Importance of Lesson(Dari)

• As previously discussed, expired medications can have negative impacts on patient care. After the expiration date the potency of the medication cannot be guaranteed.

• For medications used to treat infections this can lead to insufficient treatment and possibly death.

• This lesson will teach how to handle and account for expired medications.

Defining Expiration DatesDari

• The expiry date usually means that you shouldn’t take the medicine after the end of the month given. – E.g. expiry date is ‘January 2015’, you

shouldn’t take the medicine after January 31 2015.

• May be written on the medication packaging as:– expiry – expiry date – expires – exp – exp date – use by – use before

• Expiry dates are based on the Stability of the medication.

StabilityDari

Stability of a pharmaceutical product is defined by USP-NF as the:

“extent to which a product retains, within specified limits and throughout its period of storage and its use (i.e., its shelf life), the same properties and characteristics that it possessed at the time of manufacture.”

• Five types of stability: chemical, physical, microbiological, therapeutic and toxicological.

• Stability is based on the medication being stored under the manufacturer’s recommended conditions.

Chemical stability: the chemical structure of the active ingredient remains intact and that the potency is acceptable. (retains 90% potency)

Physical stability: the way the product appears (e.g., discoloration), tastes and smells, and if the product remains a solution or suspension.

Microbiological stability: the sterility of the product (remains free of bacterial or fungal growth.

Therapeutic stability: the product is able to exert the same actions in the body, leading to its effects against the disease it is intended to treat.

Toxicological stability: an increase in toxicity does not occur as time passes.

•The drug must continue to maintain its potency up to its expiry date.•In the United States and Canada the potency is stated in the drug monograph used to gain licensing approval.•Potency:

“ is a measure of drug activity expressed in terms of the amount required to produce an effect of given intensity”

•A highly potent drug evokes a larger response at low concentrations, while a drug of lower potency evokes a small response at low concentrations. (ie. morphine vs codeine)

“Use by” Date

• If a medicine has a use by or use before date: – Don’t take the medicine

after the end of the previous month.

– E.g. Use by date: January 2015, don’t take the medicine after December 31 2014.

Short Expiry Dates

Some medicines are given a short expiry date, such as:

– prepared antibiotic mixtures: when the pharmacist adds water to powdered antibiotic, it changes the stability of the product and the pharmacist will give it an expiry date of one or two weeks, depending on the product

– eye drops: these are usually given an expiry date of four weeks after first opening the container because your eyes are particularly sensitive to any bacteria that might get into the eye drops

• The short date becomes the new expiry date.

• The short date should be specified by the drug manufacturer in the package leaflet or on the package.

Medications After Expiry DateDari

• After the expiration date most oral medications do not pose major safety concerns.

• The most common consequence of an expired medication is loss of potency and thus effectiveness.

• This is a major concern for antibiotics, seizure and transplant medications.

• Tetracycline (antibiotic) is stated to cause nephrotoxic metabolites if used after the expiry date.– However, this is debateable.

• The National and Regional Class VIII Medical Depots will accept expired or unserviceable pharmaceuticals from the regional entities each depot supports (e.g. National Medical Hospital, Regional Hospitals, Polyclinics, Troop Medical Clinics, Battalion Aid Stations and Medical Platoons).

• Expired or unserviceable pharmaceuticals will be turned into the National and Regional Medical Depots via the use of a MoD Form 9 (per Decree 4.2, paragraph 8). The organization maintaining MoD Form 4 register will issue a document number for the MoD Form 9.
