Dear Friends, Year 2013 brought the beginning of many changes in health care. People are beginning to understand the impact of these changes and health care systems are responding to the community health needs. According to the National Association of Free and Charitable Clinics “Even after full implementation of the Affordable Care Act, there will be as many as 26 million people who are still without access to health insurance. “ The need to work together is more vital than ever to assure all people receive health care.

St. Joseph’s Medical Clinic Inc. is an independent non-profit agency that has served uninsured and low income residents of Waukesha County since 1977. We are able to provide these free or low fee services because of the support of a network of volunteers and in kind donations of regional health care systems.

New and continued donors give generously to support the residents in Waukesha County. In this report we want to share the impact of these contributions and donations. Your continued support sends the message that you value the health and quality of life for all individuals.

Many people do not realize that there are approximately 1,200 free and charitable clinics throughout the nation

Medical:• A total of 1,463 on site services were provided

including medications, clinic appointments and related support

• 711 clinic appointments were complete serving 435 unduplicated patients

• 253 medical referrals were made to network providers

• 742 lab tests were completed on clinic site.• 959 lab and diagnostic tests were completed

off site by partners and network providers• Medical case management was provided to 27

homeless people in Waukesha County• Through the Diabetes Waukesha Wellness

Working Together (DW3T) 10 diabetic patients received “wrap around” services.

• 10 classes were provided to the public and the community with a total of 25 participants

• 898 phone or walk in referrals were made to variety of resources

• 2 trainings sessions about mental illness/medications were provided to 83 law enforcement professionals

Community Partnership:• Post-secondary school internships and clinical

placements were provided to a total of 60 students.

• 58 individuals from 5 agencies or institutions donated services to improve the clinic facility

• ProHealth Community Outreach Nurses provided flu vaccine clinics and assistance with prescriptions

• In collaboration with partners, 4 community outreach events and screenings were provided

▪ 64% of patients report a household income less than $15,000. *16% of patients report they have no annual income.• 93% of patients are between 18 - 64 years of age. ▪ 49% are between 35 - 54 years of age.• 52% are female, 48% are male.• 80% of patients served by the clinic live within 10

miles of the clinic.
