Physicals
• You have to have a physical on file at Chisholm Trail High School before you can participate in tryouts.
• WHO NEEDS A PHYSICAL?
• Everyone needs a new physical turned in to Coach Holt. This is a new policy this year.
• Everyone needs to fill out medical history form and turn it in to Coach Holt
• ALL ATHLETES MUST COMPLETE ONLINE REGISTRATION

Proper court shoes are required. Knee pads are optional, but recommended. Please do
not bring your own volleyball.
Parents are welcome to attend on the last day for the last hour during competition
play and awards.
Directing the Chisholm Trail Summer Volleyball Camps will be Coach Janine Smith. Coach Smith is the head
volleyball coach for the Chisholm Trail High School Lady Rangers. She has been a head coach for 23 years.
Coach Smith started her coaching career at University of Colorado and then was the Head Coach at
University of Texas at Arlington. As a player, Coach Smith won a National Championship for the Longhorns
in 1988. This camp is sure to be a hit! Assisting with the camp will be volleyball coaches from Chisholm Trail
High School, Willkie Middle School and Creekview Middle School.

Coaches, Thank you for participating in the 2017 NTHS Volleyball Summer League. This continues to be a great tool for our athletes and teams to continue working together during the summer and getting ready for the upcoming season. The dates and other general information for the 2017 Summer League are listed below. We have a good lineup of schools this year and it will be bigger and better than ever! Let me know if you have a change in the quantity of teams you want entered in each division or if I have entered the wrong amount. Please have each athlete fill out an Assumption of Risk Form. In addition, a Roster Form needs to be filled out for each team. The easiest thing to do is to collect the completed Assumption of Risk Form from each athlete along with their $40 league fee. Complete a Roster Form for each team and attach it to the corresponding Assumption of Risk Forms and league fees and turn them all in on the first night of league play. Every team will also get a League T-Shirt. Please select the color that you would like the t-shirts to be. There is a wide range of colors so you can be creative here. I will have a color chart at check-in on the first night of the league. Below is some general information about the 2017 NTHS Summer League: League Playing Site: Tejas Volleyball Center, 100 Midway Dr. W, Euless, TX 76039 League Dates/Times: Varsity League (Tuesdays) 4:00pm-7:00pm: June 6, June 13, June 20, June 27, July 11, July 18 JV League (Thursdays) 4:00pm-7:00pm: June 8, June 15, June 22, June 29, July 13, July 20 League Cost: $40/player (made payable to Tejas Volleyball). ASSUMPTION OF RISK In consideration of TEJAS VOLLEYBALL (TVC, Inc.), NYTEX Sports Centre, the staff, and club officers hereinafter referred to as Organizers, allowing
the undersigned, hereinafter referred to as Participant, to engage in various athletic endeavors, including, but not limited to athletic games, events, practice
sessions, conditioning sessions, and activities incidental thereto, the undersigned hereby agree to the following:
1. Participant recognizes and understands that certain risks or harm are inherent and that there are dangers involved that cannot by fully foreseen
and over which the Organizer has no control, which could result in property damage, bodily injury or death.
2. Participant understands that there are dangers and inherent risks in playing or practicing to play in any sport, including VOLLEYBALL,
but are not limited to death, serious neck and spinal injuries which may result in complete or partial paralysis, brain damage, serious
injury related to the eye and/or head, serious injury to virtually all internal organs, serious injury to all bones, joints, ligaments,
muscles, tendons, and other parts of the muscular/skeletal system, and serious injury or impairment to other aspects of my body and
general health and well being.
3. The Organizers strongly suggests that the Participant seek medical advice prior to engaging in any part of the various athletic endeavors,
including but not limited to athletic games, events, practice sessions, conditioning sessions, and activities incidental thereto.
4. Participant agrees to assume all risks and responsibilities for any and all claims for damages, including personal injury or death, medical
expenses, disability, lost wages, loss of earning capacity and property damages which may be incurred by Participant while Participant engages
in athletic games, events, practice sessions, conditioning sessions, and travel beginning May 12, 2008.
5. I have read the above agreement and foregoing and have willingly signed the same for the consideration expressed and with a full understanding
of its purpose. Participant represents that he/she is 18 years of age or older and otherwise competent to execute this instrument or that his/her
legal guardian is also signing this agreement.
MEDICAL RELEASE
Further, I give permission to TVC Inc. to treat Child or arrange for medical care or treatment for my child in any situation deemed reasonably necessary by
TVC Inc. If circumstances permit, TVC, Inc. shall attempt to communicate first via telephone with the following emergency contacts for child.
