Terms of Agreement

Acknowledgement of Discount Fees: Client acknowledges that these fees are discounted and not CPIs usual & customary fees. As such, clientunderstands that these fees will only be offered for as long as client is directly paying CPI for the services and no third party insurance is being billed byCPI. If client wishes to submit receipts directly to an insurance carrier, said receipts will reflect cash fees only.

Visits Covered by Insurance: A portion of the care provided under this agreement may be covered by client's health insurance. Client understands thatpayments paid are applied to any deductible, copays and/or included care not covered by insurance. Client further understands that if their healthinsurance benefits should change, that CPI reserves the right to change this plan.

Additional Visits Provided During Plan: Additional visits during the term of this agreement may be purchased at the rates defined on page 1 of thisagreement.

Not Covered Under Program: This arrangement does not include massage services, products, or after-hours emergency care.

Early Termination of Care: Client understands that if they cancel care prior to the end of the term, clients account balance will be prorated based uponthe Pay Per Visit Fee (listed on page 1) for all services provided. If client requests early termination less than five weekdays days prior to next monthlypayment due date, and a balance is due, said monthly payment will still be drawn on due date. Any overpayment will be refunded within ten businessdays of canceling care. Penalty for Early Termination (Option 1 Only): After account adjustment, any balance due will be auto-debited from credit/debitcard on file. If credit/debit card is declined, CPI will bill client and reserves the right to charge a billing fee of $25 per billing.

Changes in Status: Client is responsible to notify CPI within 72-hours if personal circumstances change due to an injury incurred due to either work orautomobile accident.

Program Extension Due to Temporary Disruptions in Care: In the event client assigns any chiropractic health insurance benefits to CPI, or insurancebenefits become available because of a workers compensation or personal injury claim, or for any other reason; client understands that CPI willsuspend this agreement while treating client for any accident related injuries. Care provided during said extension will be at CPIs usual & customaryfees. CPI will resume treatment under this plan once client has recovered from said injury and has been released from treatment.

Vacation Credit: At the end of this plan, if the primary client has underutilized (used fewer than total visits within the program period) the programbecause client was on vacation, CPI will extend care for the time of vacation. Minimum periods of vacation time for extension is one-week. Thisextension is limited to a maximum of one-month, not to exceed total visits included.

End of Term Credit: If client does not fully utilize the cash value of care based on the Pay per Visit Cash Fee (listed on page 1), clients account balancewill be prorated based upon the Pay per Visit Cash Fee for all services provided and a CPI credit will be issued. Credit may be used towards any serviceoffered at that time by CPI.

Missed Appointments: Client must make-up all missed visits within the time period of this agreement.

This Plan is Not Insurance: This agreement does not constitute insurance and as such CPI makes no promises to treat new conditions under thisagreement.

No Guarantee of Results: Client recognizes that this agreement is not a guarantee of clinical results, and that it deals solely with financial and timeobligations. There is no guarantee that any illness, injury, or disease can be prevented or cured by participation in this program. Any balance due forservices are due regardless of results.

Complete Agreement: This agreement is non-transferable and constitutes the complete agreement between client and CPI. No other chiropracticfacilities are covered by this agreement. This agreement may be modified by CPI with 30-days written notice.
