What is FIP? Insurance for you and your family in the event of

death

An easy, safe & cost-effective way to protect your family in the event of death

How Does FIP Work? Visit the Credit Union office and complete the

application form

Select from six convenient plans

You can insure up to 6 persons including yourself

Pay your first monthly premium

Coverage is effective the first day of the month following enrolment

NO CLAIMS PAID IF THERE IS A DEATH WITHIN THE FIRST 6 MONTHS UNLESS DEATH IS ACCIDENTAL

Who Is Covered? YOU

Your spouse (includes significant other)

Up to 2 parents or parents-in-law or combination of parent & parent-in-law

Your children or those for whom you are a legal guardian

Children must be under age 26 unless they are permanently disabled and were enrolled before age 26

Parents or parents-in-law must be enrolled before age 76

How Do I Claim?

Bring in the death certificate

Request a Proof of Death form from the Credit Union which should be completed by the doctor who certified the death

Claims must be presented within 2 years of death

How is the Policy Terminated? The Credit Union terminates their participation in the

programme

The insurers terminate the Credit Union’s participation in the programme

The Credit Union fails to remit premium in accordance with the Grace Period Provision

The member withdraws from the Plan

The member ceases to be a member of the Credit Union

The member’s premium payments are over 31 days overdue

Unmarried living children and dependent children under legal guardianship of the member have reached age of 26 years

Grace Period Grace period is 31 days

If the premium is not paid on or before the due date, it may be paid during the 31 days following the due date.

During Grace Period insurance will remain in force as long as it is paid within this period

Grace Period does not apply if policy terminated by member or insurer

Duplication of Coverage IN NO EVENT MAY AN INSURED PERSON BE

INSURED THROUGH MORE THAN ONE FIP MEMBER CERTIFICATE
