Facing - and Adapting to - Change

A client living in a boarding house room barely bigger than a closet. Twobrothers sharing home-delivered meals for one because they couldn'tafford to buy food. Women at risk of losing their newborns before their firstbirthdays because they don't have safe housing and other necessities. These are some of the cases Toledo/Lucas County CareNet's communityhealth workers have encountered since the nonprofit began providing carecoordination to low-income Lucas County residents in 2015. Since itsinception as a charity care network in 2002, CareNet has continued toadjust to the needs of residents as its strives to provide access tocoordinated healthcare services.

Ohio's expansion of Medicaid eligibility to morepeople helped fuel CareNet's change. Lucas Countyresidents needed help obtaining Medicaid andutilizing their newfound healthcare coverage, alongwith getting basic necessities that everyone needs tobe healthy. CareNet began hiring community healthworkers in 2015 to ensure the network's carecoordination clients have a medical home and getconnected to needed medical and social services. Toledoan Judith Williams, who is pictured on the cover, retired in January2018. After most of Judith's savings and monthly Social Security benefitswent toward rent and utilities, her gas was turned off a few months later.She had to microwave water to bathe and didn't have enough money forfood. But Glori Puskarich, Judith's CareNet community health worker,helped her apply for Medicaid to supplement her Medicare and foodstamps. Glori also helped her get home-delivered meals and pay for utilities. With the continued support of our donors, volunteers and other partners,CareNet will continue to be Lucas County's charity care network helpingLucas County residents get the care they need. CareNet also needs thatsupport to continue providing care coordination to low-income residentslike Judith and hundreds of others.

Facing - and Adapting to - Change

A client living in a boarding house room barely bigger than a closet. Twobrothers sharing home-delivered meals for one because they couldn'tafford to buy food. Women at risk of losing their newborns before their firstbirthdays because they don't have safe housing and other necessities. These are some of the cases Toledo/Lucas County CareNet's communityhealth workers have encountered since the nonprofit began providing carecoordination to low-income Lucas County residents in 2015. Since itsinception as a charity care network in 2002, CareNet has continued toadjust to the needs of residents as its strives to provide access tocoordinated healthcare services.

Ohio's expansion of Medicaid eligibility to morepeople helped fuel CareNet's change. Lucas Countyresidents needed help obtaining Medicaid andutilizing their newfound healthcare coverage, alongwith getting basic necessities that everyone needs tobe healthy. CareNet began hiring community healthworkers in 2015 to ensure the network's carecoordination clients have a medical home and getconnected to needed medical and social services. Toledoan Judith Williams, who is pictured on the cover, retired in January2018. After most of Judith's savings and monthly Social Security benefitswent toward rent and utilities, her gas was turned off a few months later.She had to microwave water to bathe and didn't have enough money forfood. But Glori Puskarich, Judith's CareNet community health worker,helped her apply for Medicaid to supplement her Medicare and foodstamps. Glori also helped her get home-delivered meals and pay for utilities. With the continued support of our donors, volunteers and other partners,CareNet will continue to be Lucas County's charity care network helpingLucas County residents get the care they need. CareNet also needs thatsupport to continue providing care coordination to low-income residentslike Judith and hundreds of others.
