The Illinois State Board of Education in conjunction with the Illinois Department of Public Health require

that all students entering 6th grade show proof of receiving mandated vaccinations, in addition to receiving

a physical exam, and a dental exam. Please make certain that your child receives the required

immunizations and that they are recorded on his/her immunization record. These completed

nurse, Beth Jones, RN, PEL-CSN at Westfield (630.671.5315).

Physicians’ and dentists’ offices are often busy during the summer months so please make your

appointments early. The completed forms may be sent to the school’s health office or dropped off in

the Medical Forms Drop Box in the main office in your student’s building.

Attention Parents of 1st Graders

Illinois School Code requires that all 2nd graders present proof of having been examined by a dentist. If you have any questions, please contact your building nurse at your child’s school.

Physicians’ and dentists’ offices are often busy during the summer months so please make your

appointments early. The completed forms may be sent to the school’s health office or dropped off in

the Medical Forms Drop Box in the main office in your student’s building.
