Enrollment
► Eligible Dependents Legal spouse Dependent children (biological, adopted, child for whom you have legal guardianship,
children placed for adoption, step-children) Eligible children may be covered until age 26 regardless of student or marital status
► Qualified Changes in Family Status Marriage Divorce Birth or adoption Significant change in dependent’s employment or health coverage
Watch your home mail for dependent verification request. You must respond in order to cover your eligible dependents.

Personal Health Account (PHA) Plan Overview
► Two options: Seeded PHA and Unseeded* PHA Seeded PHA: Gulfstream contributes “seed” money to your HSA Unseeded PHA: Gulfstream does not contribute to your HSA – your premiums are
lowered by the seed amount
► CoreSource is the administrator of the medical plan
* Employees with coverage in a non-high deductible plan, such as Medicare and TRICARE, are not eligible to contribute to an HSA, according to IRS regulations, so they must select the unseeded PHA. Special rules apply to employees using their VA Benefits.

Preventive medications include:

► All generic medications used to treat diabetes, as well as insulin, test strips, lancets and syringes
► Preventive drugs specified by the Affordable Care Act (ACA), such as tobacco cessation and contraceptive drugs/devices

What is an HSA?
► It’s a tax-free account you can use to pay eligible healthcare expenses, administered by Fidelity
► Funds can be used tax-free for eligible expenses now or in the future (especially in retirement), includes, but not limited to: Medical, dental, vision out of pocket expenses Qualified health premiums IRS Publication 502 for full list of qualified expenses
► Investment opportunity – earnings are non-taxable
► Funds roll over year after year – no “use it or lose it” rule
► Account belongs to you; all money goes with you if you leave the company
