1.1 IntroductionMumbai Railway Vikas Corporation (MRVC), MMRDA & Doctors for You (DFY)have been working in the M-east ward of Mumbai for the past three years for improve-ment in the socioeconomic and health indicators of the PAP’s resettlement colonies.The interventions are being carried out through two health centres in the area. TheHealth centre in Natwar Parikh Area is functional since 2010 and in Lallubhai com-pound since 2013. The services are provided on daily basis by the team of doctors,paramedical staffs and outreach workers. The services are provided at the health cen-tres on daily basis like general OPD, dental OPD, Tuberculosis treatment, Immuniza-tion, Growth monitoring OPD, family planning and maternal & child health services.

In the health centre at Lallubhai compound specialized services i.e. physiotherapyand disability rehabilitation, Gynaecology clinic, Eye clinic and Skin disease treatmentetc. are also provided. The OPD services are backed up by laboratory services andphar-macy. With the opening of health centre, quality health services are easily acces-sible and affordable to residents of these PAP’s resettlement colonies and nearby slumareas.DFY has now been providing health care services to more than 1.5 lakh popula-tions.

Apart from services at the health centre, health services and awareness activitiesare also organized in the area through community meetings, rally, street plays etc. Themain aim of the project is to provide preventive and curative healthcare services to all.

1.2 Highlights of the Month

1.2.1 Global Hand Washing Program

On 15th October 2016, ‘Global Hand Washing Day’ was celebrated and a school aware-ness program was organized jointly by MRVC-DFY team & Sealed Air Pvt Ltd’s CSRteam. Awareness session was conducted with children of Shivam English MediumSchool, Mankhurd. Basic steps of hand washing were demonstrated to the children.Along with the awareness session, a total of 1012 recycled soaps along with soap con-tainers were distributed among the children as a part of ‘Soap for Hope’ Project. Themain aim of this program was to inculcate the habit of hand washing in the children.

1.2.2 Improved Pathology lab Services at MRVC DFYHealth centres

MRVC-DFY health centre, Lallubhai Compound, Mankhurd houses a Pathology Lab-oratory which provides laboratory services to patients at highly subsidized cost. Thepatients from MRVC Natwar Parikh Centre, Govandi are also provided this facility byarranging for sample collection at the centre and its timely transportation. Recentlywith the joining of Dr. Kamlesh R. Vishwakarma, MBBS, MD (Microbiology), thelaboratory services are being upgraded to higher level. Many new pathological test ser-vices namely ASO, CRP, RPR/ VDRL, Viral IgM, Anti HCV, etc have been introduced.Many new pathological tests are planned in near future KOH for skin scraping, Gramstaining, ZN staining, etc. Appropriate training is being provided to the lab Attendantin this regard. This has now added a new dimension to the MRVC-DFY Health centre.

1.2.3 Swaccha Bharat AbhiyanAs a part of ongoing Swaccha Bharat Abhiyan, cleanliness activities were organized on10th Oct 2016 at both the Natwar Parikh and Lallubhai center. The Staff participated inthe cleanliness drive at the center.

1.2.4 Launch of Outreach Mobile Immunization ser-vices at Bainganwadi area in Govandi

MRVC-DFY had been conducting outreach immunization camps at Mandala slums,Mankhurd. As a part of scaling up of outreach immunization services, new outreachspots have been identified in Bainganwadi area of Govandi in consultation with MCGM.Awareness sessions are conducted in the areas and beneficiaries are provided free Im-munization services along with Vitamin A supplementation and Deworming services.A total of 18 camps had been conducted in the month of October as a part of thisimmunization campaign. A total of 145 children were immunized, while 62 were ad-ministered Vitamin A and 65 children were given Deworming treatment. Free soapsalso were distributed to the beneficiaries for the promotion of hand hygiene under theSoap for Hope project.

1.3.8 Statistical data of Under Five ClinicThis clinic was started for the children of 0-5 year age group. This group is the mostvulnerable age group for malnutrition and subsequent mortality due to under nutritioncomplicated by infections. The aim of Under-five Clinic is to promote health of motherand children of this age group.Services:

• Measurement of Height and weight

• Treatment of common conditions of mother and child

• Assessment of immunization status

• Provide nutritional education to mother

• To provide family planning counselling and services

In this clinic both mother and child are provided comprehensive health care services atfree of cost. Since this is health promotion clinic, mother is asked to follow up everymonth with child for growth monitoring.

• Activity and Topics are covered:

1. Home environment: Importance of environment such as space, ventilation, over-crowding in the prevention of spread of TB.

2. Medicines: DOTS provider explains importance of adherence to treatment andcompleting the complete course of AKT. They also emphasize the problems as-sociated with irregular treatment and defaulter.

3. Awareness among family members: The family members are educated aboutmode of TB transmission, precaution to be taken to prevent spread of disease andimportance of timely intake of medicines and development of drug resistance dueto incomplete and inadequate treatment. Family members were also advised tovisit health centre in case of symptoms such as Cough (> 2 Weeks), Fever anddecreased appetite.

1.4.1 Physiotherapy DepartmentCase Study:A woman (Usha Abbihmannu Khandeyrao) from lallubhai Compound hadaroad traffic accident two months ago. She is 37 years old, has two kids a boy age of19 and girl age 17. As a result of the accident, she had severe pain in her back, severedifficulty and pain standing up on her right leg, unable to stand up straight and her daily

activities were restricted.As per Orthopaedic Doctor’s advice, she was told complete bed rest for two monthsand laterphysiotherapy sessions. Private physiotherapy centre in nearby areas provideservices at high cost, making it completely non affordable for Usha. She is now underthe treatment of MRVC-DFY Health Centre under Physiotherapist Dr Shivlakshmi atvery nominal charges. She has undergone 10 sessions with the centre and now she isable to walk straight and happy to do house hold works. Now she is keen to continueher treatmentand she is full of praise and thankful to MRVC DFY Health centre forproviding quality physiotherapy services in the community at such a nominal cost.

1.4.2 General OPDA 4 years female child came with the complaints of not able to walk properly to theMRVC DFY Health centre, Lallubhai Compound, in the month of June 2016. She wasdiagnosed with mineral deficiency problems and malnutrition. After getting treatmentfrom MRVC DFY OPD within 2 months she started to walk and after 4 months sheeven can run now.
