HSE case full development; HSE case sectional development and peer review/gap
analysis of client developed material; development of full MHRA reports and information such as
SOOB matrix for HSE case inclusion or as standalone
qualitative risk reports; environmental impact assessments, risk-based sensitivity
studies (QRA etc.), and vulnerability and reliability
assessments (FTA, FMEA, FMECA, RBI and RBM
programmes); on-hire pre-survey reviews and operator/owner systems

Identify Hazards MHRA Report and
SOOB Matrix Evaluate Risk Update Safety Case IADC Section 4
Compliance
Key to the BowTie
control and
assessment process is
the validation of
identified barriers by:
hardware – function,
availability, reliability,
interdependency and
survivability in an MAH
event – and process
like MOC, for safety
critical elements; and
software and
procedure –
confirmation that
procedural controls are
in place, fit for purpose
and audited as part of
the HSEMS lifecycle.
In this process Haztek
uses assessment
tables that are pre-populated sections
from hazard checklist
ISO 17776:2000(E).
Identification of
hazards.
Evaluation of risk and
identification of risk
reduction measures,
using the client risk
matrix and developed
acceptance criteria.
The SOOB matrix is
developed from the
MHRA process
assisting the worksite
in distinguishing
between a ‘stop work’
condition and a
‘proceed with caution’
condition.
The MHRA report
specifically addresses
Section 4 of the HSE
case and further
provides information to
the remaining sections
relating to hazard
management,
response requirements
and lifecycle activities.
Section 4 of the IADC
HSE case is an
integrated part of the
MHRA report and is an
underpinning
document to the
overall HSE case. It is
continuously reviewed
against any changes
from the management
or engineering
standpoint to ensure
that hazards are
identified, assessed
and managed in the
correct manner.
The HSE case
sections are updated in
parallel to the MHRA
process to ensure that
information transfer is
maximised and that
impacts or potential
changes are identified
at an early stage so
that appropriate
actions/changes can
be addressed.
The HSE case is
subject to continuous
improvement and
audit.
