CASE STUDY © 2013 Geisinger Health System www.geisingerihc.com IHC Case Presentation: A 3-year-old girl presented with a two-month history of left ankle pain and a limp. MRI findings were most compatible with calcaneal osteomyelitis and intraosseous abscess. Other etiologies, including Langerhans cell histiocytosis and Ewing’s sarcoma could not be entirely excluded. She had a biopsy of the left calcaneus that demonstrated a malignant small round cell neoplasm, as shown below. Immunohistochemical stains were completed and results are listed in Table 1. Subsequently, a CT scan revealed a 4.9 cm lobulated circumscribed heterogeneous soft tissue mass within the right kidney and axillary adenopathy. Biopsy of the right renal mass showed histology and immunostaining patterns similar to that of the bone lesion. Follow-up bilateral bone marrow biopsies were normal. Table 1.
