Academic Achievement

Present levels of Academic Achievement

Summarize where the student is in academics

Identify subject areas (math, writing, reading, spelling, and sometimes communication)

If not an area of concern, then document that the student is making adequate progress as related to same age peers

If the student is struggling for a non-academic reason, write the PLOAA in that manner

Present Levels of Performance46

Page 47: Annual Inservice September, 2011 10/31/2015 2:01 AM 1

Academic Achievement

Documentation should reflect the results of the most recent evaluation, but should not be a restatement of the evaluation results documented in the eligibility report.

For preschoolers, performance levels may include pre-academic readiness skills and developmentally appropriate cognitive skills.

Present Levels of Performance47

Page 48: Annual Inservice September, 2011 10/31/2015 2:01 AM 1

Functional Performance

Functional performance is related to activities associated with daily living.

This information should be stated in clear, concrete terms understandable to all IEP team participants.

Again, documentation should reflect the results of the most recent evaluation.

For students 14 ½ and older, describe student strengths and weakness within the context of his/her post-school goals.

Placement is not an adverse effect – “Student will not be with typical peers for a portion of the day”

Page 51: Annual Inservice September, 2011 10/31/2015 2:01 AM 1

PLOAA/FPAdverse Effects Examples

Susie’s learning disability in math reasoning and reading comprehension require her to receive small group instruction at a slower pace than same age peers. When in the general education classroom she needs accommodations such as . . .

John’s emotional disability that affects impulse control and organization requires direct instruction of alternative strategies that will positively impact his goal to be a carpenter after high school

86

Page 87: Annual Inservice September, 2011 10/31/2015 2:01 AM 1

Purpose of Program Amendment

Sometimes the school district and the parent/guardian agree not to reconvene the IEP meeting for the purposes of making changes to a student’s IEP. The Program Amendment Form is the documentation that this decision has been made and minor changes have occurred to the student’s services.

Program Amendment87

Page 88: Annual Inservice September, 2011 10/31/2015 2:01 AM 1

Things to Remember about the Program Amendment

Changes that do not significantly impact a student’s services and/or placement.

Used in place of reconvening the IEP meeting. Can not take the place of an annual review

meeting. The form will be attached to the child’s IEP.

Send copy the Amendment to the parent. Ask the parent if he/she wants another copy of the IEP

Send Perandoe the original Amendment. We will attach it to the current IEP in the temporary record.

Page 91: Annual Inservice September, 2011 10/31/2015 2:01 AM 1

Excusal of a Team Member Provides the written documentation for the excusal of

one of the required team members.

A school district and the parent/guardian may agree BEFORE THE MEETING BEGINS, in writing, to excuse an IEP team member in whole or part under the following circumstances:

If the attendance of the member is not necessary because the member’s area of curriculum or related services is not being modified or discussed in the meeting, or

If the meeting does involve a modification to or discussion of the special and related services but the team member submits, in writing, input into the development of the IEP prior to the meeting.
