cer, joined Physician Associates in 1994 as a physician recruiter and has moved her way up through the organization, holding various clinical management and supervisory positions.  She is responsible for all new business development, ongoing analysis of physician practices (comparison of individual performances to national benchmarks, comparison of individual performance against group expectations and averages, patient fl ow effi ciency and fi nancial performance), evalu-ation/management of all facilities/locations, new physician expansion, and trouble shooting and problem resolution with physician practices.  She has 15 years of experience in outpatient clinical research, has worked for fi ve years as a staff nurse, and is a member of Medical Group Management Association (MGMA).
