Workplace Investigation Initiation

Workplace investigations are conducted when the following occur:

1.Near-Miss 2.Incident 3.Accident

Near-Miss

What defines a Near-Miss Incident?

– Occupational event that is unplanned and unexpected within reason

– Has potential to cause an accident or incident

– Workplace damage, or injury /illness causing harm to an individual

Incident

What constitutes a workplace Incident?

– Unplanned and unexpected

– Results in NO lost time

– Results in NO injury

– Results in property loss that involves occupational hazards (uncontrolled hazards)

Accident

Defining a workplace Accident:

– Occupational accident that is unplanned and unexpected

– Results in an injury/illness and/or property loss

– Involved occupational hazards that are uncontrolled

People:

•You need to determine insight from what happened

•Determine their thought-process in the investigation

Equipment:

•How is it being used/operated?

• What is the maintenance schedule?

Material:

•How long has it been there?

•How is it used and who uses it?

Environment :

•Is the workplace layout suitable for the type of work

•What about the sounds, temperature, traffic?

Understand the Equipment, Material, Environment, and Process

Prior to interviewing people…Understanding the mechanic aspects of the occurrence first

will guide you to understand the human factor

Conducting Investigations

At the Scene:

•No chance of reoccurrence•Gain insight of layout and function •Account for all workers•Account for actual and possible contributing hazards and controls •Take pictures, and obtain drawing and documentation •Appraise and account for any physical damage

Witness Statements:

•Arrange for Individual Interviews

•Separate Witnesses

•Witness re-account occurrence

•Arrange for support systems

• Employee Assistance Programs (EAP)

Nature of injury:

•Understand and describe sustain injury(s)

•Understand if there are pre-existing conditions

•Measure the impact of the loss

Critical Injury Response

What is a Critical Injury?•Places life in jeopardy

•Produces unconsciousness

•Results in substantial loss of blood

•Involves the fracture of a limb

•Involves amputation of a limb

•Consists of burns to major parts of the body

•Loss of eye sight

•Hospital stay of more than two days

Critical Injury Response

• Seek Medical attention immediately

• Notify Leadership Teams or Senior Management immediately

• Notify appropriate authorities

• Be prepared to deal with Media

• Investigate with authorities

• Report
