We are delighted that you have joined us for this year’s conference. The next few days offer us an opportunity to reflect, celebrate, and look ahead.

The theme of this year’s conference is “Advancing High Value Health Care in California’s Safety Net,” and it is with enormous pride and admiration that we reflect on public health care systems’ leader-ship in doing exactly that. Whether you are a leader within a public health care system, a stakehold-er, or a supporter, you have worked alongside all of your fellow attendees to improve the health care, and the health, of millions of low-income Californians.

Our leadership continues with the opportunity to pilot new and better ways to deliver and pay for care within the safety net.

Under a renewed waiver, California’s public health systems will all participate in PRIME (Public Hos-pital Redesign and Incentives in Medi-Cal), the successor to the wildly successful DSRIP, which will help systems continue down the path to care that is integrated, coordinated, efficient, and effective.

The Global Payment Program Pilot under the waiver will be first-in-the-nation payment reform program for the remaining uninsured that will incentivize cost-effective primary care, as opposed to hospital-based care.

Some members are also leading payment reform in their Federally Qualified Health Centers through the Prospective Payment Systems pilot established by SB 147.

And the waiver’s Whole Person Care pilot program will allow Medicaid funding to be used to ad-dress the needs of some of our most challenging patients, beyond the clinical setting.

We celebrate the opportunity to continue this leadership, knowing that it won’t be easy or always clear. And we look ahead to another year of hard work and leadership as we embrace the opportu-nity to better care for the most vulnerable in the communities we serve.

Contra Costa Health ServicesAdvanced Access Bundle

The continued implementation of the Affordable Care Act has meant an influx of new patients in every one of California’s public health care systems. For Contra Costa Health Services (CCHS), this has trans-lated into 35,000 new patients in the previous fifteen months alone.

These new patients were being added to a system where “lack of timely access to clinic appointments” was already the number one complaint voiced by patients. Contra Costa’s team knew that a rede-sign of their decades-old appointment system was needed.

In most offices even today, providers’ schedules are booked weeks, if not months, in advance. This was how it worked at CCHS as well, until a newly formed Ambulatory Care Redesign Team (ACRT) determined that the system would need to be streamlined and made more adapt-able.

Seeing the need, Contra Costa’s team created Advanced Access Scheduling, a model that uses an open template, and continuously balances supply and demand to let patients, both new and old, be seen at a time that works for them. The system has very few slots reserved for special situations or patient types, a departure from how it had worked previously. The Advanced Access Bundle includes this scheduling model as well as other strategic initiatives, including a focus on consistent nurse/provider teams, the establishment of an RN telephone clinic, the extension of visit intervals for patients, and pre-visit phone calls to determine whether an in-person visit is necessary, and ensure that patients are prepared if they are going to come in.

The success of the Advanced Access Scheduling model is dependent on engaged stakeholders, including pa-tient representatives, and teams of nurses and providers engaging in strong communication. Results have been dramatic. New patient no-show rates are down 28%, and clinics have reduced their median third-next-available appointments by as much as 62%.

The Advanced Access initiative was kicked off at CCHS’s Martinez Health Center and Miller Wellness Center in January of 2015, spread to both of its Concord Health Centers in April, and then to the West County Health Center and North Richmond Health Center in July – accounting for more than 50% of the system’s total patient population. CCHS plans to spread the model across all its primary care clinics by the middle of 2016.

PERFORMANCE EXCELLENCE AWARDSanta Clara Valley Health & Hospital SystemImproving Readmission Rates for Homeless Persons in a Public Hospital System

Santa Clara County has the nation’s 4th highest chronically homeless population, with six thousand people on the streets or in insecure housing on any given night. These individuals are some of Santa Clara Valley Health & Hospital System’s (SCVHHS) most vulnerable patients, because of the often-complex nature of their health condi-tions, but also because they are often discharged into very difficult circumstances. Many are patients without social supports, who do not have access to adequate food, shelter, or a safe place to even store medications – let alone focus on recovery. These circumstances lead to a significantly higher risk of readmission and a poorer quality of life for them.

Led by SCVHHS and its Homeless Healthcare Advisory Board, the Santa Clara Medical Respite Program is a unique public-private col-laborative, which also includes eight area hospitals and a homeless shelter. The Medical Respite Program provides homeless patients recently discharged from the hospital access to a 20-bed facility that provides a safe, clean environment for recuperation from hospitaliza-tion. All referred patients at the MRP receive comprehensive health

and psychosocial assessments that allow for individualized plans and services to help link the homeless patients with comprehensive primary care in a health home, specialty care, entitled benefits, substance abuse and mental health treatment, and other needed supports and services.

The program has had 1,800 referrals and has welcomed 700 patients since its inception, and has led to an esti-mated reduction of 2,488 bed days and a cost savings of over $6 million. Among the Medical Respite Program’s patients, the 30-day readmission rate is 18%, compared to the average rate of 50% for homeless patients. But the patients themselves tell the story even better:

“Without the opportunity to stay at the Medical Respite Center, I probably would not have survived during the month of November. (This program is) saving the lives of people on the street and treating us with respect...I am no longer Willie the homeless guy. I am citizen Willie.”

“It’s time to give back to the community, and I thank respite for giving me that opportunity.”

DATA-DRIVEN ORGANIZATION AWARDSan Francisco Health NetworkImproving Specialty Care Access in a Safety Net System: Assessment, Engagement, and Innovation

It’s not easy to collect accurate data. All health systems know this, and work hard to improve their data collection policies, procedures, and practices. It’s even harder to do something productive with that data once you’ve got it. San Francisco Health Network knew that access to specialized care was a problem for many of its patients, and knew that the solution would require both collecting new data, and just as importantly, analyzing and acting upon it.

SFHN embarked on a two-year project aimed at figur-ing out how to address disparities in access to care and streamline specialty care delivery.

The first step was a needs assessment and gap analysis, which included the collection of baseline data, as well as interviews with leadership and focus groups with pa-tients. A team of physicians and nurses then developed a method for collecting, analyzing and tracking data across specialty care clinics, based on the needs that had been identified. New quality and operational dash-boards were created for all 26 specialty clinics to highlight patient satisfaction and quality metrics, with a focus on “third next available appointment” (TNAA) data, one of the industry standards for measuring access. Once these dashboards were in place and being utilized, the data started coming in, and the performance improvement work could be developed based on the analysis of that data.

This performance improvement work has included (but has not been limited to) the expansion of clinics, the use of non-physicians to evaluate patients, improving efficiencies through workplace organization, the utilization of group classes for patients, and collaboration with outside hospitals to perform the backlog of diagnostic proce-dures.

The results of this work on the system’s specialty care TNAA numbers have been staggering. Before the project, almost one quarter of clinics had a TNAA of more than 120 days. Now that number is less than 3%. At the other end of the spectrum, at the outset of the project only 14% of clinics had a TNAA of fewer than 15 days. Now it’s nearly half. And thanks to a continued emphasis on collecting and analyzing data, these numbers will keep improving.

ADVANCED PRIMARY CARE AWARDVentura County Health Care AgencyInnovative Diabetes Care with Group Medical Care

We use the phrase “advanced primary care” to describe systems that provide “right place, right time care,” especially for those with chronic conditions. Ventura County Health Care Agency, as part of its work under the Delivery System Reform Incentive Program (DSRIP), created a remarkable program at Las Islas Diabetes Center in South Oxnard.

The clinical teams at Las Islas had the goal of creat-ing a “one-stop shop” approach for their patients in group visits. They started by finding and fixing gaps in the retinal screening workflow, then implemented comprehensive group visits that included a foot exam, blood draw and dietician education.

But these visits offer more than just convenient access to services. Effective diabetes care relies heavily, if not mostly, on a patient’s ability and willingness to care for themselves. These classes

have empowered patients with the tools and social supports to help manage their own conditions.

“I actually have recommended the classes to friends,” says Linda Villegas, a patient at VCHCA. Linda had just been diagnosed with diabetes upon starting the group visits, so hearing about other patients’ experiences with their diabe-tes really helped. “Patients are encouraged to come together and socialize with the goal that they will offer support to one another,” says Karla Alcaraz, RN, CDE, the case manager who organizes group visits. “Many maintain friendships beyond the scope of the class.”

Alcaraz says the programs are showing incredible results. Three quarters of patients who were not at their LDL goal when starting the class did get to an LDL less than 100 upon completion. More than half of patients who were not at their A1c goal achieved an A1c less than 7, or at least a 1-point drop. Some patients have lowered HbA1c by more than 2 points over the course of these visits, and several have lost 15 to 20 pounds.

“The classes put you on track to manage and improve self-care,” says Villegas.

President and CEO, National Quality Forum

Christine K. Cassel, MD is President and CEO of the National Quality Forum. Dr. Cassel is a member of the President’s Council of Advisors on Science and Technology (PCAST).

She was a member of the Commonwealth Fund’s Commission on a High Performance Health System and served on the IOM committees that wrote the influential reports To Err is Human and Crossing the Quality Chasm.

She is an Adjunct Professor of Medicine and Senior Fellow in the Department of Medical Ethics and Health Policy at the University of Pennsylvania School Of Medicine, former Dean of Medicine at Oregon Health and Science University, Chair of Geriatrics at Mount Sinai School of Medicine in New York, and Chief of General Internal Medicine at the University of Chicago.

Dr. Cassel is a prolific scholar, having authored and edited 14 books and over 200 published articles.

Secretary of the California Health and Human Services Agency

Diana Dooley was appointed Secretary of the Califor-nia Health and Human Services Agency in December 2010 by Governor Jerry Brown. She leads 13 state departments within the Agency, chairs Covered California (the Health Benefit Exchange) and serves as chair or member of numerous other boards and commissions.

Previously, Dooley was President and CEO of the California Children’s Hospital Association. Dooley began her career as an analyst for the State and in 1975, she was appointed to the staff of Governor Jerry Brown for whom she served as Legislative Secretary and Special Assistant until the end of his term in 1983.

Before becoming an attorney in 1995, she owned a public relations and advertising agency. Dooley moved into health care in 2000 when she left her private law practice to serve as General Counsel and Vice President at Valley Children’s Hospital.

KEVIN HINES GEORGE LAKOFF

Speaker, Author, and Mental Health Advocate

Kevin Hines is a global speaker, author and mental health advocate who reaches audiences with his story of an unlikely survival and his strong will to live. Two years after he was diagnosed with bipolar disor-der, (at 19 years of age) he attempted to take his own life by jumping from the Golden Gate Bridge. He is one of only thirty-four (less than 1%) to survive the fall and he is the only Golden Gate Bridge jump survivor who is actively spreading the message of living men-tally healthy around the globe.

Kevin sits on honorary boards of both the Interna-tional Bipolar Foundation (dedicated to healing from Bipolar Disorder) as well as the Bridge Rail Founda-tion (dedicated to ending suicides at the Golden Gate Bridge). He is currently a member of the National Suicide Prevention Lifeline’s Consumer Survivors Sub-Committee. Kevin was a two term member of San Francisco’s Mental Health Board and has spoken in congressional hearings alongside Patrick Kennedy in support of The Mental Health Parity Bill.

Author and Professor of Cognitive Linguistics

George Lakoff is one of the world’s best-known cog-nitive scientists and linguists, and a leading expert on the framing of issues in politics. He is the Goldman Distinguished Professor of Cognitive Science and Linguistics at the University of California at Berke-ley, and the author of numerous bestselling books, including Moral Politics, Don’t Think of an Elephant, Metaphor and War, and The Political Mind.

Professor Lakoff has advised the Democratic cau-cuses in the House and Senate, helped Democratic candidates around the country, and worked with hundreds of progressive advocacy groups on issues ranging from the environment and health care to civil liberties and international relations.

In his academic life, Professor Lakoff is renowned for his discoveries on how the mind is embodied, how metaphorical thought works, what meaning is, and how meaning structures language. His current research is on how language and thought work in the brain.

GARY LANGER BURT MARGOLIN

President, Langer Research Associates

Gary Langer is president of Langer Research Associates, a New York-based company that designs, manages and analyzes rigorous, in-depth surveys of public attitudes and behavior.

Since 2011, Langer and his colleagues have produced a series of annual surveys for Blue Shield of California Foundation, evaluating the healthcare experiences of low-income Californians and examin-ing ways to improve upon these experiences through primary care redesign, focused on the principles of patient-centered care.

Langer and his staff currently are working with the Foundation to develop a patient engagement tool through which clinics throughout the state can assess and track their progress in achieving a patient-centered approach. Langer has presented the Foundation’s research at numerous conferences and webinars to audiences across the state and beyond, including two invited lectures at the Institute of Medicine in Washington, D.C.

President of The Margolin Group, Inc.

Burt Margolin is the President of The Margolin Group, Inc., a company that provides health care clients with strategic guidance and direct advocacy services at both the state and federal level. Burt Margolin began his involvement with health policy as the Chief of Staff for Congressman Henry Waxman.

He is also a former Member of the California State Assembly. During his twelve years of service (1982-1994), he chaired the Assembly Health Committee, the Assembly Insurance Committee and the Budget Subcommittee responsible for health funding.

He currently serves as the Legislative Strategist for the Los Angeles County Board of Supervisors and advises the County on a wide range of state and fed-eral funding issues. He also represents the California Primary Care Association, Santa Clara County and Planned Parenthood Affiliates of California.

SANTIAGO MUÑOZ

Chief Strategy Officer, UCLA Health, and Chair, CAPH Board of Directors

Santiago Muñoz serves as the Chief Strategy Officer of UCLA Health with responsibility for industry partner-ships, business transactions, clinical business develop-ment, and strategic initiatives to improve market position. Prior to joining the UCLA Health System, Mr. Muñoz held a similar position at UC Health in the UC Office of the President. In that capacity, he led the development of UC Health partnerships and systemwide agreements with all major commercial payers in California, including UC Health’s role in a new HMO product designed as a low-cost healthcare alternative for employees.

Mr. Muñoz has conceptualized partnerships, payment provisions, and strategies that resulted in significant pro-tections and additional payments to academic medical centers. Recent activities include playing a lead role in the development of the UC Center for Health Quality and Innovation, commissioning of the Vivity HMO, develop-ment of the California Rehabilitation Institute (122 bed acute rehabilitation hospital in Los Angeles), and suc-cessfully growing the size and scale of UCLA Health.

Chief Operating Officer, San Francisco General Hospital

Ms. Nazeeri-Simmons is currently the Chief Operat-ing Officer (COO) for San Francisco General Hospital (SFGH). In her role as COO, she provides executive oversight and management for SFGH’s clinical and administrative services.

Prior to this role she served as SFGH’s Chief Quality Officer, responsible for ensuring the hospital and clinics maintain high quality of care and service excellence focusing on patient safety, and that SFGH meets all federal, state and Joint Commission regulatory require-ments. She also serves as Adjunct Faculty at UC Berke-ley’s School of Public Health, teaching a graduate level seminar on health equity.

She has taught undergraduate and graduate courses for over fourteen years at University of Michigan, Portland State University, UC Berkeley and Portland Community College. Ms. Nazeeri-Simmons is the incoming Board Chair of The California Health Care Safety Net Institute (SNI) Board of Directors.
