Click “Responses” (in the form tab)
Click “Accepting responses”
The survey form was turned off
Click “View live form”
This is the live form that was turned off
Click “Not accepting responses” to turn on survey
Click “Only allow…” for unique response per person
Click “Change theme”
Choose themes from the list
Google Form is auto-saved in Google Drive
Click to open Google Drive
Form is displayed
Double-click to open the form
Our form is now ready, click “File”
Click “Send form”
It will show the link of the form
Click “Embed” to put the form in website
Click anywhere on the text box
Press Ctrl+C to copy and
Paste link on website
Click any social sites to share your form
Enter email addresses of recipients
Click “Done”
