What is Radial Shockwave Therapy?

Yes. Clinical research has been ongoing for decades and the published data clearly shows that Radial shockwave Therapy is a highly effective treatment option for many musculoskeletal conditions and injuries. (see research on page 9)

Radial shockwave Therapy works without the need for drugs, stimulates the body’s natural ability to heal itself, and in some cases can even help eliminate the need for invasive surgery.4

Is Radial shockwave Therapy effective?

Radial shockwaves flow through a region

extracorporeal shock Wave Therapy (esWT) is a term most commonly associated with Focused shockwave therapy, although some distributors and re-searchers also use this terminology to refer to Radial shockwave Therapy.

The first medical treatment developed using shockwaves was lithotripsy. This allowed focused shockwaves to specifically target and essentially dissolve kidney stones without surgical intervention.

More recently, focused shockwave therapy is being offered for orthopedic treat-ments. However, these systems have inherent drawbacks. To create effective thera-pies, you need to be able to precisely target both the exact areas that require treatment and the depth of the treatment being applied. This is extremely difficult even with the use of visual references like ultrasounds. Therefore, many focused shockwave treatments become “hit and miss”.

Research shows that for many conditions, Radial extracorporeal shockwave deliv-ers results as good or better than low-energy focused therapy, with the added benefits of being more consistent in application and not requiring local anesthesia 1-3. Therefore, for the balance of this document we will focus on Radial shockwave Therapy only.

Radial versus Focused shockwave and esWT

Therapeutic Radial shockwaves are simply controlled pressure waves that are created when a ballistic projectile is accelerated and then impacts against a stop-plate inside a specially-designed hand-piece.

The primary effect of these shockwaves is a direct mechanical force that occurs at a cellular level as the wave’s energy passes through tissue. These waves cause a controlled impact on the tissue being treated.1 The result is a biological reaction within the cells

Research indicates that the mechanical stimulation produced by shockwaves is capable of inducing positive reactive processes in the cellular structure of injured tissue. The most commonly observed reactions in tissue treated with shockwave Therapy are;

• Increased Metabolism

• Expression of Substance P (a growth & pain mediator hormone)

• Improvement of cellular tensile stability

• Altered cell membrane permeability

• Increased cell activity by Gene Expression

• Overall improvement of cellular structure

of that tissue which causes an increase in blood circulation through the injured site and accelerates the body’s natural healing processes. Radial shockwave treatments increase the metabolic activity around the site of pain or discom-fort. This therapy also stimulates the re-absorption of irritative calcium deposits in tendons, accelerates healing and reduces pain.

A small metal projectile is accelerated at very high speed inside the handpiece until it impacts against a stop-plate. The collision caused by this action generates a mechanical pressure wave. This pressure wave of energy is transferred through an applicator head at the end of the handpiece which directs and sends the energy into the selected treatment area.1

By adjusting specific control parameters, shockwave Therapy can be modified to treat a number of different musculoskeletal conditions and individual treatments can be customized to “fit” any patient type. Treatments can be modified and customized by adjusting;

• the amount of energy used,

• the frequency of the Shockwaves,

• the number of “shots” (or pulses) per treatment,

• and the size of the applicator head

Treatments take as little as 5 minutes with immediate results

Unlike many other forms of therapy, shockwave Therapy generates virtually immediate positive results. even after just one treatment, patients usually report;

• A significant reduction in pain.

• Improved Mobility (range of motion).

• More normalized muscle tone.

• Muscle “knots” or tightness are usually gone or noticeably reduced & relaxed.

Individual shockwave Therapy treatments usually involve delivering about 2,000 shockwave pulses or “shots” per treatment area. This takes about 5 minutes. Most conditions require between 3 and 6 treatments in total. Normally the treatment protocol recommends between 1 and 3 treatments per week. The patient’s body requires “rest & repair” time between treatments in order to allow for natural, accelerated healing.

6 Electromagnetic vs Air Compressor Systems - What should I be looking for?

Independent research shows that both forms of generating shock-waves are efficacious.3 Typically electromagnetic systems are found in the more expensive Focused shockwave units. The use of one system over the other has to do with the manufacturer’s preferred construction and not the efficacy of the therapy.

Generally, radial air compressor systems (pneumatic) generate peak pulse rises at 3.5 µs (billionth of a second), while electromagnetic peak pulses are at 13.5 µs. Both of these measures are well within the time restraints needed to a generate sufficient shockwave for treatment efficacy. The later, however has the added benefit of cre-ating less of a “stinging” sensation for the patient.

The more important factors in your equipment decision making pro-cess should be the maximum energy (Bars/mJ) and Frequency (Hz) the unit can generate, ease of use, reproducibility between profes-sionals within your clinic and requirements like portability, warran-ties, running costs and your overall budget.

I had the opportunity to use the Zimmer enPuls shock wave unit to treat members of the Canadian olympic track and field team. The enPuls had several features (in addi-tion to the clinical benefits of shock wave therapy) which made it a very practical and efficient modality for our clinic. I wanted to write and tell you my success with the Zimmer enPuls 2.0. I find it fits very well into my style of treatment. I use it on specific conditions it is “recommended” for such as plantar fasciitis, calcific tendinitis, etc, but have also got very good responses when used in areas of trigger points, generic muscle tightness, stiff joints, arthritic joints with very good results with hallux rigidus, which is a noteworthy one.

Overall I use it routinely and have good success with it. | even cured a stubborn trigger finger in about 8 sessions - the trigger finger had been persistent for over 1.5 yrs and is completely gone.

A lot of healthcare professionals find it difficult to understand all the claims made by laser manufacturers. Confusion is often com-pounded by statements that seem to be intended to misdirect your attention away from asking the right questions to make a truly informed decision. The following information is intended to shed some light on what you need to know and what questions you need to ask.

How do Lasers work?

The effects of Laser therapy are photochemical, not thermal. Photons enter the tissue and are absorbed in the mitochondria and at the cell membrane. The photonic energy is converted to chemical energy within the cell, in the form of ATP. Cell membreane perme-ablity alters, then physiological changes occur. These affect macrophages, fibroblasts, endothelial cells, mast cells, bradykinin and nerve conduction rates.

Where do I start?When considering a Laser system for your clinic you should first decide what kind of conditions you plan to treat. Different Laser systems have different specifications which determine how they will perform and therefore how they will impact a patient.

In general, all Laser systems should be evaluated on the basis of how they balance Output Power and Wavelength. There are other important factors that will help you narrow your choices even farther, but the balance between a Laser’s Output Power and the Wavelength produced by the Laser’s diodes is the most important funda-mental parameter that you need to know.

Cellular reaction to photons is very similar to photosynthesis in plants.

Reported benefits include:

• Rapid cell growth. Laser accelerates cellular reproduction and growth.

• Faster wound healing. Laser stimulates fibroblast development in damaged tissue, reducing recovery time.

• Increases metabolic activity through higher outputs of specific enzymes, greater oxygen and food particle loads on blood cells. Improves immune responses.

• Reduces fibrous tissue, reducing the formation of scar tissue following injury or surgery.

• Anti-inflammatory action for improved joint mobility.

Why is Wavelength Important?When it comes to wavelengths, the optimum “therapeutic window” for Laser therapy is from about 650nm to 1000nm. shorter wave-lengths are extremely powerful (X-Rays, Ultraviolet, etc.) and can break the bonds of atoms and produce ions. Wavelengths that are longer than 1500nm are ineffective because water has a very high absorption rate at these wavelengths and since water is a major component of muscle tissue - this radiation doesn’t penetrate much below the skin. Therefore, we are left with a range of wavelengths that are effective for treatment between 650nm and 1500nm where the radiation doesn’t cause any damaging ionization (vs. <650nm) and which are able to penetrate beneath the skin and into the deeper tissues (vs. >1500nm).

Within this range between 650nm and 1500nm, it is the 650nm to 900nm range of Wavelengths that is most effective for therapeutic treatment4 5. Wavelengths in the 650nm to 900nm range can penetrate deeper into tissues because the light beam’s photons are not heavily absorbed by the hemoglobin and water in the treatment area tissue, and yet will not produce a lot of unwanted heat like Lasers which produce wavelengths over 900nm.

It is widely accepted that longer wavelengths (800nm-900nm) are the optimal treatment range and can penetrate deeper than those at the shorter end of this spectrum (650nm-800nm)4 5. However, it is very important to always remember that output power has a major impact on a Laser’s potential depth of penetration. For example, a 2,000mW (2Watt) Laser which produces a 700nm Wavelength will actually deliver a faster and deeper treatment than a 200mW (0.2Watt) Laser producing a 900nm Wavelength. This is why it is most important when evaluating a Laser or LED device to consider the balance between its Wavelength and actual Output Power.

Total Power is NOT Output PowerTotal power (or peak power) is irrelevant. It is a Laser’s OUTPUT power which you need to know in order to assess a Laser’s true capabilities and determine if those capabilities are consistent with the kind of conditions that you plan to treat. simply put, treatment times are inversely related to the average output power of any Laser. All other things being equal, the more average output power a Laser produces – the faster it can deliver the appropriate dose of energy required to complete a treatment. Output power (in conjunction with Wave-length) is also a major factor when evaluating a system’s potential depth of penetration into tissue. Don’t confuse a laser’s TOTAL power with its true OUTPUT power.

For example, be careful when considering Laser systems that make statements about extremely high power like 25Watts - 50Watts (25,000mW - 50,000mW). These kinds of statements are usually referring to the system’s total power, NOT output power, and in reality they are commonly Super Pulsed systems with very low output power.

Understanding a Laser’s output power is vital. If a supplier can’t or won’t tell you their Laser’s actual output power (preferably broken down by individual laser diode output) – ask them – “Why not?”

What’s the difference between Laser Diodes and LeDs or sLDs?

For therapeutic treatments there are two types of light sources or Diodes which are most commonly used. They are Laser Diodes (LD) and Light emitting Diodes (LeD). some devices that call themselves Laser systems use an-other light source called an Super Luminous Diode) SLD. All of these sources usually produce Visible Red (VR) or Infra-Red (IR) light. Most of these Diodes emit light in the 600nm - 900nm Wavelength range.

There are significant differences in how these different light sources function • Laser Diodes are Monochromatic and therefore emit only a single color of light and one Wavelength within a very narrow bandwidth range of .1nm to 1 nm.

• LEDs also appear to the naked eye to emit a single light color, however they actually produce light Wavelengths in a narrow bandwidth range of about 30nm to 60nm.

• SLDs emit in an even wider range and are therefore not often used, or are used only as a “supplementary” light source to Laser or LeDs and are therefore not critical to this discussion.

Laser Diodes emit light waves that are highly organized, directional and “in-phase” which are called “coherent” light. LeDs emit light waves that are not as organized or directional, and which are “out-of-phase” called “non-coherent” light. Assuming the same Output Power, we can expect the light emitted by a Laser Diode to penetrate deeper than the light emitted by an LeD Diode, because the light from a Laser Diode is coherent, brighter and more directional than LeDs. Laser diodes are much more expensive to manufacturer and therefore, cost more than LeDs or sLDs.

CLUSTER PROBES

Research has indicated that while multi-diode “cluster” probes may be effective in reducing overall

treatment times for large areas, these device’s different diodes usually produce different

wavelengths which can lead to a degradation of the effectiveness of laser therapy.2

Beam AreaAverage Output Power (Not Total Power) & Wavelengths are necessary information, but is not enough information to accurately define the parameters of the laser system. To do this, one must also know the area of the laser beam (cm2) at the treatment surface to calculate energy density.

Continuous, Pulsed & Super PulsedAgain, don’t be confused by these terminologies. As the words suggest; “Continuous” simply refers to a Laser capable of delivering a continuous beam of light energy, while the terms “Pulsed” simply mean the Laser beam is repeatedly turned on and off. During the

How do I choose the right laser for my practice?If a supplier is not being forthcoming with their laser’s true specifications, you can turn to their international safety rating to get a basic understanding. Every laser on the market is tested for its hazard and risk level. In general, Class I Laser systems use VERY LOW Output Power and are therefore very safe, even for direct exposure to eyes and are sometimes marketed for patients to use at home. At the other end of the scale are Class IV Laser devices that use very HIGH Output Power (more than 500mW or 0.5 Watts) and re-quire the use of strict Safety Protocols.

General Treatment Guidelines• If your focus is deep tissue and musculoskeletal conditions and occasionally treat superficial conditions (wounds, etc.) – It makes sense to choose a device with true In-fraRed (IR) Laser diodes in a cluster probe with at least 500 mW of Output power or more.

• If you prefer short treatment times of 1 - 2 minutes and still want to be able to treat deep tissue conditions – You should choose a device with true Laser diodes and with output power of over 1,000 mW (1 Watt).

• If you do NOT want to treat deep tissue conditions and your focus is primarily treat-ing peripheral neuropathy and/or superficial wounds and skin conditions – You might consider an inexpensive LeD device since deep penetration is not needed and true Laser Diodes are very expensive. Keep in mind though that true InfraRed (IR) Lasers also work very well for superficial conditions, they are just more expensive and difficult to cost-justify unless you also do deep tissue treatments.

• For analgesic treatments you will require a true Laser probe with 5,000 mW – 10,000 mW of output power, or a 2,000 mW – 4,000 mW probe with slightly longer treatment times.

moments when it is on it has typically the same output power as in continuous mode, but as it is not on all the time, the average out-put power is less than when it is continuous. The average power is a function of the continuous wave power and the duty cycle (the ratio of the “on” time of the beam to the total emission (“on” + “off”) time, usually expressed as a percentage). Typical pulsed laser types are most of the gas lasers (such as the HeNe laser) and all semiconductor (diode) lasers (except the GaAs laser).

In general, pulsing is done to reduce thermal responses in surface tissue for lasers with high average output power. With these la-sers, pulsing the beam can also help to better control dosage, since they may also have extremely quick treatment times.

Super-PulsedThe original GaAs lasers were the first semiconductor lasers and required “Super Pulsing”. In order to generate laser light, the cur-rent density in the GaAs semiconductor crystal had to be extremely high. As a consequence of the high electric current the output power of this semiconductor laser is very high. Typical peak power is in the order of many watts. However, when an electric current is conducted through a material heat is generated, and with the necessary high current in this laser the crystal will burn up immediately unless the time of current conduction is extremely short, therefore super-pulsed GaAs lasers cannot work continuously. The maximal pulse time for this laser is in the order of 100 to 200 nanoseconds and, after each such pulse, a long cooling time is needed, usually about a thousand times longer than said pulse time. This form of pulsing is called super pulsing.

Even though the peak power of the super-pulsed GaAs laser may be very high, it lasts for an extremely short time compared to the pulse cycle, resulting in an average output power that is usually a thousand times lower than the peak power. For example, if the laser has 25,000 mW of Peak Power the average output power would end up being 25 mW. For clinical use, it is the output power that counts. The energy (dose) delivered from pulsed lasers is always the average output power multiplied by the exposure time.

o6Easy MaintenanceA constant monitoring system for the de-frosted water level and a defrosting feature provide a smooth-running daily operation. The air filter is easy to access; just vacuum when dusty.

Precision and EfficacyThe standard nozzle is used for semi-stationary and dynamic treatmentfrom 2 to 5 cm skin distance. The 5 and 10 mm nozzles permit precise treatment for small narrow areasand trigger-points.

The variable air flow is also used to dose the treatment efficiently.

Quick CoolingTo ensure a rapid reduction in the skin temperature, the cooled air is appliedto the skin with -30° C. This provides a drop of the skin temperature to 5° C within 10 seconds, offering a strong analgesic effect without risks.
