Long Distance Caregiving: Tips for Success

Fairfax County Services for Older Adults

If you find yourself in the long-distance caregiving role this holiday season, check out the following tips excerpted from a National Institute on Aging web posting.

Find the complete article and more on caregiving at www.nia.nih.gov/health/caregiving. You can also check out the county’s recent webinars on various aspects of caregiving at www.fairfaxcounty.gov/OlderAdults (scroll down to Services and then Caregiver Support).

Know What You Need to KnowLearn as much as you can about your family member or friend’s illness, medicines, and resources that might be available. Information can help you anticipate the course of an illness, prevent crises, and assist in healthcare management.

Plan Your VisitsYou can get more done and feel less stressed by talking to your family member or friend ahead of time and finding out what he or she would like to do. Also, check with the primary caregiver, if appropriate, to learn what he or she needs.

Remember to Actually Spend Time Visiting with Your Family MemberTry to make time to do things unrelated to being a caregiver like going to a movie or visiting with friends.

Get in Touch, and Stay in TouchMany families schedule conference calls with doctors, the assisted living facility team, or nursing home staff to keep everyone on the same page.

As we approach the holiday season, everyone starts focusing on what gifts to buy for family members and close friends. Often, people may get carried away and feel that they need to purchase extravagant gifts. Luckily, this type of gifting only happens once a year.

Many of the clients that I meet with are “adamant” about leaving an inheritance for their children, or making lifetime gifts, even if it is a detriment to their own future well-being. Because of this attitude, critical mistakes can and will be made which may affect the ability to receive long-term care needs covered by government benefits. The primary source of coverage for custodial long-term support and services, either in a nursing home or in one’s own home, for individuals with limited income and resources is Medicaid.

However, be aware, the Medicaid application includes the following question: “Have you or your spouse sold, transferred, placed in a trust/annuity, or given away any resources, such as your home or other real property, cash, bank accounts, or cars in the last sixty (60) months (5 years)?”

If the answer is “yes” to any part of this question, an applicant will incur a penalty period if that individual is otherwise Medicaid eligible. The penalty period can be a substantial number of months of ineligibility and the applicant will not have the resources to pay for private pay until the penalty period is over.

Many individuals are familiar with the phrase “annual exclusion.” An annual exclusion gift is simply a gift that qualifies for the annual exclusion

from federal gift taxes under IRS regulations. The current amount is $15,000 per person.

When Medicaid applicants are completing their applications and try to answer the question stated above, they often believe that if they have made a gift within the annual exclusion, that particular gift is not considered a disqualifying transfer. However, they are wrong. Although allowed under IRS regulations, gifting within the “look-back” period of 60 months is not allowed under Medicaid regulations.

Therefore, the gifting can create quite a dilemma. What is also very important to note is that there are certain transactions that may not “appear” to be gifts but are considered gifts under Medicaid regulations.

Here is a list of some actions that may not seem like obvious gifts but in fact are gifts:

• Putting someone else’s name (i.e. child) on your deed or your bank account as a joint owner;

• Selling/renting property that you own at less than the fair market value;

• Making a loan which is either not in writing or where you have cancelled the loan; and

• Having routinely/regularly made payments for another person’s needs (i.e. rent, utilities, food, car payments, health insurance, medical costs etc.).

Sometimes Gifting Is Not Such a Good Idea

Are there instances in which older individuals should not be making gifts of liquid assets (i.e. cash) or other types of real or tangible personal property to family members or others?

The answer is an emphatic–Yes!

According to Fairfax Water, more and more utility customers are falling victim to a nationwide bill paying scam. To increase awareness of this scam, especially as it relates to Fairfax Water, below are typical situations and what to do if someone shows up on your doorstep or calls claiming to be from Fairfax Water.

• If you receive a call offering “discounts” or

requesting account information or offering to pay your bill—this is a scam. Fairfax Water does not call asking for your account information nor do we offer discounts for service.

• Fairfax Water employees have photo identification with them at all times while on the job. Vehicles and field staff’s clothing bear their logo (pictured lower right). During the

summer months there may be Fairfax Water summer interns in your neighborhood who do not wear uniforms, however each one will have a Fairfax Water photo identification displayed prominently.

• If someone comes to your door and identifies themselves as a Fairfax Water employee, please ask for identification or call 703-698-5800 to verify before letting them into your home.

• Fairfax Water employees do not receive or accept any form of payments during service calls. They do not collect water-service fees door-to-door.

• Most water meters are located outside of a home. Therefore, Fairfax Water employees typically do not require access to a customer’s home to read a meter.

If you have any questions about Fairfax Water service or its employees, call 703-698-5800, TTY 711.

Happy Holidays Useful Shopping Tips from the FTCThe FTC has an early gift for you—tips to help you shop wisely and protect your personal information.

• Make a list and a budget. Remember incidentals, like cards and wrapping paper.

• Check out websites that compare prices for things sold online, and at stores in your area. It can help you get the best deal.

• Make sure the scanned price is right. Overcharges cost you money and time, especially if you don’t notice them right away.

• Look for rebates. Some can be redeemed at checkout, but most require you to send documentation to the manufacturer to get your rebate.

• Read reviews and recommendations to see how the product performs — or people’s experience with an online seller. But remember to consider the source.

• Be aware that shopping apps can collect a lot of personal information. Look for apps that tell you what they do with your data, and how they keep it secure.

• Check the terms of the deal when buying online — including delivery dates and refund policies. Will it arrive in time? And, if it’s not what you thought, can you send it back?

• Save receipts. When you’re shopping online, keep copies of your order number, the refund and return policies, shipping costs and warranties.

• Giving bling? Take some time to learn the terms used in the jewelry industry so you can get the best quality and value.

• Shipping to loved ones overseas? Check the U.S. Postal Service’s calendar for holiday shipping deadlines.

• Have packages delivered to a secure location. If you won’t be home, have them delivered to work, or see if a neighbor can be on the look-out for deliveries.

• Consider requiring a signature for delivery— or look for options to pick up your shipment at a local store or mailing center. For more information, check out our Shopping

and Saving page at www.consumer.ftc.gov/topics/shopping-saving.

You’re in the parking lot of a store or just walking out of the door at home with a million things on your mind and then it

happens; you close the door and instantly realize your keys are on the wrong side of the door. I can’t think of many situations more unnerving. We’ve all done it. I’ve done it.

So now you grab your phone and search for locksmiths and become overwhelmed by the number of responses. You check the number of stars, read the reviews and finally make your choice. Here is what you need to know before ordering their service: • Are they licensed in your state? There are a lot of

dishonest people posing as locksmiths.

What to Do When You’re Locked Outby Dave Newhouse, Certified Locksmith

• What is their price? Real locksmith equipment and know how isn’t cheap and a $15 service fee usually signals a bait & switch tactic.

• When will they arrive? You shouldn’t have to wait more than an hour unless you’re in a remote location.

• How will they open your door? Options are pick, bypass, or drill. A good locksmith will inspect the property for the easiest and most non-evasive way in.

• What if they can’t open your door? Will you still be charged?

Once they dispatch a locksmith you’re on the hook for their service fees, even if you get in on your own, so don’t call multiple locksmiths to come or you’ll owe them all a service fee. Cancel immediately if you get in by yourself before the locksmith arrives.

Once they arrive insist on seeing their ID and locksmith license before they start. They should ask for your ID and something that proves you belong in the house or the car such as a registration or a piece of mail with your name and address.

You don’t want to give bad guys permission to get into your vehicle or house, and, it’s unethical

and possibly illegal for a locksmith to gain entry for the wrong person. Once both are convinced that the other is legit, the locksmith has several ways to gain entry to your property.

Opening a vehicleSome locksmiths can pick the door lock with special, expensive tools. Others can make a custom key using the VIN number that can be seen through the windshield. The most common way to unlock a vehicle is using an airbag to wedge the passenger side door open wide enough to slip in a long reach tool to grab the door handle, hit the unlock button, or even pick the keys up and fish them back through the opening.

One time, I was called to work on a car with a dead battery and couldn’t get anything to unlock. Then I noticed a small handle that opened the trunk next to the driver’s seat that I was able to grab and yank with my long reach tool. I easily laid the rear seat-back down from inside of the trunk and was able to climb in and unlock a rear door.

Houses are differentIn most cases there’s an entry door knob or lever that has to be picked open using professional lock picks, an electric lock pick, or by using a special key to bump the lock open. The old credit card trick has also worked several times for me. Some locks just won’t pick so drilling and turning the lock cylinder open with a screw driver is the final option, which means more expense as the cylinder or even the whole lock needs to be replaced.

It can take a fair amount of concentration and time to open a lock so stay close by without hovering and asking, “how much longer?”

Here are a couple of tricks that you can use to prevent the embarrassment and expense of locking yourself out:• Get an all metal key made that only unlocks

Locked Out

your vehicle door and duct tape it to the back of your license plate. It’s out of the weather and easy to access by simply removing the screws that hold the plate on.

• Install a residential combination lock on one of your doors.

• Get a locking key box with a combination and bolt it to a deck post, in a shed or some other inconspicuous place. They sell for around $30 at the home Improvement stores.

• And last, do your research now to find a “reputable” locksmith and put his number in your contacts.

If there is one thing I’ve learned during my culinary travels around the world is that no matter how much I think I know, I continue to be surprised each time I share the kitchen with local cooks. Knowledge abounds and it’s fascinating how each culture uses the same old ingredients in incredibly innovative ways. It’s a journey where the process of learning never, ever stops.

Take this recipe for example. It should come as no surprise that pork and prunes are a match made in heaven. In a cooking class in Bologna, I learned an interesting technique for stuffing pork tenderloins.

Have ever looked at your knife block and thought you really ought to have a much closer relationship with your steel sharpening rod? Perhaps not you, but certainly your knives?

In this recipe the steel sharpening rod is used

as an effective tool with which to skewer the pork tenderloins so that plump prunes—soaking in marsala—can be stuffed through the entire length of tenderloins. Ingenious, isn’t it?

This recipe comes together quickly for a weeknight meal or an elegant dinner party. With the holidays nipping at our heels, it’s a boon that it presents as though you’ve been working in the kitchen all day. In reality it’s a no fuss, no muss recipe.

I recommend using

sweet Marsala wine to soak the prunes. The prunes stuffed tenderloins are roasted in a hot oven. Shallots, prunes, a splash of marsala wine and cream come together in a delicious sauce.

A simple green salad and a lovely bottle of wine is all that’s needed for a lovely dinner. Enjoy!

Roasted Pork Tenderloins. . . continued from page 12

Slice prunes in half. Soak in 1/2 cup Marsala for at least one hour.Peel and dice shallots very fine.

Method for Cooking Pork Tenderloins1. Cut each tenderloin in half for a total of four

logs of meat.2. Using the knife sharpening rod, bore a hole

through the center of the meat log to protrude out of the other end.

3. Stuff each tenderloin piece with 5-6 prunes, stuffing from both ends. Liberally rub salt and pepper all over the tenderloins.

4. Heat a cast iron pan on medium-high then add olive oil and heat until very hot. Sear the tenderloins in batches on all sides until golden brown.

5. Place a kitchen towel on the handle if it’s too hot to touch and place the pan in hot oven.

6. Roast the meat for about 15 minutes or until the internal temperature registers at 140 degrees.

7. Insert the thermometer parallel into the meaty section and not in the center where the prunes are stuffed.

8. Remove the meat from the pan to a platter, cover with foil and set aside.

9. Add the olive oil to the cast iron pan and heat on medium-high. Add shallots, 1/2 tsp., salt and sauté for 7-10 minutes until softened.

10. Add the thyme leaves, prunes and any remaining Marsala. Simmer for 5 minutes and add the stock.

11. Bring to a simmer and allow to evaporate and slightly thicken for another 5 minutes. Add salt and pepper and taste and adjust seasonings. Add the butter and whisk until the sauce slightly thickens.

12. Place the meat on the cutting board and slice at a slight angle into 1/4” thick slices. Place attractively on a platter in two rows and drizzle the sauce down the center and serve additional sauce on the side.
