2015 Community Wellness Report
This report is intended to be used by community-based Primary Health
Care Provider Teams, Clinic Teams, working groups, KTHR staff and
community leaders to inform and to develop strategies to address
unmet needs and emerging issues in the communities of Carrot River,
Arborfield and Zenon Park.
Health and wellness decisions are not simply a matter of personal
choice. What makes us and keeps us healthy is complex and
multifaceted. Determinants of health such as income, education,
environment, culture, coping skills, housing and lifestyle options all
influence a person’s ability to have access and to make the healthiest
choices.
The root cause of many illnesses in our communities stems from
poverty. For those with low incomes, poverty means more than just
living in a poor home, or having no access to affordable transportation.
There are a variety of detrimental impacts stemming from being
poor. One of the most devastating is the impact on health and well-
being that increasingly professionals across sectors are beginning to
identify. Looking at health more holistically has focused attention onto
the social determinants of health.
Many of our KTHR staff collaborate and partner with agencies and
other sectors to develop illness prevention and health promotion
strategies and work to improve the social determinants of health. For
example: Partners to End Poverty; Food For All Coalition; 0-3 Early
Childhood Task Force; Northeast Housing Research Project; Northeast
Regional Intersectoral Committee; Cost of Healthy Eating in
Saskatchewan; Tisdale Suicide Awareness Committee and Population
Health Working Groups: Eat Well, Play Well and Tobacco Reduction.

In Primary Health Care we are committed to a health care system that
puts patients first by providing Better Health, Better Care Better Values,
and Better Teams for our residents. We have clinic teams that work on
meeting the needs of those with illnesses or urgent care needs.
Ensuring all our residents have access to a main care provider has
become a regional priority. This helps to ensure patients have
continuity of care. Research tells us that people who have a family
doctor or nurse practitioner have better health outcomes. Better
health outcomes mean better quality of life.
Carrot River and Arborfield Clinic team strategies
include:
Impacting access to continuous and comprehensive care
Educating the public on the importance of attaching to a family
Dr/NP
Improving timely access to a family Dr/NP
Improving patient experience
Establishing and managing patient panels or roster. (A panel is a
list of patients that have an established relationship with a
Dr/NP. Once Dr/NP has a list of his/her patients, they can
better manage their care. For example, with screenings
(cancer, blood pressure, diabetes, etc.)
Promote Live Well with Chronic Conditions - a six week self-
management program for people who have an ongoing or
chronic medical condition. The program helps people with
chronic conditions build skills for managing their health, so that
they can maintain active and fulfilling lives. The program offers
information and teaches practical skills to better manage
symptoms and become an active partner in their care.
Participants gain confidence and motivation. In 2013/2014 and
2014/2015 the goal was to offer 1 session per year for the
communities of Carrot River, Arborfield and Zenon Park.
However due to having no local facilitator no classes were
offered. In 2015/2016 one class was offered with 5 participants
attending.
We also have Primary Health Care Provider Teams throughout the
region which brings health professionals and community
representatives together to work on unmet wellness needs in the
communities. Each team has conducted community wellness
assessments every two years since 2007 which allows the community
to tell us what the health and wellness concerns are. The 2015 survey
results will be discussed by the team and strategies to address the
various needs will be developed. Those needs that are beyond the
control of the team will be directed to the appropriate agency or group.
Our community representatives are our voice from the communities
and play an integral role when it comes to developing strategies to
address needs such as: cooking classes; reducing screen time;
promoting and supporting physical activities such as: Fun Fitness
Challenge and Trail Trek; posting health and team related information
on community bulletin boards and P.A.R.T.Y. Program (Prevent Alcohol
and Risk-Related Trauma in Youth).

Methodology:
The 2015 Community Wellness Assessment Survey was offered to
residents as follows:
Carrot River
On-line through a link on the KTHR Public website from May 7th
– June 12th
In person at Carrot River Co-op – Primary Health Care Provider
Team members were on-site to assist community residents to
complete the survey
Each team member was given 10 hard copies of the survey to
have clients/families and friends complete
In total 75 surveys were completed – 46 paper copies and 29 online surveys were completed. In comparison with the survey conducted in 2013 – 56 paper copies and 12 on line surveys were completed.
Arborfield
On-line through a link on the KTHR Public website from May 7th
– June 12th
In person at Arborfield Co-op – Primary Health Care Provider
Team members were on-site to assist community residents to
complete the survey
Each team member was given 10 hard copies of the survey to
have clients/families and friends complete
In total 59 surveys were completed – 44 paper copies and 15 online surveys were completed. In comparison with the survey conducted in 2013 – 30 paper copies and 12 on line surveys were completed.
Zenon Park
On-line through a link on the KTHR Public website and on the
Village of Zenon Park website from May 7th – June 12th
Local team member was given 10 hard copies of the survey to
have clients/families and friends complete
Paper copies were made available at the Post Office and
Association Fransaskoise de Zenon Park
Association Fransaskoise de Zenon Park offered assistance to
those wishing to complete the survey in the French
In total 25 surveys were completed – 13 paper copies and 12
online surveys were completed.
In total 160 surveys were completed within the three
communities. 104 paper copies and 56 on line surveys were
completed.

related Trauma in Youth in 2014
Health and wellness messages placed on the Primary Health
Care Provider Team’s roadside health promotion sign in Carrot
River and Arborfield
In 2014 and 2015 offered “Fun Fitness Challenge” Partnership
in communities of Carrot River, Arborfield and Zenon Park. In
2014 Carrot River was declared the winner and received the
proceeds from the 2013 Tri-Unity Challenge and in 2015 Zenon
Park was declared the winner and received the proceeds from
the 2014 Tri-Unity Challenge. Proceeds are to be used to
enhance trail systems and physical activity opportunities in
their communities
Dietitian offered monthly “Did you Know” nutrition tag card
messages at grocery stores in Carrot River, Arborfield and
Zenon Park in 2013
NP presented to Carrot River seniors group on Heart & Stroke
information in 2013
Physiotherapist/Exercise Therapist/Dietitian offered running
clinic/info at Pasquia Park in prep for 2013 Tri-Unity Challenge
NP acted as Chairperson for the 2013, 2014, 2015 Tri-Unity

community’s cumulative report and a regional cumulative report will be
made available on the KTHR Intranet and Internet.

It is hard to get in to see the doctor if you have an urgent matter we
could use more doctor time and more lab days if possible lab days
should match doctor days. There is some confusions for appointments. I
have booked an appointment in CR and gone there and found out the
appointment was in Arborfield. My husband got a bad rash and by the
time he could get into the doctor here the rash was gone and then no
cause could be found as they don't know what the rash looked like

We all work and attend school. Having to take so much time off for appointments, follow ups and tests (when told nothing appears to be wrong but we will test just because) adds up and becomes frustrating to staff, supervisors and school staff.

As mentioned previously, we have to wait too long to see our doctor. We can get in to see an alternate but a person wants the reliability & comfort of seeing the same physician every time!
Lobbying the government: - healthy foods are expensive - natural or alternative therapies (that are often preventative) are expensive and not covered by insurance plans 2. Host more preventative workshops (after hours) including natural/alternative therapies in which people can learn preventative lifestyle choices 3. Promote and encourage healthy workplaces through education of employers and employees
I feel as though our Dr.'s should listen to their patients better and be a little more thorough when diagnosing. I have had issues/heard of others having issues with this. They seem to have the idea that because they are the Dr. their patients concerns or opinions are not valid. I was asked who the Dr is when suggesting a possible diagnosis for example

Not sure how you can keep families more active. Alcohol use in very prevalent in our community. There used to be a time we just went somewhere for coffee/tea now there is always alcohol. I don't know how to change that culture.
Can't get into Carrot River Health at all on short notice if needed
The ambulance companies need to train their employees as to how to read a map, have maps of the surrounding towns they generally travel to, have RM maps with farm yard sites highlighted. and these maps need to be studied and understood. There have been too many instances of ambulances getting lost on their way. Rural people speak in terms of miles whereas younger people speak in kilometers. Are they able to convert the information?

It would be helpful to have Arborfield clinic staffed with a Nurse Practitioner more often. They are very capable; we just need to have access to them more than once a week. Would appreciate being notified of public presentations 3
I feel that when a person is not feeling good it shouldn't matter which community in the district that you come from. I was ill and phoned Tisdale clinic. The lady in charge?? Informed me people from ZP couldn’t go to the Tisdale office.
a walk-in clinic in city doesn't discriminate who you can see. Why does Tisdale? Different people have told me that this has happened to them also. Even old people have talked about how rude the Tisdale clinic people are
Produce @ our local store is never as fresh as out of town. New cooling systems???
When people from Zenon Park cannot get into see Louise in Arborfield and call Tisdale, they are told that Tisdale does not take patients from
here. We are still between 2 RMs and in the middle of Nipawin and Tisdale; I really don't think that this is acceptable.

There are not a lot of sidewalks which can pose a challenge for people who no longer drive. Seniors housing is a considerable distance from Dr. office, grocery store, post office and pharmacy
