The cranium and its constituents (blood, CSF, and brain tissue) create a state of volume equilibrium, such that any increase in volume of one of the cranial constituents must be compensated by a decrease in volume of another.” Organ otak dilindungi oleh tulang cranium dari trauma langsung dan semua pengaruh kekuatan dari luar. Sifat tulang kranium yang rigid membuat volumenya konstan, sehingga semua komponen volume intracranial menyesuaikan secara seimbang volume intracranial yang konstan. Doktrin Monro-‐Kellie-‐Burrows.
A. Dalam kondisi TIK normal. Komponen utama intracranial adalah 80% otak, darah arterial-‐vena (10%) dan cairan serebrospinal (10%). Cranium adalah bejana kaku, volume intracranial selalu konstan, dan volume normal intracranial menunjukkan TIK dengan rentang nilai (10-‐15mmHg).
B. Massa intracranial dengan kompensasi (TIK normal). Pada pasien ini terdapat massa intracranial (Space occupying lesion) berukuran sedang. Karena
volume intracranial konstan, peningkatan volume akibat massa menyebabkan kompensasi berkurangnya isi intracranial. Volume vena berkurang melalui pengeluaran vena dari intracranial ke dalam vena-‐vena jugularis. Volume cairan serebrospinal keluar melalui foramen magnum ke dalam kanalis spinalis. Otak sendiri hampir tidak dapat dikurangi volumenya, sehingga tidak terdapat perubahan volume otak yang signifikan terjadi, tidak pula perubahan pada volume arterial. Volume intracranial konstan, dan tidak ada peningkatan TIK (tekanan dikompensasi)
C. Massa intracranial dengan dekompensasi dan peningkatan TIK. Ukuran massa lebih besar, di balik pengkompensasian tekanan kapasista darah vena dan LCS, ada peningkatan TIK.
Fisiologi Aliran Darah Otak Pembuluh darah otak memiliki struktur yang besar dengan vasokonstriksi yang minimal. Pembuluh arteriol memiliki autoregulasi maksimal, serta berinteraksi dengan regulasi TIK. Mitokondria dalam endotel pembuluh darah otak berlebih. Ditemukan pula reseptor endotel untuk neurotransmitter.
Endotel Otak tidak berfenestrasi dan dihubungkan dengan taut-‐erat. Endotel ini menghasilkan vasodilator kuat seperti nitric oxide (NO), prostacyclin, carbon monoxide dan the endothelium derived hyperpolarizing factor, serta vasokonstrictor, seperti endothelin and endothelium-‐derived constrictor factor.
Autoregulasi adalah kemampuan intrinsik pembuluh darah otak untuk menjaga aliran darah serebral relative konstant terhadap tekanan perfusi serebral. Kondisi ini bertujuan mempertahankan nutrisi dan perfusi otak. Kemampuan otak mempertahankan perfusi pada MAP antara 60 – 150 mmHg.

Neurons and astrocytes are inclose proximity and arefunctionally coupled tosmooth muscle cells andendothelial cells.Theirinteraction in the normalstate163 and their coordinatedresponse to injury has led tothe concept that these cellsconstitute a functional unit,termed the neurovascularunit150,164,165.
Large cerebral arteriesbranch into smaller arteriesand arterioles that run alongthe surface of the brain (pialarteries)166.These consist of anendothelial cell layer, a smoothmuscle cell layer and an outerlayer of leptomeningeal cells,termed adventitia, which isseparated from the brain bythe Virchow-Robin space167. Asthe arterioles penetrate deeperinto the brain, this spacedisappears and the vascularbasement membrane comesinto direct contact with theastrocytic end-feet(intracerebral arterioles andcapillaries).
Cerebral endothelial cells are unique in that they are not fenestrated and are interconnected by focal adhesions,known as tight junctions.These morphological features constitute the blood–brain barrier. Endothelial cellsproduce powerful vasodilators69, such as nitric oxide (NO), prostacyclin, carbon monoxide and the endothelium-derived hyperpolarizing factor, and vasoconstrictors, such as endothelin and the endothelium-derived constrictorfactor69,168. Endothelial reactive oxygen species act as vasodilators at low concentrations, but at high concentrationsthey cause vascular dysregulation69 (see main text). Endothelial vasoactive substances are released by agonists thatactivate specific receptors, or by changes in shear stress at the cell surface produced by changes in the rate of bloodflow67. Gap junctions permit intracellular responses to be transmitted to adjacent endothelial cells169.
Smooth muscle cells and pericytes convert the chemical signals that originate from endothelial cells, neurons andastrocytes into changes in vascular diameter.These signals constrict or relax smooth muscle cells by inducingchanges in concentrations of intracellular Ca2+ and altering the phosphorylation state of light chain myosin170.Smooth muscle cells also respond to changes in intravascular pressure, constricting if it increases171.This propertyallows smooth muscle cells to counter changes in the rate of flow that are produced by increases in intravascularpressure, and underlies cerebrovascular autoregulation. Furthermore, smooth muscle cells are linked to each otherthrough gap junctions, a feature that mediates the intramural propagation of vascular signals172,173.
Astrocytic end-feet almost completely surround intraparenchymal blood vessels166,174.They are enriched in K+
channels, purinergic P2Y receptors and the water-channel protein aquaporin-4 (REF. 175), indicating key roles ingliovascular signalling and the regulation of brain water permeability. In addition, astrocytes are involved inneuronal energy metabolism176 and synaptic function177. Glutamate and GABA (γ-aminobutyric acid) released fromneurons initiate calcium waves in astrocytes that travel for several hundred micrometres53, and induce perivascularrelease of vasoactive agents that participate in neurovascular signalling (see main text).Neuronal processes are closely associated with cerebral blood vessels. Pial arteries are densely innervated by
perivascular nerves that originate from autonomic and sensory ganglia and contain many vasodilators (NO,acetylcholine, vasoactive intestinal polypeptide (VIP), calcitonin gene-related peptide (CGRP), substance P andcholecystokinin neurokinin A) and vasoconstrictors (noradrenaline, neuropeptide Y (NPY) and serotonin)178.Intracerebral arterioles and capillaries are contacted by neural processes that originate from local interneurons orfrom central pathways (intrinsic neurons).These processes contain many neurotransmitters178 (see also FIG. 3).
So perivascular neurons, astrocytes and vascular cells constitute a functional unit, the main goal of which is to protectthe brain by maintaining the homeostasis of the cerebral microenvironment.The neurovascular unit also provides afirst line of defense against the deleterious effects of cerebral ischaemia and other forms of brain injury150,165.

Blood brain barrier
Memiliki lapisan endothelial dengan tight junctions, non-‐venestrated transendotel, transport transvesikel minimal. Area non-‐BBB berfungsi sebagai sekresi dengen vaskulerisasi yang tinggi.
Perbedaan kapiler somatic dengan kapiler otak. Kapiler somatic memiliki fenestrasi antar sel-‐se endotel dan membiarkan aliran komponen plasma bebas. Selain itu, terdapat aliran bulk komponen plasma menyebrang sel endotel via vesikel pinositotik. Kapiler otak, sel-‐sel endotelialnya terhubung taut erat. Tidak terdapat fenestra yang menghubungkan. Beberapa komponen plasma menyebrang endotel bila bersifat larut-‐lemak; lainnya, semisal asam amino, dan gula ditransportasikan melalui mediator pembawa menyebrangi endothelial. Jumlah mitokondria yang besar pada endothelial menghasilkan energi yang cukup untuk transport aktif. Cerebral Edema Terdapat 4 mekanisme edema pada otak
1. Edema Vasogenik, mekanismenya melalui gangguan mekanik, blood brain barrier terganggu, permeabilitasnya meningkat. Plasma darah berkumpul di ruangan interstisial, bersifat hyperosmolar. TIK meningkat, dan sering ditemukan. Terapinya dengan perbaikan mekanik blood brain barrier.
2. Edema sitotoksik, mekanismenya akibat gangguan metabolisme intrasel, hipoksia, dan intoksikasi air. Terapinya dengan metabolisme sel diperbaiki. Terjadi hipoksia, gangguan temperature, asidosis radikal bebas, menyebabkan disfungsi NaKATPase Pump. Natrium intrasel berlebihan, gangguan osmotic, cairan intrasel berlebihan.
3. Edema interstitial, mekanismenya melalui obstruksi cairan serebrospinal. Terapinya dengan perbaikan aliran darah. Obstruksi LCS menyebabkan hidrosefalus obstruktif, aliran LCS transependim retrograde dari ventrikel, transudasi di interstisial.
4. Edema osmotic akibat cairan LCS dan cairan ekstrasel oak osmolalitasnya sedikit lebih rendah dari plasma. Misalnya kondisi hiponatremia/intake air banyak sehingga kelebihan/dilusi plasma, sindrom sekresi antidiuretic tidak tepat (SIADH), hemodialiis,
Nampak kapiler otak normal, serta perubahan yang terjadi selama edema vasogenik maupun sitotoksik. Dalam kondisi normal, taut-‐erat interseluler masih intak. Pada edema vasogenik, taut-‐erat tidak kompeten, menyebabkan kebocoran plasma ke spatium interseluler. Pada edema sitotoksik, terdapat kegagalan primer dari Pompa Na Dependen ATP yang mengakibatkan akumulasi Na dan air secara sekunder. Edema Vasogenik
