CHAPTER I
GENERAL REQUIREMENTS
Article 1
In the implementation of education at Faculty of Dentistry, Universitas Airlangga, Education
Regulations are set, including some notion as follows:
1. Higher Education is education level after the secondary education, including diploma
program, undergraduate program, professional education program, master program,
specialist program, and doctoral program organized by the Higher Education Institution;
2. The Higher Education Institution is Universitas Airlangga;
3. The Executive Board of the University is the Rector and Vice- Rectors;
4. The Rector is a leader in the management of the University;
5. Faculty Advisory Board is a unit that serves to formulate academic and scientific policies
in faculty level;
6. The Executive Board of the Faculty is the Dean and Vice Deans of Faculty of Dentistry;
7. Quality Assurance Unit is a unit that serves to help the Executive Board of the Faculty in
conducting quality assurance;
8. Accreditation is recognition of the University or study program of the University that
meets the minimum standards set by the National Accreditation Board of Higher Education;
9. Lecturers are professional educators and scientists with the main task of transforming,
developing, and disseminating knowledge and noble values;
10. Academic Personnel are people working as administrative executives, technicians,
laboratory assistants, librarians, or other professions needed by the University based on
their education and expertise. Academic Personnel are appointed and dismissed by the
Executive Board of the University;

11. Students are learners enrolled, as a result, they can participate in the process of education
at the University;
12. Alumni are graduates of one of the study programs organized by the University;
13. Academic Education is higher education directed mainly to the mastery of science,
technology and / or art, as well as the mastery and development of science and technology
branches, especially Dentistry;
14. Faculty is a unit implementing academic education, professional and / or vocational
education in the University, as well as coordinating and implementing academic activities
in one or several disciplines of science, technology and arts;
15. Dean is a faculty leader coordinating resource management and quality assurance in the
Faculty;
16. Department is a unit managing academic implementation in the field of science in the
Faculty, and also managing a study program if eligible;
17. Study Program is an integrated study plan as a guide to the organizers of academic and/or
professional education on the basis of a curriculum, intended to allow students to master
knowledge, skills and attitudes appropriate with the curriculum goals;
18. Division is a unit implementing administration at the faculty level with the aim to
coordinate, develop and/or develop an accountable, transparent and integrated
administration;
19. Laboratory is a unit supporting infrastructure for the implementation of certain academic
activities in the field of science under the Faculty or department;
20. Course Coordinator (PJMK) in Undergraduate Program is a lecturer who has a duty
and authority to formulate, implement and evaluate the learning process on certain subject;
21. Academic Advisor is a lecturer who has a duty and authority to grant academic and
professional advice to a group of students. In Faculty of Dentistry, Universitas Airlangga
(FKG, Unair), there are two kinds of Academic Advisor, namely Academic Advisor for
Dentist Education program and Academic Advisor for Dentist Professional program;
22. Professor is the highest academic functional position for a lecturer teaching in the Higher
Education Institution;
23. New Students are students first enrolled in a study program at Universitas Airlangga;
24. Credit System is a system of education in which study load of students, workload of
lecturers, and load of the study program are expressed in credit unit;
25. Credit System is a system of education in which study load of students, workload of
lecturers, and load of the study program are expressed in credit unit;

26. Academic activities in even and odd semester are conducted in 12 (twelve) weeks;
academic activities in between semesters are conducted in four (4) weeks; a replacement
holiday is in 1 (one) week; and Mid Test (UTS) and Final Test (UAS) are held in two (2)
weeks for each;
27. Semester Credit System (SKS) is a system for the provision of education by using
semester credit units (credits), to declare study load of students, workload of lecturers,
learning experiences, and load of the study program;
28. Short Semester is a period of time for academic activities composed of fourteen (14)
meetings, including evaluation, organized to help students to accelerate their study and
improve their GPA (IPK) by utilizing the spare time between semesters;
29. Semester Credit Unit (sks) is a unit of achievement of learning experience gained during
one semester through scheduled activities per week as many as 1 hour for lecture / tutorial,
or 2 hours for practicum, 4 hours for fieldwork, or 4 hours for clinical practicum, each of
which is accompanied by 1-2 hour structured activities that are not scheduled and 1-2 hour
independent activities. One hour meeting is equal to 50 minutes;
30. Curriculum is a set of plans and arrangements regarding the content and materials of the
study, lessons, as well as teaching and assessment methods used as a guide to learning
activities;
31. Core Curriculum is the group of study materials and subjects that should be covered in a
course as defined in the prevailing national curriculum;
32. Institutional Curriculum is a number of study materials and courses set by each study
program related to the circumstances and needs of the environment as well as the hallmark
of the University;
33. Learning is a process involving direct or indirect communication, practicum, clinical
fieldwork/ hospital profession-based fieldwork, experiment, and other academic tasks;
34. Personality Development Courses (Mata Kuliah Pengembangan Kepribadian-MPK)
are courses aimed to develop the Indonesian people who believe and devote their life to
God Almighty, and have noble characters, strong personality, independency, and a sense
of civic and national responsibility;
35. Scientific and Skill Courses (Mata Kuliah Keilmuan dan Keterampilan-MKK) are
courses aimed to provide a foundation of mastering certain knowledge and skills;
36. Work Skill Courses (Mata Kuliah Keahlian Berkarya-MKB) are courses aimed to
produce experts with a wealth, according to the knowledge and skills mastered;

37. Work Behavior Courses (Mata Kuliah Perilaku Berkarya-MPB) are courses aimed to
shape the attitudes and behaviors needed by students to work based on skill level, according
to the knowledge and skills mastered;
38. Community Courses (Mata Kuliah Berkehidupan Bermasyarakat-MBB) are courses
aimed to make students understand norms in society, according to the choice of expertise
in work;
39. Achievement Index (IP) is the ratio / multiplication function between credit scores and
load scores obtained, and then divided by the number of credits of the courses taken;
40. Study Load of Education Program is the amount of workload calculated in credits that
must be taken by students to complete a certain higher education;
41. Undergraduate Thesis is a task that provides learning experiences for students to make
written scientific papers by applying a scientific attitude, thinking and method in solving
scientific problems through research, and be able to present and defend the results in
writing and orally in order to complete certain study credit to obtain a bachelor's degree;
42. The main Advisor is a lecturer in the Program in question whose expertise is related to the
topic of the assignment. The main advisor must be at least Associate Professor or Professor
with Master degree or Assistant Professor with Doctoral degree. The main advisor is in
charge of chairing guiding and supervising students in completing their final project or
thesis;
43. The Second Advisor is a lecturer in the Program in question whose expertise is related to
the topic of the assignment. The second advisor must be at least Associate Professor or
Professor with Master degree or Assistant Professor with Doctoral degree. The second
advisor is assigned to assist the main advisor in guiding and supervising students to
complete their final project or thesis;
44. Academic transcript is a list of the score of learning outcomes and performance index of
all the courses taken by students during the study;
45. Academic Calendar is annual schedule of academic activities arranged in detail in each
semester;
46. Quality Assurance is a program to carry out the monitoring, evaluation, and correction as
quality improvement or quality measurement in a sustainable and systematic review of all
aspects of higher education in order to assure perfection attainable standards as stated in
the vision, mission, and goals of the University;

47. Self-evaluation is a systematic effort to collect and manage data (facts and information)
that are reliable and valid used as a measure of management to manage the continuity of
the institution or program;
48. Mid Test (UTS) is the evaluation of student’s learning outcome, held in the middle of
semester and set in academic calendar;
49. Final Test (UAS) is the evaluation of student’s learning outcome, held at the end of
semester and set in academic calendar;
50. Remedial Test (UP) is an additional test held after the Final Test based on the results of
the study evaluation in the semester before the final score of the course is determined;
51. Legitimate Reason is a reason not to participate curricular activities or tests, supported by
official document;
52. Graduate Candidacy Evaluation Meeting is a decision-making forum for determining
graduates, conducted by the faculty;
53. Graduate Candidacy Evaluation is a decision of the Dean to determine students who
have completed their study as graduates in accordance with the requirements of graduation
in the faculty, based on the results of Graduate Candidacy Evaluation Meeting;
54. Graduation is an academic event to inaugurate graduates who have completed higher
education;
55. Syllabus (GBPP) contains formulated objectives and materials of the course that includes
the name, code number, and short description of the course, general and special
competence, the main topic and the sub-topic of materials, soft skills, estimated time of
every meeting, learning method, and sources of books or journals used;
56. Lesson Plan (SAP) contains formulated objectives and principles course every meeting.
SAP must contain the name and code number of the course, estimated time of every
meeting, the serial number of meeting, general and special competence, the main topic and
the sub-topic of materials, learning method, evaluation (hard skills and soft skills), and
sources of books or journals used;
57. Course Outline is an agreement between Course Coordinator and students in the
implementation of the learning process;
58. A Face-to Face Activity is a process of interaction between lecturers and students in the
context of knowledge transfer, discussions, and similar activities carried out in the room /
classroom;
59. Practicum and Fieldwork is academic activities aimed at deepening the theory test
obtained in the class to be developed, as a result, the students can be confident;

60. Skills Lab or Medical Skills are supporting activities aimed at obtaining the deepening of
cognitive, affective and psychomotor skills in preparation for clinical work;
61. Instructors are lecturers who guide students in conducting Skills Lab, known as clinical
instructors in the study program;
62. Integrated Module Courses (PBL) are integrated courses with integrated lecture and
discussion method, namely in the form of twelve (12) classical lectures followed by twelve
(12) Problem-Based Learning discussions, in order to understand basic medical and dental
courses comprehensively to study clinical case problems;
63. Tutors are lecturers who guide students in conducting small group discussion;
64. Not-On-Time Leave Students are students who have not successfully graduated on time
from regular courses offered;
65. Team of Competency-Based Curriculum Implementation is a team formed by the Dean
of the Faculty of Dentistry, Universitas Airlangga to design, prepare, monitor and evaluate
the implementation of the learning process of Undergraduate Dentist Education Program
and Dentist Professional Study Program;
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. International Dental Course Credit Transfer Program is a credit transfer program of
Undergraduate Dentist Education program of Faculty of Dentistry, Universitas Airlangga
in collaboration with Faculty of Dentistry, Hiroshima University. Memorandum of
Agreement between the two institutions was started in 2011. This program is called "IDC
Credit Transfer Program";
2. The validity of the Memorandum of Agreement of IDC Credit Transfer Program is
five (5) years from the date of signing. The MoU will be renewed automatically at the end
of the fifth year unless one or both parties indicate otherwise. Both institutions must send
a written notice and consult each other at least 1 (one) year prior to the revision or
termination of this Memorandum of Understanding;
3. Academic Certificate issued at the end of the study period of the IDC Credit Transfer
Program is an Academic Certificate of Bachelor’s Degree in Dentistry (SKG) issued by
Universitas Airlangga and signed by the Rector of Universitas Airlangga;
4. Academic Certificate of Bachelor’s Degree in Dentistry (SKG) accompanied by a
Certificate of Recognition from Hiroshima University and academic transcript containing
details of courses and practicum in the category of Credit Earning is given to students of

the IDC Program. Certificate of Recognition signed by the Dean of the Faculty of Dentistry,
Universitas Airlangga and the Dean of the Faculty of Dentistry, Hiroshima University;
5. The Location of the IDC Program is in two universities, namely Universitas Airlangga
and Hiroshima University. The first year of the IDC Program is conducted in Faculty of
Dentistry, Universitas Airlangga, located in Jalan Mayjend. Prof. Dr. Moestopo 47
Surabaya 60132. Next, from the second year to the fifth one, the program is conducted in
Faculty of Dentistry, Hiroshima University, located in 1-2-3 Kasumi, Minami-Ku,
Hiroshima 734-8553 Japan;
6. Education curriculum used in IDC Credit Transfer Program is dental education syllabus
of Universitas Airlangga and Hiroshima University. During in Hiroshima University,
education curriculum used is the curriculum of the International Dental Course (IDC)
program of Hiroshima University;
7. IDC Credit Transfer Program is conducted for four years (48 months), started from
semester 3 to 9 with the addition of 1 semester at the beginning of the program. Odd
semester is conducted from October to March, while even semester is conducted from April
to September;
8. Undergraduate Thesis must be conducted by Students of the IDC Credit Transfer
Program, similar to students of regular program. The undergraduate thesis is called "Final
Scientific Assignment";
9. Final Scientific Assignment Advisor consists of the main advisor and the second advisor;
10. The main advisor of Final Scientific Assignment is a professor of the department of
Dentistry Faculty of Hiroshima University whose expertise is related to the topic of the
assignment. The main advisor is in charge of chairing guiding and supervising students in
completing their final project or thesis;
11. The second advisor of Final Scientific Assignment is a lecturer of Dentistry Faculty of
Universitas Airlangga whose expertise is related to the topic of the assignment. The second
advisor must be at least Associate Professor or Professor with Master degree or Assistant
Professor with Doctoral degree. The second advisor is assigned to assist the main advisor
in guiding and supervising students to complete their final project or thesis;
12. Academic transcript is a list containing the scores of student’s learning outcomes and
performance index of all the courses taken during his/her study. At the end of each
semester, Hiroshima University will send Academic Transcript of the students of
International Dental Course Program. Based on the transcript, Universitas Airlangga will

issue Diplôme Supplement that provides details of subjects classified based on Credit
Transfer (CT) Program and Credit Earning (CE) Program;
13. Credit Transfer (CT) Program is a program which offers courses given by Hiroshima
University and Universitas Airlangga;
14. Credit Earning (CE) Airlangga Program is a program which offers courses that are not
provided by Universitas Airlangga. Students, as a result, must take those courses in
Hiroshima University;
15. Credit Earning (CE) of Hiroshima Subject Needed (HSN) or CE Hiroshima Program
is a program in which students are obligated to take Hiroshima Subject Needed (HSN)
courses that are not provided by Hiroshima University. Students, as a result, must take those
courses in Universitas Airlangga;
CHAPTER II
EDUCATION LEVEL
Article 2
Faculty of Dentistry, Universitas Airlangga organizes Undergraduate Dentist Education
Program and Dentist Professional Study Program. Undergraduate Dentist Education Program
offers IDC Credit Transfer Program in collaboration with the Faculty of Dentistry, Hiroshima
University in Japan.
Article 3
Graduates of the Undergraduate Dentist Education Program must meet the following
qualifications:
a. Mastering Basic Medical Science, Medical Clinical Science, Basic Dental Science, Dental
Clinical Science, and skills in specific areas of expertise, consequently, they can discover,
understand, explain, and formulate a solution to solve problems in the area of their
expertise;
b. Able to apply the knowledge and skills they have related to their expertise in dentistry
during their productive activities and services to community with attitudes and behavior in
accordance with the norms;
c. Able to act and behave in harmony together with community during working in accordance
with their expertise in dentistry;

d. Able to update the development of science, technology, and / or art related to their expertise
in dentistry.
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. IDC Credit Transfer Program is directed to graduates who have the following
qualifications:
a. Competent, equivalent to a Bachelor's degree in Dentistry (SKG) in Indonesia;
b. Able to be a leader in research, education, and health services in the field of dentistry in
Indonesia, and able to compete in the global era.
Article 4
Dentist Professional Study Program aims to make students acquire skills and have an ability to
provide professional services in Dentistry, in accordance with their Undergraduate Dentist
Education Program;
CHAPTER III
CURRICULUM
Article 5
1. Undergraduate Dentist Education Program has two (2) sequencing sub-programs, namely
Basic Medical Program and Basic Dental Program, referred to Sub-Program I (semesters
I-IV); and Dental Professional Preparation Program, called as Sub-program II (semesters
V-VIII);
2. Dentist Professional Study Program consists of Hospital Profession-Based Fieldwork
(clinical practicum) and fieldwork (semesters IX-XII);
3. The Undergraduate Dentist Education Program consists of 148 credits, and after
completing this program, graduates get a college degree, namely Bachelor of Dentistry
(SKG);
4. The Dentist Professional Study Program consists of 36 credits, and after completing this
program, graduates get a profession title, namely Dentist (drg).
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. In International Dental Course (IDC) Credit Transfer Program, students in the first year
(semesters I and II) must take General Education at Universitas Airlangga, and then they

must study at University of Hiroshima from year 2 to year 5 (from semester IIII to semester
IX).
2. During the first six months in Hiroshima, the preparation time for International Dental
Course (IDC) program, students must take several subjects, namely Japanese Languange
and Culture, Oral and Dental Science, Life Science, Dentistry Overview, English
Communication, and International Collaboration Development for Dentistry (ICDD);
3. From semesters 3 to 9 semesters (3.5 years), students in IDC program must take Basic
Subject, Clinical Subject, and Common Core Program
4. Lectures and practicum in IDC program are conducted together with the regular program
of Japanese students
5. The teaching and learning processes are conducted in 2 (two) languages, namely English
and Japanese
6. In International Dental Course (IDC), the Undergraduate Dentist Education Program
consists of 147 credits, and after completing this program, graduates get a college degree,
namely Bachelor of Dentistry (SKG), and receive Certificate of Recognition from Faculty
of Dentistry, Hiroshima University;
Article 6
1. Curriculum used in the Faculty of Dentistry, Universitas of Airlangga is competency-based
curriculum using student-centered learning method as enacted in Decree of the Rector of
Universitas Airlangga No. 2496/H3/KR/2011 on the Determination of Curriculum in
Faculty of Dentistry, Universitas Airlangga;
2. Curriculum used in the Faculty of Dentistry, Universitas Airlangga must be in accordance
with Law No. 20 year 2003 on National Education System; Decree of the National
Education Minister No. 232/U/2000 on the Guidelines for the Development of Higher
Education Curriculum and the Assessment of Student Learning Outcomes; Decree of the
National Education Minister No. 045/U/2002 on the Core Curriculum of Higher Education;
Decree of the Indonesian Medical Council No. 22/IMC/XI/2006 on Education Standards
of Professional Dentists; Decree of the Indonesian Medical Council No. 23/IMC/XI/2006
on Competency Standards of Indonesian Dentists; and the National Curriculum
Implementation Guideline of Dental Competency-Based Education in Indonesia year 2007
(AFDOKGI 2007);
3. Based on the Competency Standards of Indonesian Dentists issued by the Indonesian
Medical Council year 2008, the Curriculum for the Undergraduate Dentist Education

Program and the Dentist Professional Study Program must involve 25% of
Professionalism Science group; 15% of Basic Medical and Dental Science group; 12% of
Clinical Skills Supporting Science group; 18% of Management and Community Science
group; 30% of Clinical Skills Science group; and 10% of Advanced Science group;
4. Curriculum that has been verified by LP3UA (Unit of Education Study and Development,
Universitas Airlangga) and approved by the Faculty Advisory Board, must be approved
and issued by the Decree of the Rector;
5. Review of the curriculum can be conducted regarding the development of science,
technology, humanities and the arts, as well as the period of the study and the needs of
community at least once in five years;
6. To support the achievement of the objectives of the education programs, the curriculum
must be applied by Semester Credit System (SKS) and measured by Semester Credit Units
(credits).
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. Based on Law No. 20 year 2013 on National Education System, national education system
must guarantee equal educational opportunities, quality improvement, and relevance and
efficiency of education management in order to meet challenges and demands for changes
in local, national, and global level; as a result, the review of education conducted in the
Faculty of Dentistry must be planned, directed, and sustained;
2. Memorandum of Understanding between Universitas Airlangga and Hiroshima University
was conducted in 2012, while Memorandum to the Basic Agreement of Exchange
Programs between the Faculty of Dentistry, Universitas Airlangga and the Faculty of
Dentistry, Hiroshima University was signed in 2011.
3. Procedures of diploma degree equivalency from an institutional or partnership program are
stated in Circular Letter of the Directorate General of Higher Education No.
970/E.E2.4./2014;
4. Join Degree Program of the Undergraduate Dentistry Program between the Faculty of
Dentistry, Universitas Airlangga and the Faculty of Dentistry, Hiroshima University has
been enacted by Decree of the Rector of Universitas Airlangga No. 1700 / UN3 / 2014.
5. In Hiroshima University, the curriculum for the International Dental Course (IDC) Credit
Transfer Program is implemented through the following methods:
a. Credit Transfer Program (CT): Students take courses given by University of Hiroshima,
also given at Universitas Airlangga

b. Credit Earning Program (CE Airlangga): Students take courses given by University of
Hiroshima, but not given by Universitas Airlangga
c. Credit Earning Program (CE Hiroshima): Students take Hiroshima Subject Needed
(HSN) courses
d. If there are courses that are not provided by Hiroshima University, they must be taken
by students at Universitas Airlangga
6. Mechanisms of the preparation and review of the curriculum is set in a separate procedure
manual.
CHAPTER IV
IMPLEMENTATION OF EDUCATION
Article 7
1. The content and topic of discussion in a course must support the achievement of
competence (learning outcomes), measured in Semester Credit Units (SKS);
2. One course can be handled by a Lecturer or Team Teaching approved by the Dean based
on the proposal of the Head of the Departments.
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. In the first year, students of IDC program must take Life Science as well as Medical and
Dental Science for about 39 credits (CT) at Universitas Airlangga. The study period of IDC
program takes 3.5 years (from semesters 3 to 9). The study period of IDC program is started
with a transition period for one semester (the first six months). During this transition period,
the students must take 19 credits. The total of credits in IDC program is 135 credits, and
159 credits for CE. In addition, the students also must take 12 additional credits for courses
that they do not get in Hiroshima (HSN). Thus, to get a Bachelor's degree in Dentistry
(SKG), the total of credits the students must take is 210 credits.
2. Additional materials (HSN) given are lab Skill 5 and Community Service (KKN) conducted
at Universitas Airlangga during the summer holiday period in August. The course materials
will be provided through video conference.
3. Final Scientific Assignment must be conducted within the period of study at Hiroshima
University.

4. Students of IDC program must take Course for dental frontier science, in which students
are not only endowed with dentistry material, but also equipped with research materials
equivalent to graduate education level (S2/Master program).
Article 8
1. In the implementation of the learning process, each course in Hiroshima University is
equipped with Syllabus made by the professor. Meanwhile, in the Faculty of Dentistry,
Unair, Syllabus (GBPP) and Course Outline are made by the Course Coordinator (PJMK);
2. Monitoring of the implementation of Syllabus (GBPP), Lesson Plans (SAP), and Course
Outline is conducted by the Head of the Departments or the Head of the Programme as part
of the quality assurance process.
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. Evaluation in IDC program is conducted through Committee Meeting and Faculty
Development Meeting at Hiroshima University to monitor the implementation of the
program and to find solutions for problems found. Evaluation of study performance is
conducted in the forms of Final Test (UAS), quizzes, independent assignments, practicum,
and Final Scientific Assignment.
CHAPTER V
LECTURES AND PRACTICUM
Article 9
1. Learning methods applied in the Faculty of Dentistry, Universitas Airlangga are Lecture,
Integrated Lecture and Discussion, Small Class Discussion, Practicum, Skills Lab, Hospital
Profession-Based Fieldwork, and Fieldwork;
2. Teaching methods can be developed and/or in combination with other methods to optimize
the achievement of learning outcomes;
3. Students of the Undergraduate Dentist Education Program must participate in Lecture,
Integrated Lecture and Discussion, Small Class Discussion, Practicum, Skills Lab, Hospital
Profession-Based Fieldwork, and Fieldwork scheduled and divided into several groups to
achieve competence;
4. Schedule of Lecture, Integrated Lecture and Discussion, Small Class Discussion, Practicum,
Skills Lab, Hospital Profession-Based Fieldwork, and Fieldwork is set for the achievement
of learning outcomes in accordance with the Semester Credit Units of each course;

5. Graduation of students of the Undergraduate Dentist Education Program is determined
after tests or other evaluation forms are conducted at the end of the study period;
6. Each group is scheduled to participate in Lecture, Integrated Lecture and Discussion, Small
Class Discussion, Practicum, Skills Lab, Hospital Profession-Based Fieldwork, Fieldwork,
and Community Service (KKN) inside or outside the campus of the Faculty of Dentistry,
Universitas Airlangga;
7. At the first meeting, the Course Coordinator (PJMK) must explain learning objectives,
Lecture methods, and evaluation methods of Lecture, Integrated Lecture and Discussion,
Small Class Discussion, Practicum, Skills Lab, Hospital Profession-Based Fieldwork, and
Fieldwork in accordance with the Course Outline;
8. Lecturers and Students must fill out the form of the attendance list in every meeting of
Lecture, Integrated Lecture and Discussion, Small Class Discussion, Practicum, Skills Lab,
Hospital Profession-Based Fieldwork, and Fieldwork;
9. Lecturer, Practicum Supervisor, Tutor, or Instructor who is unable to attend the meeting
must inform the Course Coordinator (PJMK);
10. If Lecturer, Practicum Supervisor, Tutor, or Instructor is absent for certain time due to a
task to other institutions, the Course Coordinator (PJMK) can assign a substitute lecturer
who has similar competence;
11. Lecturer, Practicum Supervisor, Tutor, or Instructor requiring replacement schedule is
required to fill Replacement Request Form;
12. Substitute Lecturer, Practicum Supervisor, Tutor, or instructor must be confirmed by the
Minutes of Replacement.

a. To provide and process library materials;

Article 13
1. To conduct dental health education, the Faculty of Dentistry, Universitas Airlangga is
required to have a partnership program with other institutions;
2. Oral and Dental Education Hospital (RSGM-P) is owned by Universitas Airlangga, which
management is under the Faculty of Dentistry, Universitas Airlangga in order to support
learning process conducted in the field of oral and dental health;
3. Oral and Dental Education Hospital (RSGM-P) is in charge of the following:

b. Providing service to community;
c. Facilitating the development of dental professional education and specialist education;
d. Providing internship program for the candidates of professional dentists and specialist

CHAPTER VII
CREDITS AND STUDY PERIOD
Article 14
1. The general objective of the implementation of Semester Credit System in Universitas
Airlangga is to make the implementation of the education and the learning process more
varied and flexible. Thus, students can have wider possibilities to select academic or
professional/expertise courses by planning the study from one semester to another
semester;
2. The special objectives of the implementation of Semester Credit System are as follow:
a. To provide an opportunity for students who are capable and keen to learn to complete
studies within a relatively short time, according to their individual abilities and plans;
b. To provide an opportunity for students to take courses according to their talents,
interests and abilities;
c. To realize the implementation of the education system with plural inputs (input) and
output (output);
d. To adjust the curriculum from time to time, in accordance with the rapid development
of science and technology as well as the fast changes of the community needs today;
e. To realize the implementation of the study evaluation system on students' learning
progress more comprehensively and objectively;
3. The number of credits in a course is expressed in the form of Semester Credit Unit (sks);

4. Activities related to lectures, or seminars, or discussion groups, practicum, research,
fieldwork, and others are also expressed in the form of Semester Credit Unit (sks);
5. Score and credits in one semester credit unit (1 credit) are determined as follows:
a. Lectures
One semester credit unit (1 credit) is determined based on a load of activities, including
three kinds of activities per week for one semester, as follows:
1. Student Activities
a. 1 (one) hour for face-to-face activities scheduled by the faculty, such as lectures;
b. 1 (one) hour for structured academic activities, that are not scheduled, but
planned by the faculty, such as homework, exercise, response activities, and
other outside-class assignments;
c. 1 (one) hour for independent academic activities performed by students
independently to explore and prepare materials, such as reading reference books.
2. Lecturer Activities
a. 1 (one) hour for face-to-face activities with students scheduled;
b. 1 (one) hour for planning and evaluation of structured academic activities;
c. 1 (one) hour for course material development through reading and writing.
b. Semester Credit Unit (SKS) for group discussion, practicum, research, fieldwork, and
Final Scientific Assignment activities are determined as follows:
a. 1 (one) credit for group discussion is equal to two-hour assignment per week for
one semester;
b. 1 (one) credit for practicum is equal to two-hour laboratorial or clinical practicum
per week for one semester;
c. 1 (one) credit for fieldwork/ clinical fieldwork/ internship in industry, agency,
company, and institution is equal to four-hour fieldwork per week for one semester,
or 80- 90 hours in one semester;
d. 1 (one) credit for research and/or final project, Final Scientific Assignment, and
thesis is equal to 3-4 hour assignment per day for one month, equal to twenty-five
working days.
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. The number of credits that must be taken by students during at Hiroshima University is 159
credits, whereas the number of credits at Universitas Airlangga is 51 credits (39 credits in
semesters 1 & 2; 12 credits of HSN).

2. Materials of lectures and practicum related to Credit Transfer (CT), Credit Earning (CE)
and Hiroshima Subject Needed (HSN) are compiled in Curriculum Document.
3. The additional materials (HSN) consists of Skill lab 5 and Community Service (KKN)
conducted at Universitas Airlangga during summer holiday period in August. The course
materials will be provided through video conference.
4. Final Scientific Assignment must be conducted within the period of study at Hiroshima
University. Standard Operating Procedure (SOP) is currently being composed by
Hiroshima University.
5. Students of IDC program must take Course for dental frontier science, in which students
are not only endowed with dentistry material, but also equipped with research materials
equivalent to graduate education level (S2/Master program).
6. The study period of students taking IDC program is longer than the regular study period in
Universitas Airlangga since the period of the semester at Hiroshima University is started
differently from that in Universitas Airlangga and students still must take several subjects
(HSN) that are not offered in Hiroshima University.
Article 15
1. 1 (one) credit is equal to three hours of work, thus, the total of credits for each semester is
generally equal to 15-24 credits, or about 18 credits per semester;
2. In determining the total of credits that must be taken, the ability of the students and the
results of studies in the previous semester reflected in the Achievement Index (IP) must be
considered.
Article 16
1. In Semester Credit System, there are two kinds of Achievement Index (IP), namely their
Grade in every semester (IPS) and Grade Point Average (IPK). Grade (IPS) is a measure
of a student's success in taking courses in one semester, while GPA (IPK) is a measure of
a student's success in taking courses from the first semester to the latest one;

3. Based on Grade (IPS) obtained in the last semester, the study load in the following semester
can be calculated, with the provisions enacted in the Regulation of the Rector of Universitas
Airlangga No. 5/H3/PR/2012.
Article 17
The total of credits in study load Dentistry Degree Program is a 144 (one hundred and forty-
four) credits within seven (7) semesters and a maximum of 14 (fourteen) semesters started from
the beginning of the education;
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
In the first year, students of the IDC Program must take studying Life Science, and Medical
and Dental Science, totally about 39 credits (CT) at Universitas Airlangga. The study period
of IDC program takes 3.5 years (from semesters 3 to 9). The study period of IDC program is
started with a transition period for one semester (the first six months). During this transition
period, the students must take 19 credits. The total of credits in IDC program is 135 credits,
and 159 credits for CE. In addition, the students also must take 12 additional credits for courses
that they do not get in Hiroshima (HSN). Thus, to get a Bachelor's degree in Dentistry (SKG),
the total of credits the students must take is 210 credits.
Article 18
The total of credits in Dentist Professional Study Program is 33 (thirty-three) credits within
three (3) semesters and a maximum of five (5) semesters after undergraduate program;

STUDENT ADMISSION
Article 19
1. The admission of new students in Undergraduate Dentist Education Program and Dentist
Professional Study Program is conducted by the University;

2. The admission of new students in Dentist Professional Study Program is from the graduates
of Undergraduate Dentist Education Program accepted directly without test;
3. Dentist Professional Study Program does not accept any graduate from Undergraduate
Dentist Education Program of other universities;
4. The admission of foreign students is conducted in accordance with the prevailing regulations
in the University.
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. The admission process of new students of the IDC Program is performed by Selection
Committee formed by the Dean with a Decree of the Rector of Universitas Airlangga
2. Faculty of Dentistry, Universitas Airlangga sets the following requirements for the
candidates of the IDC Program:
a. Candidates must be in the second semester with GPA (IPK) of at least 3.5
b. Candidates must have the latest minimum TOEFL score of 500
c. Candidates must pass psychology test
d. Candidates must pass health test
e. Candidates must attach written consent from a parent/ guardian
3. The selection process of the candidates of the IDC credit transfer program consists of the
following stages:
- Socialization program
Socialization program is conducted in several ways, as follows:
a. Disseminating to new students and students in the second semester students of the
Faculty of Dentistry, Universitas Airlangga
b. Disseminating through activities conducted by Student Executive Board (BEM) of
the Faculty of Dentistry, Universitas Airlangga, involving high school students
(Dentine)
c. Disseminating on the official website of the Faculty of Dentistry, posters, leaflets,
etc
Socialization is conducted in the beginning of academic year when new students
start their study. As a result, if there are students who are interested, they can have
enough time to prepare themselves. The socialization program involves FKG Unair
lecturers who have studied in Japan as well as FKG Unair students who are joining the
program in Hiroshima University as .
- Registration

By Final Test (UAS) in the second semester (in June), registration of the IDC
Program is opened.
- Psychology Test (in June)
- Administrative Selection
Administrative Selection is conducted after all the registration documents are
collected (in July)
- Local Interview (in July)
Local Interview is conducted by the Selection Committee of the Faculty of
Dentistry, Universitas Airlangga
- Interview with Hiroshima University (in July)
Once elected as the candidates of the IDC Program by the Selection Committee of
FKG Unair, they will be interviewed by the Selection Committee of Dentistry Faculty,
Hiroshima University via video conference.
- Student Departure (in September)
After the interview process with the Selection Committee of Dentistry Faculty,
Hiroshima University, the Selection Committee sends a letter of formal admissions.
The departure of the selected students is conducted as soon as the administrative
process of passport and student visa to Japan is done.
4. The number of students sent to the IDC Program is one student every year as enacted in the
Memorandum of the Basic Agreement (2011). But, in the first year of the program (2011),
Universitas Airlangga got the quota of two students since University of Health Sciences,
Cambodia did not send its students.
5. The number of students accepted in the IDC Program in Hiroshima University can be
increased if Hiroshima University offers admissions with Indonesian state scholarships or
private scholarships.
Article 20
1. All student candidates accepted in Universitas Airlangga must register to the Directorate
of Education to meet the prevailing requirements;
2. Candidates are officially accepted as students by the Rector of Universitas Airlangga in an
academic ceremony.

CHAPTER IX
SEMESTER CREDIT SYSTEM ADMINISTRATION
Article 21
1. Each student who participate in academic process must meet the following requirements:
a. Free from academic sanctions,
b. Paying the cost of academic administration for new students,
c. Re-registering to the faculty in the beginning of each semester according to academic
calendar,
d. Having valid KTM (Student Identity Card).
2. Students who do not meet the requirements of paragraph 1 are not allowed to participate in
any academic and non-academic processes.
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
3. Students of the IDC credit transfer program exempt from the cost of the Education
Operational Donation (SOP) during their study (4 years) in Hiroshima University
4. Students of the IDC credit transfer program remain registered as students of the Faculty of
Dentistry, Universitas Airlangga during their study (4 years) in Hiroshima University
Article 22
1. To conduct academic activities and credit system administration in each semester, students
must do the following steps, namely:
a. Re-registering and paying Operational Donation Education (SOP). Failure to make the
payment of SOP can cause the students not able to fill out their Study Plan Card (KRS)
online through Cyber Campus;
b. The filling process of the Study Plan Card (KRS) online is conducted with the following
stages:
1. Confirm to the Academic Advisor to perform approval online in Cyber Campus;
2. Print the KRS and ask the signature of the Academic Advisor;
3. Submit the printed KRS that has been signed by the Academic Advisor along with
a copy of SOP payment receipt to the Sub-Unit Academic;
c. Participating in lectures, tutorials, practicum, and Community Service (KKN);
d. Filling out Revised Study Plan Card (KPRS) online in Cyber Campus effectively 2
(two) weeks after the class starts through the following stages:

2. Print the KPRS and ask the signature of the Academic Advisor;

2. In the beginning of each semester, after fulfilling the Administration requirements of
Universitas Airlangga, students are required to re-register to Sub-Unit Academic of Faculty
of Dentistry, Universitas Airlangga;
3. After re-registration in Sub-Unit Academic, new students will obtain:
a. The Prospectus of the Faculty of Dentistry, Universitas Airlangga containing general
guideline of Lectures, Practicum, Fieldworks (PKL), Skills lab, integrated Lecture and
Discussion, Small Group Discussion, Profession-Based Fieldwork;
b. Study Plan Card (KRS) that can be accessed online in Cyber Campus;
c. Achievement Plan Card (KRP) in triplicate: 1 (one) original for Sub Division of Student
Affairs, 1 (one) sheet for the student, 1 (one) sheet for the Academic Advisor.
4. After completing the administration in Sub-Unit Academic, old students must submit:
a. SOP payment receipt;
b. Printed Study Plan Card (KRS) that has been signed by the Academic Advisor
5. Students are entitled to participate in all academic activities listed in their Study Plan Card
(KRS) and use all facilities available with their valid Student Identity Card (KTM);
6. Students must adhere to the rules and regulations issued during teaching and learning
processes, including the rules and regulations of lecture, practicum, examination,
fieldwork, fieldwork-based profession, and others legitimate activities inside and outside
the campus.
Article 23
Students must re-register to get a valid Student Identity Card (KTM), and then:
1. Students under the guidance of Academic Advisors arrange their study plan with study load
in accordance with the latest Grade (IPS), and entry their study plan online to Cyber
Campus;
2. The Academic Advisors must approve the Study Plan Card (KRS) of the students online in
Cyber Campus and sign the printed KRS;

3. Students, whose Study Plan Card (KRS) has not been approved in Cyber Campus by the
Academic Advisors and do not submit the KRS at a preset time, are not allowed to
participate in curricular activities.
Article 24
1. Students who will change their study plan (course and and lab practicum) are given an
opportunity to revise their study plan effectively 2 (two) weeks after the class starts by
filling out Revised Study Plan Card (KPRS) form;
2. The Revised Study Plan Card (KPRS) must be approved by Academic Advisor;
3. Sub-Unit Academic will obtain the List of Final Score Participants (DPNA) based on the
Study Plan Card (KRS) and the Revised Study Plan Card (KPRS) of every student in Cyber
Campus;
4. The procedures for filling Study Plan Card (KRS) are in accordance with the Work
Instructions (IK) 001- S1 FKG.UA.12:
a. Study Plan Card (KRS) filled by student with the approval of academic advisor;
b. Revising of Study Plan Card (KRS) can be conducted by filling out Revised Study Plan
Card (KPRS) form online in Cyber Campus;
c. After filling out KRS/ KPRS/ practicum course form approved by the academic advisor
and the student, the KRS/ KPRS/ practicum course form must be copied in 2 (two) and
then signed by the student and the academic advisor, 1 sheet (original) for the Sub-Unit
Academic Academic, 1 sheet (photocopy) for the academic advisor, and 1 sheet
(photocopy) for the student;
d. All consequences of any errors during filling KRS/KPRS must become the
responsibility of the student.
Article 25
1. Mid Test (UTS) and Final Test (UAS) are conducted in accordance with prevailing
regulations;
2. The Course Coordinator is required to submit the results at least two days after the Final
Test (UAS);
3. The final score is in the form of the numerical value (digits);
4. The final score will be processed and disseminated through Cyber Campus.
Article 26

1. Final Assessment as Student’s Study Result can be based on:
a. Mid Test (UTS) and Final Test (UAS), and can be added with independent assignments,
quizzes or other methods of assessment;
b. The final assessment of Modules and Skills lab is arranged in a separate guide.
2. Assessment Score for Practicum/ Profession-Based Fieldwork/ Fieldwork is based on
Benchmark Reference Rate (PAP), which has been set;
3. Measurement of the study results is determined by GPA (IPK), which is the ratio /
multiplication function between credit scores and load scores obtained, and then divided
by the number of credits of the courses taken;
4. Study Results can be accessed through an electronic information system, Cyber Campus
program;
5. The score obtained is used as evaluation of the study;
Article 27
1. Each department in the Faculty of Dentistry, Universitas Airlangga must apply academic
management information system;
2. The academic management information system aims to facilitate communication and
monitoring process, and to accelerate accuracy of decision-making process in order to
improve the quality of education.
CHAPTER X
ACADEMIC CODES
Article 28
1. Students must attend lectures/tutorials, laboratorial practicum, skills labs, integrated
lectures and discussions, small class discussions, fieldwork, and Hospital Profession-Based
Fieldwork. During those activities, students are required to do the following provisions:
a. Arrive on time, attend until the end, and do not interfere;
b. Be polite;
c. Wear clean, tidy and polite (collared) clothes;
d. Wear shoes, unless not possible;
e. Do not eat, drink, and smoke;
f. Do not leave lectures/laboratorial practicum, except with the permission of the lecturer;
g. Do not activate electronic device (Mobile, Android, iPod, MP3 and others), unless
authorized lecturers / instructors;

h. Obey prevailing regulations at the place of learning activities.
2. Students are required to attend lectures/tutorials, laboratorial practicum, skills labs,
integrated lectures and discussions, small class discussions, fieldwork, and Hospital
Profession-Based Fieldwork in accordance with KRS (Study Plan Card) or KPRS (Revised
Study Plan Card) that has been made;
3. In any lecture and laboratorial practicum, students must sign attendance list;
4. Students must attend at least 75% of lectures/tutorials or 100% of laboratorial practicum,
skills labs, integrated lectures and discussions, small class discussions, fieldwork, and
Hospital Profession-Based Fieldwork;
5. At the same time, lecturer must check the attendance list provided and immediately hand
back to Sub-Academic Unit on the same day after the course is completed;
6. Students are not allowed to take examinations / practicum assessments of one subject (with
Blocked Status) if their attendance in the subject is less than 75% lectures/tutorials or 100%
of laboratorial practicum, skills labs, integrated lectures and discussions, small class
discussions, fieldwork, and Hospital Profession-Based Fieldwork without a legitimate
reason;
7. Legitimate reason of absence from lectures/tutorials, laboratorial practicum, skills labs,
integrated lectures and discussions, small class discussions, fieldwork, and Hospital
Profession-Based Fieldwork must be attached with the letter of assignment from the
University or Faculty if being assigned or sickness certificate from a doctor if sick
addressed to the Dean no less than one week;
8. Tolerance to paragraph (3) may be granted by the Dean after hearing the consideration of
the head of the Programme and the Course Coordinator of the subject.
Article 29
1. Students must attend the whole (100%) lectures/tutorials, laboratorial practicum, skills labs,
integrated lectures and discussions, small class discussions, fieldwork, and Hospital
Profession-Based Fieldwork that have been designed, and their presence is recorded in an
attendance list;
2. Students who do not attend lectures/tutorials, laboratorial practicum, skills labs, integrated
lectures and discussions, and Hospital Profession-Based Fieldwork must give a legitimate
reason attached with the letter of assignment from the University or Faculty if being
assigned or sickness certificate from a doctor if sick. Students are not allowed to participate
in Final Test (with Blocked Status) if their attendance in the lectures/tutorials is less than

100%. Students will not get any score if their attendance in the laboratorial practicum, the

Fieldwork is less than 100%.
3. Tolerance to paragraphs (1) and (2) may be granted by the Dean after hearing the
consideration of the head of the Programme and the Course Coordinator of the subject/the
laboratorial practicum, the skills labs, the integrated lectures and discussions, the small
class discussions, the fieldwork, and the Hospital Profession-Based Fieldwork;
4. Prior to laboratorial practicum, skills lab, integrated lectures and discussions, small class
discussions, and Hospital Profession-Based Fieldwork, students must learn the instructions
of the laboratorial practicum, the skills lab, the integrated lectures and discussions, the
small class discussions, and the Hospital Profession-Based Fieldwork as well as prepare
theories and equipment relating to the theme of the material;
5. Prior to laboratorial practicum, skills lab, integrated lectures and discussions, small class
discussions, fieldwork, and Hospital Profession-Based Fieldwork, lecturers who supervise
must give explanation about activities regarding the laboratorial practicum, the skills labs,
the integrated lectures and discussions, the small class discussions, the fieldwork, and the
Hospital Profession-Based Fieldwork;
6. During laboratorial practicum, skills lab, integrated lectures and discussions, small class
discussions, fieldwork, and Hospital Profession-Based Fieldwork, students must conduct
certain activities in individual or in group that have been designed;
7. After completed laboratorial practicum, skills lab, integrated lectures and discussions,
small class discussions, and Hospital Profession-Based Fieldwork, students must report the
activities to be assessed by the lecturer who supervises the laboratorial practicum, the skills
lab, the integrated lectures and discussions, the small class discussions, and the Hospital
Profession-Based Fieldwork.
Article 30
1. Mid Test (UTS) and Final Test (UAS) are conducted in every semester as scheduled on
academic calendar.
2. The schedule and the list of participants for the Final Test must be announced by Sub-
Academic Unit one week before the test;
3. Students whose attendance is less than 75% are not allowed to participate in the Final Test
(with Blocked Status) and get a score of E;
4. Students must be present in the test room ten minutes before the test begins;

5. Students who are late for 10 minutes with an accepted reason are allowed to participate the
test without any extension of time;
6. Students must sign the attendance list provided;
7. Students must show their valid Student Identity Card (KTM);
8. During the test, students are required to work calmly, honestly and independently;
9. Students who are convicted of cheating during the test are excluded from the test room and
given a score of E as well as academic sanctions;
10. Students who do not participate in the test without a legitimate reason are considered to
have participated in the test with a score of E.
Article 31
1. During the test, every participant are required to:
a. Obey all prevailing rules and regulations during the test;
b. Obey technical guidance of the test delivered by invigilator;
c. Request approval of the invigilator in advance, before leaving the seat or the test room;
d. Submit the answer sheets to the invigilator before leaving the test room.
2. During the test, every participant are prohibited from:
a. Working together with the other participants in completing the tasks of the test;
b. Citing the answers of the other participants, or providing an opportunity for the other
participants to quote his/her answers;
c. Using notes, books, or other sources of information during the test, except as permitted
by the lecturer;
d. Disturbing the test;
e. Communicating with another participant without the permission of the invigilator;
f. Activating mobile phones and other electronic communication devices
Article 32
1. Students are prohibited from replacing their position or conducting academic and
professional education for the benefit of other students;
2. Students who engage in paragraph (1), either replacing or being replaced, are imposed
academic sanctions.
Article 33
1. Lecturer and invigilator have an authority to:

a. Arrange seat for every participant and check the attendance of the participants;
b. Determine what equipment may be used by the participants during the test;
c. Reject the presence of someone who is not served as a invigilator or as a participant,
and instruct him/her to leave the test room;
d. Report any academic dishonesty occurred in the Form of Exam Report.
2. Course Coordinator is responsible for the process of the test and the appointment of the
invigilator.
Article 34
Invigilator appointed by the Course Coordinator has an authority to impose sanctions on the
participants violating the regulations as stated in Article 35 paragraph (2), in the form of
proposing score E in their assessment to the Course Coordinator.
CHAPTER XI
EXAMINATION
Article 35
1. The objectives of conducting examination are:
a. To assess how students have understood or mastered competencies that must be
achieved in a course;
b. To classify students into categories based on their ability to achieve certain
competences in the Faculty of Dentistry, Universitas Airlangga (FKG Unair), namely
A, AB, B, BC, C, D, and E.
c. To classify students into categories based on their ability to achieve certain
competences in IDC program, namely S, A, B and C.
2. Examination can be held in the forms of written test (essay or objective test), oral test,
seminar/Final Scientific Assignment, or any other assessments. On the other hand,
practicum test can be conducted in the form of practice accompanied by written or oral test.
However, the types of the test must be in accordance with the objectives of the course, so
carried out in a certain range;
3. The types of assessment usually conducted in the regular program in FKG Unair are Mid
Test (UTS), Final Test (UAS), quiz, and self-assignment. The percentage of those
assessments is 30% for UTS, 40% for UAS, 10% for quiz, and 20% for self-assignment.
Meanwhile, the types of assessment usually conducted in IDC program are UAS, quiz, and

self-assignment Mandiri. The percentage of those assessments is 40% for UAS, 10% for
quiz, and 20% for self-assignment (40%)
4. If there is any type of test (UTS, UAS, quiz, self-assignment) not conducted or the Course
Coordinator conduct the test in another form that can be justified, the percentage of the
assessments then will be determined later by the Course Coordinator;
5. The assessment system of integrated lectures, skills lab, and Hospital Profession-Based
Fieldwork will be regulated in a separate manual;
6. Remedial Test must be participated by students obligated to join the test as announced by
Sub-Academic Unit;
7. Students obligated to join the Remedial Test are those who got the score of D or E (without
Blocked Status) for any Lecture class, preclinical practicum, integrated lecture and
discussion, and skills lab;
8. Students allowed to get the Remedial Test are those who got the score of B or C (without
Blocked Status) for any Lecture class, preclinical practicum, integrated lecture and
discussion, and skills lab;
9. Students can improve their score by taking the First Remedial Test after the Final Test
(UAS) in the semester (on going) as the time limit has not been exceeded. The final score
used is the best one with the score of B as the highest one;
10. Under certain conditions, the Second Remedial Test can be conducted in the semester (on
going) with the score of B as the highest one;
11. If necessary, the Dean will decide to conduct the Third Remedial Test with a maximum
score of B;
12. Students who fail to achieve the minimum passing limit score in the Remedial Test are
allowed to participate in Short Semester held during transition period in between semesters
by filling Study Plan Card (KRS) of the Short Semester;
13. Short Semester can be enrolled by students who have not taken any subject offered (first
taker), students with grades E, and students who are failed the Remedial Test;
14. The maximum number of credits that can be taken in the Short Semester is five credits;
15. Students can improve their score by retaking the course in their Study Plan Card (KRS) in
next regular semester or Short Semester as the time limit has not been exceeded. The final
score used is the best one with the score of A as the highest one;
16. Examination is conducted in accordance with the academic calendar. Students are allowed
to take the examination if the student has attended at least 75% of lectures and 100% of
practicum, skills lab, integrated lecture and discussion unless a dispensation from the Dean;

17. The Dean’s Dispensation can be given to students who are assigned to carry out state,
university, or faculty duties;
18. Maximum dispensation is five working days, and the student given the dispensation is given
additional task;

INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. Assessment in the Faculty of Dentistry at Hiroshima University expressed with letters S,
A, B, and C:
2. Score Improvement:
a. Score Improvement is only conducted for students within the scope of sub-program
(Sub-Program of Basic Dentistry and Sub-Program of Undergraduate Dentist
Education) by taking Remedial Test (during the semester), and/or Short Semester
before graduate candidacy evaluation;
b. During improving their score, the students will not be included into graduate candidacy
evaluation;
c. The score used as the final one is the highest score.
Article 36
1. Evaluation of the study results is conducted in the end of every semester for courses
programmed by the students as stated in their Grade (IPS);
Letters Numerical Score S 90 - 100 point A 80 - 89 point B 70 - 79 point C 60 - 69 point

2. The first evaluation of the study results is conducted in the end of the semester 4 (four)
started from the time they are enrolled as students for the first time, and announced through
Graduate Candidacy Evaluation I of Sub-Program of Basic Medical and Dental Sciences;
3. Evaluation for students of Undergraduate Dentist Education Program of Dentistry Faculty,
Universitas Airlangga is conducted after they have completed the entire program and met
the requirements of Graduate Candidacy Evaluation as follows:
a. Having completed 144 credits;
b. No score E for all courses;
c. No score D for all courses, except for the General Compulsory Courses (MKWU);
d. No score D and E for practicum;
e. The minimum score of B for Integrated Lecture and Discussion, Small Class
Discussion, Skills lab;
f. Final Scientific Assignment score > B;
g. Grade Point Average (IPK) is determined based on Decree of National Education
Minister of the Republic of Indonesia No. 232/U/2000 on the Guidelines for the
Development of Higher Education Curriculum and the Assessment of Student Learning
Outcomes, as stated in Article 13, as follows:
1. Requirements for graduation of education program are set based on the required
number of credits and the minimum of Grade Point Average (IPK);
2. Higher Education Institution sets number of credits that must be taken as referred
to in paragraph (1) based on the range of study load for each program, as enacted
in Article 5, Article 6 and Article 7.
h. The minimum of GPA (IPK) as referred to in paragraph (1) that is determined by Higher
Education Institution, equal to or higher than 2.00;
i. ELPT score > 450;
j. Statement of Acceptance from one of National Journals stating that his/her scientific
article has been accepted by the Editorial Board.
4. Evaluation for students of the IDC Program is conducted after they have completed the
entire program and met the requirements of Graduate Candidacy Evaluation to have
completed 206 credits;
5. If students have not passed the Graduate Candidacy Evaluation of the Undergraduate
Dentist Education Program, they cannot continue to Dentist Professional Study Program;

6. Evaluation for students of Dentist Professional Study Program is conducted after they have
completed the entire program and met the requirements of Graduate Candidacy Evaluation
as follows:
a. Having completed 33 credits;
b. Having taken hospital fieldwork-based profession with the minimum score of B and
GPA (IPK) > 3.00.
7. After completing the Undergraduate Dentist Education Program, students will receive a
Bachelor’s degree in Dentistry;
8. After completing the Dentist Professional Program, students will get a profession title,
namely Dentist (drg).
Article 37
Referring to Decree of National Education Minister of the Republic of Indonesia No.
232/U/2000 on the Guidelines for the Development of Higher Education Curriculum and the
Assessment of Student Learning Outcomes as also enacted in article 15, the Assessment of
Student Learning Outcomes is as follows:
1. Graduation consists of three levels, namely satisfactory, very satisfactory, and cumlaude,
which is stated on academic transcript;
2. GPA (IPK) is used as the basis for determining the level of graduation in undergraduate
and diploma programs, namely:
a. GPA 2.00 to 2.75: satisfactory;
b. GPA 2.76 - 3:50: very satisfactory;
c. 3:51 GPA - 4.00: cumlaude
3. Predicate “Cumlaude” for the undergraduate level of the IDC Program is given to students
whose study period is not more than six (6) years.
Article 38
1. If during their study, students have ever legally permitted for academic leave, in the future
then it will not be considered in determining the time limit of their study;
2. Students will be considered failed in their study if they cannot complete the education
program within the maximum time limit as enacted in Article 17 and 28;
3. Decision of failed study is stipulated by Decree of the Rector after receiving a proposal
from the Dean.

Article 39
1. Academic Leave is a condition when students are legally permitted to not follow the
academic activities;
2. Academic Leave is only granted to students who have been studying for four consecutive
semesters;
3. During their study, students are allowed to take a maximum of two inconsecutive semesters
of Academic Leave;
4. During Academic Leave, students still must be registered;
5. The period of Academic Leave is not considered in the evaluation of learning;
6. Requests for Academic Leave must be proposed at least one semester before.
Article 40
1. In the end of the program, graduation is conducted;
2. Graduates participating in graduation will receive Graduation Certificate and Academic
Certificate;
CHAPTER XII
ACADEMIC CALENDAR
Article 41
1. Academic Calendar inform the overall schedules of learning process activities
arranged in one academic year;
2. Academic Calendar aims to organize learning activities effectively and efficiently;
3. Academic Calendar contains the schedule of several activities, namely (a) the period of
registration and re-registration; (B) the period of filling Study Plan Card (KRS) and
Revised Study Plan Card (KPRS); (C) lecture, practicum, hospital fieldwork-based
professions, fieldwork, skills labs, and examination; (D) Community Service (KKN); and
(E) other supporting academic activities;
INTERNATIONAL DENTAL COURSE CREDIT TRANSFER PROGRAM
1. Academic Calendar of the IDC credit transfer program started in even semester in
Hiroshima University begins in October.

CHAPTER XIII
ACADEMIC DISHONESTY
Article 42
Students are prohibited from engaging in the following activities:
a. Cheating: acts committed by students consciously (deliberately) using or attempting to use
information materials or tools without the permission of the invigilator;
b. Falsifying: acts committed by students intentionally to change or modify the value or
academic transcripts, academic certificate, student identity card, tasks of lecture / tutorial /
practicum / test, report, or signature within the scope of academic activities;
c. Plagiarism: actions undertaken by students deliberately to use the phrase, data or other
people's work as their own work (without mentioning the original source) in an academic
activity;
d. Gratifying and/or threatening: acts committed by students to influence or attempt to
influence another person with the intent of affecting the assessment of academic
achievement;
e. Taking the place of others in academic activities: acts committed by students to replace the
position or perform a task or activity for the benefit of others on their own will;
f. Having others take the place in academic activities, namely acts committed by students by
asking someone else to take their place or perform a task or activity for their own interests
or the interests of others;
g. Cooperating with others without permission during the test either orally, by gestures, or
through an electronic device;
h. Taking the question sheet of the test without permission;
i. Duplicating the question sheet of the test.
Article 43
All perpetrators of such acts in Article 44 must get the following academic sanctions:
a. Oral or Written Extreme Caution;
b. Cancellation for scores of the course test or the academic activity in question;
c. Fail in the Course or academic activity in question;

d. Fail in all Subjects in the semester in progress;
e. Forbidden to participate in academic activities at a certain time;
f. Dismissal or removed from Universitas Airlangga.
Article 44
1. The provision of such sanctions as stated in Article 47 letters a, b, c and d are enacted by
the Dean considering the proposal of the Course Coordinator (PJMK) approved by the
Head of the Department;
2. The provision of such sanctions as stated in Article 47 letters e and f are enacted by the
Rector of Universitas Airlangga considering the proposal of the Dean of the Faculty;
3. Methods of imposing the sanctions will be regulated in Procedure Manual.
Article 45
1. The Dean gives sanctions to students as enacted in Article 47 paragraph 1, with the
following procedures:
a. Report from the Course Coordinator and the Head of the Department is made in writing
to the Dean accompanied by the minutes;
b. Examination as well as recommendation regarding sanctions for perpetrators is
performed by the Faculty Executive Board, the Head of the Department, and the Course
Coordinator;
2. The Rector gives sanctions to students as enacted in Article 47 paragraph 2, with the
following procedures:
a. Report from the Course Coordinator and the Head of the Department is made in writing
to the Dean accompanied by the minutes;
b. Examination of the report in paragraph (2a) is conducted, and the Minutes is made;
c. Examination as well as recommendation regarding sanctions for perpetrators is
performed by the Dean;
d. Based on the recommendation of the Academic Disciplinary Commission of the
Faculty, the Dean proposes sanctions to the Rector.

CHAPTER XIV
GRADUATE CANDIDACY EVALUATTION AND GRADUATION
Article 46
1. The Faculty must conduct graduate candidacy evaluation meeting;
2. Graduate candidacy evaluation meeting must be attended by all meeting participants that
portion and total are determined under the provisions of the Faculty;
3. Graduate candidacy evaluation meeting for each study program must be conducted at least
once a semester, and held at least one month prior to the graduation ceremony;
4. The requirements of graduating are set by the Faculty.
Article 47
1. Every student who has passed graduate candidacy evaluation is allowed to participate in
graduation;
2. Graduates participating in graduation will receive Graduation Certificate and Academic
Certificate.
CHAPTER XV
REPLACEMENT OF ACADEMIC CERTIFICATE AND
STUDENT IDENTITY CARD (KTM)
Article 48
1. Statement of Replacement for lost or damaged academic certificate is issued if the original
certificate is lost or damaged;
2. Statement of Improvement for mistyped academic certificate is issued if there are
typography errors on it;
3. Statement of Replacement for lost or damaged academic certificate is issued by the Rector
upon the request of the Dean.
4. Statement of Improvement for mistyped academic certificate is issued by the Rector upon
the request of the Dean.
Article 49
1. Replacement of KTM is issued if the original KTM is lost or damaged;

2. The replacement of KTM is issued by the Directorate of Student Affairs upon the request
of the Dean.
3. Extension of KTM is issued if the study period has already been expired, and the students
are given an opportunity to extend a maximum of two semesters of study.
4. The extension of KTM is issued by the Directorate of Student Affairs upon the request of
the Dean.
CHAPTER XVI
REGULATORY CHANGES OF EDUCATION
Article 50
The Dean, after obtaining approval of the Faculty Advisory Board, can propose changes of the
Education Regulation to the Rector of Universitas Airlangga.
CHAPTER XVII
ACADEMIC QUALITY ASSURANCE
Article 51
1. Each study program in the Faculty of Dentistry, Universitas Airlangga must conduct
Academic Quality Assurance;
2. Academic Quality Assurance includes several dimensions related to the quality of higher
education, namely input, process, output, and impact;
3. Academic Quality Assurance is conducted through several stages, namely planning,
monitoring, internal audit, self-evaluation, and quality control for sustainable
improvement;
4. Each study program must have and execute quality assurance documents, namely Study
Program Specification, Academic Policy, Academic Standards, Academic Regulations,
and Implementation Procedure Manual.

CHAPTER XVIII
TRANSITIONAL PROVISIONS
Article 52
1. All the academic rights and obligations of students that have already met before this
regulation is into force are continually be recognized and declared as the valid ones;
2. All the academic rights and obligations of students that have not met and been contrast to
this regulation are justified and resolved case by case by Decree of the Dean or the Rector;
3. All provisions enacted as the Education Regulation or equivalent with this regulation are
still applied as complementary regulations as long as in accordance with the Education
Regulation.
