Klinische Pharmakologie
Fortunately a surgeon who uses the wrong side of the scalpel cuts hisown fingers and not the patient; if the same applied to drugs they would have been investigated very carefully a long time ago.
(Rudolph Buchheim, 1849)

Poisons in small doses are the best medicines; and useful medicines in too large doses are poisons.
W. Withering, 1789.
If in doubt whether or not to give a drug to a person who will soon get better without it, don‘t.
Laurence and Bennett, 1992

A drug is any substance or product that is used or intended to be used to modify or explore physiological systems or pathological states for the benefit of the recipient
(WHO, 1966)

1. Inakzeptabel
2. Akzeptabel
3. vernachlässigbar
War is much to serious a thing to be left to the military
(Talleyrand 1754-1838)
Drugs are much to serious a thing to be left to the medical profession and to the pharmaceutical industry
![Page 13: Klinische Pharmakologie - Med Uni Graz · z. whether a drug is of value. z. how it may best be used. z. formal therapeutic trials. z. surveillance studies for both efficacy and adverse](https://reader036.vdocuments.us/reader036/viewer/2022070811/5f09ed027e708231d4292a92/html5/thumbnails/13.jpg)

Principal approaches to drug discovery
1. Synthesis of analogues, agonists and antagonists
2. Modification of the structures of known drugs
3. Random screening
4. Discovery of new uses for drugs already in general use
5. Combinatorial synthesis / high throughput screening (HTS)

Aufgabenbereich der Klinischen Pharmakologie
1. Pharmakologie• Pharmakodynamik: wie ein Arzneimittel auf den
Körper wirkt• Pharmakokinetik: Absorption, Distribution,
Metabolismus, Ausscheidung• Toxikologie
Pharmacodynamics is what drugs do to the body. Pharmacokinetics is what the body does to drugs.

Clinical pharmacology
1. Pharmacologypharmacodynamics: how drugs, alone and
in combination, affect the body (young, old, well, sick)
pharmacokinetics: absorption, distribution, metabolism, excretion or, how the body, well or sick, affects drugs.

32
Therapeutic evaluation
•Formal therapeutic trials (experimental cohort studies)
•Surveillance programmes, e.g. case-control studies, and observational cohort studies (large-scale monitoring for efficacy and/or adverse reactions)
![Page 33: Klinische Pharmakologie - Med Uni Graz · z. whether a drug is of value. z. how it may best be used. z. formal therapeutic trials. z. surveillance studies for both efficacy and adverse](https://reader036.vdocuments.us/reader036/viewer/2022070811/5f09ed027e708231d4292a92/html5/thumbnails/33.jpg)
33
Formal therapeutic trials
1. „explanatory“ trials
2. „pragmatic“ trials
Formal therapeutic trials are unlikely to reveal:
• adverse effects that are uncommon or occur only after prolonged use, e.g. oral contraceptives and vascular disease; renal damage due to analgesics
• effects in special population groups, e.g. pregnancy, renal or hepatic diseas, since these are generally excluded, reasonably, from formal therapeutic trials
• unexpected therapeutic effects, i.e. potential new uses
• interactions between drugs.
![Page 34: Klinische Pharmakologie - Med Uni Graz · z. whether a drug is of value. z. how it may best be used. z. formal therapeutic trials. z. surveillance studies for both efficacy and adverse](https://reader036.vdocuments.us/reader036/viewer/2022070811/5f09ed027e708231d4292a92/html5/thumbnails/34.jpg)
34
Aims of randomised controlled trial
• whether a treatment is of value
• how great its value is (compared with other remedies, if such exist)
• in what types of patients it is of value
• what is the best method of applying the treatment; how often, and in what dosage if it is a drug
• what are the disadvantages and dangers of the treatment
Important points in therapeutic trials
1. Equivalent groups of patients
2. Time and place of treatments
3. Precisely framed question
![Page 35: Klinische Pharmakologie - Med Uni Graz · z. whether a drug is of value. z. how it may best be used. z. formal therapeutic trials. z. surveillance studies for both efficacy and adverse](https://reader036.vdocuments.us/reader036/viewer/2022070811/5f09ed027e708231d4292a92/html5/thumbnails/35.jpg)
35
Types of error – confidence intervals
• Type I error, i.e. finding a difference between treatments when in reality they do not differ
• Type II error, i.e. finding no difference between treatments when in reality they do differ to an extent doctors would want to know about – the target difference
Therapeutic trials
• Fixed-sample trials
• Variable number trials (interim analysis at predetermined intervals)
![Page 36: Klinische Pharmakologie - Med Uni Graz · z. whether a drug is of value. z. how it may best be used. z. formal therapeutic trials. z. surveillance studies for both efficacy and adverse](https://reader036.vdocuments.us/reader036/viewer/2022070811/5f09ed027e708231d4292a92/html5/thumbnails/36.jpg)

Parameters of clinical trials
1. Precision or Power
2. Sensitivity
3. Meta-analysis
Parameters of clinical trials
1. Double-blind and single-blind
2. Placebo medication as a control
3. Within (cross-over) and between (parallel group) patients studies

Some mortal sins of clinical assessment
• Enthusiasm and scepticism:„A marvellous / useless drug, this“.
• Change of assessor:„Do the measurements for me Jim / Miss Jones / darling“.
• Change of time:„Don‘t worry about the assessment. Go ahead with lunch / X-rays / physiotherapy / your bath“.
• Squeezing:„You‘re much better, aren‘t you, Miss B?“ „Any indigestion yet, Miss B?“
Some mortal sins of clinical assessment
• Pride:„I‘m honest. No need for placebo in my trials“.
• Impurity:„We‘re short of cases: she‘ll have to do“. „A few aspirins won‘t make much difference“.
• Imbalance:„Sex / severity / treatment order, doesn‘t matter“.
• Error:„Not quite significant. Let‘s try sequential analysis“.

Pharmakoepidemiologie
Pharmacoepidemiology
1. Observational cohort study:patients receiving drug are collected and outcomes (therapeutic and adverse) determined – prospective
2. Case-control study:patients are collected who have the condition to be investigated and an appropriate control group is assembled. Complete drug history from each group – retrospective.

50
Classification of adverse reaction
Type A (Augmented): occuring in everyone, due to excess of normal pharmacodynamic effects Type B (Bizzare): only in some people, not part of the normal pharmacology of the drug, not dose-related. These include effects due to inherited abnormalities and immunological processes. Type C (Continuous): long-term use Type D (Delayed): e.g. teratogenesis, carcinogenesis Type E (Ending of use): e.g. rebound adrenocortical insufficiency.
Mechanism of adverse drug reactions:
Type A 80%
Type B 20%

Most common targets of adverse drug reactions:
Gastrointestinal tract
Skin
Mental alertness
Plasma K concentration
Drug-induced illness
1. Drug commonly induces an otherwise rare illness: likely to be discovered by clinical observation in pre-marketing trials
2. Drug rarely induces an otherwise common illness: likely to remain undiscovered
3. Drug rarely induces an otherwise rare illness: likely to remain undiscovered before phase 4
4. Drug commonly induces an otherwise common illness: discovered in pharmacoepidemiological studies
5. Both drug and illness rates in intermediate range: case-control and cohort studies may be needed

Adverse reactions to drugs
• 2-3% of consultations in general practice
• 0.3% of general hospital admissions (up to 3% of admissions to acute care wards)
• overall incidence in hospital 10-20%
• predisposing factors: age over 60 years or under one month, history of adverse reaction, hepatic or renal disease
• most commonly early in therapy (days 1-10)
Most common diseases of sufferers with adverse drug reactions

Milestones in Drug regulation
1. 1937: sulphanilamide as „elixir“ dissolved in diethylene glycol; no animal tests; 107 cases of death in USA.
2. 1961: thalidomide caused a total of about 10.000 cases of phokomelia worldwide, 5.000 of whom in West Germany. The drug had not been tested on pregnant animals.
Requirements of drug regulation
1. Pre-clinical testsa) animal testsb) chemical and pharmaceutical quality
2. Clinical testsa) Phase 1b) Phase 2c) Phase 3
