______________________________________________________________________________ Considering the organizational changes that have transpired in the MSLFI HR Department office
over the past years, there is a need to amend the policies and procedure in recording the absences and tardiness/under time and submission of DTR for MSLFI employees. Hence, the following are hereby prescribed:

Submission of DTR
For Promodizer all DTR must be submitted to their respective Area Coordinator on or before 11:00 AM every 11th of the month for the 1st cut off and 26th for the second cut off.
For AREA COORDINATOR all of the DTR report from their promo personnel must be submitted to the HR Department the soft and hard copy on or before 3:00 PM every 11th of the month for the 1st cut off and 26th for the second cut off.
Travel Order must also be submitted together with their promo personnel’s DTR, together with the over time form.
Incomplete attendance and delay report will be subject for investigation and will be subject for adjustment for the next cut off.
An employee who fails to log in/ log out in the Biometrics will be subject to adjustment for the next cut off plus 1 demerit point per incident.

Request for Over Time
Request for over time must be submitted first for approval before the exact date of over time. HR Department will not honor any Over Time form without the OT request and of course with the approval of the department head.
For Promo Personnel over time request must have an attachment of their DTR from the store or any Memo from the store requiring them to have an OT.
Disciplinary Action
Absenteeism - In excess of 3 times a month – 1 DPHabitual Tardiness/Under Time– In Excess of 3 times a month – 1 DP Failure to log in and out – 1 DP per incidentFrequently fails to log in or Log out- in excess of 3 times a month, an automatic 1
day suspension

.• This is to reiterate the policy regarding the Travel Order with a fix schedule
scheme that would take effect on April 11, 2014, the following schedule scheme during store visit shall be implemented.
• With in Metro Manila, Cebu and Davao Area all MSLFI Personnel that are
on store visit/ area visit is required to render at least Eight (8) hours duty inside the store premises excluding the one (1) hour lunch break. The following schedule scheme must be followed.

For outside Metro Manila, Cebu and Davao all MSLFI Personnel that are on store visit/ area visit is required to render at least six (6) hours inside the store premises excluding the one (1) hour lunch break. They may choose from the following time frame.

Failure to meet the required hours will be automatically considered as late or under time and would be deemed as willful non-compliance of the company’s policy and shall be meted with the proper disciplinary action.

Please be informed that the Office of the President of the Philippines has declared April 17
18,2014 as regular Holiday throughout the country as per Proclamation No. 655 in observance
of Maundy Thursday and Good Friday while April 19, 2014 a special non working Holiday in observance of the Black Saturday.
On this date, the Central, Warehouse, Apollo Cebu and Davao offices will not be operational. Any concern from the abovementioned officers specially customer order, kindly coordinate no later than April 16, 2014.
